Importancia de la Relación Anatómica entre el nervio Isquiático y el Margen Posterior del Acetábulo en la Luxación Posterior de la Cadera Anatomic Value of the Sciatic Nerve with the Posterior Margin of the Acetabulum in Posterior Dislocation of the Hip

Abstract: La luxación posterior de la articulación de la cadera es un trauma que puede sufrir una buena parte de la población, donde se puede comprometer el nervio isquiático, produciendo serios problemas en la vida del individuo afectado. El conocimiento de la relación anatómica entre el nervio isquiático y el margen posterior del acetábulo propiciará um diagnóstico preciso y precoz de esta lesión. Con el objetivo de estudiar la relación topográfica del nervio isquiático con el margen posterior del acetábulo y entregar antecedentes anatómicos para las cirugías ortopédicas en caso de luxaciones posteriores de la cadera, se estudiaron 40 miembros inferiores formolizados, de cadáveres humanos, adultos y de ambos sexos, localizados en los laboratorios de anatomía topográfica de la Universidad Estadual de Ciencias de la Salud de Alagoas, Brasil. En 19 miembros inferiores (47,7%) el nervio isquiático pasó directamente sobre el margen posterior del acetábulo y en los 21 restantes (53,3%), el nervio estuvo distante del margen mencionado, distancia que varió de 6 a 49 mm. Los resultados obtenidos muestran la estrecha relación entre el nervio isquiático y el margen acetabular, lo que acrecienta la posibilidad de lesión del nervio em los casos de luxación posterior de la cadera. The posterior dislocation of the hip joint is a trauma that can undergo a large part of the population, which may compromise the sciatic nerve, causing serious problems in the life of the affected individual. Knowledge of the anatomical relationship between the sciatic nerve and the posterior margin of the acetabulum will facilitate accurate diagnosis and early um this injury. In order to study the topographical relationship of the sciatic nerve with the posterior margin of the acetabulum and provide background for orthopedic anatomical in posterior dislocations of the hip were studied 40 lower limbs formolized human cadavers, adults of both sexes , located in the topographical anatomy laboratories of the Universidade Estadual de Ciencias da Saude, Alagoas, Brasil. In 19 limbs (47.7%) sciatic nerve passed directly over the posterior margin of the acetabulum and the remaining 21 (53.3%), the nerve passed distal to the acetabular margin, distance ranging from 6-49 mm. The results show the close relationship between the sciatic nerve and the acetabular margin, which increases the possibility of nerve injury in posterior dislocation of the hip.

Abstract: the objective of this study was to compare three methods to determine number of groups in studies with hierarquical cluster analysis, based at data from characterization of capsicum accessions, in order to identify those with the greatest power of discrimination. mojena method, tocher method and rmsstd method were applied to determine the optimal number of groups formed in final stage of the upgma method. forty nine capsicum chinense accessions from the vegetable germplasm bank of the universidade federal de vi?osa were analyzed, in relation to ten morphological characters for identifying the most similar accessions group, making possible the selection of superior genotypes, i.e., of commercial interest. the rmsstd method allowed to conclude on the existence of seven groups, with a greater power of discrimination for this method, compared to the tocher optimization method and the mojena method, which formed, respectively, four and three groups.

No outro extremo da ágora está Cícero, para quem a Retórica n o tem que procurar pela essência de nada, mas fazer com que a opini o do orador, seja ela verdadeira ou n o, passe a ser também a do auditório. Para tanto, a lógica, t o acalentada por Plat o, é de bem pouca valia, pois o orador, se quiser ter sucesso, deve levar os cora es da cólera à do ura ou desta àquela. Como se vê, é uma retórica alicer ada sobre o pathos, ou, como diriam os semioticistas, no estado das almas. Este artigo se prop e a pensar essa quest o utilizando o instrumental da semiótica dita tensiva com o objetivo de mostrar que, no discurso do grego, predominam as valências ligadas à extensidade, ao inteligível, às características dos objetos; ao passo que, nos ensinamentos do romano, s o as valências relacionadas à intensidade, ao sensível, que se destacam.
Meningite bacteriana aguda na infancia: fatores de risco para complica??es agudas e sequelas

Abstract: objective: to assess acute neurological complications and neurological sequelae of childhood acute bacterial meningitis in order to determine possible warning signs. methods: this retrospective study evaluated children with acute bacterial meningitis (between 1 month and 14 years of age) admitted between 2003 and 2006. results: of the 44 patients studied, 17 (38.6%) had acute neurological complications. seizure was the most frequent (31.8%) complication. patients with acute neurological complications showed a higher frequency of lower neutrophil count (p = 0.03), seizure at admission (p < 0.01), and s. pneumoniae as the etiologic agent (p = 0.01). risk factors for the development of acute neurological complications were s. pneumoniae (odds ratio [or] = 6.4, confidence interval [ci] 1.7-24.7) and neutrophil count < 60% (p < 0.01). of the 35 patients who were followed up, 14 had neurological sequelae (40%). behavioral change (22.9%) was the most frequent sequela. seizures at admission (or = 5.6, ci 1.2-25.9), cerebrospinal fluid protein concentration > 200 mg/dl (p < 0.01), and cerebrospinal fluid glucose concentration/glycemia ratio (p < 0.01) were identified as risk variables for sequelae. conclusion: neutrophil count < 60%, seizure at admission, and s. pneumoniae as the etiologic agent were identified as warning signs for acute neurological complications, while protein levels, cerebrospinal fluid glucose concentration/glycemia ratio, and seizure at admission were seen as risk factors for neurological sequelae.

Detection and Quantification of Leptospira interrogans in Hamster and Rat Kidney Samples: Immunofluorescent Imprints versus Real-time PCR

Abstract: A major limitation in the clinical management and experimental research of leptospirosis is the poor performance of the available methods for the direct detection of leptospires. In this study, we compared real-time PCR (qPCR), targeting the lipL32 gene, with the immunofluorescent imprint method (IM) for the detection and quantification of leptospires in kidney samples from the rat and hamster experimental models of leptospirosis. Using a virulent strain of Leptospira interrogans serovar Copenhageni, a chronic infection was established in the rat model, which were euthanized 28 days post-infection, while the hamster model simulated an acute infection and the hamsters were euthanized eight days after inoculation. Leptospires in the kidney samples were detected using culture isolation, qPCR and the IM, and quantified using qPCR and the IM. In both the acute and chronic infection models, the correlation between quantification by qPCR and the IM was found to be positive and statistically significant (P<0.05). Therefore, this study demonstrates that the IM is a viable alternative for not only the detection but also the quantification of leptospires, particularly when the use of qPCR is not feasible.

Abstract: the effect of glycemic state on status epilepticus (se) development was studied in animals of different ages, submitted to pilocarpine model of epilepsy. groups: i- rats with 9-day-old (p9): ia. submitted to 1se; ib. saline-treated; ic. induced- hyperglycemia; id. induced- hyperglycemia+se; ii- rats submitted to three consecutive episodes of se at p7, p8 and p9; iii- rats submitted to 1se at p17; iv- rats submitted to 1se at p21. hippocampal cell death and the expression of glucose transporter glut3 were analyzed in group i. the results demonstrated normoglycemia in the groups ia, ib and ii, hypoglycemia in group iii and hyperglycemia in group iv, showing that the glycemia during se is age dependent. induced hyperglycemia during se in p9 protected the hippocampal neurons from death and both groups ic and id presented increased glut3 expression, showing high glucose consumption by the hippocampus.
Hypomagnesemia in critically ill cancer patients: a prospective study of predictive factors

Abstract: hypomagnesemia is the most common electrolyte disturbance seen upon admission to the intensive care unit (icu). reliable predictors of its occurrence are not described. the objective of this prospective study was to determine factors predictive of hypomagnesemia upon admission to the icu. in a single tertiary cancer center, 226 patients with different diagnoses upon entering were studied. hypomagnesemia was defined by serum levels <1.5 mg/dl. demographic data, type of cancer, cause of admission, previous history of arrhythmia, cardiovascular disease, renal failure, drug administration (particularly diuretics, antiarrhythmics, chemotherapy and platinum compounds), previous nutrition intake and presence of hypovolemia were recorded for each patient. blood was collected for determination of serum magnesium, potassium, sodium, calcium, phosphorus, blood urea nitrogen and creatinine levels. upon admission, 103 (45.6%) patients had hypomagnesemia and 123 (54.4%) had normomagnesemia. a normal dietary habit prior to icu admission was associated with normal mg levels (p = 0.007) and higher average levels of serum mg (p = 0.002). postoperative patients (n = 182) had lower levels of serum mg (0.60 ± 0.14 mmol/l compared with 0.66 ± 0.17 mmol/l, p = 0.006). a stepwise multiple linear regression disclosed that only normal dietary habits (or = 0.45; ci = 0.26-0.79) and the fact of being a postoperative patient (or = 2.42; ci = 1.17-4.98) were significantly correlated with serum mg levels (overall model probability = 0.001). these findings should be used to identify patients at risk for such disturbance, even in other critically ill populations.
