Accuracy of Endosonography in the Diagnosis of Obstructive Jaundice

Abstract: Background and Objectives: Endoscopic retrograde cholangiopancreatography (ERCP) has been used as the method of choice in assessment of obstructive jaundice for a long time. Recently, the development of endoscopic ultrasonography (EUS) has attracted the attention of clinicians as an alternative accurate, safe and cheap method to replace the ERCP procedures performed only for diagnostic purposes. Methods: In this study, 100 patients underwent endoscopic ultrasonography in Digestive Disease Research Center, Shariati Hospital. The patients were divided into two groups: 1) those with operable periampullary tumors who were referred to the surgeons, 2) other patients. The final diagnosis made after ERCP and operative removal of the mass was considered as the gold standard and was compared with accuracy of endoscopic ultrasonography. Results: The overall accuracy of endoscopic ultrasonography in the diagnosis of obstructive cholestasis was 92.0%. Sensitivity and specificity of this method in diagnosis of choledocholithiasis were 95.2% and 96.4% respectively. The corresponding values for diagnosis of periampullary tumors were 96.2% and 95.8%. Conclusion: In this study the accuracy of EUS in diagnosis of obstructive jaundice and its sensitivity and specificity for diagnosis of choledocholithiasis and periampullary tumors were high. Therefore, it could be concluded that endoscopic ultrasonography is a reliable diagnostic tool for extrahepatic cholestasis.

Abstract: In this paper hybrid converters with double inputs are investigated. Mainly any possible topologies are constructed and only one of them is chosen and modeled in none ideal format, by state space averaging method. After that small signal model and equilibrium point of that model is calculated. Finally with power sharing analysis the best point in efficiency was calculated.
Using THELI pipeline in order to reduce Abell 226 multi-band optical images

Abstract: In this paper we review THELI (Erben & Schrimer, 2005), an image processing pipeline developed to reduce multi-pointing optical images taken by mosaic CCD cameras. This pipeline works on raw images by removing several instrumental contaminations, implementing photometric calibration and astrometric alignment, and constructing a deep co-added mosaic image complemented by a weight map. We demonstrate the procedure of reducing NGC3923 images from raw data to the final results. We also demonstrate the quality of our data reduction strategy using mag-count and mag-error in mag plots. Emphasis is mainly placed on photometric calibration which is of great interest to us due to our scientific case. Based on the cross-association of the extracted catalogue against a reference catalogue of stellar magnitudes, zero-point calibration is performed. Our data reduction strategy and the method employed for cross-correlating large catalogues is also presented.
Sodium and potassium allocation under drought stress in Atlas mastic tree (Pistacia atlantica subsp. mutica)

Abstract: Sodium and potassium ions have crucial roles in drought tolerant mechanisms of plants. In a pot experiment, seedlings of Pistacia atlantica subsp. mutica were cultivated in a silty clay soil (Kex=21 mg/l, Naex=143 mg/l) and exposed to different levels of soil water content (90, 65 and 45% FC). P. atlantica maintained leaf transpiration rate, and relative water content and cation (Na+ and K+) content of different organs under moderate water deficit (above permanent wilting point). At higher water shortage, the amount of transpiration rate and the relative water content of tissues were reduced, but the amount of Na+ and K+ remained unaffected even under severe drought stress (below permanent wilting point). Generally, the amount of K+ content in plant organism was higher than that of Na+. The amount of root to shoot Na+ was higher than that of K+. Potassium contents of leaves and stems were positively related with transpiration rate, whereas Na+ contents of roots and stems were positively related with root relative water content. Roots and stem of Atlas mastic tree seedlings exhibit high drought tolerance index for Na+ and K+.

Abstract: Shigellosis is a acute inflammatory colon disorder that was developed by variety strains shigella and have high prevalence in infants, preschool children and Tropical climates particularly Iran. The purpose of the present study was to determine the frequency rate of neurologic complications with shigellosis."nMaterials and Methods: In a cross sectional study, 390 patients with shigellosis that were hospitalized in Bahrami Hospital of Tehran in period of 5 years 1997-2002 were evaluated with Neurological Manifestations and Complications ."nResults: This study revealed a frequency rate of 80% for Neurological complicaton (encephalopathy alone= 15.38%, convulsion alone= 43.08%), convulsion and encephalopaty= 18.47% , Ekiri syndrome= 3.07%)."nConclusion: The reason of difference this study with other world studies is probably Restricted of this study in The hospitalized patients and over diagnosed of convulsion by parents and Medical personalies. this results suggest that neurologic complication particularly seizure commonly associated with shigellosis also Ekiri syndrome in this study has had relatively high frequency rate. For this reason serious prevention and Rapidly Treatment of shigellosis is necessary.
Risk Factors of First Acute Myocardial Infarction: Comparison of Elderly and Non-Elderly: A 24-Year Study  [PDF]

Abstract: Although risk factors of Ischemic Heart Disease (IHD) are well known, knowledge about the distribution of these risk factors in different subgroups of patients can be used for designing preventive programs. The aim of this study was to assess differences of prevalence of IHD risk factors in elderly and non-elderly patients in their first Acute Myocardial Infarction (AMI). In this retros-pective study, elderly (age > 65; n = 1060) and non-elderly (age ≤ 65; n = 2228) patients with first AMI who were admitted into four teaching hospitals in Tehran between 1982 and 2005 were recruited. Risk factors containing male gender, systolic and diastolic hypertension (HTN), diabetes, hypertriglyceridemia, hypercholesterolemia, smoking and family history of IHD were compared between the two groups. Among different risk factors, diabetes and systolic HTN were more common in elderly than their non-elderly counterparts, and male gender, smoking, family history of IHD and dyslipidemia were more common in non-elderly patients than elderly ones (p < 0.05 for all comparison). Diastolic HTN showed no significant difference between the two groups. Findings of this study, stating difference of prevalence of risk factors in two elderly and non-elderly groups, can be used by health policy makers to conduct preventive programs for AMI in this country.
Low Level Laser Therapy And Exercise Therapy In Treatment Of Chronic Low Back Pain: A Double-Blind Randomized Clinical Trial

Abstract: Background: This study was designed to compare low-level laser therapy (LLLT) + exercise therapy with LLLT alone and exercise therapy alone, and to determine whether laser therapy is a useful treatment modality for chronic low back pain (LBP). Materials and Methods: This study was a double-blind placebo-controlled randomized clinical trial. Patients with chronic LBP for at least 12 weeks were included. Visual analogue scale (VAS), Modified Oswestry Disability Questionnaire (MODQ), Schober test, flexion, extension and lateral bending were used to evaluate back pain, disability score and lumbar range of motion. Irradiation was performed with GaAlAs ( =810 nm, power density=226 mW/cm2) laser, two times a week, over a period of 6 weeks. Subjects were evaluated before the first treatment, at week 6 and 12 follow-up. Results: The reduction in pain related to motion was significantly greater in the exercise + LLLT group compared with the exercise alone group (P = 0.004) but was not significant, compared with LLLT alone (P = 0.982). Disability score in LLLT + exercise therapy reduced more than the other two groups, and the difference with exercise alone group was significant (p = 0.03). Comparison of reduction of disability between LLLT alone and exercise therapy alone was not statistically significant. Improvement of lumbar range of motion in patients treated with LLLT + exercise therapy was better than the other two groups significantly, especially by Schober test and Flexion and lateral bending. Conclusion: This study clearly shows that LLLT alone and especially LLLT combined with exercise can lead to better improvement in chronic LBP.

Abstract:

Introduction: The purpose of this study was to assess velocity-encoded cardiac magnetic resonance imaging (Ve-CMR) in a population of patients referred for cardiac magnetic resonance imaging (CMR), to determine the variability of atrial function, and to identify clinical parameters associated with left atrial function. Methods: This is a prospective study evaluating patients who were referred to our CMR center for a clinical CMR. Left atrial function was obtained via Ve-CMR thru-plane images across the mitral valve after acquiring 2 perpendicular in-plane images as “scouts”. The atrial function and mitral inflow were quantified by computer analysis (Argus, Siemens). Atrial function was defined as atrial contraction (A-wave) volume divided by total inflow volume. Left atrial volumes were calculated via computer analysis. Mitral regurgitation and left ventricular ejection fractions were assessed visually. Results: Thirty-nine patients, with mean age 56 +/- 10 years, were enrolled. The mean left atrial function was 22.9% +/-14.5%; the range in left atrial function was 0% - 57%. There was a significant positive correlation between atrial function and increased left ventricular ejection fraction (r = 0.44, P < 0.01). There was a significant negative correlation between atrial function and severity of mitral regurgitation (r = -0.60, P < 0.01), as well as left atrial volume (r = -0.36, P = 0.02). Conclusion: Our results indicate a wide variability in left atrial function and a significant association between left atrial function and left ventricular ejection fraction, left atrial volume and mitral regurgitation.

Female carpet weavers' visual acuity and effective factors: Fordu rural area of Qom

Abstract: Background: Healthy vision of workers is one of the important elements in carpet weaving industry which has essential role in improving of the job quality and efficiency. Visual acuity is the primary outcome measure in most studies involving eye diseases. Ihe aim of this study was determination of visual acuity and its effective factors in women carpet weaver in fordu rural area of Qom has been investigated. Methods: In a cross-sectional (descriptive-analytical) study visual acuity of 177 women carpet weaver has been determined. Job hours per day, job history, age, literacy, trauma history, diabetes history, family history of eye diseases, eye diseases history and family size of subjects were determined .Results: The results of study indicated that the right eyes visual acuity of 72.4 % of women were desirable (0 - 0.8) and 27.6 % were undesirable (0.9-1.2). These results for the left eyes were 69.5 % and 30.5 % respectively. In addition, the result showed that with increasing the job hours and history and age, percent of women with undesirable both eyes visual acuity were increased. With higher literacy levels, percent of women with undesirable both eyes visual acuity decreased. In subjects with truma history, the undesirable visual acuity was higher. In this study the relation between visual acuity and job history, age, literacy, truma history and eye diseases history were statistically significant (in all cases P<0.05).Conclusions: High percentage of women carpet weaver were in undesirable visual acuity and in this study the relation between visual acuity and job history, age, literacy, trauma history and eye diseases history were statistically significant (in all cases P<0.05).
