Evaluación externa del desempe?o en el tamizaje de bancos de sangre en Argentina: resultados y estrategias para mejorarlo

Abstract: objectives: because there was no program for the external evaluation of the quality of the screening for transfusion-transmitted infections (ttis) in blood banks in argentina, in 1999 the hemotherapy service of garrahan hospital, in buenos aires, launched an ongoing external evaluation program, with the support of the pan american health organization and the argentine hemotherapy and immunohematology association. methods: a panel of 12 samples that were reactive to all the markers screened for in argentine blood banks was distributed. the panel was delivered to 52 laboratories in 1999, 102 laboratories in 2000, and 118 laboratories in 2001. the participating laboratories were classified into one of four categories according to their performance: a: 0 or 1 false positive (fp) results; b: 2 or 3 fps; c: 4 or more fps; and d: at least 1 false negative result. workshops were held with the participants in order to jointly analyze the results. results: out of the laboratories that received the panel of samples, the percentage of laboratories that sent in their results was 92.3% in 1999, 92.2% in 2000, and 83.9% in 2001. these response levels demonstrate the interest in evaluation activities. however, the annual average percentage of the laboratories that received a "d" classification was very high (30%), which indicates that there are problems in the performance of tti screening. this poor showing could be related to the difficulty in taking corrective measures throughout the system, given the large number of laboratories involved. conclusions: evaluations of this type should be included in argentina's national blood program as a tool for the ongoing assessment of the blood banking situation, for decision-making, and for monitoring the quality of screening.

Abstract: OBJETIVOS: Debido a la falta de un programa para la evaluación externa de la calidad del tamizaje de infecciones transmisibles por transfusión (ITT) en los bancos de sangre, el Servicio de Hemoterapia del Hospital Garrahan, de Buenos Aires, Argentina, inició en 1999 un programa continuo con el apoyo de la Organización Panamericana de la Salud (OPS) y la Asociación Argentina de Hemoterapia e Inmunohematología (AAHI). MéTODOS: Se distribuyó un panel de 12 muestras con reactividad para todos los marcadores tamizados en los bancos de sangre argentinos. El panel se entregó a 52, 102 y 118 laboratorios en 1999, 2000 y 2001, respectivamente. Los laboratorios participantes se clasificaron según su desempe o en: A: menos de 2 resultados positivos falsos (RPF); B: de 2 a 3 PF; C: más de 3 RPF; y D: algún resultado negativo falso. Se efectuaron talleres con los participantes para analizar conjuntamente los resultados. RESULTADOS: Los porcentajes de respuesta de cada a o fueron 92,3, 92,2 y 83,9, respectivamente; estos porcentajes de respuesta indican interés en las actividades de evaluación. Sin embargo, el promedio de los laboratorios con clasificación D resultó muy alto (30%), lo que evidencia la presencia de problemas en el desempe o del tamizaje de las ITT. Esto podría asociarse con la dificultad de tomar medidas correctoras en todo el sistema, dado el elevado número de laboratorios involucrados. CONCLUSIONES: Se deben incluir evaluaciones de este tipo en el Programa Nacional de Sangre para el diagnóstico continuo de la situación, la toma de decisiones y el seguimiento de la calidad del tamizaje.

Abstract: the umbilical cord blood (ucb) is a rich source of hematopoietic progenitor cells (hpc). the unrelated program has been recently established and it has received financial support by the ministry of health to acquire new equipment. this program was aimed at collecting and preserving ucb from altruistic donors. the units are potentially used for allogeneic bone marrow transplantation in those patients who lack a suitable compatible donor. from august 2005 to january 2006 110 donors have been enrolled and 96 units collected. units processed (n=70) yield a mean recovery of 76.4 ± 7.4% of the total nucleated cell (tnc) collected with a final count of 0.8 ± 0.3 x 109 tnc and 2.79 ± 2.57 x 106 cd34+ cells per unit. by hla analysis, those variants common in the argentine population (a68, b3519, dr08) along with the less frequently found alleles were detected. this program will make it possible to increase the availability of units from hispanic-latin american background that are scarce in registries from other countries.

The GeV to TeV view of SNR IC443: predictions for Fermi

Abstract: We present a theoretical model that explains the high energy phenomenology of the neighborhood of SNR IC 443, as observed with the Major Atmospheric Gamma Imaging Cherenkov (MAGIC) telescope and the Energetic Gamma-Ray Experiment Telescope (EGRET). We also discuss how the model can be tested with observations by the Fermi Gamma-ray Large Area Space Telescope. We interpret MAGIC J0616+225 as delayed TeV emission of cosmic-rays diffusing from IC 443 and interacting with a known cloud located at a distance of about 20 pc in the foreground of the remnant. This scenario naturally explains the displacement between EGRET and MAGIC sources, their fluxes, and their spectra. Finally, we predict how this context can be observed by Fermi.
Multi-messenger model for the starburst galaxy M82

Abstract: In this paper, a consistent model of the multifrequency emission of the starburst galaxy M82, from radio to gamma-rays is presented and discussed. Predictions for observations with Fermi, MAGIC II/VERITAS and CTA telescopes are made. The model is also used to self-consistenty compute the (all flavors) emission of neutrinos resulting from this starburst galaxy, what can be used in considerations of the diffuse contributions of such objects.
The GeV to TeV connection in the environment of SNR IC 443

Abstract: We have recently interpreted the source MAGIC J0616+225 as a result of delayed TeV emission of cosmic-rays diffusing from IC 443 and interacting with a cloud in the foreground of the remnant. This model was used to make predictions for future observations, especially those to be made with the Fermi satellite. Just recently, AGILE, Fermi, and VERITAS have released new results of their observations of IC 443. In this work, we compare them with the predictions of our model, exploring the GeV to TeV connection in this region of space. We use Fermi data to consider the possibility of constraining the cosmic-ray diffusion features of the environment. We analyze the cosmic-ray distributions, their interactions, and a possible detection of the SNR environment in the neutrino channel.

Abstract: comparing field epidemiology and social epidemiology, we pretend to think about the no explicit images and meanings operating in both necessary convergent fields, about the obstacles present in epidemiological practice to fulfil its social function and about the necessity of changing epistemological, methodological and practice grounds, beginning with field epidemiologists teaching programmes. field epidemiology would tend to act in an absent theoretical frame. on the other hand, social epidemiology would tend to prioritize theoretical developments (thinking and research about social determinants) without correspondent action, because of the limits to change public policies. other differences are found at intervention level (micro-macrospace), its aim (outbreak control vs. inequalities control) and the way to communicate with society. they are similar in the methodological concern, the predominance of orientation based on positivism and framed through statistic methods, but in process of epistemological opening, the stress experienced between the alternative relationship to a virtual world of data bases or to the real society, their peripherical situation in relation of the political, social, institutional and professional system and the tendency to professional frustration. finally, we ask ten questions to the field epidemiologists related with their present practice, in order to consider if they are developing social epidemiology, and propose some changes in epidemiologist teaching and practice.

Abstract: based on the reports of the chilean foreign mission in canada, this article deals with the relations between chile and canada during the period in which both countries decided to exchange embassies. it explains that before those years, canada was interested in latin america mainly in commercial topics, while chile showed more interest on the diplomatic side. this difference is due to the fact that chile found itself in a rather vulnerable position before 1939. the second world war was the main reason that led to the establishment of embassies in both countries. the article analyzes also the first impressions of the chilean representatives in ottawa, showing that it was not easy for chilieans to understand the nature of a country like canada. it ends saying that in the short term, the establishment of embassies did not lead to an increase in the commercial or the political relations between both countries
