Abstract: introduction: haemodialysis (hd) lines are traditionally primed with sodium heparin and saline solution, which entails the haemorrhagic risk associated to the passing of heparin into the blood. aim: to compare hd dialyser priming with and without sodium heparin, in terms of coagulation, dialytic efficacy, and economic efficacy. material and method: a randomized, prospective studio of 3482 sessions with 109 hd patients (x 66.8±15 years, 40% women, 44% diabetic, 45% with high-flow tunnelled catheter, 50% native iaf, 5% prosthetic iaf). 1827 sessions carried out with fifty-nine patients were included in the control group (primed with saline solution + sodium heparin) and 1455 sessions with patients in the study group (primed with saline solution). the monitoring period was 5 months. variables related to the coagulation of the circuit and the efficacy of the dialysis were analysed: selected blood flow (qb) and effective qb, final status of the dialyser, number of changes of dialyser, number of changes of the arterial line, number of changes of the venous, average kt/hour measured by ionic dialysance and heparinization of the circuit. the efficacy in economic terms was also studied. results: there are no significant differences in the results related to coagulation and efficacy of the dialysis obtained in both groups, except x of heparin with low molecular weight (2476±1089 iu in the study group compared to 2854±1414 iu in the control group). priming without heparin represents a saving of 0.50 € per session. conclusions: priming with saline solution does not affect the hd dose, and does not entail a greater risk of coagulation of the system compared to a priming solution to which heparin has been added, avoids the risk of heparin passing into the bloodstream, reduces handling in the connection protocols and reduces costs.

Viral sequences of the cerebrospinal fluid were amplified by using olygonucleotides that are homologous to region 5′of the Enteroviruses. The results showed a great variability in the amplified sequences, suggesting the presence of enteroviral quasi-species with altered biological properties in the stressed individuals that may play an important role in the neurological damage.

MediSur , 2011,
Abstract: Background: the study of dietary patterns in adolescence becomes of great importance because at this stage individuals acquired eating habits that they will keep throughout their lives. These habits, if not adequate, may increase the risk of chronic diseases in adult ages. Objective: To determine nutritional patterns of eighth grade students and their impact on dietary adequacy. Methods: descriptive study including a universe integrated by all eight grade students in the 5 de Septiembre Junior High School of Cienfuegos. Through simple random sampling, 59 teenagers were selected to be administered two surveys: one on food preferences and another one on a recall of their meals in the last 24 hours. The surveys were prepared by the Institute of Nutrition and Food Hygiene. Results: students preferred fried and sugary foods; only 3.38% received the recommended iron intake; 49, 15% of children received more than 60% of energy from carbohydrates and 59.32% received low amounts of proteins. Conclusions: There are still gaps in these students' nutritional behavior. The major concerning points are the low energy intake and the fact that it is mainly based on carbohydrates, while the deficit in proteins intake, iron and fat has reached a critical point in more than half of our patients.
Mitigating an undesirable immune response of inherent susceptibility to cutaneous leishmaniosis in a mouse model: the role of the pathoantigenic HISA70 DNA vaccine

Veterinary Research , 2012, DOI: 10.1186/1297-9716-43-59
Abstract: Leishmania major is the major cause of cutaneous leishmaniosis (CL) outside of the Americas. In the present study we have cloned six Leishmania genes (H2A, H2B, H3, H4, A2 and HSP70) into the eukaryotic expression vector pCMVβ-m2a, resulting in pCMV-HISA70m2A, which encodes all six pathoantigenic proteins as a single polyprotein. This expression plasmid has been evaluated as a novel vaccine candidate in the BALB/c mouse model of CL. The DNA vaccine shifted the immune response normally induced by L. major infection away from a Th2-specific pathway to one of basal susceptibility. Immunization with pCMV-HISA70m2A dramatically reduced footpad lesions and lymph node parasite burdens relative to infected control mice. Complete absence of visceral parasite burden was observed in all 12 immunized animals but not in any of the 24 control mice. Moreover, vaccinated mice produced large amounts of IFN-γ, IL-17 and NO at 7 weeks post-infection (pi), and they showed lower arginase activity at the site of infection, lower IL-4 production and a weaker humoral immune response than infected control mice. Taken together, these results demonstrate the ability of the HISA70 vaccine to shift the murine immune response to L. major infection away from an undesirable, Th2-specific pathway to a less susceptible-like pathway involving Th1 and Th17 cytokine profiles.

BMC Microbiology , 2012, DOI: 10.1186/1471-2180-12-215
Abstract: Background Group A Streptococcus (GAS) causes human diseases ranging in severity from uncomplicated pharyngitis to life-threatening necrotizing fasciitis and shows high rates of macrolide resistance in several countries. Our goal is to identify antimicrobial resistance in Spanish GAS isolates collected between 1994 and 2006 and to determine the molecular epidemiology (emm/T typing and PFGE) and resistance mechanisms of those resistant to erythromycin and tetracycline. Results Two hundred ninety-five out of 898 isolates (32.8%) were erythromycin resistant, with the predominance of emm4T4, emm75T25, and emm28T28, accounting the 67.1% of the 21 emm/T types. Spread of emm4T4, emm75T25 and emm28T28 resistant clones caused high rates of macrolide resistance. The distribution of the phenotypes was M (76.9%), cMLSB (20.3%), iMLSB (2.7%) with the involvement of the erythromycin resistance genes mef(A) (89.5%), msr(D) (81.7%), erm(B) (37.3%) and erm(A) (35.9%). Sixty-one isolates were tetracycline resistant, with the main representation of the emm77T28 among 20 emm/T types. To note, the combination of tet(M) and tet(O) tetracycline resistance genes were similar to tet(M) alone reaching values close to 40%. Resistance to both antibiotics was detected in 19 isolates of 7 emm/T types, being emm11T11 and the cMLSB phenotype the most frequent ones. erm(B) and tet(M) were present in almost all the strains, while erm(A), mef(A), msr(D) and tet(O) appeared in less than half of them. Conclusions Spanish GAS were highly resistant to macrolides meanwhile showed minor resistance rate to tetracycline. A remarkable correlation between antimicrobial resistance and emm/T type was noticed. Clonal spread of emm4T4, emm75T25 and emm28T28 was the main responsable for macrolide resistance where as that emm77T28 clones were it to tetraclycline resistance. A wide variety of macrolide resistance genes were responsible for three macrolide resistance phenotypes.

Natural Resources (NR) , 2013, DOI: 10.4236/nr.2013.48059
Abstract:

The mechanism through which exit options and outside opportunities affect cooperation outcomes has not been well studied in the resource management literature, since a deep analysis of the concepts of exit options and outside opportunitiesis missing. This article analyzes these concepts across the common pool resource management literature, investigates the factors that underlie opting out decisions, and reviews potential ways to operationalize these concepts for empirical analysis. It also explores how the relationship of cooperation and exit options may be influenced by broad economic processes, such as economic integration.
