Submental method for orotracheal intubation in treating facial trauma

Abstract: objective: to demonstrate an alternative method for intubating patients with fractures of maxilla and nose, prior to surgery. design: cases report. participants: we studied 10 patients with facial fractures that affected maxilla and nose. intervention: the patients were submitted to surgery under general anesthesia and submental oro-tracheal intubation. results: this type of intubation allowed the surgical team to work on the whole face of the patient and left no visible scar. conclusion: this procedure is indicated for patients with fractures of maxilla and nose who need surgical intervention under general anesthesia.
Determina o de volumes e press es de balonetes de tubos traqueais insuflados com ar ambiente ou óxido nitroso

Abstract: JUSTIFICATIVA E OBJETIVOS: A press o exercida pelo balonete do tubo traqueal contra a parede da traquéia deve permitir fluxo capilar adequado e prevenir escapes de ar ou aspira o pulmonar. Esta pesquisa procurou determinar as varia es de press o do balonete insuflado com ar ambiente ou com óxido nitroso a 100%. MéTODO: Trinta pacientes foram selecionados para receber anestesia geral balanceada com intuba o orotraqueal. O balonete foi insuflado conforme critérios clínicos. As medidas de base foram realizadas após 15 minutos do início da anestesia com um man metro aneróide calibrado em cm de H2O e forneceram os valores iniciais de press o e volume. Os pacientes foram divididos em dois grupos: balonete reinsuflado com ar ambiente, grupo A, ou com óxido nitroso, grupo B. As medidas de press o foram obtidas em intervalos até a primeira hora e os resultados comparados. RESULTADOS: Os grupos mostraram-se comparáveis para idade e sexo. Em ambos os grupos os valores basais médios para press o foram próximos de 40 cmH2O com 8 ml de volume. No grupo com ar ambiente, as press es aumentaram até 36 cmH2O em uma hora. No grupo de balonete insuflado com N2O, as press es diminuíram abaixo de 20 cmH2O entre 20 e 30 minutos de anestesia. CONCLUS ES: O uso de N2O a 100% para insufla o do balonete de sonda traqueal n o constitui método seguro, acarretando progressiva perda da capacidade de veda o. O uso de ar ambiente promove aumento de volume e de press o no balonete, aumentando possibilidade de les o da mucosa traqueal.

Abstract: Quali sono le questioni chiave, le criticità che interessano la città storica di Bologna? Cosa succede, cosa cambia nella città storica? Una città nota nel mondo per la valorizzazione del suo centro storico e per le politiche sociali rivolte ai suoi abitanti. Una prima considerazione riguarda il superamento del concetto di centro storico e l’allargamento alla città storica che comprende anche la città consolidata otto-novecentesca rappresentata dal Quartiere della Bolognina e dalla città giardino pedecollinare. Questo cambiamento dello sguardo dell’urbanistica sulla città storica - dal centro storico alla città storica - ha comportato un criterio di dilatazione della storicità. Sono infatti da comprendere nella città storica anche le testimonianze architettoniche del moderno che hanno contribuito alla trasformazione urbana.
Ceramic materials lead to underestimated DNA quantifications: a method for reliable measurements

Abstract: In the context of investigating cell-material interactions or of material-guided generation of tissues, DNA quantification represents an elective method to precisely assess the number of cells attached or embedded within different substrates.Nonetheless, nucleic acids are known to electrostatically bind to ceramics, a class of materials commonly employed in orthopaedic implants and bone tissue engineering scaffolds. This phenomenon is expected to lead to a relevant underestimation of the DNA amount, resulting in erroneous experimental readouts. The present work aims at (i) investigating the effects of DNA-ceramic bond occurrence on DNA quantification, and (ii) developing a method to reliably extract and accurately quantify DNA in ceramic-containing specimens.A cell-free model was adopted to study DNA-ceramic binding, highlighting an evident DNA loss (up to 90%) over a wide range of DNA/ceramic ratios (w/w). A phosphate buffer-based (800mM) enzymatic extraction protocol was developed and its efficacy in terms of reliable DNA extraction and measurement was confirmed with commonly used fluorometric assays, for various ceramic substrates. The proposed buffered DNA extraction technique was validated in a cell-based experiment showing 95% DNA retrieval in a cell seeding experiment, demonstrating a 3.5-fold increase in measured DNA amount as compared to a conventional enzymatic extraction protocol.In conclusion, the proposed phosphate buffer method consistently improves the DNA extraction process assuring unbiased analysis of samples and allowing accurate and sensitive cell number quantification on ceramic containing substrates.
Electroweak Physics in Six-Fermion Processes at Future Colliders

Abstract: Recent developments in the field of complete electroweak tree-level calculations for six-fermion final states in e+ e- collisions are briefly reviewed. Particular attention is given to top-quark and Higgs boson physics, which are items of primary importance at the Next Linear Collider. The relevance of electroweak backgrounds and finite-width effects is discussed, showing the importance of complete calculations for precision studies at the colliders operating in the TeV energy range.
Tutoria com médicos residentes em anestesiologia: o programa da Irmandade da Santa Casa de Misericórdia de S?o Paulo

Abstract: background and objectives: medical training programs may bring emotional distress. the main objective of this study was describes a mentoring program during the residence training period of physicians in anesthesiology. methods: the program was instituted at the central hospital of the irmandade da santa casa de misericórdia, s?o paulo. in this program were included 27 residents in anesthesiology and 4 health professionals that made up the mentoring team. the study was made through a evaluation of a questionnaire applied on first and second-year residents' opinion about education in anesthesiology. it was developed, with twelve questions and three answers for each question, which has been applied before and after the program. residents were divided in four groups: first year residents; second year residents; first and second-year residents (2 groups). mentoring meetings were monthly held on thursdays, 7 a.m. and lasted for one hour. results: residents routine was considered adequate. there has been decreased frustration and an improved self-confidence. a high number has reported higher enthusiasm for anesthesiology and higher expectations as to post-residency opportunities. mentors have reported the importance of mandatory meetings, the increase of discussion about humanization and the possibility of mentors' un-preparedness as a limiting factor. conclusions: mentoring programs may be tools for adapting residents to the training program in anesthesiology.
Angiodisplasia do ceco na terceira década de vida

Abstract: the vascular malformations represent one of the main causes of lower gastrointestinal bleeding in elderly patients. the incidence of the disease in patients with less than thirty years is a rare described event. the objective of the present report is to show a case of angiodysplasia, located in the cecum region, in-patient on his third decade of life, revealed by preoperative selective angiography and confirmed by histopathological study. men, 29th years old, presented complaining a chronic anemia of obscure origin for several years and refractory to the clinical and transfusion therapy. with the objective to investigate a possible digestive origin he was submitted to the abdominal selective angiography that demonstrated the presence of vascular malformation located in the cecum. a right colectomy was done and then it was possible to confirm the localization of the disease for postoperative angiography of surgical specimen and the diagnosis of angiodysplasia for the anatomopathologic study. after surgical procedure presented favorable evolution, being currently in the 5th postoperative month without presenting signals of anemia. despite of an uncommon possibility the presence of angiodysplasia has to be always considered in the clinical investigation of anemia of obscure cause in young patients.

Abstract: objective: to estimate the prevalence of visiting doctors and to analyze associated factors. methods: cross-sectional, population-based study performed in the city of pelotas, southern brazil, between december 1999 and april 2000. a total of 1,962 individuals of both sexes, aged between 20 and 69 years and living in the urban area were included in this study. data were collected with standardized, pre-coded questionnaires. the outcome "visited doctor during the 12 months prior to interview" was analyzed with socioeconomic and demographic factors, presence of chronic diseases and psychiatric disorders, nutritional status, smoking, alcohol consumption, and hospitalizations in the year prior to interview. poisson regression was used, according to a hierarchical model, controlled by confounding variables, and considering a significance level of <0.05. results: among interviewees, 1,395 (70.9%) had visited a doctor in the period analyzed. multivariate analysis among men revealed that individuals who showed higher prevalences of medical visits had a per capita family income higher than 10 minimum wages per month, were over 60 years of age, suffered from diabetes mellitus, had a body mass index equal to or above 25kg/m2, and had been hospitalized. women who had a per capita family income higher than six minimum wages per month, were over 60 years of age, white and non-smokers, suffered from hypertension and diabetes, and had been hospitalized showed higher outcome prevalences. conclusions: health inequalities were identified in relation to ethnicity and family income. in addition, there was high prevalence of medical visits, especially among individuals who were older and suffered from certain chronic, non-communicable diseases.
Tutoria com médicos residentes em anestesiologia: o programa da Irmandade da Santa Casa de Misericórdia de S o Paulo

Abstract: JUSTIFICATIVA E OBJETIVOS: A residência médica pode trazer desgaste emocional. Este artigo descreve um programa de tutoria durante o período de treinamento dos médicos em Anestesiologia. MéTODO: O programa foi instituído na Irmandade da Santa Casa de Misericórdia de S o Paulo. Vinte e sete residentes de Anestesiologia participaram do programa e quatro profissionais de saúde constituíram a equipe de tutoria. Um questionário sobre as impress es dos médicos residentes de primeiro e segundo anos, a respeito da forma o médica em Anestesiologia foi desenvolvido com doze perguntas e três respostas para cada um, sendo aplicado antes e no fim do programa. Foram constituídos quatro grupos de residentes: residentes do 1o ano; residentes do 2o ano; residentes do 1o ano e 2o ano (2 grupos). As reuni es de tutoria ocorreram mensalmente às quintas-feiras, 7h00, com dura o de uma hora. RESULTADOS: O cotidiano do residente foi considerado adequado. Houve redu o na frustra o e melhoria na auto-confian a. Um número elevado descreveu maior entusiasmo pela Anestesiologia e expectativas mais elevadas em rela o ao futuro após a residência. Os tutores relataram a importancia de reuni es obrigatórias, a possibilidade discutir a humaniza o e a possibilidade de despreparo dos tutores como um fator da limita o. CONCLUS ES: O programa de tutoria pode ser considerado como um instrumento para a adapta o dos residentes no programa de Anestesiologia.
