Regions (forms of territorial autonomy) in the theory of law and law history

Abstract: This discussion has two main parts: theoretical and empirical. The task of the first part is to determine the notion of the region as such (the problem not sufficiently cleared up so far). Namely, it is necessary to delimit the region both from the notion of local self-administration and from the notion of state. The region differs from local self-administration in possessing a qualitatively higher degree of authority, authority for the original regulation of legal relations, legislation in the material sense. The region differs from the state in the fact that the authority of the subject with statal (constitutional) authority in principle has above it only the social legal norm as the content of the joint (collective) legal act of a stronger part of the nation in a state. On the contrary, the region has to be subjugated to the constitutional authority of the state in whose borders it is located. There are two basic types of regions: the region as a state fragment and region as a public service. The former is similar to the state, because it has its own state organs (organs with their own authority of coercion), while the latter does not have such organs. Furthermore, regions could be comprehensively divided into non-incorporated autonomous territories, separate original parts of a state and the regions included into the regional state. This discussion accepts as politically most, relevant the division of regions into the regions within monarchies and the regions within republics. (Due to the spatial limitations the third category, regions under the regime of international law, could not have been included into the discussion). Naturally, this discussion could not have been comprehensive when it comes to regions, so it discussed only the most interesting examples. Thus as examples of the regions in monarchies, it presented dominions within the British Empire and Finland within the Russian Empire, and of the regions in republics, the regions in Italy.
Optimization of the triple-pressure combined cycle power plant

Abstract: The aim of this work was to develop a new system for optimization of parameters for combined cycle power plants (CCGTs) with triple-pressure heat recovery steam generator (HRSG). Thermodynamic and thermoeconomic optimizations were carried out. The objective of the thermodynamic optimization is to enhance the efficiency of the CCGTs and to maximize the power production in the steam cycle (steam turbine gross power). Improvement of the efficiency of the CCGT plants is achieved through optimization of the operating parameters: temperature difference between the gas and steam (pinch point P.P.) and the steam pressure in the HRSG. The objective of the thermoeconomic optimization is to minimize the production costs per unit of the generated electricity. Defining the optimal P.P. was the first step in the optimization procedure. Then, through the developed optimization process, other optimal operating parameters (steam pressure and condenser pressure) were identified. The developed system was demonstrated for the case of a 282 MW CCGT power plant with a typical design for commercial combined cycle power plants. The optimized combined cycle was compared with the regular CCGT plant.
Use of the modified fries technique in the reconstruction of the lower lip after the removal of a malignant tumor

Abstract: Background/aim: Lip carcinoma makes about 1/4 of all oral carcinomas. Primary treatment of the lower lip carcinoma means radical excision of the full thickness of the lower lip tissue together with the tumor. The reconstruction of the defect accomplished after the removal of the lower lip tumor is a challenge for the surgeon and requires a detailed preoperative planning, the right choice of the method for reconstruction and the knowledge of the adequate surgical techniques needed. The aim of this study was to present a ten-year clinical results and experience concerning the reconstruction of lower lip defects longer than 4 cm by means of the modified Fries method in patients treated for the lower lip carcinoma. Methods. The reconstruction of the lower lip by means of the modified Fries method was performed in 37 patients. The following parameters were analyzed: sex, age, the time elapsed from the occurrence of the first symptoms until the first consultation with the doctor, the size of tumors, the size of the postexcision defect, TNM classification, the application of side diagnostic procedures, pathohistological diagnosis of postoperative complications. The sensibility of the lower lip region was monitored, as well as the obtained esthetic effects. Results. The results classified according to the mentioned parameters were processed, analyzed and displayed in detail. Clinical experience concerning the application of this reconstruction technique was presented and analyzed. Conclusion. The modified Fries technique can successfully be applied in the reconstruction of the lower lip defects longer than 4 cm because its application leads to satisfactory functional and acceptable esthetic results.
The growing stock of the Republic of Serbia: State and problems

Abstract: Based on the data of the National Forest Inventory, this paper presents the basic characteristics of forest ecosystems in Serbia by different qualitative and quantitative parameters. The following parameters were dealt with: land use, forest ownership, origin, naturalness, preservation status, mixture and structural form of forests, percentage of tree species, stand category, volume distribution by diameter degrees, as well as the quantity of dead wood and carbon stock in the forests of Ser-bia. The analysis of the presented state enables the definition of some of strategic problems which individually and acting together burden our forest ecosystems, and whose detailed elaboration, measures and methods of enhancement will be fully expressed in the highest planning document - National Forest Programme. A special section, but without being sufficiently analytical, presents the main data on the growing stock of the Autonomous Province Kosovo and Metohija taken from the Report of the Norwegian Forestry Group.
Radical retropubic prostatectomy: Experience of two urological departments

Abstract: We have analyzed oncological and functional results of the patients who underwent radical retropubic prostatectomy for treatment of localized prostate cancer. Material and methods: In the period of 4 years, from 2007. to 2011. on two analyzed urological departments there have been 32 patients who underwent surgical treatment for treatment of localized prostate carcinoma. All of the patients underwent radical retropubic prostatectomy. Patient age was in range from 57 to 70 years of age, with average value of 63 years of age. Gleason score values were: GS 8 - 2 patients, GS 7 - 6 patients, GS 6 - 10 patients, GS 5 - 9 patients, GS 3 - 4 patients GS 10 - 1 patient. Duration of the surgery was in the range of 55 to 95 minutes with the average blood loss between 220 and 640 ml. Fifty percent of the patients did not required blood transfusion. Results: After radical retropubic prostatectomy all patients are continent. Average urinary catheter removal time was 12 days after surgery. Postoperative surgical wound healing was prolonged for 4 patients. One patient had stenosis of urethrovesical anastomosis with was resolved with urethral dilatation. Control PSA values for 29 patients tree months after surgery was under 0, 2 ng/ml. Three patients were diagnosed with local tumor recurrence who underwent radiotherapy and latter on with hormonal therapy protocol. All patients are alive. Conclusion: Results of surgical treatment of localized prostate carcinoma have shown that the radical retropubic prostatectomy is the safe method for treatment of the localized prostate cancer, with good oncological response and small amount of postoperative complications.
Tumors of the horseshoe kidney: Characteristics and literature review

Abstract: The aim of this paper is to point out the significance of the rare urinary system anomaly such as horseshoe kidney. Tumors of the horseshoe kidney are rarely described as single cases in literature. Bilateral tumors of the horseshoe kidney can present a health and therapeutically challenge. In this paper we have analyzed characteristics of the horse shoe kidney tumors and their diagnostics. Primary means of the horseshoe kidney tumors treatment are surgical with different techniques depending on the size, presentation of the tumor and other indications. The review of the available literature is also included.
Resynchronisation therapy in patients with heart failure: Our results

Abstract: INTRODUCTION. Resynchronisation therapy with biventricular permanent pacing stimulation is one method of treating patients with systolic heart failure, with echocardiograph signs of ventricular asynchrony and a prolonged QRS of longer than 120 milliseconds. This method has been accepted in most medical centers around the world and was instigated in our Pacemaker Centre in December 2001, 3 months after FDA approval for human use. OBJECTIVE. The aim of the study was to present this new procedure and the results obtained from our own group of patients. METHOD. A multi-site, biventricular pacemaker, with a special electrode for left-half heart stimulation was implanted in the coronary sinus of 17 patients who had suffered systolic heart failure (12 male and 5 female, average age 59.9 years). For all of them, the duration of the QRS interval was longer than 120 ms, with left bundle branch morphology, and an ejection fraction below 30%. All the patients were NYHA class II or III. Prior to and after the implantation, a 12-channel ECG and ECHO were carried out, a b-minute hall walk test was performed, additionally, the total walked distance on a flat surface was measured, the general condition of the patient was evaluated, the number of medications being taken was noted, as was the number of days of hospitalization. RESULTS. The average time from diagnosis to implantation was 22 months, and the average post-operative follow-up was 14 months. Two of the patients died 10 and 7 months after the implantation, due to a new myocardial infarction and refractory heart failure. In addition, one patient did not show any improvement after the implantation of the multi-site pacemaker (there were three "non-responder" patients). All the other patients felt much better: decreased NYHA class for I - II class, increased left ventricle ejection fraction, reduced use of diuretics, increased b-minute hall walk distance and general walk distance on a flat surface, and decreased number of days of hospitalization. CONCLUSION. Resynchronisation heart failure therapy in the majority of patients with systolic left ventricular dysfunction and a prolonged QRS interval considerably improves cardiac function, in addition to reducing symptoms and hospital stays.
Infective endocarditis - maybe yes, maybe no: Case report

Abstract: Introduction. Infective endocarditis (IE) is a rare disease which manifests in different ways. Case Outline. We are presenting a female patient who was suspected of IE based on the presence of fever, accelerated erythrocyte sedimentation rate, increased levels of C-reactive protein and echocardiographic findings of filamentous structures on the aortic valve which were assumed to be vegetation. Because of the well-known fact that in the pre-antibiotic era IE was almost always a fatal disease, empirical antibiotic therapy was conducted despite the absence of clear criteria for IE and it resulted in a satisfactory outcome. The course of the disease and the persistence of echocardiographic findings with a completely competent aortic valve, suggested us to consider the diagnosis of Lambl’s excrescences. There was no indication for surgical treatment in our patient; so that in the absence of pathological confirmation our diagnostic dilemma was left unresolved. Conclusion. In patients with typical clinical features of IE and filamentous structures on the cardiac valves that are completely competent, Lambl’s excrescences should be kept in mind as a possible differential diagnosis.
Gigantic transitional cell carcinoma of the renal pelvis in the region of endemic nephropathy

Abstract: AIMS: To present the rare case of transitional cell carcinoma of the renal pelvic in the region of endemic nephropathy. PATIENT REPORT: A 64-year old man with gigantic urothelial carcinoma in renal pelvis and ureter. Patient underwent right nephrectomy. Hystopathological analysis after right nephrectomy revealed urothelial transitional cell carcinoma, grade III-IV with infiltration of right ureter. After one month, because of the hystopatological findings, patient underwent ureterectomy on the right side. RESULTS: Two years after surgery there are no signs of primary disease or metastasis and the cystoscopy findings are normal. CONCLUSION: Despite the poor prognosis, two years after surgery patient is without signs of primary disease or metastasis.

Abstract: In the previous five-year period in the area of Ni , Pirot and Toplice region there have been 252 suicides with known profession. In production workers, electricians and drivers the increase of suicide prevalence has been registered during the analyzed period. The dominant way of carrying out suicidogenic intents in housewives has been poisoning, in workers employed in production, workers in service industries, policemen and soldiers a shot from firearms, and in all other professions it has been carried out by hanging. Farmers, contrary to expectations, more often than not choose hanging as a safer and cheaper way of committing a suicide in regard to poisoning with pesticides.
