Técnica de fixa??o de tubos nasogástrico e nasoentérico

Abstract: nasogastric and nasoenteral tubes are necessary in medical practice, but they are unpleasant and hard to be tolerated during long periods. this paper describes a technique of fastening the nasogastric or nasoenteral catheter that is easy to be performed, diminishes the discomfort and avoids most of nasal complications related to its location. benjoim solution moistens the tube and the skin of the upper lip, nose, forehead or deltopeitoral region. a micropore adhesive tape measuring 13 centimeters length and one centimeter width surrounds the tube and fastens it to the upper lip. this fastening centralizes the catheter into the nasal hole. the nasoenteral catheter is fastening on the upper lip, and then it is turned up, to be attached to nose and forehead with two other adhesive tapes. the larger nasogastric tube, used for gastric drainage, is fastened to the upper lip and attached on the deltopeitoral region with another adhesive tape. the nasogastric or nasoenteral tube fastening technique on the upper lip is easy, efficacious, inexpensive and can be performed in any environment with less discomfort of the patient.

Abstract: background: the most common incisions for parotidectomy consist of opening or pre periauricular extended to the submandibular or cervical region. they can accompany themselves ragged scars, causing local skin deformities. aim: the purpose of this study was to evaluate the treatment of parotid surgical diseases using periauricular incision. method: thirty nine patients with parotid tumors were reviewed. pleomorphic adenoma (20 cases) was the most common disease followed by other benign tumors (9 cases), carcinoma (5 cases), parotid cyst (3 cases) and chronic parotiditis (2 cases). all parotidectomies were performed through a periauricular incision. results: the parotid tumors were removed in all cases without complementary skin approach. the incisions had good aesthetic result and almost imperceptible scars were verified after six months. the patients were very satisfied with the appearance of the operative scar. all patients complained hypostesia of the operative area during a period not longer than six months. temporary postoperative facial weakness occurred after 28 operations and was permanent in three of patients operated on for carcinoma. transitory ear discomfort occurred in 22 patients. two cases of local infection were recorded. all of these complications have been described previously by other authors using other incisions and are mostly due to removal of the gland. conclusion: periauricular incision is a good and highly aesthetic option for surgical approach of the parotid.

Abstract: background: the large wounds of the body wall, due to traumas, removal of tumors or prolonged laparostomies are a difficult surgical challenge with complex treatment. aim: presents the efficacy of the closure of large surgical wounds using rubber elastic bands. method: one or two circular rubber elastic bands were sutured under mean tension at the opposite edges of 21 large wounds located in different body sites. these rubber strips were replaced when they were broken or refixed when they have lost their tension until the complete closure of the wounds. results: complete closure of the wounds was achieved in all cases without any other surgical procedure or device. no major complication due to this treatment was verified. conclusion: the synthesis of large wounds with rubber elastic bands kept under mean tension is a simple, efficacious and unexpensive surgical option that may be useful for treatment in several circumstances.

Abstract: introduction: to publish became almost compulsory in medicine. there is no doubt about the importance of publishing research, but the ordering of its authors is not easy. the lack of internationally accepted criteria led to the establishment of several groups or conventions particularized medical and scientific sectors. objetive: to present numerical method to establish rule of value to people who carried out the research, and whether or not incorporated as authors. method: the proposed score is based on the needs of each step when conducting a scientific work. they were divided into topics in which the main ones were: 1) scientific criteria for authorship; 2) create the idea that originated the work and develop hypotheses; 3) structure the method of work; 4) guiding the work; 5) write the manuscript; 6) coordinate the group that carried out the work; 7) reviewing the literature; 8) suggestions incorporated into the work; 9) to solve fundamental problems of labor; 10) to collect data; 11) presentation at scientific meetings; 12 ) lead the job and raise funds; 13) providing patients or material; 14) to do the routine needs; 15) specific fee to participate; 16) criteria for ranking the authors; 17) honorary author; 18) usurpation of the main authorship, 19) acknowledgments . conclusions: it is important to emphasize that, to prevent major conflicts, the group that is willing to conduct a scientific work should establish at the outset, as objectively as possible, the criteria to be adopted for distribution of authorship. the subjective criteria here proposed avoid interference and prevent conflicts of interest.

Abstract: objective: although the radiological features of acute appendicitis are well documented, the value of the plain radiography has not been fully appreciated yet. the present study was aimed at determining the frequency of association between acute appendicitis and a radiological sign characterized as an image of fecal loading in the whole cecum. materials and methods: plain abdominal radiographs of 100 consecutive adult patients with acute appendicitis proved by surgery and histology were assessed. the presence of fecal loading characterized by hypotransparency in the whole cecum, and sometimes also in the ascending colon, was observed. results: the image of fecal loading in the cecum was found in 97% of cases of acute appendicitis, independently of age, gender, skin color or stage of appendicitis. conclusion: the present study suggests that the presence of radiological images of fecal loading in the cecum may be a useful sign for the diagnosis of acute appendicitis.

Abstract: idiopathic myelofibrosis is a chronic myeloproliferative disorder that may evolve to myeloid hepatosplenomegaly, which is also called myeloid metaplasia, involving several organs. occasionally the spleen reaches giant proportions and must be withdrawn. however, this procedure is followed by greater morbidity and mortality. partial splenectomies, preserving the splenic pedicle, have been proposed in order to reduce postoperative complications. after an early postoperative improvement, some patients develop with symptomatic recurrence of their disease. the present communication shows two alternatives for the treatment of complicated myeloid splenomegaly: partial splenectomy, preserving the upper splenic pole supplied only by the splenogastric vessels and total splenectomy followed by autotransplants of splenic tissue. we performed the subtotal splenectomy in five patients. their ten-year postoperative follow-ups and improvement in health suggest that this operation should be considered for the treatment of huge spleens due to myelofibrosis with myeloid splenomegaly. another patient was submitted to total splenectomy and autologous splenic transplantation on the greater omentum. the three-year follow-up was uneventful related to the splenic remnants. however, this last patient required intense hematological control of his severe disease. in conclusion, whether an operation is indicated to complement the hematological therapy of myeloid splenomegaly, a splenic conservative procedure must be performed. subtotal splenectomy or total splenectomy and autotransplants of splenic tissue are worth considering in such cases.

Abstract: complications of splenomegaly in gaucher's disease, such as hypersplenism and mechanical compressions, have been treated by partial splenectomy. however, the splenic remnant, vascularized by the hilar vessels, may enlarge leading to recurrent splenomegaly and all its adverse effects. more than eighteen years of experience treating portal hypertension, splenic trauma, myeloid splenomegaly, splenomegalic somatic and sexual hypodevelopment, chronic lymphocytic lymphoma and cystadenoma of pancreatic tail by subtotal splenectomy, maintaining the upper pole supplied only by the splenogastric vessels showed that the splenic remnant does not increase its size. based on evidence that the hilar vessels may contribute to the increase of the splenic remnant, we performed in five cases with gaucher's disease subtotal splenectomy, maintaining the upper pole supplied only by the splenogastric vessels. the splenic remnant did not modify in size during a postoperative follow-up of more than twelve years, and the hematological parameters remained normal. another patient whose splenogastric vessels were insufficient to maintain the upper splenic pole, was submitted to total splenectomy and autologous splenic tissue transplantation on the greater omentum. the autotransplant survived and presented with function. all six patients had uneventful postoperative follow-up in relation to the splenic conservative procedure. in conclusion, in presence of giant splenomegaly with severe hematologic and clinical symptoms the spleen must be resected in a conservative fashion such as subtotal splenectomy or total splenectomy followed by autotransplants of splenic tissue.

Abstract: in order to diminish the adverse effects of total splenectomy, a conservative approach must be performed whenever possible. subtotal splenectomy and splenic autotransplantation are good alternatives in such cases. the purpose of the present study was to compare the phagocytic function of the upper pole remnant of subtotal splenectomy and of autologous spleen tissue implanted into the greater omentum. this investigation was carried out on 35 adult holtzman rats of both sexes. all animals were submitted to subtotal splenectomy, preserving the upper pole supplied by the splenogastric vessels and autotransplantation of the removed splenic segment on the greater omentum. the phagocytic function of the splenic remnants was verified by two different methods: scintigraphy with 99mtc sulfur colloid and microscopic counting of macrophages containing carbon colloid. during the first three months, the phagocytosis was higher in the remnant upper pole. after this time, there was no difference between the upper pole and the splenic autotransplant. in conclusion, the remnant of subtotal splenectomy and the splenic autotransplant maintain the filtering function of the spleen by removing colloid particles from the blood.
Critérios quantitativos para analisar o valor da publica??o de artigos científicos

Abstract: background: the assessment of the publication of scientific works is a hard challenge when it is necessary to classify a curriculum vitae or even to verify the performance of an institution, such as a medical school, a postgraduate program or a research department. most of time, the evaluation is based on personal feelings and experience, without established standard pattern. objective: to present a guideline to evaluate the publications of scientific papers, without foreign influence. methods: the guideline to assess the value of each publication is based on the multiplication of three factors. the first factor is the value of the author corresponding to his place among the authors in the authorship of the work. the second factor is the classification of the type of publication. finally, the product of these two factors is multiplied by ten times the "impact factor" of the journal. when the "impact factor" is zero, 0.1 should substitute it. results: this method has been used by us since 1994, with good results and without any problem related to the judgment. conclusion: this criterion prevents subjective interference. conflicts of interest are avoided when clearly presented from the beginning of the judgment process.

Abstract: background. during the development of a scientific research, one of the main difficulties is to choose the authors of the paper from all participants of the investigation. the organization of the authorship, based on the contribution of each investigator is still more complex. objective: to propose a justified indicator for the authorship of a scientific paper. method:the author investigated the main research centers of brazil in order to know their philosophy related to the authorship of a research. an assessment of the literature completed this study. results: we present and justify the order of each investigator in the authorship or acknowledgment of the paper, based on his or her participation on the investigation conclusion: the investigator deserves to be included as an author of a scientific paper only if he or she has an intellectual participation in the work and also effectively contributed with the execution and accomplishment of the research.
