Osseous Metaplasia in Castleman's Disease: A Case Report

Abstract: Background. Castleman's disease is a benign lymphoproliferative disorder. The disease may be localized or multicentric. Three histologic variants are recognized: hyaline vascular, plasma cell, and mixed types. Case. A 20-year-old man presented with large left axillary mass. The histologic examination of the resected mass showed follicular pattern with large nodules of mantle cells arranged concentrically around atrophic and vascularized germinal centers. There was also some benign-appearing bone trabeculae interspersed with lymphoid tissue. The diagnosis of Castleman's disease, hyaline-vascular type with osseous metaplasia, was made.
Pulmonary Extramedullary Hematopoiesis in a Patient with Chronic Asthma Resembling Lung Cancer: A Case Report

Abstract: Background. Extramedullary hematopoiesis is most often seen in reticuloendothelial organs specially spleen, liver, or lymph nodes, and it is rarely seen in lung parenchyma. Almost all reported cases of pulmonary extramedullary hematopoiesis occurred following myeloproliferative disorders specially myelofibrosis. Other less common underlying causes are thalassemia syndromes and other hemoglobinopathies. There was not any reported case of pulmonary extramedullary hematopoiesis in asthmatic patients in the medical literature. Case. Here we reported a 65-year-old lady who was a known case of bronchial asthma with recent developed right lower lobe lung mass. Chest X-ray and CT studies showed an infiltrating mass resembling malignancy. Fine needle aspiration cytology of mass revealed pulmonary extramedullary hematopoiesis. The patient followed for 10 months with serial physical examination and laboratory evaluations which were unremarkable. Conclusion. Extramedullary hematopoiesis of lung parenchyma can be mistaken for lung cancer radiologically. Although previous reported cases occurred with myelofibrosis or hemoglobinopathies, we are reporting the first case of asthma-associated extramedullary hematopoiesis.
Sweet Syndrome Accompanying Inflammatory Dehydrogenase Deficiency and Its Association Bowel Disease in a Child

Abstract: Acute neutrophilic dermatosis, first described in 1964 byRobert Douglas Sweet, is characterized by sudden onset fever,neutrophilic leukocytosis, and well demarcated erythematouspapules, nodules, and plaques with dense neutrophilic infiltrateson histologic evaluation.Here is a report of a 7-year-old girl who presented withhigh grade fever, and discrete erythematous papular skin eruptions,which gradually increased in number and involved theface, trunk, extremities, palms, soles, hard palate, and palataltonsils. The skin eruptions evolved to pustules and after coalescingcaused large crusted plaques, with mild tenderness butwithout any pruritus. White blood cells were 36900/ml with92% neutrophils. Skin biopsy test was compatible with acutefebrile neutrophilic dermatosis, so prednisolone (1 mg/kg/day)was started that led to a rapid defervescence and significantimprovement of dermatosis. After a few days, the patient presentedwith fever and arthritis of right elbow, both ankles, andwrists, so she was re-admitted. She also developed bloody diarrheaduring the hospital stay. Colonoscopy and intestinalbiopsy were performed, which confirmed the diagnosis of ulcerativecolitis. Prednisolone, sulfasalazine, and naproxenwere prescribed. The fever and diarrhea stopped after a fewdays and joint swelling decreased. She was discharged 2weeks after the admission with a rather good general condition.Inflammatory bowel disease can be one of the severalconditions accompanying sweet syndrome.
Hemolysis Induced by Glucose-6-Phosphate Dehydrogenase Deficiency and Its Association with Sex in Children

Abstract: Background: Glucose-6-phosphate dehydrogenase (G6PD)deficiency is the most common enzyme disorder in human.The aim of this study was to determine the prevalence ofG6PD deficiency among children and evaluate its associationwith ABO/Rh blood groups.Method: Blood samples of 3401 asymptomatic children wereanalyzed and compared with 317 children who were admitted tohospital because of hemolysis resulted fromG6PD deficiency.Results: Among asymptomatic children 375 (11%) were G6PDdeficient. Male to female ratio for this group was 4.2:1 and forthe hemolytic group was 2.5:1 (P=0.004). Two hundred andsixty-seven (84.2%) of the patients with hemolysis wereyounger than 2 years, with the peak age of hemolysis between 2and 3 years (27.7%). The overall rate of hemolysis caused byG6PD deficiency was 12.3% during the 3 consecutive monthsof fresh Fava bean consumption. Blood groups O+, A+, and B+together constituted 87.1%, 87.7%, and 84% of the bloodgroups among normal children, asymptomatic G6PD deficientsubjects, and those with G6PD deficiency related hemolysisrespectively (P=0.367). Seven percent of the normal childrenand asymptomatic G6PD deficient subjects were Rh- vs 9.7 %of G6PD deficient children with hemolysis (P=0.16).Conclusion: The prevalence of G6PD deficiency among thechildren was 11%. Male to female ratio was greater in nonhemolyticvs hemolytic group so that the female share was higherin hemolytic group than in the other two groups (P=0.004).The distribution of ABO blood groups was similar amongasymptomatic non-G6PD deficient, asymptomatic G6PDdeficient,and G6PD-deficient children with hemolysis. Thedistribution of Rh- types among the G6PD-deficient childrenwith hemolysis and the other two groups was similar (9.7% vs7%, P=0.16).
MRI Evaluation of Liver Iron Concentration in Patients with β-Thalassemia Major

Abstract: β-thalassemia major is a hereditary anemia, characterized by a genetic deficiency in the synthesis of the β-globin chain (1). The main complication of the multiple blood transfusions to these patients is iron overload and the deposition of iron in various organs, such as the reticuloendothelial system, the liver, the heart and the endocrine glands (2).he most reliable method of calculating body iron stores is the biochemical or histochemical assessment of iron in a liver biopsy specimen. Since liver biopsy is an invasive procedure, attempts have been made to use imaging for detection and quantification of liver iron content (4-7). Magnetic resonance imaging (MRI) is a technique for the diagnosis of hemochromatosis. Many investigators have shown that hepatic T2 relaxation or intensity ratios in MR images have a significant correlation with hepatic iron content (8-11).In this study, we have evaluated the correlation of the hemochromatosis grades in liver MRIs and liver biopsies to evaluate the precision of liver MRIs in grading hemochromatosis.
Tracheal replacement by autogenous aorta

Abstract: We replaced 5 cm of cervical trachea in 10 dogs with harvested infra-renal aorta and repaired the aortic defect with Dacron graft.Necropsy of the grafted aorta and anastomotic site revealed well healed anastomosis in all animals together with ciliated columnar epithelium coverage of grafted aorta and neovascularization of aortic wall.Aortic graft is preferable to other substitutes because of less antigenicity, less vascularity, and no mucous secretions or peristalsisTracheal defects may occur after trauma or prolonged intubation. Resection of tracheal tumors also poses a major challenge for substitution. In an effort to solve this problem, different techniques have been tried with little success. The various tracheal substitutes and techniques of reconstruction were analyzed by Grillo [1], who classified them in five categories: foreign materials, nonviable tissues, autogenous tissues, tissue engineering, and tracheal transplantation. Attempts with foreign materials led to problems of chronic infection, airway obstruction, migration of the prosthesis, erosion of major blood vessels and proliferation of granulation tissue. Implantation of nonviable tissues, either chemically treated, frozen or lyophilized has been associated with poorly functional results. Reconstructions with autogenous tissues such as skin, fascia lata, pericardium, costal cartilage, bladder, esophagus or bowel are complex procedures, which have been associated with disappointing results. More recently, efforts have been made to induce the formation of cartilaginous tubes covered with epithelial cells, but to date this type of tissue engineering has not provided reliable results. Finally, tracheal allotransplantation has been also disappointing so far due to complication of necrosis or stenosis of the graft. Diagnostic pitfalls in tibial adamantinoma: two cases with a clinicopathological review

Abstract: Adamantinoma is a rare primary bone tumor that commonly arises in the jaw and has also been described in the appendicular skeleton such as the tibia. We report 2 cases of tibial adamantinomas that were originally misdiagnosed; one as fibrous dysplasia of the tibia and the other as a cutaneous eccrine carcinoma in a groin mass, which was metastatic adamantinoma to the inguinal lymph nodes. Such metastatic adamantinoma to the groin lymph nodes is extremely rare. The clinical and pathological data with a review of the available literature on inguinal lymph node metastases from primary tibial adamantinoma are reported. Increased clinical awareness and accurate recognition of such uncommon patterns of inguinal nodal metastases are imperative for appropriate planning of therapeutic strategies and risk management in these patients.
The Use of Multi-Objective Genetic Algorithm Based Approach to Create Ensemble of ANN for Intrusion Detection  [PDF]

Abstract: Due to our increased dependence on Internet and growing number of intrusion incidents, building effective intrusion detection systems are essential for protecting Internet resources and yet it is a great challenge. In literature, many researchers utilized Artificial Neural Networks (ANN) in supervised learning based intrusion detection successfully. Here, ANN maps the network traffic into predefined classes i.e. normal or specific attack type based upon training from label dataset. However, for ANN-based IDS, detection rate (DR) and false positive rate (FPR) are still needed to be improved. In this study, we propose an ensemble approach, called MANNE, for ANN-based IDS that evolves ANNs by Multi Objective Genetic algorithm to solve the problem. It helps IDS to achieve high DR, less FPR and in turn high intrusion detection capability. The procedure of MANNE is as follows: firstly, a Pareto front consisting of a set of non-dominated ANN solutions is created using MOGA, which formulates the base classifiers. Subsequently, based upon this pool of non-dominated ANN solutions as base classifiers, another Pareto front consisting of a set of non-dominated ensembles is created which exhibits classification tradeoffs. Finally, prediction aggregation is done to get final ensemble prediction from predictions of base classifiers. Experimental results on the KDD CUP 1999 dataset show that our proposed ensemble approach, MANNE, outperforms ANN trained by Back Propagation and its ensembles using bagging & boosting methods in terms of defined performance metrics. We also compared our approach with other well-known methods such as decision tree and its ensembles using bagging & boosting methods.
On the Design of Circular Fractal Antenna with U-Shape Slot in CPW-Feed  [PDF]

Abstract: A new ultawideband circular fractal antenna with notched-band characteristics is presented. A notched-band characteristic is achieved by employing a U-shape slot in 50 Ω feed lines. The ultra-wideband impedance matching and compact size have been obtained by using CPW-feed technique and the fractal concept. The measured result of proposed fractal antenna exhibits the ultra wideband characteristics from 3.0 to 18.0 GHz at VSWR 2:1 except notched-band frequency. The proposed antenna has been analyzed theoretically and experimentally with respect to design parameters. The measured radiation pattern of fractal antenna is nearly omnidirectional in azimuth plane throughout the operating frequency. This antenna is useful for UBW communication system.

Abstract: Recently an object-oriented approach has been applied in the fields of finite element analysis with a view to treating the various complexities within these. It has been demonstrated that finite element software designed using an object-oriented approach can be significantly more robust than traditional codes. This paper describes a special kind of implementation of object-oriented programming which is rather hybrid in nature, in the development of a finite element code for engineering analysis of metal working problems using C++, and discusses the advantages of this approach.
