Revista de Saúde Pública: quarenta anos da produ??o científica no Brasil

Abstract: objective: to recognize the characteristics and path taken by the through analysis of the scientific production it has published over the period from 1967 to 2005. methods: scientometric methods were used to analyze reference data on the articles published in the revista, retrieved from the databases isi/thomson scientific (web of science), national library of medicine (pubmed) and scientific electronic library online (scielo). results: the revista is the only brazilian publication in the field of public health that is indexed by isi/thomson scientific. it is prominent as a medium for publishing brazilian scientific production in public health and is displaying a geometric increase in publication and citation, with annual rates of 4.4% and 12.7%, respectively. the mean number of authors per paper has risen from 2 to 3.5 over recent years. although original research articles predominate, the numbers of reviews, multicenter studies, clinical trials and validation studies have been increasing. the number of articles published in foreign languages has also increased, accounting for 13% of the total, and the leading countries originating these are the uk, usa, argentina and mexico. the number and diversity of journals citing the journal has been increasing, many of which are non-brazilian. authorship per author shows good fit to lotka's law, but the parameters suggest greater concentration and less dispersion than would be expected. among the fields of interest of published papers, the following topics account for more than 50% of the total volume: infectious-parasitic diseases and vectors; health promotion, policies and administration; and epidemiology, surveillance and disease control. conclusions: the revista shows great dynamism, without signs of abating or reaching a plateau any time soon. there are signs of progressively increasing complexity in the studies published, and more multidisciplinary work. the revista seems to be widening its outreach and recognition, wh

Abstract: OBJETIVO: Reconhecer as características e trajetória da Revista de Saúde Pública pela análise da produ o científica por ela veiculada, no período de 1967-2005. MéTODOS: Foram analisados por métodos cientométricos dados referenciais dos artigos publicados na Revista e recuperados nas bases de indexa o do Institute for Scientific Information (ISI)/Thomson Scientific (Web of Science), National Library of Medicine (PubMed) e Scientific Eletronic Library Online (SciELO). RESULTADOS: A Revista, sendo a única na área da saúde pública no Brasil, indexada pela ISI/Thomson Scientific, destaca-se como veículo de divulga o da produ o científica brasileira em saúde pública e exibe crescimento geométrico de publica es e cita es, cujas taxas anuais s o de 4,4% e 12,7%, respectivamente. O número de autores cresceu de uma média de cerca de dois por item publicado até 3,5 para os dias atuais. Embora os artigos originais de pesquisa predominem, nos últimos anos aumentou o número de revis es, estudos multicêntricos, ensaios clínicos e de valida o; cresceu também o número de artigos publicados em língua estrangeira, com 13% do total, destacando-se a participa o da Inglaterra, Estados Unidos, Argentina e México. Aumentou o número e diversidade de revistas que citam a Revista de Saúde Pública, com expressiva participa o de revistas internacionais. A distribui o de autorias por autor da Revista ajusta-se muito bem à Lei de Lotka, mas seus parametros sugerem maior concentra o e menor dispers o do que o esperado. Entre os temas de interesse dos itens publicados, doen as infecto-parasitárias e vetores, promo o, políticas, administra o de saúde e epidemiologia, vigilancia e controle de doen as responderam por mais de 50% do volume total. CONCLUS ES: A Revista mostra grande dinamismo sem sinais que sugiram qualquer esgotamento ou estabilidade em futuro próximo. Há sinais de progressivo aumento de complexidade dos estudos publicados, bem como de maior multidisciplinaridade. A Revista parece alargar seu alcance e repercuss o embora continue fiel aos temas de interesse da saúde pública do Brasil.
Avalia??o de estratégias de gest?o de ciência e tecnologia: um estudo de caso

Abstract: the health department of the state of s. paulo has implemented a management strategy on research and development (r&d) for its research institutes which comprises three different items: 1) project registration, 2) support and 3) promotion of r&d activities. the present study explores the ability of this r&d management system to provide information and evaluates the performance of the third item which has been responsible for the assignment of research grants to professionals whose projects are selected through a peer review. the assessment of the grant's distribution was conducted by comparing the mean scientific production recorded in literature data bases for grant holders and equal an number of controls matched by institute of origin. the ability of the system to produce information was studied by the application of cluster analysis. the results show that grant holders have a scientific production significantly higher than their controls. concerning information production, it is shown that proper raw data processing may render suggestions for decision making and that the institutes may be grouped in clusters of those that adopt similar management strategies.
Pertussis booster vaccine for adolescents and young adults in S?o Paulo, Brazil

Abstract: objective: to develop a model to assess different strategies of pertussis booster vaccination in the city of s?o paulo. methods: a dynamic stationary age-dependent compartmental model with waning immunity was developed. the "who acquires infection from whom" matrix was used to modeling age-dependent transmission rates. there were tested different strategies including vaccine boosters to the current vaccination schedule and three of them were reported: (i) 35% coverage at age 12, or (ii) 70% coverage at age 12, and (iii) 35% coverage at age 12 and 70% coverage at age 20 at the same time. results: the strategy (i) achieved a 59% reduction of pertussis occurrence and a 53% reduction in infants while strategy (ii) produced 76% and 63% reduction and strategy (iii) 62% and 54%, respectively. conclusion: pertussis booster vaccination at age 12 proved to be the best strategy among those tested in this study as it achieves the highest overall reduction and the greatest impact among infants who are more susceptible to pertussis complications.

Abstract: A partir de implementa o de um modelo de gest o de ciência e tecnologia em institutos de pesquisa do Estado de S o Paulo, Brasil, foi estudada a capacidade desse sistema para gera o de informa es gerenciais e avaliado o incentivo à produ o científica e tecnológica, através do qual vem-se atribuindo bolsas a pesquisadores cujos projetos s o selecionados através de uma revis o por pares. Para avaliar a corre o do processo de sele o de bolsistas, a produ o científica média registrada em bases de dados de literatura nos últimos 5 anos, desses bolsistas, foi comparada por meio de análise de varian a à produ o média de igual número de controles n o-bolsistas, pareados por instituto de origem. Foi estudada a capacidade do sistema de gerência em produzir informa es por meio da aplica o de análise por aglomera o. Os resultados mostraram que os bolsistas s o pesquisadores que registram produ o científica significativamente superior aos n o-bolsistas, permitindo inferir que a sele o de bolsistas esteja se processando adequadamente. Quanto ao potencial do sistema de gerência para produzir informa es, os resultados sugeriram que o processo de produ o de pesquisa pode ser caracterizado com detalhes relevantes para a tomada de decis es administrativas e de planejamento e que os institutos podem ser reunidos em grupos que comportam estratégias distintas de interven o.

Abstract: objective: the aim of the present scientific study is to evaluate the patterns of antiretroviral technology adoption by the brazilian public health system (sus). methods: based on previous articles published in scientific medical literature, three indicators to assess antiretroviral technology adoption by sus were proposed: knowledge-adoption interval; critical mass of knowledge; and validation-adoption interval. using the databases from the sus department of information technology (datasus) and from the brazilian logistic center for medication control (siclom), two pharmaceutical groups were selected (antiretroviral medications and a group of high cost medications). results: antiretroviral medications were adopted faster than the high cost medication group when assessed on the basis of "knowledge-adoption" interval and "validation-adoption" interval. yet, they require a lower "critical mass of knowledge" before adoption. conclusion: antiretroviral medications have been adopted faster and based on a lower number of scientific medical articles than a selected group of high cost medications.

Abstract: introduction: an urgent need for the management of science and technology production in the health field has been recognised in brazil since 1994, when the federal government called a national conference on the subject. the present study presents a methodology for the identification of items of priority in planning such management. material and method: a survey was conducted in the research institutes belonging to the s. paulo state health department among a sample of research professionals with a view to collecting data on two different scenarios: present and expected situation. eighteen concepts, assembled in four different groups, were assessed is terms of an average percentage approval or disapproval, for each scenario. consistency for the measurement of each of these groups was examined by the use of cronbach's alpha coefficient and inconsistent concepts were disregarded in the analysis. the average percentage of approval/disapproval was further calculated as scenario co-ordinates for each concept and institute entering the study so that priority maps for concepts and institutes could be constructed. results: results suggest that the present situation meets with disapproval, though not strongly so, while a high degree of expectation is expressed with significant emphasis as regards research infrastructure management. the main priorities are given as: acknowledgement of performance, support for publication, resource allocation by research project and methodological advice for research analysis. conclusion: it is concluded that the study methodology was helpful in eliciting management priorities and could be applied elsewhere with due adjustment of content regarding selection of concept and their grouping.
Lógica fuzzy e regress?o logística na decis?o para prática de cintilografia das paratiróides

Abstract: objective: to develop and compare two mathematical models, the first one based on logistic regression and the second one on fuzzy sets theory, aiming at defining a laboratory testing-based measure of indication for submitting patients to parathyroid scintigraphy. methods: one-hundred and ninety-four patients with serum calcium and parathyroid hormone available were identified from the data registry of parathyroid scintigraphy of a diagnostic laboratory in s?o paulo, southern brazil, in the period between january 2000 and december 2004. the logistic regression model was developed using spss and the fuzzy model was developed using matlab software programs. the performances of both models were compared using roc curves. results: the performance of both models were statistically different (p=0.026). the area under the roc curves were 0.862 (95% ci: 0.811-0.913) for the logistic regression model and 0.887 (95% ci: 0.840-0.933) for the fuzzy model. the latter had the advantage of allowing to making decisions based on parathyroid hormone information within a non-discriminating range of calcium values. conclusions: the mathematical model based on fuzzy sets theory seemed to be more useful than the logistic model in the decision making for scintigraphy indication. however, inferences can be made only regarding model comparison and not for parathyroid scintigraphy itself since the data analyzed was not representative of any population.
Sistema Nacional de Inova??o em Saúde: rela??es entre áreas da ciência e setores econ?micos

Abstract: health science and technology is currently the subject matter of government and university actions. such actions should converge to the establishment of a national health innovation system, which still calls for acknowledgment from the economic sector counterpart. a study was carried out with the purpose of describing the relations between scientific fields and economic sectosr as a means of learning more about this system. records from the brazilian directory of research groups (version 4.1) were examined and selected when health was a keyword either to field of knowledge or economic sector. data were compiled into multiresponse variables and analyzed in contingency tables using residual, correspondence, and cluster analyses. it was found that the brazilian national health innovation system constitutes a sectorial system where competitiveness is more socially than economically-oriented, making this system favorably in tandem with the national unified health system as well as responsive to public policies focused on social welfare.

Abstract: INTRODU O: A necessidade de gest o da produ o científica e tecnológica em saúde está bem estabelecida no Brasil desde a realiza o da I Conferência Nacional de Ciência e Tecnologia (C&T) em Saúde, em 1994. O presente estudo apresenta uma estratégia metodológica para identifica o de prioridades institucionais no planejamento dessa gest o. MATERIAL E MéTODO: Questionário aplicado a uma amostra de pesquisadores dos Institutos de Pesquisa da Secretaria de Saúde do Estado de S o Paulo recolheu impress es sobre dois cenários alternativos: situa o atual e expectativa. Dezoito conceitos de gest o de C&T, compondo quatro temas distintos, foram medidos como percentagem média de aprova o ou reprova o em cada cenário. A contribui o de cada conceito nos quatro temas foi avaliada por exame de consistência interna através do coeficiente alfa de Cronbach e a composi o final dos temas foi depurada de conceitos inconsistentes. As médias de aprova o/reprova o foram ainda calculadas como coordenadas de cada cenário tanto para os conceitos quanto para os institutos estudados, permitindo a constru o de mapas de localiza o de prioridades de gest o por conceitos e por institutos. RESULTADOS: Observou-se que a situa o atual, ainda que n o expressivamente, merece reprova o, enquanto identificam-se altos níveis de expectativa, com destaque significativo para a gest o de infra-estrutura para pesquisa. As principais prioridades de gest o de C&T s o identificadas como: reconhecimento de mérito, apoio à divulga o, planejamento de recursos por projetos e assessoria metodológica. CONCLUS O: A estratégia metodológica adotada no estudo provê as informa es necessárias ao planejamento podendo ser aplicada a outras institui es de pesquisa através de adapta o do conteúdo temático e conceitual.
