Single center experience in laparoscopic colectomy for cancer

Abstract: SUMMARY Objective: The application of laparoscopic colectomy for colorectal carcinoma has been disputable regarding its efficacy as an oncological procedure. The aim of this study was to assess the early and long-term results of laparoscopic colectomy for malignant disease. Method: Between 1990 and 1997, 243 patients underwent laparoscopic colectomy for colon carcinoma, in our center. Morbidity and mortality data were analyzed retrospectively along with disease-free and overall patient survival. Results: Laparoscopic colectomy was successfully completed in 216 of 243 patients (89%). A perioperative morbidity rate of 22% and mortality rate of 1.4% were observed. The overall estimated 1, 2, 3, 5, and 7- year survival rates were 92%, 82%, 74%, 65%, and 45%, respectively. Only one case (<0.5%) with a trocar site recurrence was observed in this series. There were no recurrences at the wound extraction site, when the wound was protected. The overall estimated 5- and 7-year disease-free survival rates were 79% and 76%, respectively. Conclusion: Laparoscopic colectomy for cancer can be performed safely and effectively with perioperative morbidity, mortality and survival rates comparable to open colon resection. Abstract: there is increasing interest in peripheral nerve blocks (pnb) because of potential benefits relative to interactions of anticoagulants and central neuraxial techniques. among all the regional anesthesia procedures, pnb of the lower limb, and specially sciatic nerve block, are the less known and rarely used. in this paper, we summarize the recent advances in lumbosacral plexus block, with special remark on continuous catheter blocks for postoperative analgesia and chronic pain treatment. the use of long action local anes-thetics combined with a weak motor block, like ropivacaine, permits the combination of a single skin injection technique to achieve an adequate intraoperative analgesia with continuous infusion ones for postoperative analgesia. a detailed knowledge of anatomy as well as cutaneous nerve distribution is needed for the performance of these techniques. the introduction of several image-based techniques, principally ultrasound imaging, may improve the accuracy of nerve localization and needle placement. it also can help to prevent accidental puncture to vessels and other neighbouring structures. continuous plexus and peripheral nerve blocks offer the potential benefits of prolonged analgesia with fewer side effects, greater patient satisfaction, and faster functional recovery after surgery.

Abstract: Existe un interés creciente por la realización de los bloqueos de nervio periférico (BNP) debido a sus potenciales beneficios como los concernientes a las interacciones de los fármacos anticoagulantes y los bloqueos neuroaxiales. Los BNP de la extremidad inferior, y sobre todo, los bloqueos periféricos del nervio ciático son el pariente pobre de las técnicas de anestesia regional y, en general, son poco conocidos y por tanto poco utilizados. En este artículo se realiza una revisión de los bloqueos del plexo lumbosacro, realizando especial énfasis en los bloqueos continuos mediante catéteres para analgesia postoperatoria y para el tratamiento del dolor crónico. La utilización de anestésicos locales de larga duración de acción, asociada a un escaso bloqueo motor, como es el caso de la ropivacaína, nos permite combinar técnicas de punción única para conseguir una adecuada analgesia intraoperatoria, con las técnicas de perfusión continua para analgesia postoperatoria. Es necesario un conocimiento anatómico preciso, así como de los territorios cutáneos de inervación de las ramas del plexo lumbosacro, para la realización de estas técnicas de bloqueo. La introducción de diferentes técnicas de imagen, fundamentalmente la ultrasonografía, para la localización de las estructuras nerviosas, facilita la realización de estos bloqueos y disminuye el riesgo de lesiones de los órganos adyacentes. La realización de los bloqueos continuos de nervio periférico ofrece el beneficio de una analgesia postoperatoria prolongada, con menores efectos adversos, mayor grado de satisfacción del paciente, y una recuperación funcional más rápida después de la cirugía. The equivalence theorem and the production of gravitinos after inflation

Abstract: We study the high-energy equivalence between helicity 1/2 gravitinos and goldstinos in order to calculate the production of gravitinos in time-dependent scalar and gravitational backgrounds. We derive this equivalence for equations of motion, paying attention to some subtleties, mainly due to external sources, that are not present in the standard proofs. We also propose the Landau gauge as a simplifying alternative to the usual gauge choices, both for practical calculations and in the equivalence theorem proof.
Production of high-energy gravitinos during preheating

Abstract: We present a new technique for the calculation of helicity $\pm 1/2$ gravitino production during preheating. It is based on the equivalence between goldstinos and helicity $\pm 1/2$ gravitinos at high energies. The problem is thus reduced to the standard (Majorana) fermion production after inflation. Comparison with the results obtained in the unitary gauge is also presented.

$η\to π^0 γγ$ decay within unitarized chiral perturbation theory

Abstract: We improve the calculations of the $\eta \to \pi^0 \gamma \gamma$ decay within the context of meson chiral lagrangians. We use a chiral unitary approach for the meson-meson interaction, thus generating the $a_0(980)$ resonance and fixing the longstanding sign ambiguity on its contribution. This also allows us to calculate the loops with one vector meson exchange, thus removing a former source of uncertainty. In addition we ensure the consistency of the approach with other processes. First, by using vector meson dominance couplings normalized to agree with radiative vector meson decays. And, second, by checking the consistency of the calculations with the related $\gamma \gamma \to \pi^0 \eta$ reaction. We find an $\eta \to \pi^0 \gamma \gamma$ decay width of $0.47\pm 0.10$ eV, in clear disagreement with published data but in remarkable agreement with the most recent measurement.
Caracterización óptica De Nano Fluidos En Soluciones Iónicas

Abstract: Se estudia la transferencia de electrones fotoinducida entre nanopartículas y los compuestos de clorofila y el Ioduro 3,5-dimetil-1-fenil-2-metoxietano-pirazolio. Este sistema tiene potencial uso en electroquímica, en el desarrollo de pilas basadas en la generación de una corriente inducida por luz. Las nanopartículas se generan por medio del método de ablación láser al hacer impactar luz de alta potencia sobre placas de metales sumergidas en soluciones acuosas de un medio estabilizante. Las suspensiones coloidales de nanopartículas se caracterizan por medio de espectroscopia UV-Visible y TEM, de manera de determinar su tama o y morfología. Se evalúa la transferencia electrónica a través de los espectros de fluorescencia de muestras de diferente composición y concentración. Debido a que existe una dependencia de la cantidad de nanopartículas y la morfología de las mismas con respecto a la concentración del estabilizador, se observó que hay una marcada influencia de este parámetro con respecto a la intensidad de emisión fluorescente, lo cual se interpreta como una inhibición de fluorescencia producida por la transferencia electrónica de los estados mas excitados del compuesto hacia las nanopartículas de oro. El proceso dominante para esta inhibición es atribuida a la transferencia fotoinducida de electrones de los compuestos excitados (Clorofila y el Ioduro 3,5-dimetil-1-fenil-2-metoxietano-pirazolio) hacia las nanopartículas de oro.
Nature of the axial-vector mesons from their Nc behavior within the chiral unitary approach

Abstract: By describing within the chiral unitary approach the s-wave interaction of the vector meson nonet with the octet of pseudoscalar Goldstone Bosons, we find that the main component of the axial vector mesons b_1(1235), h_1(1170), h_1(1380), a_1(1260), f_1(1285) and the two states associated to the K_1(1270) does not follow the QCD dependence on the number of colors for ordinary qqbar mesons.
