Diabetes autoinmune del adulto en diabéticos tipo 2: frecuencia y características LADA in type 2 diabetics: frequency and characteristics

Abstract: Se realizó este trabajo para conocer frecuencia, características clínico-bioquímicas, inmunológicas y genéticas de la diabetes autoinmune en adultos (LADA) en 1 000 diabéticos tipo 2 con edades 3 35 a os con distintos tiempos de duración de la diabetes. Se les determinó glucemia, anticuerpos antiislotes pancreáticos (ICA), anti-GAD65, anti-ICA512bdc/IA2, antimicrosomales tiroideos (AMT), antigástricos parietales (AGP), antinucleares (AN), microalbuminuria y péptido C en ayunas. Se encuestaron y se registraron algunas características clínicas. Se dividieron en 2 grupos según la presencia de ICA. Todos los diabéticos tipo 2 + para autoanticuerpos antiislotes (ICA y/o anti-GAD65) fueron identificados como LADA. Se detectó el 3,4 % de diabético tipo 2 con ICA +, en los diabéticos tipo 2 ICA- el 22,0 % presentó anticuerpos anti-GAD65. Se encontró que los diabéticos tipo 2 ICA+ eran más jóvenes, la duración de su diabetes era menor, presentaron menor IMC, disminución de los niveles de péptido C en ayunas, menos antecedentes familiares (padres) de DM2, valores menores en las tensiones arteriales diastólicas y sistólicas, mayor presencia de anticuerpos anti-GAD65, AMT y AGP en comparación con los diabéticos tipo 2 ICA-. Se observó que los diabéticos tipo 2 ICA+ (LADA) tienen características específicas que los asemejan a los diabéticos tipo 1, esto implicaría variaciones importantes en su tratamiento y evolución con respecto a los diabéticos tipo 2 ICA-. Se observó una baja frecuencia de ICA y alta de GAD en los diabéticos tipo 2 cubanos, las cuales fueron diferentes a la encontrada en poblaciones caucasianas. Los anticuerpos anti-GAD65 fueron superiores a los ICA para detectar los LADA. Las características clínicas e inmunológicas de estos pacientes muestran la lenta progresión de la destrucción autoinmune de las células b con implicaciones terapéuticas. Abstract: En el presente trabajo se realiza una revisión del conocimiento actual sobre la regulación de las concentraciones intracelulares del ion calcio, los principales mecanismos de entrada y salida de este a través de la membrana plasmática, con especial atención en el intercambiador Na+/Ca2+, y la función de este importante segundo mensajero en la secreción de insulina, así como la muerte celular programada de las células pancreáticas. Helicobacter pylori y anticuerpos antiislotes pancreáticos en la diabetes mellitus

Abstract: 66 ica-positive patients were studied: 21 type 1 diabetics, 24 diabetics initially classified as type 2 and 21 first-degree relatives of type 1 diabetics. they were compared with 101 ica-negative patients: 20 type 2 diabetics, 20 first-degree relatives, 21 children and 40 non-diabetic adult controls to analyze if the immunolgical response to h. pylori is associated with islet cell antibodies (ica). the ica were determined by the indirect immunofluorescence method with prolongued incubation and the levels of antibodies (igg) versus h. pylori (hp) by an elisa using a commercial kit. it was observed a frequency of antibodies (igg) versus h. pylori of 38.0 % (25/66) in ica-positive patients and of 39.6 % (40/101) in ica-negative patients. no correlation was found between the presence of antibodies (igg) versus h.pylori and the ica in the studied groups. there were no differences as regards the presence of igg anitbodies versus h. pylori between diabetes mellitus and the controls (47.7 % [31/65] vs. 40.9 % [ 25/61]). it was observed a high percentage of antibodies (igg) vs. h. pylori in type 1 diabetics (33 %, 7/21) in relation to the controls (14 %, 3/21), but with no significant differences. the frequence of antibodies vs. helicobacter in adult controls is higher than in children (55 %, 22/40 vs. 3/21, p=0.0025). the frequency of antibodies vs. h. pylori is much more elevated in the adult controls may be because of the greater possibility of exposure to reinfections. in spite of these results, we do not exclude the idea that the infection due to h. pylori is related to type i diabetes, since some studies suggest that igg versus h. pylori does not persist for a long time.

Abstract: this paper was aimed at knowing the frequency, clinico-biochemical, immunologic and genetic characteristics of autoimmune diabetes in adults (lada) in 1 000 type 2 diabetic patients aged 35 or over with different times of duration of diabetes. glycemia, anti-pancreatic islet cell antibodies (ica), anti-gad65 antibodies, anti-ica512bdc/ia2 antibodies, anti-microsomal thyroid antibodies (amt), anti-gastric parietal antibodies (agp), antinuclear antibodies (an), microalbuminuria and peptide c during fasting were determined. these patients were surveyed and some clinical characteristics were registered. they were divided into 2 groups according to the presence of ica. all the type 2 + diabetics for anti-islet cell autoantibodies (ica and/or antigad65) were identified as lada. 3.4 % of type 2 ica + were detected. 22.0 % of type 2 ica - diabetics had anti-gad65 antibodies. it was found that type 2 ica + diabetics were younger, that their diabetes was shorter, that they had lower bmi, reduced levels of fasting peptide c, less dm2 history family (parents), lower values of diastolic and systolic arterial pressure, higher presence of anti-gad65 antibodies, amt and agp in comparison with type 2 ica - diabetics. it was observed that type 2 ica+ diabetics (lada) have specific characteristics that make them similar to type 1 diabetics, which would lead to important variations in their treatment and evolution as regards type 2 ica - diabetics. among the cuban type 2 diabetics it was detected a low frequency of ica and a high frequency of gad, which were different to those found in the caucasian populations. the anti-gad65 antibodies were higher than ica to detect lada. the clinical and immunological characteristics of these patients show the slow progression of the autoimmune destruction of b-cells with therapeutic implications.

Abstract: introduction: the antithyroperoxidase (tpoab) and antitransglutaminase (ttgab) antibodies are useful markers of autoimmune thyroid disease and celiac disease, respectively. its presence in first-degree relatives of type 1 diabetes patients has not been described in cuba. objective: to determine the tpoab and ttgab frequencies in first-degree relatives of type 1 diabetes patients and its relation to some clinical, biochemical and immunological features. methods: in a group of 285 subjects we measured tpoab and in 262 subjects we measured ttgab. the cases included aged between 2 and 65. data were registered on age, sex, skin color, personal backgrounds, and a family history of obesity, type 2 diabetes, thyroid disease and celiac disease. symptoms were look for and clinical signs of celiac disease and autoimmune thyroid disease were explored. fasting glucose, fasting insulin, tpoab, ttgab and type 1 diabetes associated autoantibodies (agad and aia-2) were determined as well as the insulin resistance according the homa-ir index. results: the frequencies of positive tpoab and ttgab were of 5,3 and 1,9%, respectively. the family history of thyroid disease, slight muscular tremor and exophthalmos are related to presence of tpoab. intestinal malabsorption, persistent diarrhea, recurrent abdominal pain and personal background of liver disease were associated with presence of ttgab. there was an association between ttgab and aia-2. insulin resistance was not associated with the presence of both antibodies. conclusions: in first-degree relatives of type 1 diabetes patients, frequencies of tpoab and ttgab are low. some backgrounds, symptoms and signs linked to celiac disease and autoimmune thyroid disease may be practical indicators previous to perform these autoantibodies.
Glutamine synthetase and glutamate synthase activities in relation to nitrogen fixation in Lotus spp.

Abstract: lotus corniculatus, l. tenuis, l. pedunculatus, and l. subbiflorus inoculated with mesorhizobium loti nzp2037 strain were grown in a growth chamber. the plants dry weight (dw), the nodule fresh weight (fw), the nitrogenase activity, the nodule glutamine synthetase (gs) and glutamate synthase (gogat) activities, as well as the leghemoglobin content and the amino acid in the stem were measured 28 days after inoculation. the highest dw of plants was measured in l. tenuis and the highest fw of nodules was measured in l. pedunculatus. nitrogenase activity in l. tenuis, l. pedunculatus and l. subbiflorus was six fold the activity in l. corniculatus. nodule gs and gogat activities did not follow this same pattern. l. tenuis had the highest values of gs and gogat activities in the nodule, and a high nitrogenase activity which is consistent with its high plant dw. the four species of lotus were compared and no correlation between nitrogen fixation parameters and ammonia assimilation enzymes was found, but the gs/gogat ratio has a positive and significant correlation (r2=0.82**) with the amino acid content in stems.
