Abstract: Esophageal replacements in children usually are necessary in patients with of esophageal atresia or those develop inflammatory scars after consumption of caustic substances. For the substitutive procedure diverse organs are employed, like small or thick intestine and stomach. We presented the Good Clinical Practices Guideline for Esophageal replacement, approved by consensus in the the 3th National Good Clinical Practices Workshop in Pediatric Surgery (Camagüey, Cuba; February 23 - 26, 2004). Las sustituciones esofágicas en ni os frecuentmente son necesarias en pacientes portadores de atresia esofágica y en aquellos que desarrollan cicatrices inflamatorias posteriores a ingestión de sustancias cáusticas. Para el procedimiento sustitutivo se emplean diversos órganos, como intestino delgado, grueso y estómago. Se presenta la Guía de Buenas Prácticas Clínicas para sustituciones esofágicas, aprobada por consenso en el 3er Taller Nacional de Buenas Prácticas Clínicas en Cirugía Pediátrica (Camagüey, 23 al 26 de febrero de 2004).
Torsión testicular perinatal: resultados de 10 a?os de experiencia

Abstract: introduction: the perinatal testicular torsion is defined as a twist of testicle in prenatal period or within the first 28 days of life. objective: to assess the data obtained from the medical records and to propose the principles of clinical and surgical treatments. methods: retrospectively the medical records of newborn operated on diagnosed with testicular torsion were reviewed admitted in the pediatric surgery service of the "william soler" teaching children hospital from january, 2000 to december, 2009. results: eighteen patients were eligible for study, 17 had a unilateral torsion with a testicle of increase size, hard and painless (94 %), the 55 % had contralateral hydrocele, 16 patients born at term weighing more than 3 100 g (88 %), the extravaginal torsion and left testicle involvement was observed in 13 (72 %). a case had bilateral asynchronism (6 %), since the left testicle was twisted in the inguinal region with ischemia but not necrosis and could be preserved. two cases (12 %) had torsion of intravaginal type and one of them was clinically presented as acute scrotum. the affection was diagnosed in ages from 0 to 7 days in the 72 % of cases. an orchiectomy was performed without contralateral exploration in 16 patients (88 %). evolution was satisfactory in 17 cases (94 %). conclusions: the perinatal testicular torsion accounts for the 3,4 % of neonatal surgical affections and may to involve a und-descent testicle. authors suggest making a protocol of preoperative doppler ultrasound and the exploration and fixation of contralateral testicle.

Abstract: the pleuropulmonary blastoma (ppb) ia a rare and aggressive tumor affecting children at early ages. the case of a 3-year-old male patient that was diagnosed ppb of the left lung in our hospital (july, 1995) and required 5 operations during the 6 years of survival due to the trend to relapse of this type of tumor, which is a challenge for the pediatric surgeon, is presented. pediatricians and specialists in general comprehensive medicine are given information on the therapeutic conducts and the current trends in a child with cancer of long time of evolution.

Abstract: Introducción: la torsión testicular perinatal es definida como la torsión del testículo en período prenatal, o dentro de los primeros 28 días de vida. Objetivo: evaluar los datos obtenidos de las historias clínicas, y proponer principios de tratamientos clínicos y quirúrgicos. Métodos: fueron revisadas retrospectivamente las historias clínicas de los recién nacidos operados con diagnóstico de torsión testicular en el servicio de cirugía pediátrica del Hospital Pediátrico "William Soler", entre enero de 2000 y diciembre de 2009. Resultados: 18 pacientes fueron elegibles para el estudio, 17 tenían torsión unilateral con testículo aumentado de tama o duro e indoloro (94 %), 55 % tenían hidrocele contralateral, 16 pacientes nacieron a término, con más de 3 100 g de peso (88 %), y la torsión extravaginal y la afectación del testículo izquierdo se observó en 13 (72 %). Un caso fue bilateral asincrónico (6 %), pues el testículo izquierdo estaba torcido en la región inguinal, isquémico, pero no necrótico, y fue preservado. Dos casos (12 %) tenían torsión de tipo intravaginal, y uno de ellos se presentó, clínicamente, como escroto agudo. Se diagnosticó la afección en edades de 0 a 7 días en el 72 % de los casos. Se realizó orquiectomía sin exploración contralateral a 16 pacientes (88 %). La evolución fue satisfactoria en 17 (94 %). Conclusiones: la torsión testicular perinatal representa el 3,4 % de las afecciones quirúrgicas neonatales, y puede afectar un testículo no descendido. Se sugiere protocolizar el uso del ultrasonido doppler preoperatorio, y la exploración y fijación del testículo contralateral. Abstract: El blastoma pleuropulmonar (BPP) es un raro y agresivo tumor maligno que afecta a los ni os de edad temprana. Se trata de un paciente masculino que a los 3 a os de edad se le diagnostica en nuestro hospital (julio de 1995) BPP del pulmón izquierdo, y requirió durante los 6 a os de sobreviva de 5 intervenciones quirúrgicas a causa de la tendencia a recidivar de esta variedad de tumor, lo que constituye un reto para el cirujano pediatra. Se ofrece a los pediatras y a los especialistas de medicina general integral, una información sobre las conductas terapéuticas y las tendencias actuales ante un ni o con cáncer de largo tiempo de evolución. Abstract: introduction: present like a real surgical emergence, the neonatal gastric perforation is a challenge for all physicians working in this field. objective: to show the results of our experience with the treatment of newborns presenting with gastric perforation and also to review the current state of knowledge on this nosology. methods: a retrospective and descriptive research was conducted in all the newborns admitted in the surgical neonatology ward of the "william soler" university children hospital, diagnosed with a gastric perforation, from january, 1990 to december, 2010. results: eight patients were treat concluded that three were idiopathic. the fundus of the stomach and the organ's anterior face were the more frequently involved sites. by his part, the prematurity hasn't an evident relationship with the occurrence of perforation. in most of patients a primary repair of the perforation without gastrotomy was carried out. the general morality in present study was of 62 % and the peritonitis was the more frequent complication. conclusions: the neonatal gastric perforation is a highly lethal entity in which the different alternatives of surgical treatment are not anyway by themselves, the solution to achieve an appropriate survival.

Abstract: Introducción: presentándose como una verdadera emergencia quirúrgica, la perforación gástrica neonatal constituye un reto para todos los galenos que laboran en este campo. Objetivo: exponer los resultados de nuestra experiencia en el tratamiento de recién nacidos con perforación gástrica, y revisar el estado actual del conocimiento sobre esta nosología. Métodos: se realizó una investigación descriptiva, retrospectiva, con la totalidad de los recién nacidos ingresados en la sala de neonatología quirúrgica del Hospital Pediátrico Universitario "William Soler", diagnosticados con perforación gástrica, durante el período comprendido entre enero de 1990 y diciembre de 2010. Resultados: se trataron 8 pacientes, de los cuales 3 se consideraron como idiopáticos. El neumoperitoneo estuvo presente en la totalidad de los pacientes. El fundus gástrico y la cara anterior del órgano constituyeron los sitios más frecuentemente afectados. Por su lado, la prematuridad no guardó una clara relación con la ocurrencia de la perforación. En la mayoría de los pacientes se realizó una reparación primaria de la perforación sin gastrostomía. La mortalidad general en el estudio fue de un 62 %, y fue la peritonitis la complicación más frecuente. Conclusiones: la perforación gástrica neonatal constituye una entidad altamente letal, en la cual las distintas alternativas de tratamiento quirúrgico no son, en definitiva, por sí solas, la solución para lograr una adecuada sobrevida. Abstract: INTRODUCCIóN: se presentan las 4 primeras pacientes operadas de hemitiroidectomía videoendoscópica en el Hospital "Hermanos Ameijeiras", para introducir una técnica que elimine la cicatriz en el cuello de los(as) pacientes que necesitan un tratamiento quirúrgico por una enfermedad tiroidea. MéTODOS: se escogieron 4 pacientes con nódulos benignos del tiroides menores de 3 cm, remitidas para tratamiento quirúrgico por el servicio de endocrinología de nuestro centro, de enero a junio de 2009. Se les explicó en detalles el proceder y firmaron un consentimiento informado para participar en el estudio. RESULTADOS: en las 4 pacientes se pudo realizar la operación planificada, el promedio de tiempo quirúrgico fue de 2,9 h. El efecto cosmético fue mayor en las pacientes 2, 3 y 4 al no quedar ninguna cicatriz en el cuello. La laringoscopia posoperatoria demostró movilidad normal de las cuerdas vocales en las 4 pacientes. CONCLUSIONES: la cirugía videoendoscópica del tiroides es una realidad que no podemos obviar, y realmente tiene un único impacto demostrado hasta el momento actual, que es la posibilidad de eliminar la cicatriz residual en el cuello que tanto preocupa a los(as) pacientes jóvenes. INTRODUCTION: these are the cases of the first patients operated on of Hemithyroidectomy by videoendoscopy in "Hermanos Ameijeiras" Clinical Surgical Hospital to introduce a technique eliminating the neck scar of patients needing surgical treatment by thyroid disease. METHODS: four patients presenting with benign thyroid nodules referred to surgical treatment by Endocrinology service of our institution from January to June, 2009. Procedure was explained in details and they signed the informed consent to participate in study. RESULTS: in four patients it was possible to perform the planned operation; the surgical time average was of 2, 9 hr. Cosmetic effect was greater in patients 2, 3 and 4 without neck scar. Postoperative laryngoscopy showed a normal motility of vocal cords in the four patients. CONCLUSIONS: thyroid videoendoscopy surgery is a reality that cannot be obviated and with a unique impact proved until present time that is the possibility to eliminate the neck residual scar so worrying for young patients.

Abstract: the laparoscopic splenectomy (ls) offers the advantages of the laparoscopic surgery. the best indication of ls are the bening hematological diseases. 22 splenectomies were performed by the laparoscopic method from october, 1996, to august, 2000. the most frequent indications were the bening hematological diseases: idiopathic thrombocytopenic purpura in 9 patients, hereditary microspherocytosis in 4, autoimmune hemolitic anaemia in 5, hodgkin lymphoma staging in 2, 1 case of cystic spleen tumor and 1 case of hypersplenism of undemonstrated cause. the results obtained in these first 22 cases are analyzed and the surgical technique is described. the average age was 34.3 years old with a range from 16 to 73 years old. the average surgical time was 147 min. open surgery was performed in 4 patients due to postoperative bleeding. surgical accidents occurred in 4 patients. no postoperative complications were reported. the procedure was associated with laparoscopic cholecystectomy in 6 patients and with biopsy of mesenteric ganglion with liver biopsy in 2 patients. the average transoperative bleeding was 467 ml. the ls is a reproducible and safe technique having the advantages of laparoscopic surgery (lower morbidity and postoperative mortality), mainly in the bening hematologic diseases with spleen of normal size or splenomegaly not exceeding 25 cm, in spite of the negative effect of the learning curve
Análisis de la continencia fecal en 83 pacientes operados de malformaciones anorrectales

Abstract: we made a retrospective study of 83 patients operated on for anorectal malformations( 46 males and 37 females) in "william soler" university pediatric hospital from january 1984 to december 1997. this research work analyzed fecal continence using kelly′s evaluation method after the posterior sagital anorectoplasty. fecal continence per anatomical variety of malformation and sex, follow up time and association with sacro bone alterations were shown. the best results were achieved in low defects, longer period of recovery and in those children with normal sacro bone.
