Aprendizaje práctico de la medicina en entornos clínicos para alumnos de tercer curso: Consecución de objetivos de conocimiento

Abstract: aim. to present the main results obtained in the cognitive objectives in the project called 'basic clinical initiation centered medical practice', a teaching innovation developed in the introduction to internal medicine subject of medical degree during the academic year 2009-2010. subjects and methods. 50 students were selected in the third course of medicine degree for the development of the project, which aimed to equip them with basic knowledge and skills to start on health care by conducting guided and supervised clinical practice in real clinical settings, with specific targets and under the supervision of medical experts. results. the results were excellent, showing the purchase of all proposed objectives. most students felt that it was a very positive and enriching experience. conclusion. the proposed methodology, based on the achievement of specific goals, contributes to improve learning. the satisfaction of both students and teachers has been very high with the results obtained after implementation of the project.

Abstract: Objetivo. Presentar los principales resultados obtenidos en la consecución de los objetivos de conocimiento en el proyecto denominado 'Iniciación clínica básica centrada en la práctica médica', una innovación docente desarrollada en la asignatura 'Patología general' de la licenciatura de medicina durante el curso académico 2009-2010. Sujetos y métodos. Se seleccionaron 50 alumnos de tercero de medicina para el desarrollo del proyecto, cuya finalidad era contribuir a dotarles de los conocimientos y habilidades básicas para iniciarse en la atención sanitaria mediante la realización de prácticas clínicas orientadas y supervisadas en contextos asistenciales reales, con unos objetivos concretos y bajo la supervisión de médicos expertos. Resultados. Los resultados obtenidos fueron excelentes, mostrando la adquisición de todos los objetivos propuestos. La gran mayoría de los alumnos consideraron muy positiva, a la par que enriquecedora, la experiencia. Conclusión. La metodología propuesta, basada en las prácticas clínicas orientadas a la consecución de objetivos concretos, mejora el aprendizaje. La satisfacción tanto de los alumnos como de los profesores ha sido muy alta con los resultados obtenidos tras la implantación del proyecto. Aim. Abstract: Describimos el caso clínico-radiológico de una paciente de 25 a os que desarrolló una leucoencefalopatía posterior reversible (LPR) en el periodo posparto, sin evidencia de preeclampsia-eclampsia o hipertensión arterial crónica. La LPR se asocia a entidades clínicas diversas, incluyendo la eclampsia. La paciente presentó, diez días después del parto, un cuadro clínico compuesto por cefalea, hipertensión arterial y convulsiones. El edema vasogénico reversible que afecta a la sustancia blanca de las regiones posteriores constituye el hallazgo característico en la resonancia magnética cerebral. Aunque el pronóstico es favorable, el tratamiento debe ser precoz y agresivo, efectuando un control rápido de las convulsiones y de la hipertensión arterial, con la finalidad de evitar el desarrollo de isquemia e infarto cerebral. Es necesario tener un alto índice de sospecha y considerar el diagnóstico de eclampsia posparto y LPR en mujeres que presenten convulsiones y otros síntomas neurológicos en el puerperio. We report the clinical-radiological case of a 25 year-old female patient who developed reversible posterior leukoencephalopathy syndrome (RPLS) in the postpartum period, without evidence of preeclampsia-eclampsia or chronic arterial hypertension. RPLS is associated with diverse clinical entities including eclampsia. Ten days after giving birth, the patient presented with clinical symptoms of headache, elevated blood pressure and seizures. Reversible vasogenic oedema affecting the white matter in the posterior regions was the characteristic finding in magnetic resonance imaging (MRI) of the brain. Although the prognosis is favourable, treatment needs to be early and aggressive, with rapid control of the convulsions and arterial hypertension, with the aim of preventing ischemia and cerebral infarct from developing. There is a need to be highly alert and to consider the diagnosis of RPLS in women presenting with convulsions and other neurological symptoms in postpartum.

Clinical and Virological Efficacy of Etravirine Plus Two Active Nucleos(t)ide Analogs in an Heterogeneous HIV-Infected Population

Abstract: Etravirine (ETV) is recommended in combination with a boosted protease inhibitor plus an optimized background regimen for salvage therapy, but there is limited experience with its use in combination with two nucleos(t)ide reverse-transcriptase inhibitors (NRTIs). This multicenter study aimed to assess the efficacy of this combination in two scenarios: group A) subjects without virologic failure on or no experience with non-nucleoside reverse-transcriptase inhibitors (NNRTIs) switched due to adverse events and group B) subjects switched after a virologic failure on an efavirenz- or nevirapine-based regimen. The primary endpoint was efficacy at 52 weeks analysed by intention-to-treat. Virologic failure was defined as the inability to suppress plasma HIV-RNA to <50 copies/mL after 24 weeks on treatment, or a confirmed viral load >200 copies/mL in patients who had previously achieved a viral suppression or had an undetectable viral load at inclusion. Two hundred eighty seven patients were included. Treatment efficacy rates in group A and B were 88.0% (CI95, 83.9–92.1%) and 77.4% (CI95, 65.0–89.7%), respectively; the rates reached 97.2% (CI95, 95.1–99.3%) and 90.5% (CI95, 81.7–99.3), by on-treatment analysis. The once-a-day ETV treatment was as effective as the twice daily dosing regimen. Grade 1–2 adverse events were observed motivating a treatment switch in 4.2% of the subjects. In conclusion, ETV (once- or twice daily) plus two analogs is a suitable, well-tolerated combination both as a switching strategy and after failure with first generation NNRTIs, ensuring full drug activity. Abstract: cereal-legume combinations offer proteins of high quality due to the compensation of the existing essential amino acids. quinua (chenopodium quinua willd) and lupin (lupinus albus l.) were selected with the aim of designing a formulation of high protein content with additives and chicken flavoring in order to obtain a physical powder mixture that, when added to water, resulted in a baby food type cream destined for 2-5 year old children, whose amino acid content would fulfill 35-40% of the daily requirement. linear programming was applied for the formulation. in the selected formulation, the nutrients were determined through proximal analysis, and some physical and microbiological tests were performed. sensorial tests by attributes in semi-trained adults were carried out and a 5 points hedonic face scale was applied to preschool children from three day care centers at antofagasta, chile. protein content of the lupin flour (49.77g×100g) surpassed in 4.6 times that of quinua (10.82g×100g), but not that of the essential amino acids, and the complement of both granted a protein of good quality. sanitary quality of the product was corroborated by satisfactory microbiological results. the cream obtained a score between "i like it slightly" and "i like it very much", showing a good acceptance by 2-5 year old children. the cream provided a protein digestibility corrected amino acid score (pdcaas) of 0.9 for 90 days after produced, because the essential amino acids, triptophan and the set metionine + cistine, did not fulfill the established fao requierement.

Abstract: In this paper we obtain $L^1$-weighted norms of classical orthogonal polynomials (Hermite, Laguerre and Jacobi polynomials) in terms of the zeros of these orthogonal polynomials; these expressions are usually known as quadrature rules. In particular these new formulae are useful to calculate directly some positive defined integrals as several examples show.

Abstract:

The diagnosis delay in new cases of HIV infection is a frequent fact. Our objective was to detect and analyse the lost opportunities and describe the characteristics of these patients. Method: The search was done by a revision of personal histories of new diagnosis of HIV infection from 1st January to 31st December 2011 in the database of VACH. We selected those that had consulted a doctor in the previous year in the Emergency area, Primary Care and Specialised Consultations in the database of the histories of the Public Health Service. We called low attendance if they came 1 - 3 times and high if over 3. We grouped patients into those that fulfilled criteria of diagnosis delay by count of CD4s. We called no diagnosis delay to those that had count of CD4 over 350, diagnosis delay under 350 and advanced disease under 200. Results: There were 107 new cases. The global percentage of DD was 61.7% of cases. From these, 45.38% fulfilled criteria of AD. It was possible to find information about the existence of previous sanitary attendance in 59 patients. From these 58% were diagnosed with delay, fulfilling criteria of AD in 27%. The predominant means of infection was sexual. 35 patients attended a healthcare level, 19 two and 5 three. 47.5% consulted over 3 times. They requested a total of 274 consultations. Discussion: The diagnosis delay is a reality. It took our attention that from 59 patients having requested previous medical assistance 58% were diagnosed with delay and 27% fulfilled criteria of AD. We found that almost half of them had been attended in 4 and up to 14 times, in some occasions with suggestive symptoms of HIV infection. Facing this discovery we think that some interventions should be undertaken to get an early diagnosis and the control of the outbreak.

Desde entonces el sue o de muchos investigadores en cáncer ha sido el de desarrollar terapias basadas en la exquisita especificidad del sistema immunitario con el fin de eliminar tumores sin la toxicidad asociada con la quimio y/o radioterapia.
