Transposition of the great arteries

Abstract: The diagnosis is confirmed by echocardiography, which also provides the morphological details required for future surgical management. Prenatal diagnosis by foetal echocardiography is possible and desirable, as it may improve the early neonatal management and reduce morbidity and mortality. Differential diagnosis includes other causes of central neonatal cyanosis. Palliative treatment with prostaglandin E1 and balloon atrial septostomy are usually required soon after birth. Surgical correction is performed at a later stage. Usually, the Jatene arterial switch operation is the procedure of choice. Whenever this operation is not feasible, adequate alternative surgical approach should be implemented. With the advent of newer and improved surgical techniques and post operative intensive care, the long-term survival is approximately 90% at 15 years of age. However, the exercise performance, cognitive function and quality of life may be impaired.Transposition of the great arteries; physiologically uncorrected transposition; complete transposition; atrioventricular concordance with ventriculoarterial discordance. The European paediatric cardiac code for this disease is 01.05.01.The transposition of the great arteries was first described by Mathew Baillie in 1797, in the second edition of the book "The Morbid Anatomy of Some of the Most Important Parts of the Human Body". However, the term transposition was only applied in 1814, by Farre, meaning that aorta and pulmonary trunk were placed (positio) across (trans) the ventricular septum.In fact, this congenital cardiac malformation is characterised by atrioventricular concordance and ventriculoarterial discordance. In other words, the morphological right atrium is connected to the morphological right ventricle which gives rise entirely to or most of the aorta; the morphological left atrium is connected to the morphological left ventricle from where the pulmonary trunk emerges [1].The term congenitally corrected transposition

Abstract: the article addresses the production of medical literature that offered advice on how mothers should raise their children. doctors specializing in puericulture - educating mothers about topics like food, dress, hygiene, and behavior - believed that love alone was not enough to provide children with good training and education. as representatives of scientific truth and technique, these doctors tried to teach mothers to act according to the principles of puericulture, contributing primarily to the construction of the mother-nurse, an ideal endorsed by the authors of the three manuals published between 1938 and 1963 that are analyzed in this article.

Abstract: this article deals with the production of specialized knowledge about the female body between the 19th and 20th centuries. its main goal is to analyze the production of images displayed in obstetrics treatises and manuals published in europe and used by brazilian medical students and doctors. it seeks to understand the realism of the medical-scientific images of the female body as a means of expression of a new relationship between doctors and women, resulting from the investigation methods used in anatomy-pathology laboratories and in the clinical examination of pregnant women.

Abstract: Este artigo analisa cartas escritas por pessoas comuns para o presidente Getúlio Vargas com o objetivo de receber o abono familiar. Motivados pela política familiar do governo definida pela Constitui o de 1937 e pelo Decreto-Lei 3200, de 1941, pais e m es enviavam cartas e fotografias para contar suas histórias de pobreza, mas também para provar como estavam adequados à política demográfica e social do governo. This article analyzes letters written by ordinary people to President Getúlio Vargas with the aim to receive the family allowance. Motivated by government’s family policy defined by the Constitution of 1937 and Decree no. 3200 of 1941, fathers and mothers sent letters and photographs to tell their stories of poverty, but also to prove they conformed to the government’s demographic and social policy. Este artículo analiza las cartas escritas por personas comunes al presidente Getúlio Vargas, con el objetivo de recibir el abono familiar.Estimulados por la política familiar del gobierno, definida por la Constitución de 1937 y por el Decreto Ley 3200 de 1941, padres y madres enviaron cartas y fotografías no sólo para contar sus historias de pobreza, sino también para mostrar cómo se habían adecuado a la política demográfica y social del gobierno.
How Growth Ability of Multidrug-Resistant Escherichia coli Is Affected by Abiotic Stress Factors  [PDF]

Abstract:


The ability of multidrug-resistant Escherichia coli to adapt and grow in a wide range of different environmental conditions may be crucial to the global spread of antimicrobial resistance. The aim of this study was to evaluate the survival ability of 54 multidrug-resistant E. coli strains, isolated from three different biotopes (clinical setting, gull intestine, river water) when subjected to variations in pH (from 3 to 11) and salinity (from 0.5% to 6% of NaCl) and to nutrient deprivation. The growth of each isolate as well as of a reference strain was assessed during 168 h in every varying condition. Slight variations in the growth ability under some abiotic stress factors were recorded among the isolates from the different biotopes. Multidrug-resistant isolates from gull feces were found to be the more tolerant to environmental abiotic changes, while isolates from river water were the less tolerant. In addition, it was notorious that the carriage of antimicrobial resistance has a clear fitness cost in comparison with the susceptible (reference) strain, highlighting the necessity of reducing the selective pressure exerted by antibiotics. This study underlines the ecological hardness of multidrug-resistant E. coli isolates with a consequent ability to reach and colonize new host and environments.


A case study applying process and project alignment methodology

Abstract: software process improvement (spi) is one of the main software development challenges. however, spi standards and models (cmmi, spice) have not been always adopted with success. the current problem is a lack of strategy to implement successfully these standards and models. to undertake this objective is essential observe real life experiences and detect process and project mutual relationships. without this alignment it will not be possible to find out how process management is really important to achieve organization's strategic objectives. this paper proposes a methodology that allows the definition, evaluation and improvement of an organization software development process. this proposal, called a process and project alignment methodology (propam), allows the specification of an organization development process, as well process and project alignment. propam presents the following life cycle: (1) process definition; (2) project definition considering a base process model; (3) project coordination and monitoring and (4) process improvement assessment. this paper also provides an overview of the action plan to be taken within the software organizations that intent to conduct a spi initiative. this plan includes two distinct phases: (1) specify the development process and (2) analyze projects, starting an spi effort. in order to evaluate propam, a study case is undertaken. the case study is performed following the action plan and presents all the steps of the propam. final results show that, when the organization started using propam, process and project alignment reduced project planning time and effort. propam also introduced new organizational practices that result in a spi program.
Estudo comparativo e varia??o sazonal da ictiofauna na zona entremarés do Mar Casado-Guarujá e Mar Pequeno-S?o Vicente, SP

Abstract: through a monthly beach seine survey programme carried out in the coastal beach of mar casado (guarujá-sp) and in the estuarine beach of mar pequeno (s?o vicente-sp), from may, 1984 to may, 1985, it was yielded a total of 5723 individuals of 42 species of marine and estuarine fish. a list of species, seasonal occurrence and relative abundances is presented. for each area, we found only five dominant species as: in mar casado, trachinotus carolinus and t. falctus (carangidae), juveniles of mugilidae, harengula clupeola (clupeidae) and eucinostmus melanopterus (gerreidae); in mar pequeno, juveniles of mugilidae, xenomelaniris brasilienses (atherinidae), opisthonema oglinum and harengula clupeola (clupeidae). in mar pequeno we found the greatest richness of species, the highest values of individuals's number, and cpue, apparently related with water's temperature that are lower in winter and higher in summer-fall. the shannon-weaver index was calculated to determine the diversity of ichthyofauna; it varies seasonally and seems not to be related with any hydrological parameters.

Abstract: the present work characterized the crystallographic texture of a commercial 3003 aluminum alloy produced by the twin roll caster process. the different stages of metal processing: solidification, plastic deformation and recrystallization, have different crystallographic textures. the crystallographic texture at the end of the manufacturing process contains information about the process itself. the texture characterization was carried out by electron back scatter diffraction (ebsd). results showed that there was a significant difference between the specimen's texture at the surface and that of its center.
