Preventive health services for systemic lupus erythematosus patients: whose job is it?

Abstract: Over the last few decades, the survival of patients with systemic lupus erythematosus (SLE) has improved dramatically. With these patients living longer, medical care has moved from merely treating the primary condition of the patient to a comprehensive approach that includes the provision of preventive services. How good we are at providing these services and who is responsible for providing them (the specialist as opposed to the generalist) have not been previously addressed. Lupus-related problems that need to be addressed without delay and the limited time allowed per patient visit may curtail the ability of the specialist to provide such services. But poor communication with the specialist may limit the generalist in providing these services, particularly if the latter assumes incorrectly that all health-related issues (preventive services included) are being taken care of by the former. Such assumptions are not uncommon in a less-than-ideal and oftentimes fragmented health care system, such as the one we currently have in the US.Thus, it is gratifying to note the study by Yazdany and colleagues [1] in this issue of Arthritis Research & Therapy. In this study on the provision of preventive health services to patients with SLE, two categories of preventive services were addressed: cancer prevention and immunizations. The authors studied patients from the Lupus Outcome Study, a longitudinal observational study of more than 1,000 English-speaking patients recruited from academic and non-academic centers and community-based sources in Northern California [2]. Data from the California Health Interview Survey were used for comparison. Only patients for whom a specific service would apply (for example, mammography or colon screening) and the respective population-based data were included in each of the analyses. Caracterización de personalidad de mujeres adolescentes infractoras de ley: un estudio comparativo

Abstract: this study aimed to characterize a group of female juvenile offenders emphasizing differences from male and female juvenile offenders with clinical problems. participants were 90 adolescents from 13 to 18 years old, who belonged to three groups: female juvenile offenders, male juvenile offenders and female with clinical problems. participants were evaluated through the millon adolescent clinical inventory (maci) and the escalation and risk record card (fer) using a descriptive correlational design. results showed that females perpetrated fewer and less severe offences than did males; however, they present more risk factors. the female personality profile revealed an active, unruly style, similar to males' and more severe emotional disorders than those found in females with clinical problems. this female personality profile is congruent with international studies about juvenile delinquency and relevant for guiding specialized intervention programs for female young offenders.
Estilos de Personalidad y Desadaptación Social Durante la Adolescencia

Abstract: the present study was aimed at identifying personality traits in adolescent offenders. with this purpose, millon's adolescent clinical inventory (maci) was administered to a sample of 86 chilean male adolescents who had broken de law. data obtained were analyzed by a combination of an hj-biplot multivariate analysis and an analysis by conglomerates. these analyses yielded five groupings of personality profiles: (a) transgressor-delictual type "t-d", (b) the opossitionistic-selfdestructive type "o-a", (c) the inhibited-avoiding type "i-e", (d) the anxious-dependent type "d-a", and (e) the subclinical type "sub". these results are discussed in terms of the tentative importance that personality profiles may have as predictors of a greater probability of recursiveness and harmfulness of crimes committed by adolescents, as well as in terms of the relevance gained by personality assessment in adolescents, particularly in the context of judicial work.

Abstract: El presente estudio tuvo por objetivo identificar características de personalidad en adolescentes reincidentes en conductas antisociales. Con ese propósito se administró el Inventario Clínico para Adolescentes de Millon MACI (Millon, 1993) a una muestra de 86 adolescentes varones chilenos infractores de Ley. Los datos obtenidos fueron analizados mediante la combinación de un análisis de conglomerados y un análisis multivariado HJ-Biplot, resultando en la detección de las siguientes cinco agrupaciones de perfiles de personalidad: a) el tipo Transgresor Delictual "T-D", b) el tipo Oposicionista-Autodestructivo "O-A", c) el tipo Inhibido-Evitativo "I-E", d) el tipo Dependiente-Ansioso "D-A" y e) el tipo Subclínico "Sub". Estos resultados se discuten en términos de la importancia que pueden tener la evaluación psicológica de adolescentes en contextos judiciales y los perfiles de personalidad en interacción con factores contextuales, los que pueden constituir un factor de riesgo relevante para una mayor reincidencia y peligrosidad en los delitos cometidos por adolescentes. Evaluación diagnóstica de dos proteínas purificadas de Fasciola hepatica mediante ELISA en la fasciolosis ovina

Abstract: dos proteínas antigénicas de fasciola hepatica, una de 14 kda y otra de 29 kda, fueron purificadas por la técnica de electroelución a partir de un antígeno de excreción-secreción (es) de fasciolas adultas. estas proteínas fueron evaluadas por separado, mediante una técnica de elisa utilizando para ello sueros de ovinos con fasciolosis, sueros de ovinos sin fasciolosis y sueros de ovinos con otras parasitosis. la proteína de 14 kda tuvo una sensibilidad baja de un 60% y una especificidad del 100%, mientras que su valor predictivo positivo fue de un 100% y el negativo de 77%. los valores de sensibilidad y especificidad obtenidos para la proteína de 29 kda fueron de un 94% y 98% respectivamente, mientras que el valor predictivo positivo fue de un 97% y el negativo de un 96%. finalmente, se pudo concluir que la proteína de 14 kda no mostró ser útil en el inmunodiagnóstico de esta enfermedad mediante elisa. en cambio la proteína de 29 kda, debido a sus altos valores de sensibilidad y especificidad, puede ser utilizada en el inmunodiagnóstico de la fasciolosis animal mediante la técnica de elisa.

Abstract: Dos proteínas antigénicas de Fasciola hepatica, una de 14 kDa y otra de 29 kDa, fueron purificadas por la técnica de electroelución a partir de un antígeno de excreción-secreción (ES) de fasciolas adultas. Validez predictiva del instrumento de evaluación de riesgos y recursos para la intervención FER-R, en adolescentes chilenos infractores de ley / Predictive Validity at the Instrument Risk and Resources (FER-R) for Intervention Assessment with Young Offenders: Preliminary Study

Abstract: Se estudió la validez predictiva del instrumento Ficha de Evaluación de Riesgos y Recursos (FER-R) para adolescentes infractores de ley, en un lapso de dos a os (2009-2011). La FER-R consta de 60 ítems y permite el registro de 10 dominios, riesgos criminogénicos, recursos y el registro de un índice de escalada delictiva. La muestra estuvo conformada por 101 adolescentes varones, entre 14 y 19 a os de edad, judicializados y deriva- dos a programas de intervención en medio libre. Los principales resultados muestran que el índice general de riesgo medido por la FER-R, presenta un 68.3 % de aciertos, con un área bajo la curva ROC = 0.73, indicando una magnitud predictiva fuerte de la escala total para predecir reincidencia en los adolescentes infractores chilenos.
Desempe o en lectura inicial de escolares con TEL que presentan problemas fonológicos o morfosintácticos

Abstract: The objective of this study is to give more information about the impact of primary school student's phonological problems and morphosyntactic problems in initial reading with Specific Language Impairment (SLI). We used a study group of 18 school students with SLI. and a control group of 30 individuals with typical language development. Among the children with SLI, twelve presented phonological problems and the other six had morpho-syntactical problems. All were evaluated in phonology, morphosyntax and initial reading. The results indicate that children with SLI have a significantly lower performance in initial reading when compared with the control group. Moreover, it was noted that the initial reading problems and word decoding problems were mainly in school students with morphosyntactic problems.
Sensitivity of Venturia inaequalis Chilean Isolates to Difenoconazole, Fenarimol, Mancozeb, and Pyrimethanil

Abstract: apple scab (venturia inaequalis) is the most important disease in chilean apple (malus domestica borkh.) orchards where fungicide management is the main tool to control disease. sensitivity tests to difenoconazole, fenarimol, mancozeb, and pyrimethanil fungicides were conducted on v. inaequalis wild isolates to construct local baseline sensitivity distributions, and compare them with a collection of isolates from seven different commercial orchards with no evidence of practical resistance. in vitro assays were conducted and sensitivity was measured as inhibition of mycelial growth (difenoconazole, fenarimol, pyrimethanil) or conidial germination (mancozeb). departure from the baseline distribution to less sensitivity was found for difenoconazole, fenarimol, and mancozeb with resistance factors of 4.7, 5.8, and 2.1, respectively. the highest difference between baseline and orchard populations was observed for fenarimol which showed the highest shift to less sensitivity. discriminatory doses of 0.04, 1.0, 0.6, and 0.2 μg ml-1 are proposed for in vitro monitoring of sensitivity to difenoconazole, fenarimol, mancozeb, and pyrimethanil, respectively.

Abstract: this study goes in search of the protective factors of stress in the work environment that protect and benefit health in stressful situations. our objective was to know the stress protector factors that are present in the work environment, as perceived by the health staff who work in public and private hospitals in the araucanía region, chile. it is a qualitative study based on glasser y strauss theory, which included in depth interviews to eight important informers: heads of medical centers, nurses, aids and physicians who work in critical and non critical areas, public and private, medium and high complexity in the araucanía region. they were recorded under permission and transcribed to summarize what they perceived. after this process the interviews were put on a software for textual analysis, assigning codes for specific themes and then we wrote down comments. code categories were built on and categories were compared and relations were established between the commentaries and categories in order to analyze them together. we founded five domains of stress" protector factors in the organization. they were: organizational structure, clear definition of role, leadership, physical environment, and teamwork. the results pretend to be the first step to reveal the positive aspects of useful work to objectively orientate the future strategies to promote a healthy working environment for the employees.
