Abstract: Proteases are present in all living forms as they are involved in various metabolic processes. They are mainly involved in hydrolysis of the peptide bonds (Gupta et al. 2002). Proteases are classified into six types based on the functional groups in their active sites. They are aspartic, cysteine, glutamic, metallo, serine, and threonine proteases. They are also classified as exo-peptidases and endo-peptidases, based on the position of the peptide bond cleavage. Proteases find a wide range of applications in food, pharmaceutical, leather and textile, detergent, diagnostics industries and also in waste management (Rao et al. 1998). Thus, they contribute to almost 40% of enzyme sales in the industrial market. Though proteases are found in plants and animals, microbial proteases account for two-third of share in the commercially available proteases (Kumar and Takagi 1999).Proteases are also classified as acidic, neutral or alkaline proteases based on their pH optima. The largest share of the enzyme market is occupied by detergent proteases, which are mostly alkaline serine protease and active at neutral to alkaline pH range. Alkaline serine proteases have Aspartate (D) and Histidine (H) residues along with Serine (S) in their active site forming a catalytic triad (Gupta et al. 2002). Serine proteases contribute to one third of the share in the enzyme market and are readily inactivated by Phenyl Methane Sulfonyl Fluoride (PMSF) (Page and Di Cera 2008). Based on the sequence and structural similarities, all the known proteases are classified into clans and families and are available in the MEROPS database (Rawlings and Barrett 1993).Several microbial proteases from the culturable organisms have been characterized. Functional outcome of corpectomy in cervical spondylotic myelopathy

Abstract: Background : Cervical spondylotic myelopathy (CSM) is serious consequence of cervical intervertebral disk degeneration. Morbidity ranges from chronic neck pain, radicular pain, headache, myelopathy leading to weakness, and impaired fine motor coordination to quadriparesis and/or sphincter dysfunction. Surgical treatment remains the mainstay of treatment once myelopathy develops. Compared to more conventional surgical techniques for spinal cord decompression, such as anterior cervical discectomy and fusion, laminectomy, and laminoplasty, patients treated with corpectomy have better neurological recovery, less axial neck pain, and lower incidences of postoperative loss of sagittal plane alignment. The objective of this study was to analyze the outcome of corpectomy in cervical spondylotic myelopathy, to assess their improvement of symptoms, and to highlight complications of the procedure. Materials and Methods: Twenty-four patients underwent cervical corpectomy for cervical spondylotic myelopathy during June 1999 to July 2005.The anterior approach was used. Each patient was graded according to the Nuricks Grade (1972) and the modified Japanese Orthopaedic Association (mJOA) Scale (1991), and the recovery rate was calculated. Results: Preoperative patients had a mean Nurick′s grade of 3.83, which was 1.67 postoperatively. Preoperative patients had a mean mJOA score of 9.67, whereas postoperatively it was 14.50. The mean recovery rate of patients postoperatively was 62.35% at a mean follow-up of 1 year (range, 8 months to 5 years).The complications included one case (4.17%) of radiculopathy, two cases (8.33%) of graft displacement, and two cases (8.33%) of screw back out/failure. Conclusions: Cervical corpectomy is a reliable and rewarding procedure for CSM, with functional improvement in most patients.

Abstract: This study aims to assess the impact of religiosity on female citizens based on the Theory of Planned Behavior (TPB) that finds the factors influencing the behavioral intention of them toward bike-sharing in Comilla, a city corporation in Bangladesh. A survey of 277 female citizens was conducted in a city corporation area of Bangladesh for data and to test the proposed model. Structural equation modeling (SEM) was used to analyze this study. The findings pointed out that attitude, subjective norm, and perceived behavioral control have significant positive impact on women’s intention toward shared fleet of bike while religiosity influences female users’ intention negatively. To the best of our knowledge, it is the first study which evaluated the effects of religiosity on female users’ intention toward bike-sharing.
Pre-operative echocardiogram in hip fracture patients with cardiac murmur- an audit

Abstract: All hip fracture patients (N = 349) between 01/06/08 and 01/06/09 were included in the study. 29 patients had pre-operative echocardiogram (echo group). A computer generated randomised sample of 40 patients was generated from N, 'non-echo' group. Data was obtained from medical records and the Hospital Information Support System (HISS). The groups were compared using Student's t test. Approval was obtained locally from the clinical governance department for this project.Age and gender distribution were similar in both groups. Indication for echo was an acute cardiac abnormality in 4 cases. 25 patients had echo for no new cardiac problem (indication being cardiac murmur in 23 patients and extensive cardiac history in 2 cases). Cardiology opinion was sought in 5 cases. No patient required cardiac surgery or balloon angioplasty preoperatively. Patients having pre-operative echo had significant delay to surgery (average 2.7 days, range 0-6 days) compared to 'non-echo' group (average 1.1 days, range 0-3 days), (p < 0.001). There was no significant difference in length of stay (p = 0.14) and mortality at 30 days (p = 0.41) between the groups.We have developed departmental guidelines for expediting echo requests in hip fracture patients with cardiac murmur. A liaison has been established with our cardiology department to prioritise such patients on the Echocardiography waiting list, to prevent unnecessary avoidable delay. Careful patient selection for pre-operative echocardiography is important to avoid unnecessary delay to surgery.The incidence of hip fractures in the elderly population is on the rise. Abstract: We reviewed forty patients with forty-two extremities having ipsilateral fracture of femur and tibia atChristian Medical College Ludhiana. Patients were grouped according to type of injury and the method oftreatment Three percent femoral and ten percent tibial fractures developed delayed union out of whichbone grafting was done in two femoral and seven tibial fractures. Three(7.2%) tibial fractures developedinfected gap non union which were subsequently managed by distraction osteoneogenisis. Eight (29.1%)patients had stiffness of knee with range of movement less than 90 degree. Bad results were found inpatients having compound injuries and in juxta articular involvement of both fractures. In our studyexternal fixator was the mainstay of treatment in operative group comprising of ten(23%) femoral and twentythree(55%) tibial fractures because of compound nature of injury and many limbs were salvaged whichotherwise could have amputated. Patients in whom rigid internal fixation was used to treat both fracturesproduced excellent results.

Abstract: In 2000, acquired immunodeficiency syndrome (AIDS) overtook tuberculosis (TB) as the world's leading infectious cause of adult deaths. In affluent countries, however, AIDS mortality has dropped sharply, largely because of the use of highly active antiretroviral therapy (HAART). Antiretroviral agents are not yet considered essential medications by international public health experts and are not widely used in the poor countries where human immunodeficiency virus (HIV) takes its greatest toll. Arguments against the use of HAART have mainly been based on the high cost of medications and the lack of the infrastructure necessary for using them wisely. We re- examine these arguments in the setting of rising AIDS mortality in developing countries and falling drug prices, and describe a small community-based treatment programme based on lessons gained in TB control. With the collaboration of Haitian community health workers experienced in the delivery of home-based and directly observed treatment for TB, an AIDS-prevention project was expanded to deliver HAART to a subset of HIV patients deemed most likely to benefit. The inclusion criteria and preliminary results are presented. We conclude that directly observed therapy (DOT) with HAART, "DOT-HAART", can be delivered effectively in poor settings if there is an uninterrupted supply of high-quality drugs.
On the screening of static electromagnetic fields in hot QED plasmas

Abstract: We study the screening of static magnetic and electric fields in massless quantum electrodynamics (QED) and massless scalar electrodynamics (SQED) at temperature $T$. Various exact relations for the static polarisation tensor are first reviewed and then verified perturbatively to fifth order (in the coupling) in QED and fourth order in SQED, using different resummation techniques. The magnetic and electric screening masses squared, as defined through the pole of the static propagators, are also calculated to fifth order in QED and fourth order in SQED, and their gauge-independence and renormalisation-group invariance is checked. Finally, we provide arguments for the vanishing of the magnetic mass to all orders in perturbation theory.

Abstract: A sufficiently large star will collapse to form a Black Hole Singularity due to Gravitational Pressure beyond Neutron Degeneracy. A Black Hole exhibits extremely strong Gravitational attraction that no particle or electromagnetic radiation can escape from it. The boundary of the region from which no escape is possible is called Event Horizon. In this work it is proposed that there exists a Neutron star smaller than Event Horizon, which is termed as Black Neutron Star. Furthermore an alternative method is proposed to ascertain the maximum permissible mass limit of the Neutron Star and the minimum mass limit of the naturally occurring gravitationally collapsed Black hole.
