Antibiogram for Haemodialysis Catheter-Related Bloodstream Infections

Abstract: Background. Haemodialysis (HD) catheter-related bloodstream infections (CRBSIs) are a major complication of long-term catheter use in HD. This study identified the epidemiology of HD CRBSIs and to aid in the choice of empiric antibiotics therapy given to patients with HD CRBSIs. Methods. Patients with HD CRBSIs were identified. Their blood cultures were performed according to standard sterile technique. Specimens were sent to the microbiology lab for culture and sensitivity testing. Results were tabulated in antibiograms. Results. 18 patients with a median age of 61.0 years (IQR: 51.5–73.25) were confirmed to have HD CRBSIs based on our study criteria. Eight (44.4%) patients had gram-negative infections, 7 (38.9%) patients gram-positive infections, and 3 (16.7%) patients had polymicrobial infections. We noted that most of the gram-negative bacteria were sensitive to ceftazidime. Unfortunately, cloxacillin resistance was high among gram-positive organisms. Coagulase-negative Staphylococcus and Bacillus sp. were the most common gram-positive organisms and they were sensitive to vancomycin. Conclusion. Our study revealed the increased incidence of gram-negative organism in HD CRBSIs. Antibiogram is an important tool in deciding empirical antibiotics for HD CRBSIs. Tailoring your antibiotics accordingly to the antibiogram can increase the chance of successful treatment and prevent the emergence of bacterial resistance. 1. Introduction Chronic kidney disease (CKD) is a major public health burden [1]. The contribution of CKD to the global burden of disease may be underestimated due to the lack of significant importance in certain kidney disease classifications and failure to realize the relationship of CKD with cardiovascular disease [1]. The prevalence of end-stage renal disease (ESRD) is increasing exponentially worldwide. USA, Japan, and Taiwan had the highest rate of prevalence of ESRD [2]. In Malaysia, dialysis is the main modality of renal replacement therapy. There were about 26,000 patients on dialysis in 2011 with the prevalence of 900 per million populations [3]. Haemodialysis (HD) accounts for about 89% of dialysis patients and most of them were accepted to centre HD [4]. Unfortunately, not all patients enter HD program with a native vascular access. Many patients still presented late and HD access catheter, either cuffed or noncuffed catheters, is needed to perform HD. The use of HD catheter had increased from 3% in 2002 to 8.1% in 2011 of all vascular accesses [5]. We cannot argue that HD catheter plays a very important role in the treatment of

Abstract: US Presidential Election 2012 has been a very tight race between the two key candidates. There were intense battle between the two key candidates. The election reflects the sentiment of the electorate towards the achievements of the incumbent President Obama. The campaign lasted several months and the effects can be felt in the internet and twitter. The presidential debates injected new vigor in the challenger's campaign and successfully captured the electorate of several states posing a threat to the incumbent's position. Much of the sentiment in the election has been captured in the online discussions. In this paper, we will be using the original model described in Choy et. al. (2011) using twitter data to forecast the next US president.
Control and Treatment Profiles of 70,889 Adult Type 2 Diabetes Mellitus Patients in Malaysia - A Cross Sectional Survey in 2009

Abstract: Background: Understanding the current diabetes care in Malaysia is the first step towards successful control. Many local studies have contributed toward knowledge of diabetes control and treatment profile but none had reported from such a large cohort of patients as in this registry.Aim & Objectives: To describe glycaemic control, risk factors, and treatment profile for quality assessment of diabetes care in Malaysia.Methods: This is a descriptive study based on secondary data from the online Adult Diabetes Control and Management (ADCM) looking into the control and treatment profiles of type 2 diabetes mellitus patients aged 18 years old and above from 1st January until 31st December 2009. Clinical characteristics included were age, sex, diabetes duration and treatment, glycaemic control, blood pressure, body mass index, and treatment for co-morbidity. Results: A total of 303 centres contributed a total of 70889 patients. Fifty-nine percent was female. The study population included 61.9% Malay, 19% Chinese and 18% Indian. The mean age at diagnosis was 52.3 years old (SD 11.10) and the mean duration of diabetes was 5.9 years (SD 5.56). The mean of HbA1c was 8.3% (SD 2.10); only 18.1% attained HbA1c < 6.5%. There were 61.8% diabetic hypertensive patients based on blood pressure measurements and about one third were treated to target BP ≤ 130/80 mmHg. The mean of low density lipoprotein-cholesterol was 3.2 mmol/L (SD 1.10); one-third achieved the target of ≤ 2.6 mmol/L. Eighteen percent was on more than two anti-hypertensive agents and 41.6% were on anti-lipid agents. Metformin (83.2%), angiotensin-converting enzyme inhibitors (63.9%) and statin (89.8%) were the mostly prescribed anti-diabetic, anti-hypertensive and anti-lipid agents respectively.Conclusion: Diabetes and its co-morbidities (hypertension and hyperlipidaemia) were less satisfactorily controlled. The choice of drugs was appropriate but probably inadequate. More effort and resources are needed to improve diabetes care in this country especially in the primary healthcare.
The Effectiveness of Acupuncture in Prevention and Treatment of Postoperative Nausea and Vomiting - A Systematic Review and Meta-Analysis

Abstract: Background Acupuncture therapy for preventive and treatment of postoperative nausea and vomiting(PONV), a condition which commonly present after anaesthesia and surgery is a subject of growing interest. Objective This paper included a systematic review and meta-analysis on the effect of different type of acupuncture and acupoint selection in PONV prevention and treatment. Methods Randomised controlled trials(RCTs) comparing acupuncture with non-acupuncture treatment were identified from databases PubMed, Cochrane, EBSCO, Ovid, CNKI and Wanfangdata. Meta-analysis on eligible studies was performed using fixed-effects model with RevMan 5.2. Results were expressed as RR for dichotomous data, with 95%CI. Results Thirty RCTs, 1276 patients (intervention) and 1258 patients (control) were identified. Meta-analysis showed that PC6 acupuncture significantly reduced the number of cases of early vomiting (postoperative 0-6h) (RR=0.36, 95%CI 0.19,0.71; P=0.003) and nausea (postoperative 0-24h) (RR=0.25, 95%CI 0.10,0.61; P=0.002), but not early nausea (postoperative 0-6h) (RR=0.64, 95%CI 0.34,1.19; P=0.150) and vomiting (postoperative 0-24h) (RR=0.82, 95%CI 0.48,1.38; P=0.450). PC6 acupressure significantly reduced the number of cases of nausea (RR=0.71, 95%CI 0.57,0.87; P=0.001) and vomiting (RR=0.62, 95%CI 0.49,0.80; P=0.000) at postoperative 0-24h. PC6 electro-acupoint stimulation significantly reduced the number of cases of nausea (RR=0.49, 95%CI 0.38,0.63; P<0.000) and vomiting (RR=0.50, 95%CI 0.36,0.70; P<0.000) at postoperative 0-24h. Stimulation of PC6 with other acupoint(s) significantly reduced the number of cases of nausea and vomiting (RR=0.29, 95%CI 0.17,0.49; P<0.000) at postoperative 0-24h. Stimulation of other acupoint(s)(non PC6) also significantly reduced the number of cases of nausea and vomiting (RR=0.63, 95%CI 0.49,0.81; P=0.000) at postoperative 0-24h. However, the quality of study was generally low in studies of PC6 combined with other acupoint(s) and other acupoint(s). Details of blinding were not reported in most reports. Conclusions Besides PC6, PC6 combined with other acupoint(s) and other alternative acupoint(s) might be beneficial in prevention and treatment of PONV, the evidence justifies future high-quality studies.
A sentiment analysis of Singapore Presidential Election 2011 using Twitter data with census correction

Abstract: Sentiment analysis is a new area in text analytics where it focuses on the analysis and understanding of the emotions from the text patterns. This new form of analysis has been widely adopted in customer relation management especially in the context of complaint management. With increasing level of interest in this technology, more and more companies are adopting it and using it to champion their marketing efforts. However, sentiment analysis using twitter has remained extremely difficult to manage due to the sampling bias. In this paper, we will discuss about the application of using reweighting techniques in conjunction with online sentiment divisions to predict the vote percentage that individual candidate will receive. There will be in depth discussion about the various aspects using sentiment analysis to predict outcomes as well as the potential pitfalls in the estimation due to the anonymous nature of the internet.
Revisiting the Cellulosimicrobium cellulans yeast-lytic β-1,3-glucanases toolbox: A review

Abstract: Several bacteria have been reported to be able to lyse and grow on viable yeast and fungal cells by producing a variety of cell-wall degrading enzymes such as endo-β-1,3-glucanases, proteases, β-1,6-glucanases, mannanases, and chitinases. The structural complexity of the yeast cell wall, which is mainly composed of complex polymers of β-1,3- and β-1,6-glucans, mannoproteins, and smaller amounts of chitin [1,2], implies that the synergistic action of these enzymes is necessary to hydrolyse its components into assimilable substrates. Nevertheless, endo-β-1,3-glucanases [EC 3.2.1.39 and EC 3.2.1.6] have been considered to play a major role in yeast cell lysis [2,3].The actinomycete Cellulosimicrobium cellulans (also known with the synonyms Cellulomonas cellulans, Oerskovia xanthineolytica, and Arthrobacter luteus), has been regarded as a major source of yeast-lytic enzymes, particularly endo-β-1,3-glucanases, proteases and mannanases. Notably, several commercially available yeast-lytic glucanases preparations derived from this organism, namely Lyticase, Zymolyase, and Quantazyme, have been widely used for yeast protoplast preparation and yeast DNA isolation. Endo-β-1,3-glucanases are the major component of such enzyme preparations. Only one of these commercially available preparations (Quantazyme, Quantum Biotechnology, Canada) is produced recombinantly and protease-free. Abstract: Systems biology is increasingly generating a quantitative knowledge base of cell physiology, offering, for the first time, insights into molecular/cellular processes and function at a cell-wide scale. In order for metabolic/cellular engineers to embrace the potential that systems biology offers, an understanding of a variety of analytical and mathematical/computational tools is required [9-12]. However, whilst our knowledge on the systems components (genes, proteins, metabolites) has increased significantly, data integration in computer models with appropriate mechanistic and molecular detail to enable in silico experiments of sufficient predictive capability is still limiting the increase in the success rate of microbial cell factories engineering strategies.Microbial Cell Factories is already contributing to individual systems biology-driven technological and methodological advances, experimental (at the transcriptomic [13-15], proteomic [16] and metabolomic/fluoxomic levels [17-20]) or computational [21,22]. Whilst strengthening this trend, we would also like to expand our field of interest to the integration of experimental data with computational and theoretical methods by encouraging the publication of research and review articles covering core aspects of the application of systems biology to the engineering of microbial cell factories (i.e. metabolic or cellular engineering). Moreover, the growing number of host cell systems being explored as factories, the continuous improvement of genetic tools, progress in de novo synthesis of increasingly complex biological entities (synthetic biology [23]) and, the growing diversity of bioproducts, is creating an emerging interest to extend our knowledge base to cell factories other than the classic model organisms. Abstract: Love for an unknown person in a distant location has featured spectacularly throughout the history of literature. One of the emblematic authors who dealt with falling in love in such a blind fashion was the troubadour Jaufré Rudel, to the extent that he may be considered to be a literary paradigm. When analysing his work and its repercussions, it is impossible to escape a comparison with the type of amorous relationships that can be established via chat, the common denominator of which is that the interlocutor is unknown. In addition to the mystery, related to a high component of idealisation, we believe that there are other comparative links from a metaphorical perspective, such as distance, the identity of the participants and the specific nature of the code used. In this article, we will set out some ideas with a view to stimulating further analyses in that respect.
Global Businesses ‘from Below’: Ethnic Entrepreneurs in Metropolitan Areas

Abstract: The urban migration in the North American cities and many European cities displays a great diversity of urban ethnic landscapes. These territories do not just show to the concentration of resident foreign-born population in certain areas inside the cities or in the suburbs but also the relevance of the economic activities developed by the immigrants. The role of migrant entrepreneurs has been critical in the revitalization of certain derelict urban cores and peripheries which have been transformed sometimes into vibrant ethnic business spaces. Some of these spaces show the importance of concentration of ethnic businesses and coethnic residents; some others present different geographic patterns, such as concentration or dispersion of ethnic businesses among a non-coethnic population; other times it is the concentration of specialized businesses. The combination of the different factors: concentration vs dispersion, diversity vs specialization and the majority vs minority relationship between ethnic businesses and the residents (monoethnicity vs multiethnicity) creates a set of different types of ethnic business spaces. The paper will explain theses types of ethnic business spaces with examples, such as Petite Asie in Paris, or multiethnic Ravalistan in Barcelona’s old town, and the geographical factors that are shaping them. The methodology is based upon a review of contemporary literature and other research findings on ethnic landscapes mainly in metropolitan North America but also in European cities.
