Papel del adipocito en la inflamación del síndrome metabólico

Abstract: the adipocyte, traditionally considered as a cell with little metabolic activity, is now recognized as an important player in metabolic, hormonal and inflammatory processes. its ability to secrete pro-inflammatory cytokines and to produce peptides in charge of regulating body weight and energy expenditure (adipocytokines) as well as vasoactive substances such as angiotensin ii (atii) is well known. the inflammatory markers produced by the adipocytes include c-reactive protein (crp), interleukin 6 (il6) and tumor necrosis factor alpha (tnfa). these substances participate in the pathophysiology of the metabolic syndrome, highly prevalent both in developed as well as in developing countries, and are the main focus of this review.

Abstract: Tradicionalmente considerada una célula de poca actividad metabólica, el adipocito es reconocido como un importante actor de procesos metabólicos, hormonales e inflamatorios. Al momento es bien conocida su capacidad secretora de citoquinas pro-inflamatorias, secreción de péptidos reguladores del peso corporal y del gasto energético (adipocitoquinas), y de sustancias vasoactivas como la angiotensina II (ATII). Los marcadores inflamatorios generados por el adipocito son la proteína C reactiva (PCR), la interleuquina 6 (IL6) y el factor de necrosis tumoral alfa (TNFa). Estas sustancias participan en la fisiopatología del síndrome metabólico, una patología de alta prevalencia tanto en países desarrollados como en vía de desarrollo y son objeto de revisión en este artículo. Abstract: Durante los últimos a os, el Instituto de Investigaciones de la Fundación Cardiovascular de Colombia ha centrado sus proyectos en el estudio de las diferencias en los mecanismos etiofisiopatológicos de la hipertensión inducida por el embarazo y del síndrome metabólico en poblaciones de países desarrollados y en vía de desarrollo, así como en el peso específico de los factores de riesgo que determinan la presentación de estas enfermedades. Los resultados obtenidos de las investigaciones realizadas en la población, sugieren que los cambios de hábitos de vida ocasionados por la sociedad consumista, son el principal determinante del riesgo aumentado de preeclampsia y enfermedades cardiovasculares que al momento presenta la población colombiana. The Research Institute of the Colombian Cardiovascular Foundation has centered its projects during the last years in the study of the differences in the etio-physiopathologic mechanisms of pregnancy induced hypertension and in the metabolic syndrome in populations of developed and underdeveloped countries, as well as in the value of the risk factors that determine the appearance of these diseases. The results obtained from the investigations realized in the population suggest that changes in life costumes due to a consumer society are the main determinant of the increased risk of pre-eclampsia and cardiovascular diseases that the Colombian population presents at this moment.
Determinación del índice de resistencia a la insulina mediante homa y su relación con el riesgo de hipertensión inducida por el embarazo

Abstract: objective: to assess whether insulin resistance determined by homeostatic model assessment (homa) is an early predictor of the development of pregnancy induced hypertension in colombian pregnant women. methods: we conducted a nested case control study in a prospective cohort of four hundred and thirty eigth normotensive primigravidae women, with gestational age < 30 weeks. the homa was calculated using fasting plasma concentrations of glucose and insulin, determined by glucose-oxidase and chemoluminiscence methods, respectively. results: twenty-three pregnant women developed pregnancy induced hypertension (5.25%). two normotensive pregnant women were selected as controls for each case, matched by gestational and maternal age at enrollment. the women who subsequently developed pregnancy induced hypertension had higher levels of homa (1.48 ± 0.98 vs 0.96 ± 0.70, p<0.001), which was associated with an increased risk of developing pregnancy induced hypertension (or: 3.8, ic95%: 1.1-12.8 p=0.01). systolic blood pressure levels at enrollment were significantly higher in pregnant women who later developed pregnancy induced hypertension (106 ± 12 vs 97 ± 10, p <0.01), showing an interaction with the levels of homa in the multiple logistic regression model. conclusions: women who subsequently developed pregnancy induced hypertension were more insulin-resistant before the onset of clinical manifestations of the disease. the homa index could be a useful method to screen women at risk of developing pregnancy induced hypertension.

Abstract: A randomized, double blind, cross-over, placebo-controlled, clinical trial was designed to assess the effects of Candesartan (up to 32 mg/day during 6 months) on the Homeostasis Model Assessment (HOMA) index, lipid profile, protrombotic state, oxidative stress and plasma levels of inflammatory markers. The participants will be recruited in the "Fundación Cardiovascular de Colombia". Subjects who fullfil selection criteria will receive permanent educational, nutritional and exercise support during their participation in the study. After a 15 days-run-in period with placebo and life-style recommendations, the patients who have a treatment compliance equal or greater than 80% will be randomlly assigned to one of the treatment groups. Group A will receive Candesartan during 6 months and placebo during 6 months. Group B will receive placebo during the first 6 months, and then, Candesartan during the last 6 months. Control visits will be programed monthly and all parameters of interest will be evaluated every 6 months.Treatment with Candesartan, could improve the HOMA index, the response to the oral glucose tolerance test and reduce the plasma levels of adipoquines, oxidative stress and prothrombotic markers, in non diabetic, non hypertense subjects with dysglycemia and abdominal obesity, recruited from a population at high risk of developing insulin resistance. These effects are independent of the changes in arterial blood pressure. Abstract: this paper presents a general outlook on the mainstream orientations that prevail nowadays in mexico city studies. these orientations fail to recognize the functioning of the city as a totality, operating on the basis of a serious fragmentation of the object under study. the author holds that the distrito federal is only a part of a real city, formed by the metropolitan zone of the valle de méxico. because of this, it is necessary to develop these kinds of studies in a metropolitan scale or, at least, putting these fragmentary studies in a general context. the article also deals with some fields of research that the strategist should take into account for the structural change of the metropoli.
