Can Endarterectomy Be Useful in Peripheral Arterial Occlusive Disease with Critical Limb Ischemia?  [PDF]

Abstract: Importance ofthrombendarterectomy (TEA) had declined with the advent of bypass techniques and availability of prosthetic grafts, in patients with Peripheral arterial occlusive disease (PAOD). Recently, there had been a significant shift towards lower limb revascularization using endoluminal techniques. However, previously available data evaluating the long leg bypass or combined endoluminal and bypass procedures have been too anatomically heterogeneous to be easily applied to patients with infrainguinal disease and tissue loss. Clinical decision making in Complex multilevel or diffuse peripheral arterial occlusive disease with multiple co-morbidities especially associated coronary arterial disease is challenging. We describe twelve patients of iliofemoral arterial occlusive disease with tibiopopliteal arterial occlusive disease along with multiple co-morbidities like coronary arterial disease and diabetes mellitus who showed marked improvement with minimal post-operative?morbidities after iliofemoral or ileopopliteal bypass grafting with endarterectomy of the tibiopopliteal segment and related review of the literature.
Efecto del almacenamiento de las semillas de cafeto (Coffea arabica L. var. Catuai amarrillo) sobre la emergencia

Abstract: in order to determining the effect of storage on coffee seeds the germination, for it stored seeds in sisal and white polyethylene packings during 0, 2, 4, 6, 8 and 10 months in three conditions: condition 1 (20°c and 78% hr); condition 2 (26°c and 65% hr) and condition 3 (10°c and 55 % hr), when finalizing the test could be appreciated as the period of storage had a significant effect on the germination, since as passed the time, a well-known reduction of the percentage of germination in the lots of seeds took place significantly, being in addition affected so much by the conditions of storage like by the type of used packing. nevertheless, during the first 6 months of storage in conditions 1 and 2, the percentage of germination was relatively high (91.1% and 92.2% respectively), in comparison to the condition 3 that presented/displayed 5.8% as of the fourth month of storage. when finalizing the test the percentage of germination were of 58.1%; 31.4% and 2.2% for conditions 1, 2, and 3 respectively. being the condition of 20°c with 78% hr and the most favorable packing of sisal to maintain the physiological quality of the seed by a period of up to ten months.

Abstract: El presente trabajo tuvo como finalidad determinar el efecto del almacenamiento de las semillas de cafeto sobre la emergencia. Para ello se almacenaron semillas en empaques de sisal y polietileno blanco durante periodos de 0, 2, 4, 6, 8 y 10 meses, bajo tres condiciones de temperatura y humedad relativa: condición 1 (20°C y 78% HR); condición 2 (26°C y 65% HR) y la condición 3 (10°C y 55% HR). Se pudo apreciar que a medida que transcurrió el tiempo de almacenamiento se produjo una notoria reducción del porcentaje de plántulas emergidas, resultando además significativamente afectada tanto por las condiciones de almacenamiento como por el tipo de empaque utilizado. Sin embargo, durante los primeros 6 meses de almacenamiento de las semillas en las condiciones 1 y 2 la emergencia fue alta (91,1 y 92,2% respectivamente), en comparación a la condición 3 que presentó 5,8% a partir del cuarto mes de almacenamiento. Al finalizar las evaluaciones (10 meses) los porcentajes de emergencia fueron de 58,1; 31,4 y 2,2% para las condiciones 1, 2 y 3 respectivamente. Siendo la condición 1 (20°C con 78% HR) y el empaque de sisal los más favorables para mantener la calidad fisiológica de las semillas por un período de hasta diez meses. Abstract: Thoracoplasty has been a useful procedure to control and cure recalcitrant lung lesions often as a last measure to treat pulmonary infections like post-tubercular chronic empyema with fibrocavitary lesions and residual pleural space infections after pulmonary resections. Long-standing empyemas with persistent discharge on drainage often respond poorly after decortication and?debridement alone due to non-expanding underlying lung tissue due to fibrotic changes. These cases often develop recurrent empyemas with sepsis with high morbidity and mortality. Similarly chronic space-occupying lung lesions with adjacent pulmonary damage and fibrosis leave a considerable residual space after surgery of these lesions and these persistent residual space leads to pleural space infections and empyema. In the presence of a large apical abscess and destruction of the apical pulmonary tissue forming a potential space confined by the bony thoracic cage and limited inferiorly by the rigid lining of the abscess, the cure and management of the chronic empyema especially the post-tubercular ones are particularly difficult. We address these cases in our series by performing partial thoracoplasty tailored to the site of the lesions in addition to other procedures. Partial thoracoplasty was found beneficial for selective cases of chronic empyema, obliteration of residual cavity after resectional surgery and management of the apical abscess.

Abstract: A 64-year-old woman with IgA kappa multiple myeloma was treated with thalidomide-dexa- methasone. Due to progression of the disease, bortezomib, doxorubicin and dexamethasone were administered, followed by autologous stem cell transplantation. Although near-complete remission was achieved, 5 months later, neuro- logical symptoms appeared and the patient was diagnosed with multiple myeloma with cells in- filtrating the spinal cord. Bis-chloronitrosourea, bortezomib and lenalidomide were then admin- istered and although the patient remained neu- rologically asymptomatic, she died 3 months later becausee of disease progression. Lena- lidomide entered into the cerebrospinal fluid (confirmed by ultrasensitive high-performance liquid chromatography), although did not im- prove the poor prognosis of multiple myeloma involving the central nervous system.
Sindromes de compresión neurovascular descompresión microvascular: Nuestra experiencia (2003-2009)

Abstract: Objective. To present and evaluate the surgical results of microvascular decompression in the treatment of trigeminal neuralgia, glossopharyngeal neuralgia and hemifacial spasm. Material and Method. We present 15 patients with compressive syndrome of cranial nerve during a period of 6 years: 11 patients with trigeminal neuralgia, 2 patients with glossopharyngeal neuralgia and 2 patients with hemifacial spasm. Results. We achieved total improvement in 8 patients and partial in 4. The most frequent symptom after surgery was hypoesthesia in V2 in patients with trigeminal neuralgia. 3 patients presented temporary facial paresis. Conclusion. Microvascular decompression is an effective treatment with low morbidity in patients with compressive syndrome of cranial nerves. The goal of treatment should be complete disappearance of symptoms.
Schwannomas intracraneanos: Nuestra experiencia (2000-2009)

Abstract: Objective. To present and evaluate the surgical results in the treatment of intracranial schwannomas. Method. We present 26 patients with intracranial schwannomas for a period of 10 years treated with surgery, analyze kind of presentation and postoperative results. Results. One of the cases corresponded to schwanoma of III pair, another case to schwanoma of the V pair, two other cases corresponded to schwanoma of the IX pair and twenty two cases corresponded to schwanomas of the VIII pair. Average age was 51,95 a os (19-73). Total resection was made in 25 patients, partial resection was made in other. Facial nerve preservation was made in the remaining 22 patients. Preservation auditory nerve function was made in all the cases. We had 3 cases of cerebrospinal fluid leak.There were no mortal cases associated to the surgical procedure. Conclusion. The goal of treatment of intracranial schwanomas is total resection with anatomical and functional preservation of neurological structures around. This could be accomplished by retrosigmoid approach by experienced neurosurgeons.
Neuralgia del glosofaríngeo presentación de dos casos

Abstract: Objective. To present 2 cases of glossopharyngeal neuralgia treated by microvascular decompression. Description. Case one. 37-year- old male suffering pain in the posterior region of the left half of the tongue for 3 years. The diagnosis was left glossopharyngeal neuralgia. We performed microvascular decompression with Teflon felt. The patient has improved however he is still under medication. Case two. 59-year-old female suffering pain in the posterior third of the left half of the tongue. The diagnosis was left glossopharyngeal neuralgia. We performed microvascular decompression with Teflon. Symptoms completely disappeared. Conclusion. Glossopharyngeal neuralgia is infrequent. It can be treated with microvascular decompression with good results.

Abstract: the correlations among the total length, pré-umbonal length, width and height of the shell of neocorbicula limosa (maton, 1811), based on the examination of 100 shells from the rio guaíba basin are established. the radio index among these measures was calculated and the maximum, intermediary and minimum values are presented. as a result of data analysis it was pointed out that the relationship among the measures are positive and there is no allometry. it is also suggested that the proportion indexes could be used as helpful diagnostic character in conchologic studies.
