Abstract: the way in which psychology has been acquiring recognition, worth and social usefulness from its beginning as university degree, is the starting point of the problems that, after coming into effect the new law " ley de ordenación de profesiones sanitarias" and the decree 1277/003, are suffering the clinical psychologists: limited opportunities for sanitary jobs, secondary posts in the national health service, overcrowded universities and scanty connexion between academic and clinical scope. if we wanted to repair such deficiencies we should focus on a strategy for a middle and long-term, which develops the sanitary aspect of the psychology in the context of the national health service and we should focus on improving the specialization of clinical psychology through specialized training that the government establishes for all sanitary professions.

Abstract: El modo en cómo la Psicología ha ido adquiriendo reconocimiento, valor y utilidad social a partir de su configuración como licenciatura universitaria están en la base de los problemas que tras la reciente entrada en vigor de la Ley de Ordenación de Profesiones Sanitarias y del Real Decreto 1277/2003, están padeciendo los psicólogos clínicos: posibilidades limitadas de trabajo profesional en lo sanitario, posición periférica en el Sistema Nacional de Salud, facultades masificadas y escasa relación entre el ámbito académico y clínico. Si queremos subsanar dichas carencias deberemos apostar por una estrategia que a medio y largo plazo desarrolle lo que de sanitario tiene la Psicología en el contexto del Sistema Nacional de Salud por la potenciación de la especialidad de Psicología Clínica a través de la Formación Especializada que el Estado establece para todas las profesiones sanitarias.

Abstract: this paper presents the development of a climate dynamics model for a greenhouse located on the bogota plateau. a black-box model was estimated from experimental data, considering a novel system structure derived from a first principles model and experimental tests. it considered two control volumes, one given by the air over the crop and a second one formed by the air trapped by crop foliage. the model was selected from a set of linear, discrete-time, statevariable systems using subspace identification methods. the estimated system was able to predict climate dynamics for both control volumes, having errors below 8%. such performance was comparable to previous work reported in literature while the obtained model was a low-complexity linear system

Abstract: En este trabajo se presenta el desarrollo de un modelo dinámico del clima en un invernadero ubicado en la Sabana de Bogotá. Se realiza la estimación de un modelo tipo caja negra obtenido a partir de datos experimentales. Se propone una estructura a partir de principios físicos que divide el sistema en dos volúmenes de control dados por el aire libre dentro del invernadero y el aire atrapado por el follaje. El modelo es seleccionado, dentro de una familia de sistemas línea- les descritos en variables de estado, a través de métodos de identificación por subespacios. El modelo obtenido es capaz de explicar el comportamiento del clima en el aire y en el follaje con errores inferiores al 8%. Este desempe o es comparable al de otros estudios reportados en la literatura, con la ventaja de que el modelo empleado es lineal. Syphilis in HIV-infected patients: predictors for serological failure and serofast state

Abstract: Purpose: HIV-infected patients treated for syphilis may be at increased risk for serological failure and serofast state. Our aim was to analyse serological response to treatment in HIV-infected patients diagnosed with syphilis, and factors associated with serological cure and serofast state. Methods: Open-label, no controlled study of a series of HIV-patients diagnosed with syphilis during 2004–2011. Patients were categorized by rapid plasma reagin titer (RPR) into success (4-fold decrease in RPR by 12 or 24 months after treatment of early or late syphilis), serofast (success with persistently stable reactive RPR), and failure/re-infection (failure to decrease 4-fold in RPR by 12 or 24 months after treatment or sustained 4-fold increase in RPR after treatment response). Results: 141 HIV-patients were diagnosed with syphilis during the study period (104 early syphilis, 36 late or indeterminate latent syphilis). The mean age was 36.3 years, 98.5% were male, and 87.2% homosexual men. In 46 (32.6%) cases, HIV and syphilis infection diagnosis were coincident (mean CD4 457/mm3 and HIV-VL 4.72 log10). Among patients with prior known HIV infection, 65 were on antiretroviral therapy (ART) at syphilis diagnosis (mean CD4 469/mm3, 76.9% undetectable HIV-VL). 116 patients satisfied criteria for serological response analysis (89 early, 24 late/indeterminate). At 12 months of early syphilis treatment (89.2% penicillin) there were 16 (18%) failures, and at 24 months of late/indeterminate syphilis (91.7% penicillin) there were 5 (18.5%) failures. Overall, 36 (31.0%) patients presented serofast state. Treatment failure was related with lower CD4 count (295 vs 510/mL; p=0.045) only in patients with coincident diagnosis. Serofast state was related with older age (41 vs 36 years; p=0.024), and lower CD4 count (391 vs 513/mm3; p=0.026). Conclusions: In this series of HIV-infected patients, with many patients on ART and with good immunological and virological parameters, serological failure and serofast state were frequent. Immunological status, and age could influence on serological response to syphilis treatment in HIV-infected patients.

Abstract: Objective: To examine the association between elevated levels of red cell distribution width (RDW) and cardiovascular risk factors (CVRF) and metabolic syndrome (MS) in HIV-patients. Methods: Cross-sectional study including all asymptomatic HIV-outpatients under follow-up during 2007. Patients completed a questionnaire about CVRF, underwent a physical examination, and an 8-hour fasting blood analysis. Elevated RDW was defined as ≥75th percentile. Patients with and without an elevated RDW were compared. Results: 666 patients (79.3% men) were included: mean age 44.7 years, mean CD4 506/mm3 and 87.5% on antiretroviral therapy (85.3% with undetectable viral load). Mean RDW was 13.7% (range: 7.7% - 33.6%; 75th percentile, 14.1%). The prevalence per quartile of MS was 15.7%, 9.3%, 18.8% and 16.6% and of patients with CVRF > 20% was 8.4%, 4.0%, 4.4%, and 6.4%, respectively (p > 0.05); 23.4% of the patients had an elevated RDW (>14.1%). The top percentile of RDW was associated with AIDS (OR 1.6; 95% CI, 1.0 - 2.4; p = 0.02), detectable viral load (OR 1.5; 95% CI, 1.01 - 2.4; p = 0.04) and hypertension (OR 2.3; 95% CI, 1.4 - 4.0; p = 0.001). Conclusions: In HIV-outpatients, higher RDW is related with detectable viral load and with AIDS. Although it was associated with hypertension, we found no relation with MS nor with higher cardiovascular risk.

Abstract: Purpose: To assess platelet activity in HIV-patients with and without antiretroviral therapy (ART), analysing the influence of the presence or absence of ABC in the regimen. Methods: Observational, pilot study, including 30 asymptomatic HIV-patients: 20 on ART for at least 24 weeks and with undetectable HIV viral load - 10 on ABC, 10 on tenofovir (TDF) - and 10 na?ve patients, and a control group of 10 HIV-negative subjects. No subject was receiving drugs with antiagregant activity. Platelet activity was assessed by measuring time-dependent platelet aggregometry (electrical impedance on fasting whole blood), induced by ADP (1.25, 2.5 μM), collagen (0.5, 1 μg/mL), arachidonic acid (100, 200 μM), and U46619 (receptor agonist of the tromboxano A2) (1.25, 2.5 μM). Statistic program: SPSS, 16.0. Results: Demographic, anthropometric data, and cardiovascular risk factors were similar in all groups, but older age and longer time of HIV infection in the ABC group (50.4 vs 36.1, 34.2 and 42.7 years, respectively; p < 0.05, and 140.3 vs 88.1 and 48.3 months in the two other groups of HIV patients; p < 0.05). Mean CD4 cells count was 564/mm3. Platelet aggregation with exposure to U46619 was higher in the ABC compared with the TDF group (11.1 vs 4.4%; p = 0.007), na?ve patients (11.1 vs 5.7%; p = 0.014), and the HIV-negative group (11.1 vs 6.5%; p = 0.04). These differences remained significant when controlled for age and time of HIV infection. Conclusions: ABC increases platelet aggregability possibly in relation with the receptor of tromboxano. Wider studies are needed to confirm this hypothesis.
Guidelines for the microbiological quality of treated wastewater used in agriculture: recommendations for revising WHO guidelines

Abstract: three different approaches for establishing guidelines for the microbiological quality of treated wastewater that is reused for agriculture are reviewed. these approaches have different objectives as their outcomes: the absence of faecal indicator organisms in the wastewater, the absence of a measurable excess of cases of enteric disease in the exposed population and a model-generated estimated risk below a defined acceptable risk. if the second approach (using empirical epidemiological studies supplemented by microbiological studies of the transmission of pathogens) is used in conjunction with the third approach (using a model-based quantitative risk assessment for selected pathogens) a powerful tool is produced that aids the development of regulations. this combined approach is more cost-effective than the first approach and adequately protects public health. the guideline limit for faecal coliform bacteria in unrestricted irrigation ( < 1000 faecal coliform bacteria/100 ml) is valid, but for restricted irrigation < 105 faecal coliform bacteria/100 ml is recommended when adult farmworkers are exposed to spray irrigation. a limit of < 103 faecal coliform bacteria/100 ml is recommended if flood irrigation is used or children are exposed. the guideline limit for nematode eggs for both types of irrigation is adequate except when conditions favour the survival of nematode eggs and where children are exposed; in these cases it should be reduced from < 1 egg/l to < 0.1 egg/l.

Abstract: objective: to evaluate the flexible program implemented for the medical internship at school of medicine, universidad del rosario during the period 1997-2002. methodology: a descriptive study was performed to summarize the choices of medical clerkships made by the interns during the whole studied period. the coincidence with the further choice of a determined medical specialty was assessed. conclusions: most of the last year's students remain preferring a conservative approach to their career, by choosing clerkships in a basic area, such as internal medicine, pediatrics, gynecology and obstetrics or general surgery. the coincidence between the type of internship or clerkships a student performs and the future election of a specialty is high.
Inflammation and perforation of a solitary diverticulum of the cecum: A report of 5 cases and literature review

Abstract: solitary diverticulum of the cecum is a benign condition uncommon in the western world, and with a higher incidence in asian population. they are usually asymptomatic, and manifest clinically only with complications such as inflammation, perforation, or bleeding. they are a rare cause of acute abdominal pain, clinically similar to acute appendicitis, with tenderness in the right lower quadrant, fever, and leukocytosis. in spite of the information provided by ultrasonography or ct scans, a correct preoperative diagnosis is still difficult to reach, and is usually arrived at in the operating theater; differentiation from a neoplasm may be also sometimes complicated, and a wide surgical resection is usually required for such cases. we report on 5 cases of inflammation and perforation of a solitary cecal diverticulum, and perform a literature review.
