Dada a agressividade do melanoma, a sobrevida depende do diagnóstico precoce. A biópsia do linfonodo sentinela tem contribuído na decis o de se realizar ou n o a linfadenectomia completa, e alguns autores já est o utilizando a mesma em crian as.

Abstract: objective: to present a case of a child who was subjected to sentinel lymph node biopsy for cutaneous melanoma. description: a 12 year-old child with dysplastic nevus syndrome developed melanoma on the lumbar region. the excision biopsy revealed a melanoma with depth of 1.5 mm. the patient was submitted to amplification of the margins 2 cm in all directions and the sentinel node was also excised. the histopathological exam did not show residual disease. sentinel on exam did not show metastases either under hematoxylin-eosin stain or immunohistochemistry (s-100 and hmb45). therefore, rt-pcr for tyrosinase mrna was positive. the patient has been followed for twelve months without evidence of recurrence. comments: childhood melanoma is rare, corresponding to less than 1% of malignant tumors in children. data point to a worldwide increase in its incidence. melanoma occurs in melanocytic lesions in 70% and in the remaining 30% it occurs de novo. melanoma is very aggressive, so the survival depends on an early diagnosis. sentinel lymph node biopsy has selected patients to complete lymphadenectomy. some authors have been using this technique in childhood melanoma.
Feelings and Senses Given to the Music Present at the Hospital during Hemodynamic Procedures: Cardiac Catheterization and Coronary Angioplasty  [PDF]

Open Journal of Medical Psychology (OJMP) , 2017, DOI: 10.4236/ojmp.2017.61003
Abstract: Several studies indicate that music has soothing effects and is beneficial in reducing stress and anxiety in coronary patients. It is known that reactions to music are considered to be subjective, however, results of previous studies suggest that variables such as changes in heart rate, in cerebral blood flow and heart rate variability increase with a faster musical tempo, regardless of individual preferences. The effects of stress on the cardiovascular system have also been proven. However, the meanings assigned to music when used during hemodynamic procedures are unknown, as are the meanings of the experience of these procedures. The aim of this research is to understand the senses and feelings of music for patients undergoing hemodynamic procedures, identify and interpret the related fantasies and emotions and study the possibility of deploying in hospitals the “Musical Method for Hemodynamic Procedures”, being developed by the author. This research is based on a Clinical-Qualitative Methodology. The sampling method is the theoretical saturation. The semi structured interview was used in order to obtain data that were submitted to content analysis. The subjects are patients undergoing hemodynamic procedures in the Hospital SEMPER (Permanent Medical Service), Belo Horizonte, Brazil. We conclude that within the experience of listening to music while undergoing catheterization, 100% of the patients interviewed claimed they had overcome the experience of stress and felt calm, tranquility, peace and happiness. Some patients described the music as a companion, as something that diverts their attention from fear, transporting them to an imaginary place, to another dimension. The episodic memory, the capacity to recognize a musical excerpt for which the spatiotemporal context surrounding its former encounter could be recalled, was also important, with surprising results, in the case of patients who underwent catheterization in the presence of music and, later, angioplasty without the presence of music.

Abstract: four families of carbon fiber reinforced composites (cfrc) used in aeronautical industry were evaluated by flexural and interlaminar shear tests. it is also characterized three families of non-conditioned and conditioned cfrc by compression test. the composites were obtained by hand lay-up process in autoclave by using prepregs based on epoxy matrices (f155 and f584) and carbon fiber fabric reinforcements (pw-"plain weave" and 8hs-"eight harness satin"). the f155-epoxy matrix was cured at 121 °c and the f584-epoxy type at 177 °c. after molding, the laminates were cut in specimens attending the astm d790 for the flexural test, the astm d2344 for the interlaminar shear test (ilss) and the astm d3410 for the compressive test. the compressive tests were performed for testing the specimens before and after hygrothermal conditioning. the results show that the f584-epoxy matrix laminates present higher mechanical properties when compared to the f155-epoxy ones. the shear-tested samples observed by scanning electron microscopy and that ones tested in flexural, analyzed by stereoscopy, revealed that the fractured surfaces present typical aspects. the compressive results show that the hygrothermal conditioning caused the decrease of the compressive strength in, approximately, 8-20% depending on the laminate type. the failure modes of the tested specimens were evaluated showing good agreement with the literature.

Abstract: Patrícia Paiva-Lima,1 Rafael M Rezende,1 R mulo Leite,2 Igor DG Duarte,1 YS Bakhle,3 Janetti N Francischi11Department of Pharmacology, Laboratory of Inflammation and Pain, Biological Sciences Institute, Federal University of Minas Gerais, Brazil; 2Department of Pharmacology, Faculty of Pharmacy, Federal University of Ouro Preto, Minas Gerais, Brazil; 3Leukocyte Biology, National Heart and Lung Institute, Faculty of Medicine, Imperial College, London, UKBackground: Celecoxib exerted analgesic effects (hypoalgesia) reversed by opioid receptor antagonists in a model of inflammatory pain. To analyze this celecoxib-induced hypoalgesia further, we assessed the effects of several disruptors of cytoskeletal components in our model of inflammation. Methods: Hyperalgesia to mechanical stimuli was induced in rat hind paws by intraplantar injection of carrageenan and measured using the Randall-Selitto method over the next 8 hours. The effects of systemic pretreatment with celecoxib and a range of cytoskeletal disruptors (colchicine, nocodazole, cytochalasin B, latrunculin B, acrylamide, each given by intraplantar injection) on carrageenan-induced hyperalgesia were similarly investigated. Morphine and other selective cyclo-oxygenase 1 (SC-560), cyclo-oxygenase 2 (SC-236), and nonselective cyclo-oxygenase (indomethacin) inhibitors were also tested under similar conditions. Results: None of the cytoskeletal disruptors affected the peak intensity of carrageenan-induced hyperalgesia, and its duration was increased only by nocodazole and colchicine. Pretreatment with celecoxib 30 minutes before carrageenan reversed the hyperalgesia and raised the nociceptive threshold (hypoalgesia). All analgesic effects of celecoxib were blocked by nocodazole, colchicine, cytochalasin B, and latrunculin B. Pretreatment with morphine also induced hypoalgesia in carrageenan-inflamed paws, an effect reversed by colchicine and cytochalasin B. However, the analgesic effects of indomethacin were not reversed by disruption of actin filaments with cytochalasin B or latrunculin B.Conclusion: These data strengthen the correlation between cytoskeletal structures and the processes of pain and analgesia. Keywords: hypoalgesia, celecoxib, morphine, cytoskeleton, actin, cyclo-oxygenases
