Abstract: objective: to assess (1) the consistency between self-classified and interviewer-classified color/race according to socioeconomic and demographic variables and (2) the magnitude of the ethnic-racial inequalities of income and socioeconomic status using self-classified and interviewer-classified color/race. methods: a cross-sectional population-based study was carried out among individuals of both sexes aged >20 years (n=3,353), living in the urban area of a city in southern brazil, in 2005. a two-stage sampling scheme was adopted and data collection was performed at participants' homes. face-to-face interviews were conducted using standardized precoded questionnaires. the consistency between self-classified and interviewer-classified color/race was checked by means of concordance proportions and kappa statistics. ethnic-racial inequalities of income and socioeconomic status were estimated using linear and ordinal logistic regression models, adjusting for sex, age and schooling. results: the response rate was 93.5%. despite the high reproducibility observed between self-classified and interviewer-classified color/race, a tendency towards whitening was seen among the interviewees. self-classified brown (pardo) and black individuals were 1.4 and 1.5 times more likely to be classified as whites than as blacks and browns (pardos), respectively. socioeconomically deprived strata presented higher kappa values. ethnic-racial inequalities of income and socioeconomic status were found, and these were slightly greater using interviewer-classified color/race. conclusions: racial classification presents a tendency towards the whitening of participants by interviewers. browns (pardos) and blacks were socioeconomically disadvantaged in comparison with whites.

Abstract: the epidemiological literature has been limited by the absence of a theoretical framework reflecting the complexity of causal mechanisms for the occurrence of health phenomena / disease conditions. in the field of oral epidemiology, such lack of theory also prevails, since dental caries the leading topic in oral research has been often studied through a biological and reductionist viewpoint. one of the most important consequences of dental caries is dental pain (odontalgia), which has received little attention in studies with sophisticated theoretical models and powerful designs to establish causal relationships. the purpose of this study is to review the scientific literature on the determinants of odontalgia and to discuss theories proposed for the explanation of the phenomenon. conceptual models and emerging theories on the social determinants of oral health are revised, in an attempt to build up links with the bio-psychosocial pain model, proposing a more elaborate causal model for odontalgia. the framework suggests causal pathways between social structure and oral health through material, psychosocial and behavioral pathways. aspects of the social structure are highlighted in order to relate them to odontalgia, stressing their importance in discussions of causal relationships in oral health research.

Esta associa o leva à uma forma grave de insuficiência respiratória cujo sucesso do desmame da ventila o mecanica é dependente da capacidade da musculatura respiratória remanescente em compensar o trabalho respiratório excedente. Fisioterapia respiratória intensiva com treinamento da musculatura, corre o dos distúrbios metabólicos, tratamento das infec es respiratórias associadas à ventila o mecanica e otimiza o do suporte nutricional foram fundamentais para o sucesso do desmame.
Oral health studies in the 1982 Pelotas (Brazil) birth cohort: methodology and principal results at 15 and 24 years of age

Abstract: the aim of this study was to describe the methodology and results of oral health studies nested in a birth cohort in pelotas, southern brazil. for the oral health studies a sub-sample (n = 900) was selected from the cohort and dental examinations and interviews were performed at ages 15 (n = 888) and 24 years (n = 720; 81.1%). data collection included dental outcomes, dental care, oral health behaviors, and use of dental services. mean dmf-t varied from 5.1 (sd = 3.8) to 5.6 (sd = 4.1) in the study period. the proportion of individuals with at least one filled tooth increased from 51.9% to more than 70%. individuals who had always been poor used dental services less and had fewer healthy teeth on average than those who had never been poor. individuals with decreasing or increasing family income trajectories showed intermediate values. an increase was seen in the number of healthy teeth from age 15 to 24 only among those who had never been poor. a history of at least one experience with poverty had a negative impact on oral health in adulthood.

Abstract: the association of bilateral diaphragmatic paralysis and scapular girdle paralysis is a rare complication after aortic aneurysm correction. such a complication induces to a severe form of hypercapnic respiratory failure in which the success of weaning from mechanical ventilation depends on the capacity of remaining respiratory muscles to compensate extra respiratory work load. intensive respiratory physiotherapy with muscle training, hydroelectrolityc inbalances correction, treatment of ventilation associated infections and a well planned nutritional support were essential to the successful weaning of mechanical ventilation.

Abstract: objective: to describe the progress of patients with diabetes mellitus seen by health care team members who followed the staged diabetes management framework. methods: this descriptive, prospective, and longitudinal study was conducted in a period of 12 months. the sample consisted of 54 patients with diabetes mellitus. data were collected in three occasions through interviews: p0 - at beginning of the study; p6 - in six months; and, p12 - at the end of the study. results: there was an increase in adherence to diet from 61.1% in the p0 to 92.6% in the p12, in adherence to physical activities from 57.4% in the p0 to 66,7% in the p12, and in the use of medication. conclusion: the use of the sdm framework among this sample of patients with diabetes promoted better adherence to diet, physical activities, and the use of medication.

Abstract: the first description of severe pain in the distribution of the glossopharyngeal nerve is credited to weisenberg, in 19101, in a patient with cerebellopontine angle tumor. however, it was harris, in 19262, who coined the term glossopharyngeal neuralgia to describe this rare condition characterized by paroxysms of excruciating pain located laterally at the back of the tongue, soft palate, throat, and lateral and posterior pharynx, radiating to the ear. swallowing, coughing, yawning or chewing may trigger pain, which usually lasts from seconds to minutes. the association between glossopharyngeal neuralgia and syncope is very rare, being identified by brief episodes of bradycardia, asystole, and hypotension. such an association, with this same pathophysiology, was first described by riley et al3 in 1942.

Abstract: Osteoma is a benign tumor that often affects the maxillofacial region. It exhibits slow growth and remains asymptomatic for prolonged time and may cause facial asymmetry or functional disorders when it reaches vast proportions. When osteomalacia is diagnosed it is important to check about Gardner’s Syndrome because up to 90% of patients with this syndrome may present skeletal abnormalities and osteomas. The objective of this paper is to report a case of a 54-year-old woman that related an 8-year evolution of a volume increase in the face. Clinical examination showed facial asymmetry and presence of nodular lesion located in the left region of the mandibule, painless and hard on palpation. Panoramic radiography showed radiopaque image limited to the left mandibular body and angle, suggestive of osteoma. Gastrointestinal review was requested and discarded intestinal polyps and Gardner’s Syndrome. The patient was submitted to excisional biopsy and microscopic examination of the tumor that confirmed the diagnosis of osteoma. Osteoma needs special attention due to the possible association with malignant lesions intestine and also by psychological and functional problems, when it reaches great proportions.

Abstract: this manuscript discusses the implications of the productive paradigm changes in health and environment of rural area of brazil. it analyzes production, economy and demography data, facing social and structural determinants with health and environmental reflexes, revealing a serious panorama that includes: a) environmental impacts associated to large productive chains, such as corn, soy-bean, cotton and sugar cane; b) chemical contamination of diverse environment compartments due to pesticides and fertilizers used in agriculture; c) the use of natural land in the expansion of agriculture and cattle farming activities; and d) the reflexes of these actions on individuals, workers or not, that are more and more forced to deal with extreme vulnerability situations because of the risk of rural work processes. thus, the present article points that macro-determinants (or driven-forces) tends to configure the health status of rural brazil inhabitants which, day after day, face a series of challenges in order to guarantee dignity in their work and quality of life.
