El debate entre externistas e internistas acerca de la racionalidad inferencial

Abstract: several arguments have been presented against content externalism. i deal here with one of these arguments, which -like some others- comes from boghossian. it is related to our inferential rationality (to use his terms). according to his reasoning, the apriority of our logical abilities is inconsistent with certain externalist assumptions. this conclusion is not justified. the problem posed by boghossian constitutes an important challenge for any theory of content, not just for externalism. furthermore, when we examine what internalists may propose to solve the problem, we see that externalists have at their disposal a more promising repertoire of possible replies than internalists. in that sense, insofar as boghossian's scenario is relevant to the debate externalism/internalism, it can be seen -against boghossian's original intention- as providing additional evidence for content externalism.

Abstract: i approach in this article one of the problems which purportedly would show the incompatibility of externalism and authoritative self-knowledge. the problem is based on some alleged consequences of externalism regarding the existence of substances or objects external to the thinker: if externalism is a priori, then a subject can know a priori that water exists by knowing a priori (by introspection) his thought about water. the two main compatibilist responses i work out exploit the role played by experience in a priori knowledge. taking into account the experience required to entertain the relevant thoughts makes less puzzling that a subject could acquire a priori knowledge about water's existence, or about a weaker related condition which is the real commitment of externalism.

Abstract: a bibliographic review of professional competence was made and concepts of competence, evaluation and conditions to create evaluative tools were analyzed. the different levels of professional performance were described according to miller′s model. reference was made to the newest evaluation test, the objective structured clinical examination, which is one of the most complete methodologies at present, although it is made clear that the evaluation is carried out under a created and simulated situation. it is concluded that the bases for making the evaluative tools that will measure the competence or incompetence of our medical sciences professionals should be structured.
Experiencia de trabajo en la Unidad de Cuidados Intensivos ante las manifestaciones psiquiátricas de la malaria

Abstract: a research work was carried out on malaria cerebral affecting internationalist soldiers who were admitted to the intensive care unit of luanda′s general hospital. this work was made in coordination with the psychiatric ward of convalescents of "dr luis galván soca" military hospital. in the sample of 18 cases that had associated psychiatric manifestations, the analyzed variables were: age, length of stay in the people′s republic of angola, personal and familiar pathological history, most frequent symptoms and progression. it was concluded that if there are acute cerebral alterations (blackouts, disorientation, hallucinations, psychomotor agitation, delirious and negative ideas, and neurotic symptoms) in patients with acute malarial episodes, then malaria with psychiatric alterations is present. on this occasion, recovery was favorable without any sequelae. there was good response to combined therapy of antimalarials and psychotropic drugs.

Abstract: Se realizó una investigación sobre el paludismo cerebral en los combatientes internacionalistas que ingresaron en la Unidad de Cuidados Intensivos del Hospital General de Luanda en coordinación con la Sala de Psiquiatría de Convalecientes del Hospital Militar "Dr. Luis Galván Soca". En la muestra seleccionada de 18 casos que tenían asociadas manifestaciones psiquiátricas, las variables analizadas fueron: edad, tiempo de estancia en la República Popular de Angola, antecedentes patológicos personales y familiares, síntomas más frecuentes y evolución. Se concluye que si existen alteraciones cerebrales agudas (toma de conciencia, desorientación, alucinaciones, agitación psicomotriz, ideas delirantes, negativas y síntomas neuróticos) en los pacientes con episodios agudos palúdicos, está presente la enfermedad de la malaria con alteraciones psiquiátricas. En esta experiencia, la evolución fue favorable sin dejar secuelas. Hubo buena respuesta ante la combinación del medicamento antipalúdico y psicofármacos. Abstract: Se realizó una revisión bibliografía sobre la competencia profesional, que analizó los conceptos de competencia, evaluación y condiciones para crear instrumentos evaluativos. Se describieron los diferentes niveles de actuación profesional según el modelo de Miller. Se hizo mención a la prueba de evaluación más novedosa, el examen clínico objetivo estructurado, que es una de las metodologías más completas de las que se dispone en la actualidad, aunque no deja de se alarse que la evaluación se realiza en una situación creada y simulada. Se concluye que deben estructurarse las bases para la confección de los instrumentos evaluativos, los que medirán el grado de competente o incompetente de nuestros profesionales en ciencias médicas. Abstract: congenital adrenal hyperplasia (cah) is one of the most frequent autosomal recessive disorders. it is characterized by a deficiency of an enzyme involved in cortisol synthesis and in 95% of patients the cause is 21-hydroxylase deficiency. a diagnostic marker is elevated levels of 17-hydroxyprogesterone, a precursor of cortisol. cah has several clinical forms, and classical or severe forms manifest in the neonatal period with symptoms due to excess adrenal androgen production such as virilization and ambiguity of the external genitalia in affected girls. in more than 70% of patients, there is associated salt wasting (aldosterone deficiency), which can be fatal in males without an early diagnosis. we summarize the various forms of presentation of 21-hydroxylase deficiency and describe diagnosis and treatment. screening programs avoid incorrect sex assignment in the newborn and can save the lives of males with severe forms and salt wasting. we discuss the genetic-molecular diagnosis of cyp21a2 (chromosome 6p 21.3). we review future recommendations for the study and management of this disease, including several treatments such as, growth hormone, and gonadotrophin antagonists. prenatal diagnosis and treatment in affected female fetuses are feasible and the results are encouraging. we also discuss the management of the transition to adulthood and the importance of follow-up of women with cah during pregnancy.
Estudio de casos: conocimientos físicos de los estudiantes cuando terminan la escuela secundaria: una advertencia y algunas alternativas

Abstract: initial knowledge of mathematics and physics is usually regarded by university physics instructors as a prerequisite for successful student learning. this article describes incoming university student's understanding of some basic laws of physics at the universidad de alcalá, spain. a 13-item single response multiple choice test has been constructed and used to measure the conceptual knowledge of physics topics normally included in high school physics. it also contains 5 items on mathematics subjects thought to be of relevance for physics instruction. the results show that these student's sample still hold, after high school instruction, major misconceptions of the tested topics. a comparison is made between this measured understanding and the conceptual knowledge adquired by high school students enrolled in classes that followed active learning methods. the significant differences in learning gains between traditional and active learning instruction strongly indicate that traditional approaches to teaching physics in high schools should be replaced by field-tested, active learning methodologies. it is argue that this study can be considered, in the field of physics, complementary and reaffirmatory of the recently released results of the pisa 2006 survey.
