Evaluación de la competencia en médicos intensivistas

Abstract: a bibliographic review of professional competence was made and concepts of competence, evaluation and conditions to create evaluative tools were analyzed. the different levels of professional performance were described according to miller′s model. reference was made to the newest evaluation test, the objective structured clinical examination, which is one of the most complete methodologies at present, although it is made clear that the evaluation is carried out under a created and simulated situation. it is concluded that the bases for making the evaluative tools that will measure the competence or incompetence of our medical sciences professionals should be structured.
Experiencia de trabajo en la Unidad de Cuidados Intensivos ante las manifestaciones psiquiátricas de la malaria

Abstract: a research work was carried out on malaria cerebral affecting internationalist soldiers who were admitted to the intensive care unit of luanda′s general hospital. this work was made in coordination with the psychiatric ward of convalescents of "dr luis galván soca" military hospital. in the sample of 18 cases that had associated psychiatric manifestations, the analyzed variables were: age, length of stay in the people′s republic of angola, personal and familiar pathological history, most frequent symptoms and progression. it was concluded that if there are acute cerebral alterations (blackouts, disorientation, hallucinations, psychomotor agitation, delirious and negative ideas, and neurotic symptoms) in patients with acute malarial episodes, then malaria with psychiatric alterations is present. on this occasion, recovery was favorable without any sequelae. there was good response to combined therapy of antimalarials and psychotropic drugs.

Abstract: Se realizó una investigación sobre el paludismo cerebral en los combatientes internacionalistas que ingresaron en la Unidad de Cuidados Intensivos del Hospital General de Luanda en coordinación con la Sala de Psiquiatría de Convalecientes del Hospital Militar "Dr. Luis Galván Soca". En la muestra seleccionada de 18 casos que tenían asociadas manifestaciones psiquiátricas, las variables analizadas fueron: edad, tiempo de estancia en la República Popular de Angola, antecedentes patológicos personales y familiares, síntomas más frecuentes y evolución. Se concluye que si existen alteraciones cerebrales agudas (toma de conciencia, desorientación, alucinaciones, agitación psicomotriz, ideas delirantes, negativas y síntomas neuróticos) en los pacientes con episodios agudos palúdicos, está presente la enfermedad de la malaria con alteraciones psiquiátricas. En esta experiencia, la evolución fue favorable sin dejar secuelas. Hubo buena respuesta ante la combinación del medicamento antipalúdico y psicofármacos. Abstract: Se realizó una revisión bibliografía sobre la competencia profesional, que analizó los conceptos de competencia, evaluación y condiciones para crear instrumentos evaluativos. Se describieron los diferentes niveles de actuación profesional según el modelo de Miller. Se hizo mención a la prueba de evaluación más novedosa, el examen clínico objetivo estructurado, que es una de las metodologías más completas de las que se dispone en la actualidad, aunque no deja de se alarse que la evaluación se realiza en una situación creada y simulada. Se concluye que deben estructurarse las bases para la confección de los instrumentos evaluativos, los que medirán el grado de competente o incompetente de nuestros profesionales en ciencias médicas. Resistencia de Helicobacter pylori a metronidazol, claritromicina y amoxicilina en pacientes colombianos

Abstract: helicobacter pylori (h. pylori), is a universal pathogen that infects more than half the world population. in the last two decades, the recommended treatment for its eradication, as first-line scheme is the standard triple therapy, consisting of an inhibitor of the proton pump, clarithromycin and amoxicillin or metronidazole. in recent years the effectiveness of this therapy has declined, especially due to the resistance of bacteria to metronidazole and clarithromycin. objectives: in this study, we evaluated the prevalence of primary resistance of colombian h. pylori isolates to metronidazole, clarithromycin, amoxicillin. in addition, the vaca and caga genotypes of strains isolated were determined and associated to correlate the virulence markers and antibiotic resistance. methods: minimum inhibitory concentration (mic) for metronidazole, clarithromycin and amoxicillin were determined by e-test method. genomic dna was extracted, and allelic variants of vaca and caga were identified by the polymerase chain reaction (pcr). results: resistance to metronidazole was 81.01 % (ic95% 70.3%-88.6%), to amoxicillin 3,8% (ic 95% 0-8,6%), and to clarithromycin 17.72% (ic95% 10.37-28.29). no significant correlation between pathogenicity and resistance or susceptibility was detected when mic values for each antibiotic were compared with different vaca and caga genotypes. conclusion: we find a high rate of resistance to three principal antibiotics used in the majority of the successful schemes of eradication of the infection, which implies the need to investigate with priority new schemes of treatment for the eradication of the infection in colombia.

Abstract: Helicobacter pylori (H. pylori) es un patógeno universal, que infecta a más de la mitad de la población mundial. En las últimas dos décadas, el tratamiento recomendado para su erradicación, como esquema de primera línea, es la triple terapia estándar, constituida por un inhibidor de la bomba de protones, amoxicilina y claritromicina o metronidazol. En los últimos a os la eficacia de esta terapia ha declinado, debido especialmente a la resistencia de la bacteria a metronidazol y a claritromicina. Objetivos: En este estudio, se evaluó la prevalencia de resistencia primaria de cepas colombianas de H. pylori a metronidazol, claritromicina, amoxicilina. Además, se analizaron los genotipos de vacA y cagA de las cepas aisladas y la correlación entre los marcadores de virulencia y la resistencia a claritromicina, amoxicilina y metronidazol. Métodos: La resistencia a metronidazol, amoxicilina y claritromicina fue determinada por el método de E-test. Se extrajo el ADN genómico y variantes alélicas de vacA y cagA fueron identificadas por la técnica de reacción en cadena de la polimerasa (PCR). Resultados: La resistencia a metronidazol fue de 81,01% (IC 95% 70,3%-88,6%), a amoxicilina de 3,8% (IC 95% 0-8,6%) y a claritromicina de 17,72% (IC 95% 10,37-28,29). No se encontró asociación significativa entre el genotipo de patogenicidad y la resistencia o susceptibilidad a los antimicrobianos cuando los valores de CIM de cada antibiótico se compararon con los diferentes genotipos cagA y vacA. Conclusión: Encontramos una alta tasa de resistencia a los tres principales antibióticos utilizados en la mayoría de los esquemas exitosos de erradicación de la infección, lo cual implica la necesidad de investigar, con prioridad, nuevos esquemas de tratamiento para la erradicación de la infección en Colombia. Abstract: the traumatisms that affect the extremities represent 80% of all the vascular traumatisms approximately. the inadequate handling of these it contributes to fateful consequences as the loss of the life or of the function of the extremity. the vascular lesions for firearm constitute one of the main causes with the appearance of arteriovenous fistula. we present the case of a 36 year old masculine patient with antecedents of having hurt 1 year ago by firearm in the root of the left thigh goes to present increase of volume of the extremity and difficulty to the march with sensation of fatigue and gravity. to the physical exam increase of volume of the limb more marked affection was verified in the root of the thigh, in the area related with the bullet impact thrill is felt and blow holosistólico is auscultated. the echo doppler and the tomography with contrast use evidenced the presence of a arteriovenous fistula. with this diagnosis was carried out surgical exploration and repair by means of veins bond for exclusion and interference of ilio-femoral arterial implant of politetrafluoroethylene (ptfe). the postoperative evolution was satisfactory with regression of all the clinical signs.

Abstract: a case of a patient of the nephrology service is presented that is integrated to the hemodialysis program since five years ago by presenting an irreversible chronic renal failure, not able for transplant, with multiple conventional vascular accesses carried out (direct and prosthetic) that have failed by progressive and generalized decay of the vascular system (arterial and venous), due to the effect of the associated pain to the renal disease and the triggering ones by the therapeutic intervention. because of that was surgically intervened and a vascular access at the level of the right inguinal region was performed with the objective to connect the venous system with the arterial one by means of an artificial prosthesis of polytetrafluoroethylene that was implanted in the form of a loop and in superficial planes to facilitate the access to the same one. the use of prosthetic implants of polytetrafluoroethylene in the surgery of vascular access for hemodialysis is a first election alternative when the conventional methods fail.

Abstract: this is a systematic review of literature of the results of sensibility (s) specificity (e), (vpp) positive predictive value, and (vpn) negative predictive value, of original articles published between 2003 and 2008, of the test of fecal antigens and urea breath test with the hematoxilin and eosin and giemsa stains in histology in pre and post treatment for the detection of helicobacter pylori. the results showed s=98%, e 95% for the test of fecal antigens in pre treatment, e=100% y s=100% for the urea breath test of in pre treatment, e=100% y s=100% for the test of fecal antigens in post treatment and finally e=100% y s=100% for the urea breath test in post treatment. the graphs of funnel plot in 3 of the 4 groups revealed asymmetry, and the test of heterogeneity in the studies of the 4 groups were homogeneous so much for s as for e. in conclusion the histology continues being the best alternative for the diagnostic of the infection before the treatment; on the contrary the urea breath test and the test of fecal antigens are the best options to verify the eradication of the infection. the test of fecal antigens can be implemented easily in routine laboratories in developing countries as colombia where there are not realized tests of the routine form to verify the eradication of the infection.

Abstract: type 1 neurofibromatosis (nf1) is one of the most common autosomal and dominant diseases and is produced by defects in the nf1-gen located in 17q11.2 chromosome. a descriptive and transversal study was carried out in patients with nf1 in pinar del río during the year 2005, aimed at describing the brain clinical and radiological characteristics of this entity, selecting 42 patients suffering from nf1 associated with dimorphism, mental retardation, malignant degeneration, plexiform neurofibromas and masses of the cns, predominating the milk-coffee colour spots, the inguinal and axillary freckles and lisch's node, being frequent acromegaly, the mental retardation and the decrease in visual acuity; the increase of sella turcica was one of the most relevant radiological finding. the total of the predominant clinical characteristics in the study are diagnostic criteria of nf1. the radiological findings are a principal tool for diagnosis, predominating in those cases from a new mutation.
