Diabetes autoinmune del adulto en diabéticos tipo 2: frecuencia y características LADA in type 2 diabetics: frequency and characteristics

Abstract: Se realizó este trabajo para conocer frecuencia, características clínico-bioquímicas, inmunológicas y genéticas de la diabetes autoinmune en adultos (LADA) en 1 000 diabéticos tipo 2 con edades 3 35 a os con distintos tiempos de duración de la diabetes. Se les determinó glucemia, anticuerpos antiislotes pancreáticos (ICA), anti-GAD65, anti-ICA512bdc/IA2, antimicrosomales tiroideos (AMT), antigástricos parietales (AGP), antinucleares (AN), microalbuminuria y péptido C en ayunas. Se encuestaron y se registraron algunas características clínicas. Se dividieron en 2 grupos según la presencia de ICA. Todos los diabéticos tipo 2 + para autoanticuerpos antiislotes (ICA y/o anti-GAD65) fueron identificados como LADA. Se detectó el 3,4 % de diabético tipo 2 con ICA +, en los diabéticos tipo 2 ICA- el 22,0 % presentó anticuerpos anti-GAD65. Se encontró que los diabéticos tipo 2 ICA+ eran más jóvenes, la duración de su diabetes era menor, presentaron menor IMC, disminución de los niveles de péptido C en ayunas, menos antecedentes familiares (padres) de DM2, valores menores en las tensiones arteriales diastólicas y sistólicas, mayor presencia de anticuerpos anti-GAD65, AMT y AGP en comparación con los diabéticos tipo 2 ICA-. Se observó que los diabéticos tipo 2 ICA+ (LADA) tienen características específicas que los asemejan a los diabéticos tipo 1, esto implicaría variaciones importantes en su tratamiento y evolución con respecto a los diabéticos tipo 2 ICA-. Se observó una baja frecuencia de ICA y alta de GAD en los diabéticos tipo 2 cubanos, las cuales fueron diferentes a la encontrada en poblaciones caucasianas. Los anticuerpos anti-GAD65 fueron superiores a los ICA para detectar los LADA. Las características clínicas e inmunológicas de estos pacientes muestran la lenta progresión de la destrucción autoinmune de las células b con implicaciones terapéuticas. Abstract: Se estudiaron 66 sujetos ICA +: 21 diabéticos tipo 1, 24 diabéticos clasificados inicialmente como de tipo 2 y 21 familiares de primer grado de diabéticos tipo 1, se compararon con 101 individuos ICA -: 20 diabéticos tipo 2, 20 FPG, 21 ni os y 40 adultos controles no diabéticos para analizar si la respuesta inmunológica a H. pylori está asociada con anticuerpos antiislotes pancreáticos (ICA). Los ICA se determinaron por el método de inmunofluorescencia indirecta con incubación prolongada y los niveles de anticuerpos (IgG) contra H. pylori (HP), por un ELISA utilizando un kit comercial. Se observó frecuencia de anticuerpos (IgG) contra H. pylori del 38,0 % (25/66) en los sujetos ICA + y del 39,6 % (40/101) en los ICA-. No se halló correlación entre la presencia de anticuerpos (IgG) contra H. pylori y los ICA en los grupos estudiados. No hubo diferencias en relación con la presencia de anticuerpos IgG contra H. pylori entre la DM y los controles (47,7 % [31/65] vs. 40,9 % [25/61]). Se encontró alto porcentaje de anticuerpos (IgG) contra H. pylori en los diabéticos tipo 1 (33 %, 7/21) en relación con los controles (14 %, 3/21), pero sin diferencias significativas. Existe mayor frecuencia de anticuerpos contra Helicobacter en los controles adultos, en comparación con los ni os (55 %, 22/40 vs. 14 %, 3/21, p = 0,0025). La frecuencia de anticuerpos contra H. pylori está mucho más elevada en los controles adultos quizá por la mayor posibilidad de exposición a reinfecciones. A pesar de estos resultados, no excluimos que la infección por H. pylori esté relacionada con la diabetes tipo 1, porque algunos estudios sugieren que la IgG contra H. pylori no persiste por largo tiempo. 66 ICA-positive patients were studied: 21 type 1 diabetics, 24 diabetics initially classified as type 2 and 21 first-degree relatives of type 1 diabetics. Abstract: 66 ica-positive patients were studied: 21 type 1 diabetics, 24 diabetics initially classified as type 2 and 21 first-degree relatives of type 1 diabetics. they were compared with 101 ica-negative patients: 20 type 2 diabetics, 20 first-degree relatives, 21 children and 40 non-diabetic adult controls to analyze if the immunolgical response to h. pylori is associated with islet cell antibodies (ica). the ica were determined by the indirect immunofluorescence method with prolongued incubation and the levels of antibodies (igg) versus h. pylori (hp) by an elisa using a commercial kit. it was observed a frequency of antibodies (igg) versus h. pylori of 38.0 % (25/66) in ica-positive patients and of 39.6 % (40/101) in ica-negative patients. no correlation was found between the presence of antibodies (igg) versus h.pylori and the ica in the studied groups. there were no differences as regards the presence of igg anitbodies versus h. pylori between diabetes mellitus and the controls (47.7 % [31/65] vs. 40.9 % [ 25/61]). it was observed a high percentage of antibodies (igg) vs. h. pylori in type 1 diabetics (33 %, 7/21) in relation to the controls (14 %, 3/21), but with no significant differences. the frequence of antibodies vs. helicobacter in adult controls is higher than in children (55 %, 22/40 vs. 3/21, p=0.0025). the frequency of antibodies vs. h. pylori is much more elevated in the adult controls may be because of the greater possibility of exposure to reinfections. in spite of these results, we do not exclude the idea that the infection due to h. pylori is related to type i diabetes, since some studies suggest that igg versus h. pylori does not persist for a long time.

Abstract: this paper was aimed at knowing the frequency, clinico-biochemical, immunologic and genetic characteristics of autoimmune diabetes in adults (lada) in 1 000 type 2 diabetic patients aged 35 or over with different times of duration of diabetes. glycemia, anti-pancreatic islet cell antibodies (ica), anti-gad65 antibodies, anti-ica512bdc/ia2 antibodies, anti-microsomal thyroid antibodies (amt), anti-gastric parietal antibodies (agp), antinuclear antibodies (an), microalbuminuria and peptide c during fasting were determined. these patients were surveyed and some clinical characteristics were registered. they were divided into 2 groups according to the presence of ica. all the type 2 + diabetics for anti-islet cell autoantibodies (ica and/or antigad65) were identified as lada. 3.4 % of type 2 ica + were detected. 22.0 % of type 2 ica - diabetics had anti-gad65 antibodies. it was found that type 2 ica + diabetics were younger, that their diabetes was shorter, that they had lower bmi, reduced levels of fasting peptide c, less dm2 history family (parents), lower values of diastolic and systolic arterial pressure, higher presence of anti-gad65 antibodies, amt and agp in comparison with type 2 ica - diabetics. it was observed that type 2 ica+ diabetics (lada) have specific characteristics that make them similar to type 1 diabetics, which would lead to important variations in their treatment and evolution as regards type 2 ica - diabetics. among the cuban type 2 diabetics it was detected a low frequency of ica and a high frequency of gad, which were different to those found in the caucasian populations. the anti-gad65 antibodies were higher than ica to detect lada. the clinical and immunological characteristics of these patients show the slow progression of the autoimmune destruction of b-cells with therapeutic implications.

Abstract: in this work a formal methodology for the extraction of enterprise knowledge from data that companies posess, is presented. the data that are kept in company′s databases are the most valuable knowledge sources in an enterprise, so the efficient management of databases becomes of special importance. the proposed methodology has been named km-iris and consists of five steps. in each step, the main objectives are clearly stated, and also the techniques and tools to be used.this scheme facilitates the inmediarte application of the methodology, following the proposed steps, to calibrate and verify the proposal, the methodology has been applied to some typical cases, such as the extraction of knowledge from people, documents and data.

Abstract: Este trabajo se presenta una metodología formal para la extracción del conocimiento a partir de los datos que dispone una empresa. Los datos que residen en las bases de datos corporativas pueden ser una de las fuentes de conocimiento más importantes que hay en las empresas por lo que su manejo eficiente es de especial importancia. La metodología propuesta se ha denominado KM-IRIS y consta de cinco fases. En cada fase se proponen los objetivos a cumplir y las técnicas y herramientas que se pueden aplicar. Este planteamiento facilita la aplicación inmediata de la metodología siguiendo simplemente las pautas expuestas. Con el propósito de calibrar y validar la metodología, se ha aplicado el método desarrollado a varios casos típicos, como pueden son la extracción del conocimiento a partir de las personas, los documentos o los datos. Abstract: This work uses data from the Spanish Tourism Demand Segments Survey (N=6900) conducted by the IESA-CSIC for Turismo Andaluz, SA. The objective of the paper is to develop a statistical segmentation or typology of Spanish tourists based on objective aspects of tourist behaviour measured in the survey including destinations visited, theme of the trip, lodging, transportation and travel group. Initial categorical data are reduced using multiple correspondence analysis and grouped through cluster analysis. Afterwards, identified segments are evaluated to analyse their tourist profiles with a view to examining sociological perspectives of tourist behaviour.

Abstract: The goal of this report is to look in more depth at the effects that the relationships between indigenous people and immigrants can have on the attitudes of the former towards the latter. The study uses the results of a national opinion poll on the perception of Spaniards towards immigration. Following the traditional perspective of contact theory developed by G.W. Allport, two central questions are posed here: first, if more frequent contact between indigenous and immigrantcommunities brings with it a more positive perception of the migratoryphenomenon and second, if the type of relationship established between the two groups influences this perception. In other words, to what extent does intergroup contact contribute to reducing stereotypes and prejudices towards immigrants?
Beyond harvests in the commons: multi-scale governance and turbulence in indigenous/community conserved areas in Oaxaca, Mexico

Abstract: Some important elements of common property theory include a focus on individual communities or user groups, local level adjudication of conflicts, local autonomy in rule making, physical harvests, and low levels of articulation with markets. We present a case study of multi-scale collective action around indigenous/community conserved areas (ICCAs) in Oaxaca, Mexico that suggests a modification of these components of common property theory. A multi-community ICCA in Oaxaca demonstrates the importance of inter-community collective action as key link in multi-scale governance, that conflicts are often negotiated in multiple arenas, that rules emerge at multiple scales, and that management for conservation and environmental services implies no physical harvests. Realizing economic gains from ICCAs for strict conservation may require something very different than traditional natural resource management. It requires intense engagement with extensive networks of government and civil society actors and new forms of community and inter-community collection action, or multi-scale governance. Multi-scale governance is built on trust and social capital at multiple scales and also constitutes collective action at multiple scales. However, processes of multi-scale governance are also necessarily “turbulent” with actors frequently having conflicting values and goals to be negotiated. We present an analytic history of the process of emergence of community and inter-community collective action around strict conservation and examples of internal and external turbulence. We argue that this case study and other literature requires an extensions of the constitutive elements of common property theory.
