Caracterización de Lactantes con infección del Tracto Urinario. 2009-2010. Hospital Regional de Antofagasta.

Abstract: INTRODUCCION: Las infecciones del tracto urinario (ITU) son una de las enfermedades bacterianas más frecuentes en pediatría. La incidencia de ITU en el paciente pediátrico no se conoce con exactitud. En recién nacidos se estima en 1% con una mayor proporción de varones, mientras que en lactantes aumenta entre 3 a 5% con igual proporción en ambos sexos. OBJETIVO: Caracterizar el perfil clínico-epidemiológico de lactantes con diagnóstico de ITU en el servicio de Pediatría del Hospital Regional de Antofagasta. PACIENTES Y METODOS: Estudio descriptivo retrospectivo de corte transversal. Se analizaron fichas clínicas de pacientes de la unidad de lactantes del Servicio de Pediatría del Hospital Regional de Antofagasta con diagnóstico de ITU al egreso del periodo Mayo 2009 - Mayo 2010. La muestra final fue 88 pacientes. Los datos fueron analizados y tabulados en una planilla Excel2007. RESULTADOS: Un 71,5% fueron sexo femenino. La edad promedio fue de 8,9 meses con rango entre 1 mes y 23 meses. Los síntomas más frecuentes fueron fiebre (97,7%), vómitos (30,7%), diarrea (23,1%), inapetencia (21.6%) y llanto/irritabilidad (16%). Las alteraciones de laboratorio fueron PCR elevada (96%), piuria (94,3%), leucocitosis (87,5%) y nitritos positivos (48%). Sólo fueron consignados el 85,2% de los urocultivos, de los cuales el patógeno más frecuente fue Escherichia coli en el 84%. DISCUSION: La ITU en lactantes en nuestro estudio fue más frecuente en el sexo femenino, teniendo como signo preponderante la fiebre. El germen más común fue Escherichia coli, correlacionándose con la literatura. Valoramos la importancia de solicitar urocultivos para la confirmación microbiológica.

Abstract: The cohort design is a major methodological tool in epidemiologicaland medical research. This article introducesthe basic concepts and statistical methods supporting thisdesign. After describing the fundamental ideas of prospectiveand retrospective cohort designs and of fi xed and variablecohort designs, this article focuses on the basic tools used invariable cohort designs. These tools are part of the so calledsurvival analysis. The concepts of censoring, life table, survivalfunction, the Kaplan-Meier estimator and the cumulativehazard function are then described. Statistical methods forcomparing two survival functions such as the Cochran-Mantel-Haenszel and log-rank tests and the log-rank stratifi edtest are also explained. Finally, the alternative person-yearsmethod is introduced. The concepts and methods are illustratedwith a retrospective cohort study of the age of onsetof daily smoking in patients with schizophrenia

Abstract: the use of ontologies to facilitate semantic annotation of medical images has been a widely used approach. a particular limitation of this approach is the lack of ontologies with a high level of completeness, mainly because the problem that ontology evolution represent. this article proposes an approach that facilitates the evolution of ontologies from the contributions made by domain experts through social tagging of medical images. the method guides the collaborative process during the discovery of change. additionally, we present a tool build on web protégé to support the proposed method.
The immunopathology of human schistosomiasis-III: immunoglobulin isotype profiles and response to praziquantel

Abstract: immunoglobulin (ig) isotype (igg, igg1, igg2, igg3, igg4, igm, igd and ige) levels were investigated, both pre- and post-treatment with praziquantel (pzq), in 43 adults and children chronically infected with schistosoma mansoni , by means of a two-site, isotype-specific immunoenzymometric assay. the patients were classified as responders (r) or non-responders (nr) on the basis of their circumoval precipitin test (copt) results 12 months after treatment. in comparison with controls, pre-treatment r children showed significantly higher levels of igg, igg1, igg4 (p<0.001) and ige (p<0.01), and diminished igg2 (p<0.05), while nr children showed significantly elevated levels only of ige (p<0.05). twelve months after therapy, r children maintained significantly lower levels of igg2, but showed significantly decreased levels of igg, igg1, igg4, and ige, while the ig isotype profile of nr children was unaltered. adult r and nr showed similar isotype profiles before chemotherapy, with the exception of significantly elevated igm levels in r. twelve months after therapy, r adults showed significantly decreased levels of igg, igg1, and igg4, while nr adults showed only diminshed igg4 levels. these results reveal different ig isotype profiles in untreated adults and children chronically infected with s. mansoni. the results further show that the pre-treatment ig isotype profile may be significantly modified after an effective r to chemotherapy, accounted for by down regulation of the igg1 isotype in association with negative seroconversion of the copt in r patients. the copt reaction has been associated with the highly specific egg glycoprotein antigen w1, which shows a significant reduction in reactivity six months after treatment. igg1 may thus play a main role in the response against the w1 antigen.

Abstract: El uso de ontologías para facilitar la anotación semántica de imágenes médicas ha sido un enfoque ampliamente utilizado. Una limitación particular de este enfoque, es el reducido número de ontologías con un alto nivel de completitud, debido en parte, a la dificultad que representa su evolución. En este artículo se propone un método que facilita la evolución de ontologías a partir de las contribuciones hechas por expertos de dominio mediante el etiquetado social de imágenes médicas. El método guía el proceso colaborativo durante el descubrimiento del cambio. Adicionalmente, se presenta una herramienta construida sobre Web Protégé para dar soporte al método propuesto.
Synthesis and Characterization of Polyphosphazenes Modified with Hydroxyethyl Methacrylate and Lactic Acid

Abstract: Poly(dichlorophosphazene) was prepared by melt ring-opening polymerization of the hexachlorocyclotriphosphazene. Poly[bis(2-hydroxyethyl-methacrylate)-phosphazene] and poly[(2-hydroxyethyl-methacrylate)-graft-poly(lactic-acid)-phosphazene] were obtained by nucleophilic condensation reactions at different concentrations of the substituents. The properties of the synthesized copolymers were assessed by FTIR, 1H-NMR and 31P-NMR, thermal analysis (DSC-TGA), and electron microscopy (SEM). The copolymers have a block structure and show two 's below room temperature. They are stable up to a temperature of 100°C. The type of the substituents attached to the PZ backbone determines the morphology of the polymers. 1. Introduction Polyesters, polyorthoesters, polyanhydrides, poly(R-amino acids), and polyphosphazenes are degradable polymers that have been investigated for a variety of biomedical applications such as sutures, drug delivery systems, and scaffolds for tissue engineering [1]. Useful properties can be obtained by blending two different polymers. However, compatible polymer blends require strong molecular interactions between polymer chains [2]. Poly(organophosphazenes) offer an appealing platform for the design and synthesis of novel biodegradable polymers as well as critical advantages for the design of biologically functional macromolecules with a broad structural diversity [3], high functional density, and tailored biodegradability [4]. These polymers are of scientific and technological concern since the first work of synthesis reported by Allcock et al. [5, 6]. Polyphosphazenes (PZs) posses special characteristics, including flame-retardant properties, high resistance to oil and solvents, and feasibility for tailored properties according to the choice of organic, inorganic, or organometallic side groups [7]. As biomaterials they have inherent advantages, due to their biocompatibility and fast degradation rate. In addition, degradation residues, phosphate, ammonia, and side groups are either nontoxic when they are present in small quantities or are easily metabolized by the human body [8, 9]. Polyphosphazenes are hybrid polymers with a flexible inorganic backbone of alternating phosphorus and nitrogen atoms and organic side groups. Their composition vary from 3 to 10,000 (–N=P–) repetitive units having two substituents (–R) attached to the phosphorus atom [7].
