Abstract: There are many national medical journals published in our country. Especially the journals representing our country in the international arena not only makes proud of us but also provide guidance on what can we do. After each issue, we have continued our way with the excitement what can we make in order to do the best for the new issue. We have shared these ideas with our 8500 readers. In this process, we have also taken into account your suggestions, contributions and criticisms. Day by day JCAM is processing on the way to becoming an international source for journals with increasing the number of the articles. One of our main goals is to share interesting and unusual cases with our readers like many other journals. Before publication of a case report, an ambitious researcher passes through a difficult process including variable criticism. Therefore, JCAM scientific committee asses the case reports with this responsibility. In table 1, we showed three main medical branches of the most published case reports between 2010 and 2011. JCAM will go on to change in order to protect and furtherance of its position in the life of medical publication. On the behalf of JCAM, we will content to get feedback from you as acting on a sense of common ownership.
Necrotizing Pancreatitis with Hypertriglyceridemia Development Results: A Case Report

Abstract: Acute pancreatitis due to hypertriglyceridemia is a relatively rare clinical entity. Acute pancreatic necrosis is a life threatening form of acute pancreatitis in which early recognition and treatment is important. Necrotising pancreatitis should be treated immediately. We presented a case of pancreatic necrosis due to hypert riglyceridemia which required surgical intervention. We performed necrosectomy. After surgery the patient recovered. We presented the case in order to mention necrotising pancreatitis arising from hypertriglyceridemia and requiring surgical exploration.
Squamous Cell Carcinoma Arising in Etiology of Chronic Pilonidal Sinus Disease:A Case Report

Abstract: Pilonidal sinus disease is usually placed in sakrokoksigeal area. The etiology is unknown exactly. The pathogenesis is assumed to deciduous hair penetration into the subcutaneous tissues and a foreign body-type reaction may then lead to formation of an abscess. Cellulitis, abscesses, fistulas are common complications in pilonidal sinus disease. Chronic inflammation may be factors in the development of some malignancies. Malignant degeneration is very rare complication in chronic pilonidal disease. We reported in this case, a squamous cell carcinoma in caused by untreated chronic pilonidal sinus.
Warfarin Overdose Induced Intramural Small-Bowel Hematoma (Case Report)

Abstract: Uncontrolled use of anticoagulants may cause bleeding. Warfarin- dependent spontaneous intramural hematoma of the small intestine is a rare complication. Although warfarin using patients with abdominal pain were usually treated medically, surgical interventions should be considered in selected cases with intestinal intramural hematoma. Here we present a patient who was treated surgically to bring to mind this rare condition.
A Successful Treatment of Thromboembolic Ischemia in Persistent Sciatic Artery Through Surgical and Endovascular Intervention

Abstract: A 50-year-old man, presented with sudden onset of right leg coldness, numbness and pain for 1.5 months. It was found that the right superficial femoral artery was thrombosed, which ended at the mid-thigh level and a continued as the popliteal artery in magnetic resonance angiography. Persistence of the sciatic artery is extremely rare vascular anomaly. It is prone to undergo early atherosclerotic changes, occlusive embolism, and aneurysm formation. In this article we report a patient with acute artery occlusion developed in popliteal and crural arteries in a patient with persistent sciatic artery which was successfully treated through surgical and endovascular intervention.
Mean platelet volume in patients with acute pancreatitis

Abstract: Objectives: Platelet contributed tissue damage and inflammations have been of increased interest. It was previously suggested that platelets have an important role in patients with acute pancreatitis (AP). We aimed to assess the mean platelet volume (MPV) which is described as an indicator of platelet activation and platelet count in patients with AP.Materials and methods: The study group consisted of 30 patients with AP and 30 control subjects who were matched for age, gender, body mass index (BMI). MPV values and platelet counts were measured on admission.Results: Mean platelet volume was significantly higher among patients with AP when compared with control group (8.82±1.33 vs. 7.94±0.54 fL respectively; p < 0.01). Platelet count was significantly lower among AP patients when compared with control group (223.0±46.2 vs. 295.3±58.9 ×109/L respectively; p < 0.001). The level of CRP was significantly higher in patients with AP compared with control group. Platelet and MPV were correlated with serum CRP levels in correlation analysis (p<0.01; r = -0.364, r = 0.406 respectively). We have shown increased MPV, an indicator of platelet activation, in patients with AP. The platelet count was found to be decreased in patients with AP. MPV values and platelet counts were correlated with serum C-reactive protein level that is known as an inflammation marker.Conclusion: In conclusion, increased MPV level in AP may be a marker for the diagnosis of acute pancreatitis. Transmural Migration of a Retained Sponge Through the Rectum: A Case Report

Abstract: Retained surgical sponge in the abdomen following abdominal and pelvic surgery is an uncommon condition. Here we present a case of retained surgical sponge with unusual presenting symptoms. A 27-year old female patient presented to our department with a foreign body localized in the anal region. She had a past history of a myomectomy 1 year earlier. Clinical examination and radiographic workout revealaed a sponge migrating towards the rectum. The sponge was removed under visual guidance of rectoscopy without laparotomy. The patient was discharged without complications.

Abstract:

Energy will be the main determinant of the economic and political relations between the regions and nations during the years to come. The need for energy and the consumption of energy related to this need continues increasingly. The fossil fuels which meet the energy of many of the growing economies in the world, especially Chinabeing in the first place will continue to be the main energy source until the year2050. Intoday’s world, countries are classified as those which have rich reserves of natural resources and which obtain most of their revenues by selling these natural resources, those which have large amounts of consumption of natural resources and which import natural resources in spite of their own natural resources and as those which are dependant on outside resources but are on the route of transmission of natural resources because of their geopolitical positions. Turkey’s place is in the third group.Turkeyis the leading country among the countries which are situated on the route of the transmission of the natural resources.Turkeyis in a geographical region where its neighbors have rich natural resources, and this geographical region has approximately 70% of the fossil fuels which comprise a great deal of the energy needs in the whole world. Therefore, Turkey is at the center of the transmission corridor of the natural resources between the north and the south, and between the west and the east. TheUnited States of Americainstalled itself and settled down in the region at the beginning of the 90’s because of the first Gulf War, and theUnited Stateshas taken steps and some measures in order to support the establishment of new pipeline corridors for the transmission of natural resources. In the background of this support lies the thought and idea that by virtues of these pipelines the natural resources that exist in the Central Asian countries should be transmitted to the western world countries and to the international markets without any need for dependence onRussia. Another idea behind this is to put an end to the dominance of Russia over these countries.
