Abstract: OBJETIVO: Estabelecer um modelo experimental de cultura primária de melanoma metastático por pun o aspirativa por agulha fina (PAAF). MéTODOS: Duas culturas primárias de células de melanoma metastático através de PAAF foram desenvolvidas a partir de pacientes submetidos à excis o de les es metastáticas e identificadas por imunohistoquímica utilizando os marcadores S-100 e MB-45, específicos para melanoma. RESULTADOS: O diagnóstico da cultura foi confirmado através da imunohistoquímica. A técnica oferece vantagens como pequena morbidade e invasibilidade, além do mais, a mesma les o pode ser puncionada seqüencialmente durante o tratamento do doente. CONCLUS O: O estabelecimento de cultura primária de melanoma metastático a partir de pun o aspirativa por agulha fina mostrou-se uma técnica viável.
Vital dye is enough for inguinal sentinel lymph node biopsy in melanoma patients

Abstract: PURPOSE: The importance of gamma probe detection (GPD) combined with vital dye for sentinel node (SN) biopsy is well accepted. We evaluated the efficacy of patent blue dye (PBD) in identifying inguinal SN. METHODS: Ninety-four cutaneous melanoma patients with inferior extremity lesions were submitted to SNB according to a established protocol. Patients were randomized in two groups: Blue group, where SN was identified by PPD and Probe group, where SN was identified by GPD. The median age was 44.2 years and median Breslow thickness was 2.1 mm. Preoperative lymphoscintigraphy, lymphatic mapping with PBD and intra-operative GPD was performed on all patients. Histological examination of SN consisted of hematoxylin-eosin and immunohistochemical staining. If micrometastases were present complete lymphadenectomy was performed. The SN was considered as identified by PBD if it was blue and identified by GPD if it demonstrated at least ten times greater radioactivity than background. RESULTS: It was explored 94 inguinal lymphatic basins, 145 SN were excised (70 guided primarily by blue dye and 75 guided primarily by probe). All SN identified by preoperative lymphoscintigraphy were excised. In the Blue group PPD identified all SN and all of them were hot. In the Probe group all SN were identified by probe and were blue. The coincidence of PPD and GPD was 100%. CONCLUSION: Patent blue dye is enough to identify superficial inguinal SN in cutaneous melanoma.

Abstract: Pneumomediastinum is a rare and life-threatening complication that may occur following dental procedures. We report a case of acute subcutaneous emphysema that extended through the superior mediastinum, after a root canal treatment and air syringe use. Awareness of such complication and prompt prophylactic antibiotic therapy are extremely important and can save lives.

Abstract: OBJETIVO: verificar se há alguma rela o entre o estado histopatológico do linfonodo sentinela, a recorrência e a mortalidade decorrente do melanoma espesso em pacientes submetidos à BLS ao longo de um seguimento significante. MéTODOS: Oitenta e seis pacientes portadores de melanoma espesso submetidos à BLS foram selecionados de um banco de dados prospectivo. A linfocintilografia, o mapeamento linfático e a detec o gama intraoperatória foram realizados em todos pacientes. O linfonodo sentinela (LS) foi analisado por HE e por imunoistoquímica. Linfadenectomia total foi indicada para os pacientes com LS positivo. O estado histopatológico do LS foi relacionado à taxa de recorrência e de mortalidade por melanoma. RESULTADOS: Cento e sessenta e seis LS foram retirados dos 86 pacientes. As idades variaram de 18 a 73 anos. Havia 47 mulheres e 39 homens. Micrometástases foram encontradas em 44 pacientes. Quarenta e dois pacientes foram submetidos à linfadenectomia total. Sete pacientes tiveram outro linfonodo positivo. Entre os 44 pacientes com LS positivo houve 20 recorrências e 15 mortes. Houve 18 recidivas e 12 mortes no grupo de LS negativo. A espessura de Breslow n o apresentou correla o com o estado histopatológico do LS. O estado histopatológico do LS n o interferiu nas taxas de recorrência e de mortalidade (teste de Fisher, p=1.00). A mediana de seguimento foi 69 meses. CONCLUS O: Considerando a falta de evidência de benefício, a BLS n o deve ser indicada para pacientes com melanoma espesso fora de estudos clínicos. OBJECTIVE: To ascertain whether there is any relationship between the state of the sentinel lymph node histopathology, recurrence and mortality from thick melanoma in patients undergoing SLNB over a long follow-up. METHODS: Eighty-six patients with thick melanoma undergoing SLNB were selected from a prospective database. Lymphoscintigraphy, lymphatic mapping and intraoperative gamma probe detection were performed in all patients. The sentinel lymph node (SLN) was analyzed by HE and immunohistochemistry. Complete lymphadenectomy was indicated for patients with positive sentinel node. The histopathological SLN status was related to the rate of recurrence and mortality from melanoma. RESULTS: One hundred and sixty-six SLNs were taken from the 86 patients. Ages ranged from 18 to 73 years. There were 47 women and 39 men. Micrometastases were found in 44 patients. Linfonodo sentinela em melanoma de crian a: relato de caso

Abstract: Objetivo: apresentar um caso de melanoma cutaneo na infancia, submetido à biópsia de linfonodo sentinela. Descri o: crian a de 12 anos de idade, portadora da síndrome do nevo displásico, desenvolveu melanoma em dorso. A biópsia excisional revelou melanoma (Breslow = 1,5 mm), sendo submetida a tratamento cirúrgico da les o, com amplia o de margem de 2cm e biópsia de linfonodo sentinela. O exame anatomopatológico n o mostrou doen a residual. O exame do linfonodo sentinela n o mostrou metástases ao exame histopatológico por hematoxilina eosina, nem à imunohistoquímica (S100 e HMB45). No entanto, a pesquisa de RNA mensageiro da tirosinase por RT-PCR se mostrou positiva. A paciente n o mostrou sinais de metástase ou recidiva local nestes doze meses iniciais de seguimento. Comentários: o melanoma é raro em crian as, corresponde a menos de 1% dos tumores da infancia. Inúmeros trabalhos mostram o aumento da incidência mundial do melanoma. Em cerca de 70%, se originam de nevos melanocíticos pré-existentes, e nos 30% restantes, eles surgem de novo. Dada a agressividade do melanoma, a sobrevida depende do diagnóstico precoce. A biópsia do linfonodo sentinela tem contribuído na decis o de se realizar ou n o a linfadenectomia completa, e alguns autores já est o utilizando a mesma em crian as.

Abstract: the main purpose of a study is its publication on a scientific journal. research financing agencies are important institutions so that studies can be developed and published. the most important research financing agencies that are discussed in this article are: "coordena??o de aperfei?oamento de pessoal de nível superior" (capes), "conselho nacional de desenvolvimento científico e tecnológico" (cnpq) and "funda??o de amparo à pesquisa do estado de s?o paulo" (fapesp). capes' activities can be grouped in four different strategy lines: a) it evaluates the stricto sensu, at the post-graduation level; b) it provides access and development of scientific research; c) it provides investment on the development of high qualified human resources in brazil and abroad, and d) it promotes international scientific cooperation. although capes does not support directly scientific publications, almost all actions of this agency contribute to the development of scientific research and publication. cnpq has two main purposes: financing researches and development of human resources. it provides the researchers with financial aid to scientific publication. the grants for editing were specifically created for supporting the national scientific and technical publications edited by brazilians institutions or societies. cnpq can also support congresses, symposiums and similar short-term courses. the plataforma lattes is also a branch of cnpq on which the curriculum lattes is available. this site has the curriculum vitae of the scientific community and is of great value for researchers. fapesp also finances journal publications, articles and books that bring up original results of studies made by researchers from the state of s?o paulo. it finances, partially, the travel expenses of innovative papers authors in meetings within the country or abroad. brazilian authors are increasing the number of international publications. universities, research institutes, financing agencies and private com

Experimental model in rat for sentinel node biopsy

Abstract: although sentinel node procedure has been used world wide, there are many aspects to be defined and better standardized. this study address if the experimental model in rats is appropriate for sentinel node biopsy. in this model, the lymph nodes are showed by lymphoscintigraphy, they are dyed by patent blue and identified by intraoperative gamma probe detection. it isn?t necessary to use magnification for the procedure. the model demonstrated that sentinel node biopsy in rats is feasible. so, besides allowing researches in this field, the model is useful for training and diffusing this technique.
Recorrência em pacientes portadores de melanoma cutaneo submetidos a biópsia de linfonodo sentinela: seguimento mediano de 37 meses

Abstract: background: sentinel node biopsy (snb) has been considered as an advance in surgical oncology for microstading melanoma. we report our experience with this procedure focusing on recurrence. methods: snb was performed in 133 cutaneous melanoma patients submitted to preoperative lymphoscintigraphy, lymphatic mapping and gamma probe detection. histologycal samples were analysed by he and immunohistochemistry (ihc). results: the sentinel node was detected in 126 patients (96.2%). micrometastasis were diagnosed in 20 patients (15.6%). there were nine recurrence, four in negative sentinel node group (108 patients). in this group, there was one systemic recurrence and three (2.8%) on lymphatic drainage region (false negative). in positive sentinel node group (20 patients) there were five recurrences. the recurrence was lower among negative sentinel node patients (p=0.0048). ulceration (p=0.029) and positivity of the sentinel node (p=0.003) were considered significant risk factor by logistic analisys. only sentinel node positivity maintained significance on multivariate analysis (p=0.024). the median follow up was 37 months. conclusion: patients with positive sentinel node presented more recurrence than negative sentinel node patients. the false negative rate was 2.8% and patients didn't have long-term sequelae ,allowing us to consider snb as a secure and accurate procedure for melanoma staging.
