A responsabilidade profissional na assistência ao parto: discursos de enfermeiras obstétricas

Abstract: the objective of this qualitative, exploratory study was to identify obstetrics nurses' knowledge concerning professional delivery responsibilities. eleven nurses in hospital delivery wards and/or home birthing in santa catarina, brazil were interviewed from march - august, 2009 for data collection. after analysis, using discourse of the collective subject, central ideas emerged concerning themes related to relationships among obstetrics nurses, physicians, and institutions; professional responsibility; and legal and moral repercussions of errors. this study verified that nurses understand little about legal repercussions of potential delivery errors. assuming their role in assisting deliveries, they must dedicate their total attention to competence limits and predictable error prevention, maintaining that they will also assume responsibility for their failures. an opportunity to bring these nurses up-to-date concerning their legal responsibilities is as important as scientific and may contribute to greater professional self-confidence, creating more efficient overall nursing performance.

Abstract: the objective of this literature review is to identify how aspects related to legal responsibilities of the obstetrics nurse and the obstetrics nursing assistant have been approached in brazilian publications. after researching the following databases for the period of 1980 to 2009; bdenf, cinahl, lilacs, and scielo, seven articles which dealt with judicial aspects were selected for this study. it was clear that none of these focused upon legal responsibilities for obstetrical nurses. error prevention was highlighted among the majority of these publications, as well as civil and ethical responsibilities. however, the majority point out administrative sanctions and penal responsibilities. these aspects need to be researched in greater depth and reported upon, for they may contribute to more qualified formal education for nurses concerning the legal implications of their practice.

Abstract: the debate over the differences between quantitative and qualitative methods is frequent, holding favorable and opposite positions concerning their integration. outlining a research that contemplates both approaches generates doubts and restlessness about how to use them without damaging the methods' rigor, specificity, as well as the methodological and reflective sophistication of each. the purpose is to report and discuss using the quantitative (randomized controlled clinical trial) and the qualitative approach to analyze and understand the practice of including a companion chosen by the woman during her labor and childbirth, performing the role of support provider. using both methods allowed for approximating the multiple facets involved in this practice and evaluating both the explicative dimension and the comprehension, since it could be performed with complementary views.

Abstract: the objective of this systematic review was to evaluate the maternal and neo-natal results generated from the utilization of non-pharmacological methods for pain alleviation concerning the strains of delivery, classified as light-to-hard technology. research was carried out in the following databases: cinahl, medline, lilacs, scielo, scopus, and isi web of science. this study is composed of 12 randomized eligible clinical tests published between 1980 and 2009 which evaluated immersion baths, massages, and aromatherapy. results show that the immersion bath should be initiated after 3 cm of dilation in order not to prolong the strains of delivery and harm neo-natal results. massage is efficient in alleviating anxiety, pain, and stress, being most effective in reducing pain when utilized in the beginning of the latent phase. aromatherapy diminishes anxiety and fear. it is necessary to establish parameters for the application of each method in order that maternal and neonatal results are positive and contribute to the woman's satisfaction.
Puerperal women's perceptions on vertical and horizontal deliveries

Abstract: this qualitative study aims to better understand the perceptions of puerperal women regarding their experiences in vertical and horizontal deliveries and identify positive and negative aspects of each position. semi-structured interviews were carried out with ten puerperal women hospitalized in the rooming-in unit of a teaching hospital. after thematic analysis - collective subject discourse - positive aspects of the vertical position emerged, namely: greater comfort, freedom of movement, reduction of the expulsive effort, favors women's participation. negative aspects were listed as discomfort and lack of obstetric intervention. positive aspects of the horizontal position were reported as quickness, feelings of security and of being helped. negative aspects were related to discomfort and difficulty in exerting strength. positive aspects of the vertical position and negative aspects of the horizontal position stood out more intensely and frequently, and are in accordance with scientific evidence.

Abstract: Se trata de una revisión sistemática con el objetivo de evaluar los resultados maternos y neonatales de la utilización de métodos no farmacológicos para aliviar el dolor durante el parto, que son clasificados como tecnología blanda y dura. Se realizó una búsqueda en las bases de datos CINAHL, MEDLINE, LILACS, SciELO, SCOPUS y el ISI Web of Science. Se incluyeron 12 ensayos clínicos aleatorios elegibles, publicados entre 1980 y 2009, en los que se evaluaron: el ba o de inmersión, el masaje y la aromaterapia. Los resultados mostraron que ba o de inmersión debe ser iniciado después de 3 cm de dilatación para no prolongar el trabajo de parto y afectar el resultado neonatal. El masaje es efectivo para aliviar la ansiedad, el dolor y el estrés, siendo más eficaz en la reducción del dolor cuando se usa temprano en la fase latente. La aromaterapia reduce la ansiedad y el miedo. Se deben establecer parámetros para cada método de aplicación en la que los resultados maternos y neonatales sean positivos y contribuyan a la satisfacción de las mujeres.

Abstract: This work shows how students of UFSC s Master s Course in Nursing Assistance, 96/1 class, succeeding in keeping the group together and mentally healthy during the elaboration phase of the conclusion s course work. In monthly meetings in locals that were chosen by the group in advance, we had informality and happy moments, with a facing strategy, group integration, experiences exchange and a reinforcement of the self . That even not having all the group participation in the meetings, it has being possible to build a relationship based on friendship, companionship, respect, and the valuing of others, which has been contributing in our healthy living process.
Evidências sobre o suporte durante o trabalho de parto/parto: uma revis?o da literatura

Abstract: the effects of support for women during labor and delivery provided by health professionals, lay women, and doulas on the maternal and neonatal outcomes have been evaluated through randomized clinical trials, meta-analyses, and systematic reviews. this article presents a review of these studies, focusing on the principal characteristics, support provider, simultaneous presence of the woman's spouse and/or family members during labor and delivery and the outcomes. the analysis included studies published from 1980 to 2004 which explicitly approached these aspects. in general, the results of such support were favorable, highlighting a reduction in the cesarean rate, analgesia/ medication for pain relief, duration of labor, and utilization of oxytocin and an increase in maternal satisfaction with the experience. the benefits were greater when the support provider was not a health professional. the available studies did not evaluate the specific companion chosen by the woman as a support provider, which constitutes a gap in the knowledge that should be filled by future research.

Abstract: objective: to understand health care providers' perception on managing laboring women in the presence of a labor companion of their choice, and the labor companion's perception of this experience. methods: a qualitative study was conducted based on a controlled randomized clinical trial. sampling was intentional and determined through information saturation. semi-structured interviews were carried out with 11 health care providers and 16 laboring companions in the obstetric unit of a maternity facility at the hospital complex, in campinas, southern brazil, between october 2004 and march 2005. the thematic analysis of discourse was applied using the following methodological figures: central idea, key expressions and discourse of the collective subject. results: the most remarkable central ideas of health care providers were: no difference was observed in managing laboring women with a labor companion; positive changes were noted in labor management when a labor companion was present; the companion provided emotional support to laboring women who were more pleased, and felt safer and peaceful; many positive aspects were seen in the behavior and involvement of laboring women with a labor companion; the companion caused no problems and encouraged health providers to engage in a more humane and less routine attitude. the main central ideas of labor companions were: positive feelings, emotions, a sense of satisfaction with the experience; being with the laboring woman was a great opportunity to provide her emotional support; and they felt welcome by health care providers. conclusions: health providers considered positive the support provided by a labor companion and had no problems in managing laboring women in the presence of their companions. labor companions were pleased and happy with this experience. there was no conflicting opinions.
