Rhino-Orbito-Cerebral Mucormycosis in a Diabetic Patient with Idiopathic CD4 + Lymphocytopenia  [PDF]

Abstract:

A 54-year-old diabetic male, with idiopathic CD4 + lymphocytopenia, was presented with a two-month history of headache and periocular pain followed by vertigo, left hypoacusia and left peripheral facial palsy and hypoesthesia. More than a month after admission, a palate ulcer appeared and Infectious Diseases consultation was required. Mucormycosis was suspected and the diagnosis was confirmed by histologic examination. Despite early surgery and combination antifungal therapy, the patient did not survive. This case report illustrates the difficulty in diagnosing a rare condition with non-specific clinical manifestations and underlines the importance of a timely multidisciplinary approach in order to recognise this highly fatal disease earlier. It also describes a previously non-reported situation of mucormycosis in a patient with idiopathic CD4 + lymphocytopenia.

Abstract: placenta vascular lesions are a group of distinct yet related entities that include chorangiomas and diffuse multifocal chorangiomatosis. chorangioma is an expansive nodular lesion with an incidence of about 1%. diffuse multifocal chorangiomatosis is rare (0.2%) and mostly seen in placentas before the 32nd gestational week. the authors present a case of a monochorionic/biamniotic twin pregnancy, in which, at the 26th gestational week, one fetus developed intrauterine growth restriction (iugr), hydrops, and anemia associated with a tumor of the placenta with increased vascularization in the doppler study. pathological examination of the placenta diagnosed diffuse multifocal chorangiomatosis. this rare case report of diffuse multifocal chorangiomatosis with prenatal manifestations resembling those of a chorangioma proves that prenatal ultrasound detection of a placenta tumor, with increased vascularization at doppler study, must raise other diagnostic possibilities beside chorangioma.

Abstract: the use of remote sensing in oceanography, has been largely increased, mainly because the synoptic look, the accuracy of the data as well as the economics aspects. this paper shows the features and the first results of some experiments performed in the offshore region near santos bay and s?o sebasti?o island, in the southeast coast of brazil. the technique seems to be powerfull in coastal circulation studies, as well as, in every process that uses the temperature as a natural tracer. the tide currents seem to have large influence in the area near the santos bay, with a surface mean speed of about 1.2 m/s, which can reach distances of about 50 n.m. one can note that the area can be divided, at least in surface terms, in two subareas, with different thermal behaviours, with the interface standing normal to the coast in front of santos.

Abstract: the aim of this study was to examine the relative associations between proactive coping skills, emotional distress, selfesteem, functioning and quality of life among persons diagnosed with schizophrenia. twenty three persons diagnosed with schizophrenia were assessed with the following instruments: quality of life scale from the world health organization, life skills profile, proactive coping inventory, anxiety, depression and stress scale, and rosenberg self-esteem scale. through correlative analysis we found that proactive coping, reflective coping, strategic planning and preventive coping correlated positively with different dimensions of functioning and quality of life. avoidant coping wasn?t correlated with any functional outcome. we also didn?t found significant correlations between distress symptoms, functioning and quality of life, excepting depressive symptoms, which were correlated with communication skills and social relationships, and stress symptoms, which significantly correlated with the psychological domain of quality of life. self-esteem was positively associated with several domains of quality of life but not with objective dimensions of functioning.

Abstract: Metastases are synonymous of malignant neoplasm in advanced stage and so associated with a poor prognosis. As the first manifestation of cancer disease, the muscle metastasis is very rare. The definitive diagnosis of its presence is made based on histology. The authors presented the case of a patient with a lung adenocarcinoma, being with first manifestation of it, a muscle metastasis only about a month later there were clinical manifestations with imaging appearance that led to the diagnosis of a primary neoplasm. The rare manifestation of the disease motivated the study and surveillance of the patient and lead to the diagnosis.

Abstract: INTRODU O: A associa o de dilata o da papila de Vater com bal o TTS-CRE (through the scope-controlled radial expansion) com esfincterotomia (ETE) tem sido usada para remo o de cálculos de grandes dimens es da via biliar principal (VBP). OBJECTIVOS: Apresenta o da casuística do nosso Servi o durante doze meses, em que se procedeu a ETE seguida de dilata o papilar com bal o para remo o de cálculos da VBP. Pretendeu-se avaliar a eficácia na remo o dos cálculos, a necessidade de litotrí-cia mecanica e a ocorrência de complica es. MéTODOS: Entre Outubro de 2009 e Setembro de 2010 foi realizada dilata o papilar com bal o precedida de ETE em 25 doentes. Os doentes incluídos apresentavam no colangiograma por colangiopancreatografia retrógrada endoscópica (CPRE) cálculos na VBP com dimens o superior ou igual a 10mm. Usaram-se bal es TTS-CRE de 12 a 20mm de diametro. RESULTADOS: Dos 25 doentes, 12 (48%) eram do sexo feminino. A média de idades foi de 73 anos e 65% dos doentes apresentavam dois ou mais cálculos. O tamanho médio dos cálculos foi de 15mm. Seis doentes apresentavam litíase residual do colé-doco com ETE prévia. Em 100% dos casos foi conseguida remo o completa dos cálculos numa única sess o. N o houve necessidade de litotrícia mecanica. N o se registaram complica es. CONCLUS O: A associa o dos dois métodos mostrou-se eficaz, rápida e segura na remo o de cálculos de grandes dimens es da VBP. INTRODUCTION: Papillary balloon dilation with TTS-CRE balloon (through the scope-controlled radial expansion) in association with sphincterotomy (EST) has been used for removal of large bile duct stones. OBJECTIVES: We present the 12 month experience of our Department wherein we performed a EST followed by papillary balloon dilation for removal of bile duct stones. Efficacy, need for mechanical lithotripsy and occurrence of complications were evaluated. METHODS: Papillary balloon dilation preceded by EST was performed in 25 patients from October 2009 to September 2010. Patients included in the present study had bile duct stones with a diameter greater than or equal to 10mm detected on cholangiogram obtained during endoscopic retrograde cho-langiopancreatography (ERCP). We used TTS-CRE balloons with 12 to 20mm. RESULTS: Of the 25 patients, 12 (48%) were female. Average age was 73 years and 65% of patients had two or more stones. The average size of the stones was 15mm. Six patients had residual bile duct stones with prior EST. In 100% of cases complete removal of the bile duct stones was achieved in a single session.
