Abstract: INTRODU O: A associa o de dilata o da papila de Vater com bal o TTS-CRE (through the scope-controlled radial expansion) com esfincterotomia (ETE) tem sido usada para remo o de cálculos de grandes dimens es da via biliar principal (VBP). OBJECTIVOS: Apresenta o da casuística do nosso Servi o durante doze meses, em que se procedeu a ETE seguida de dilata o papilar com bal o para remo o de cálculos da VBP. Pretendeu-se avaliar a eficácia na remo o dos cálculos, a necessidade de litotrí-cia mecanica e a ocorrência de complica es. MéTODOS: Entre Outubro de 2009 e Setembro de 2010 foi realizada dilata o papilar com bal o precedida de ETE em 25 doentes. Os doentes incluídos apresentavam no colangiograma por colangiopancreatografia retrógrada endoscópica (CPRE) cálculos na VBP com dimens o superior ou igual a 10mm. Usaram-se bal es TTS-CRE de 12 a 20mm de diametro. RESULTADOS: Dos 25 doentes, 12 (48%) eram do sexo feminino. A média de idades foi de 73 anos e 65% dos doentes apresentavam dois ou mais cálculos. O tamanho médio dos cálculos foi de 15mm. Seis doentes apresentavam litíase residual do colé-doco com ETE prévia. Em 100% dos casos foi conseguida remo o completa dos cálculos numa única sess o. N o houve necessidade de litotrícia mecanica. N o se registaram complica es. CONCLUS O: A associa o dos dois métodos mostrou-se eficaz, rápida e segura na remo o de cálculos de grandes dimens es da VBP. Efficacy, need for mechanical lithotripsy and occurrence of complications were evaluated. METHODS: Papillary balloon dilation preceded by EST was performed in 25 patients from October 2009 to September 2010. Patients included in the present study had bile duct stones with a diameter greater than or equal to 10mm detected on cholangiogram obtained during endoscopic retrograde cho-langiopancreatography (ERCP). We used TTS-CRE balloons with 12 to 20mm. RESULTS: Of the 25 patients, 12 (48%) were female. Average age was 73 years and 65% of patients had two or more stones. The average size of the stones was 15mm. Six patients had residual bile duct stones with prior EST. In 100% of cases complete removal of the bile duct stones was achieved in a single session. Abstract: on the basis of a questionnaire survey carried out among portuguese children, this article aims to characterise their possession and usage of the internet. the survey ascertained that the children can be grouped under four distinct user profiles - ?confirmed cybernauts?, ?applied students?, ?inveterate gamers? and ?beginners? - and demonstrates that these profiles are highly conditioned by variables such as a child?s sex and age and his or her parents? educational levels and socio-professional categories. these profiles also correspond to different forms of parental mediation and different representations regarding the internet.

Abstract: O tumor de Buschke-L wenstein (TBL) é uma variante rara de condiloma acuminatum que possui comportamento maligno pelo crescimento local, mas n o apresenta potencial metastático. Afeta primariamente a área genital e, menos frequentemente a regi o perianal. Os fatores de risco para o seu desenvolvimento incluem a terapêutica imunossupressora e infe o VIH. Possui elevada taxa de recorrência e de transforma o maligna, sendo a cirurgia o tratamento inicial recomendado. Apresenta-se um caso de TBL perianal e anal num doente com infe o VIH cuja histologia n o revelou transforma o maligna e para o qual a terapêutica cirúrgica isolada foi eficaz. Revê-se o conhecimento atual sobre esta entidade tendo em aten o a história natural nos indivíduos infetados pelo VIH. The Buschke-L wenstein tumor (TLB) is a rare variant of condyloma acuminatum that presents local malignant behavior but no metastatic potential. It affects primarily the genital area and less frequently the perianal region. Risk factors for its development include immunosuppressive therapy and HIV infection. This tumor has a high rate of malignant transformation and the recommended initial treatment is surgery. We present a case of perianal and anal TBL in a HIV patient whose histology revealed no malignant transformation and that was treated successfully with surgery alone. We review the current knowledge about this entity taking into account its natural history in individuals infected with HIV.

Probabilistic Key Management Practical Concerns in Wireless Sensor Networks

Abstract: The subject of Key Management in Wireless Sensor Networks has gained increased attention from the security community around the world in the last years. Several proposals were made concerning the peculiarities of resource constrains inherent to sensor devices. One of the most accepted proposals is based on random distribution of keys among the sensor nodes, which was followed by some variants in order to increase its security. In this paper we introduce the mathematical concepts behind this class of proposals through a step-by-step mathematical analysis. This leads to some practical concerns about its applicability to real world applications where the technological constrains strictly compromise the mathematical theoretical models. We demonstrate that the number of communication links needed to assure near 100% network connectivity, is in fact, impractical in nowadays applications.
