Prolapso de tuba uterina após histerectomia vaginal: relato de caso

Abstract: O prolapso de tuba uterina é complica o rara após histerectomia, com aproximadamente 80 casos descritos na literatura. A sintomatologia é inespecífica, podendo incluir sangramento genital, dispareunia e dor pélvica cr nica. O diagnóstico diferencial deve ser feito com granuloma de cúpula vaginal e carcinoma de vagina. O tratamento deve ser individualizado, podendo ser realizado por via vaginal, abdominal ou laparoscópica. Relatamos o caso de uma paciente, 47 anos, com miomatose uterina, submetida a histerectomia vaginal, evoluindo com prolapso de tuba uterina após 11 meses de pós-operatório. O exame especular evidenciava les o vegetante, friável e sangrante localizada na cúpula vaginal. Esses achados clínicos sugeriam o diagnóstico de prolapso de tuba uterina. A paciente foi submetida a nova interven o cirúrgica, com ressec o da tuba uterina por via vaginal. O exame natomopatológico confirmou o diagnóstico e a paciente evoluiu com remiss o completa da sintomatologia.
Pubovaginal sling in the treatment of stress urinary incontinence for urethral hypermobility and intrinsic sphincteric deficiency

Abstract: PURPOSE: This study was undertaken to evaluate the use of pubovaginal sling for the treatment of female stress urinary incontinence in patients with intrinsic sphincteric deficiency and patients with urethral hypermobility. MATERIALS AND METHODS: Sixty-two patients aging 22 to 73 years-old (mean = 49.6) with a median parity of 4.1 (range 0 - 14) who underwent pubovaginal autologous fascial sling procedures for stress urinary incontinence from August/1999 to August/2002 were prospectively analyzed. Objective pre and postoperative urodynamic evaluation was performed in all cases. The patients were divided into 2 groups: thirty-nine patients (62.9%) with urethral hypermobility (Valsalva leak point pressure equal or superior to 60 cm of H2O) and twenty-three patients (37.1%) with intrinsic sphincteric insufficiency (Valsalva leak point pressure below 60 cm of H2O). RESULTS: The average follow-up period was 24.8 months, ranging from 3 to 38 months. Three patients (4.8%) had detrusor overactivity before the operation, and 36 patients (58.1%) had voiding dysfunction before surgery. The postoperative objective cure rate was 88.7% for stress urinary incontinence. The study also showed that 32.2% of the patients had voiding dysfunction and 11.3% had detrusor overactivity. The mean hospital stay was 3.1 days (range 2 - 4). No difference in the above parameters was noticed between patients with intrinsic sphincteric deficiency and those with urethral hypermobility. CONCLUSION: Construction of a pubovaginal sling is an effective technique for the relief of severe stress urinary incontinence, for both patients with urethral hipermobility and with intrinsic sphincteric deficiency, having a cure rate of 88.7%. The high frequency of postoperative voiding urgency was not related to the detrusor overactivity as evaluated by urodynamic studies.

Abstract: fallopian tube prolapse is a rare complication after hysterectomy, with approximately 80 cases described in the literature. the symptoms are nonspecific including vaginal bleeding, dyspareunia and chronic pelvic pain. the differential diagnosis must be done with granulation tissue of the vaginal cuff and vaginal carcinoma. the treatment should be individualized, and is possible to be done by vaginal, abdominal or laparoscopic approach. we report a case of a 47-year-old woman with myoma who was submitted to a vaginal hysterectomy and evolved with fallopian tube prolapse 11 months after surgery. specular examination showed a fungating, friable and bleeding lesion at the vaginal cuff. the clinical findings suggested the diagnosis of fallopian tube prolapse. the patient was submitted to a new surgical intervention with resection of the left fallopian tube. the histologic examination confirmed the diagnosis and the patient evolved with complete remission of the symptoms.

Abstract: OBJETIVO: avaliar os resultados do uso do pericárdio bovino na cirurgia de sling pubovaginal para o tratamento da incontinência urinária genuína de esfor o. MéTODOS: avalia o prospectiva de cinco pacientes, com diagnóstico de incontinência urinária genuína de esfor o submetidas à cirurgia de sling pubovaginal com utiliza o de faixa de pericárdio bovino, no período de outubro/2001 a dezembro/2001. A média de idade foi de 48,2±11,5 anos (33-69 anos). RESULTADOS: o tempo cirúrgico médio foi de 45±35,3 minutos, com média de 36±12,4 horas de interna o (24-48 horas). N o ocorreram complica es per-operatórias ou no período pós-operatório recente. Todas as pacientes apresentaram resultado inicial satisfatório, apresentando mic es normais e sem perda. Ocorreram complica es pós-operatórias em todas as pacientes, sendo evidenciadas deiscência da ferida operatória vaginal com expuls o total da faixa em duas pacientes e exterioriza o de parte da faixa em três pacientes. Todas as pacientes evoluíram com incontinência urinária de esfor o, sendo submetidas à nova cirurgia de sling, com a utiliza o de fáscia do reto abdominal. Após a segunda cirurgia as pacientes evoluíram sem intercorrências e com melhora da perda involuntária de urina em quatro delas. CONCLUS O: o sling pubovaginal com uso de pericárdio bovino associa-se a altas taxas de complica es, n o devendo ser mais utilizado no tratamento da IUE.
Necrosante periodontal disease: ulcerative gengivitis necrosante – case report

Abstract: This project proposes to discuss the clinical characteristics ofGUN and its differentiated diagnostics with the PUN under thenew optical classification of necrosante periodontal disease. Dealswith a clinical case of an 18 year old female patient, with symptomsof limited gingival necroses at the gingival vestibule edge and theinter-dental papilla in the upper back region, with painfulsymptoms, gum bleeding when touched, dental calculus, largequantity of visible bacterial plaque and bad breath not significant,with the absence of linfoadenopathy, fever, pseudomembrane, andno discomfort. During the discussion of the patient’s medicalhistory she reports that she is healthy, except for the possibledevelopment of emotional stress. The treatment plan isconservative (educational orientation on health, coronary scraping,and smoothing out of the root on the entire oral cavity, prophylaxis,topical chemical treatment, of the bacterial plaque with hydrogenperoxide H2O2 and oral hygiene monitoring utilizing the visibleindex of plaque – IPV – and the edge gum bleeding – ISG). Antibiotictherapy was not used. The treatment is weekly. At the end of thisperiod there is a complete reestablishing of the periodontal healthso, her return for preventive periodontal maintenance wasscheduled, in 4 months. For this reason, this case agrees with thedecision of the committee responsible for the elaboration of thenew classification for the periodontal disease for it to be placed inthe same category “necrosante periodontal disease”.

Abstract: purpose: to evaluate the results of the use of bovine pericardium in the pubovaginal sling procedure for treatment of stress urinary incontinence. methods: a prospective analysis of five patients who underwent pubovaginal sling with the use of bovine pericardium strip for stress urinary incontinence in the "hospital das clínicas of ufmg" from october/2001 to december/2001. the mean age was 48.2±11.5 years (33 to 69 years). results: the mean surgical time was 45±35.3 min and the mean hospital stay was 36±12.4 h (24 to 48 h). complications in the periperative or immediate postoperative period did not occur. all patients initially presented satisfactory results with normal voiding and without stress incontinence. postoperative complications occurred in the 5 patients (100%), with dehiscence of the vaginal wound and total expulsion of the strip in 2 patients (40%) and partial expulsion in 3 patients (60%). all patients presented stress urinary incontinence and were submitted to a new sling procedure using the rectus fascia. the patients then progressed without complications and with improvement of urinary continence in 4 patients (80%). conclusions: pubovaginal sling with the use of bovine pericardium was associated with high rates of complications. therefore, its use is not recommended in the treatment of stress urinary incontinence.

Abstract: the objective of this paper is to present a game theory analysis of the incentives a state member of the european union may have to disrespect the stability and growth pact (sgp), thus keeping an excessive deficit, in spite of the political and financial risks associated with the non-compliance. several games are analyzed: complete and incomplete information between a country and the european union and incomplete information between the union and two countries. this last case brings about an equilibrium that involves reputation building. the main conclusion is that the european union must be strong and enforce the pact?s sanctions, even if that imposes a high political cost. indeed, if the european union lacks the determination to compel the members to adjust their deficits, one may expect a general non-compliance of the sgp, which will lead to a fiscal collapse of the economic and monetary union.

Abstract: this article presents a game theoretic analysis of informality in the labor market based on an infinite dynamic game between employers and workers in which the judiciary system enters parametrically. we find two classes of subgame-perfect nash equilibria. in the first class, the worker first gets an informal contract but he eventually becomes a formal worker. we show that the more effective the judiciary system, the faster the labor relationship is formalized. in the second class informal labor is maintained, coupled with high work turnover. again, the more efficient the judiciary system, the less likely is that equilibrium.

Abstract: acute and latent infections with the brazilian la031 strain of aujeszky?s disease virus (adv) were established in mice. ultraviolet irradiated adv administered subcutaneously was a successful way to establish latent infection. the presence of adv was detected by pcr. two sets of 22-mer primers were synthesized and used to amplify gg glycoprotein gene sequences in acute and latent infected trigeminal nerve ganglia. the specificity of the amplification was verified by dot-blot hybridization.
