PRK versus LASIK para corre??o de miopia baixa e moderada

Abstract: purpose: to compare, through a prospective, randomized and bilateral study, the effectiveness, safety and predic-tability of photorefractive keratectomy (prk) and excimer laser in situ keratomileusis (lasik) for correction of low and moderate myopia. methods: seventeen patients (34 eyes) with preoperative refractional spherical equivalent (se) ranging from -2.00 to -6.00 diopters (d) were enrolled in this study. each patient received prk in one eye and lasik in the other. both eyes were treated by the same surgeon during the same operative session with the excimer laser visx 20/20b and the chiron automated corneal shaper microkeratome.results: preoperatively the mean se was the same for both groups (-3.57 ± 1.17d and -3.71 ± 0.98d, for prk and lasik groups, respectively). pain was reported only by the prk cases (8 eyes: 47.1%). the average follow-up was 14 ± 5 months (6 to 24) for the prk group, and 14 ± 6 months (3 to 24) for the lasik group. the mean residual refractive error was similar after both procedures (-0.34 ± 0.45d and -0.29 ± 0.52d, for prk and lasik, respectively). at 1 day after surgery, 15 lasik eyes (100%) and 2 prk eyes (13.3%) had an uncorrected visual acuity (ucva) 3 20/40 (n=15). at last follow-up, 15 eyes from the lasik group (88.2%) and 11 from the prk group (64.7%) reached a ucva 3 20/20 (n=17). the best spectacle corrected visual acuity remained unchanged in all cases. conclusion: both prk and lasik were found to be similarly safe, effective and predictable for the correction of low and moderate myopia. patients preferred lasik because of fast and painless visual recovery.
Sight-dependent quality of life and ophthalmic findings in a group of Brazilian patients with multiple sclerosis

Abstract: objective: to assess the visual quality of life in patients with multiple sclerosis (ms), and to observe whether this parameter could be correlated to the findings of the ophthalmologic examination. method: the translated and validated 25-item national eye institute visual function questionnaire (vfq-25) was used to assess the visual quality of life. clinical data on ms, visual acuity, visual fields, optic coherence tomography (oct) and disc cupping were used for assessing correlation with vfq-25. results: the mean vfq-25 value was 78.6±18.2% in 27 patients. vfq-25 did not correlate with patients' ages, with disability (edss), disease duration or medication use. visual acuity showed a relatively poor (<60%) correlation to vfq-25, while no correlation could be established between visual fields, oct and disc cupping with vfq-25. conclusion: ms patients present several alterations in their eyes and sight that cannot be assessed by isolated measures. ophthalmological examination of these patients must include many parameters not usually used in standard ophthalmologic consultations.

Abstract: purpose: to evaluate the changes due to aging in normal human lens density and thickness, and in anterior chamber depth. methods: using scheimpflug image processing, 120 eyes of 60 patients were examined. the patients were divided into 6 groups, namely, group i, 10 to 19 years old; group ii, 20 to 29 years old; group iii, 30 to 39 years old; group iv, 40 to 49 years old; group v, 50 to 59 years old; and group vi, 60 to 69 years old. anterior chamber depth, density and thickness of lens were analyzed. the densitometry was performed dividing the lens into five areas: anterior capsular (area 1), anterior cortical (area 2), nuclear (area 3), posterior cortical (area 4) and posterior capsular (area 5). these were correlated with age. results: the anterior chamber depth decreased and the lens thickness increased with age. the lens density also increased with age, except in area 5, where values decreased. there was a strong positive correlation between increasing age and the density in all lens areas (area 1: r = 0.78, p<0.001; area 2: r = 0.87, p<0.001; area 3: r = 0.88, p<0.001; and area 4: r = 0.70, p<0.001) except in the posterior capsular area, where correlation was negative (area 5: r = -0.62; p<0.001). in addition, there was a significant correlation between increasing age and lens thickness (r = 0.73; p<0.001). correlation between age and anterior chamber depth (r= -0.34; p<0.001) was negative. the correlation coefficient for lens thickness and anterior chamber depth was also negative (r = -0.60; p<0.001). conclusions: this study shows changes due to aging in human lens density and thickness as well as in anterior chamber depth using scheimpflug system. these findings should be considered when studying anticataractogenic drugs, cataractogenic potential of drugs and cataract patients.

Abstract: Objetivo: Avaliar altera es do cristalino relacionadas à idade, em olhos normais, analisando-se sua densidade e espessura, além da profundidade da camara anterior (PCA). Métodos: Foram estudados 120 olhos de 60 pacientes hígidos por meio do sistema Scheimpflug. Os pacientes foram distribuídos em 6 grupos: Grupo I, 10 a 19 anos; Grupo II, 20 a 29 anos; Grupo III, 30 a 39 anos; Grupo IV, 40 a 49 anos; Grupo V, 50 a 59 anos ; e Grupo VI, 60 a 69 anos. Foram analisadas as seguintes variáveis: profundidade da camara anterior, densidade do cristalino (DC) e espessura do cristalino (EC). Para avalia o da densidade, o cristalino foi dividido em 5 áreas: cápsula anterior, córtex anterior, núcleo, córtex posterior e cápsula posterior. Estas variáveis foram correlacionadas com a idade. Resultados: A profundidade da camara anterior diminuiu, ao passo que a espessura do cristalino aumentou com o envelhecimento. A densidade do cristalino aumentou com a idade em todas as áreas estudadas, exceto na cápsula posterior, onde ocorreu diminui o nos valores. Quando correlacionadas idade e densidade do cristalino observou-se forte correla o positiva nas seguintes áreas: cápsula anterior (r = 0,78; p<0,001), córtex anterior (r = 0,87; p<0,001), núcleo (r = 0,88; p<0,001) e córtex posterior (r = 0,70; p<0,001). A correla o entre idade e DC na área capsular posterior foi negativa (r = -0,62; p<0,001). Encontrou-se correla o significante entre idade e EC (r = 0,73; p<0,001). Os coeficientes de correla o para PCA e EC (r = -0,60; p<0,001) e PCA e idade (r = -0,34; p<0,001) foram negativos. Conclus es: Este estudo mostrou, por meio do sistema Scheimpflug, as altera es da profundidade da camara anterior, densidade do cristalino e espessura do cristalino decorrentes do envelhecimento. Deve-se ter aten o com estas mudan as relacionadas à idade na realiza o de estudos envolvendo drogas anticataratogênicas, drogas tóxicas ao cristalino e pacientes com catarata.
Ceratoplastia lamelar profunda com viscodissec??o da membrana de Descemet

Abstract: objective: to evaluate the effect of the deep lamellar keratoplasty technique in preserving descemet's membrane and the patient's endothelial cells, using corneoscleral donor button. methods: 14 lamellar keratoplasties were performed with viscodissection of the receptor's descemet's membrane using 4% chondroitin sulphate and 3% sodium hyaluronate. ten (10) patients presented advanced keratoconus, 3 had herpes simplex leucoma and 1 had corneal irregularity due to a previous radial keratotomy. postoperative follow-up varied from 12 to 48 months (average 24 ± 10.5 months). results: after all sutures had been removed, the spherical equivalent on the last ophthalmic examination was -2.0 ± 3.6 diopters (d) (-10,3 d to +4,74 d). final astigmatism varied from -6,0 cd to -0,75 cd with an average of -3,3 ± 1,9 cd. twelve of 14 patients presented a best spectacle corrected visual acuity of 20/40 or better on the last examination. all patients gained snellen lines. postoperatively there was no corneal edema, endothelial decompensation, rejection, interface opacity nor deposits. two patients developed descemet's folds accounting for low visual acuity. conclusion: in spite of technical difficulties related to lamellar keratoplasty and viscodissection of descemet's membrane, we believe that this is a procedure of choice in patients whose endothelial cells population is preserved. this technique results in better final visual acuity when compared to the traditional lamellar transplant, and less immunologic reaction when compared to penetrating keratoplasty. because the receptor's endothelium was preserved, surgeries resulted in transparent corneas, regardless of the donor's tissue quality.

Abstract: Objetivo: Avaliar a eficácia da técnica de ceratoplastia lamelar profunda preservando a membrana de Descemet e as células endoteliais do paciente, com a utiliza o de bot o esclero-corneano. Métodos: Foram realizadas 14 ceratoplastias lamelares com viscodissec o da membrana de Descemet do receptor usando sulfato de condroitina 4% e hialuronato de sódio 3%. Dez (10) pacientes apresentavam ceratocone avan ado, 3 tinham leucoma por herpes simples e em 1 havia irregularidade corneana pós-ceratotomia radial. O acompanhamento pós-operatório variou de 12 a 48 meses (média 24 ±10,5 meses). Resultados: Após a retirada de todas as suturas a média do equivalente esférico no último exame oftalmológico foi de -2,0 ± 3,6 dioptrias (D) (-10,3 D a +4,74 D). O astigmatismo final variou de -6,0 DC a -0,75 DC com média de -3,3 ± 1,9 DC. Dos 14 pacientes 12 apresentaram na visita final acuidade visual com corre o de 20/40 ou melhor. Todos os pacientes ganharam linha de vis o pela tabela de Snellen. N o houve presen a de edema corneano, descompensa o endotelial ou rejei o. Nenhuma opacidade ou depósito na interface foi observado. Dois pacientes apresentaram dobras na membrana de Descemet com baixa da acuidade visual. Conclus o: Apesar das dificuldades técnicas relacionadas à ceratoplastia lamelar e viscodissec o da membrana de Descemet, acreditamos que este seja um procedimento de escolha em pacientes cuja popula o de células endoteliais esteja preservada. Esta técnica resulta em melhor acuidade visual final quando comparada ao transplante lamelar tradicional e menor rea o imunológica quando comparada a ceratoplastia penetrante. Independentemente da qualidade do tecido doador, conseguimos córneas transparentes como resultado, pois o endotélio do receptor foi preservado.
Analysis and Design of Thermally Actuated Micro-Cantilevers for High Frequency Vibrations Using Finite Element Method  [PDF]

Abstract: Vibrational behavior of thermally actuated cantilever micro-beams and their mechanical response at moderately high frequency under a non-harmonic periodic loading is studied in this paper. Two different configurations are considered: 1) a straight beam with two actuation layers on top and bottom which utilizes the bimorph effect to induce bending; 2) a uniform beam with base excitation, where the beam is mounted on an actuator which moves it periodically at its base perpendicular to its axis. Generally, vibrating micro-cantilevers are required to oscillate at a specified frequency. In order to increase the efficiency of the system, and achieve deflections with low power consumption, geometrical features of the beams can be quantified so that the required vibrating frequency matches the natural frequencies of the beam. A parametric modal analysis is conducted on two configurations of micro-cantilever and the first natural frequency of the cantilevers as a function of geometrical parameters is extracted. To evaluate vibrational behavior and thermo-mechanical efficiency of micro-cantilevers as a function of their geometrical parameters and input power, a case study with a specified vibrating frequency is considered. Due to significant complexities in the loading conditions and thermo-mechanical behavior, this task can only be tackled via numerical methods. Selecting the geometrical parameters in order to induce resonance at the nominal frequency, non-linear time-history (transient) thermo-mechanical finite element analysis (using ANSYS) is run on each configuration to study its response to the periodic heating input. Approaches to improve the effectiveness of actuators in each configuration based on their implementation are investigated.

Abstract: Assessment of seismic safety of the nuclear power plants requires knowledge of plant fragilities. In the paper, preliminary analysis is made on use of the cumulative absolute velocity in modelling of fatiguetype seismic damage. The dependence of the cumulative absolute velocity on the strong motion parameters is analysed. It is demonstrated that the cumulative absolute velocity is an appropriate damage indicator for fatigue failure mode. Failure criteria are defined in terms of cumulative absolute velocity using various fatigue failure theories.
Children's Perception Of Parenting Styles In Relation To Schooling Background

Abstract: A parenting style is a psychological construct representing standard strategies that parents use in their child rearing. Parents create their own style from a combination of factors, and these may evolve over time as the children develop their own personalities and move through life's stages. A total sample of 210 children in the age group of 8-10 years were chosen for the study which was equally divided among 3 different schools i.e. Municipal, Private, & Aided with the help of purposive sampling. Aself- structured questionnaire was prepared on children's perception on parenting styles with 10 hypothetical statements. The tool was validated and the reliability of the tool was found Father: r = 0.979, Mother: r= 0.983. The data was quantitatively analyzed. The findings indicate that the schooling background doesn't affect the parenting styles. Most of the parents use authoritative parenting styles, want their children to be responsive and mature, they develop a sense of independence in their children, but still have limits and consequences.
