Benign afebrile cluster convulsions with gastroenteritis: an observational study

Abstract: We report the occurrence of such afebrile convulsions in association with viral gastroenteritis without dehydration or electrolyte imbalance, over a 3-year period, in a cohort of 14 British children.The children (5 males and 9 females, 10 Caucasians and 4 Asians) were aged 9 to 60 months (median 14.5 months). All 14 had a normal neurological examination and normal serum biochemistry. Twelve children had generalised seizures and 2 had, in addition, absence seizures. The number of seizures per child ranged from 1 to 8. Most convulsions were short with 85.7% of children having the longest seizure not longer than 4 minutes. The longest duration for a seizure was 10 minutes and occurred in 2 children. Convulsions did not recur after the first day in 10 children, 3 children had recurrences the second day and one child on the fourth day. No convulsions recurred after 4 days.Cerebrospinal fluid studies, computed tomography and electroencephalogram (EEG) were performed on two children who had prolonged seizures and the results were normal. No pathogenic bacteria were grown in any of the stools. Enzyme immunoassay detection of Rotavirus in the stools was positive in 7 of the 10 children where it was tested. All 14 children recovered spontaneously within a few days. On long-term follow of up to 31 months (median 16 months), none had further convulsions and all had normal development milestones.Afebrile seizures in association with viral gastroenteritis do also occur outside Asia. Recognition of this entity should lead to reassurance of the parents. As in previously published series, investigations such as lumbar puncture, neuroimaging and EEG are usually normal and may not be necessary in most cases. Likewise, published data indicate that long-term anticonvulsant therapy is not usually warranted and the prognosis seems to be reassuring.Febrile seizures are common in children between the age of 6 months and 6 years and carry a benign prognosis. Abstract: this is the first report of a series on foraminifera of brazil. the animals were collected on the brazilian continental shelf and only the miliolidae, peneroplidae and alveolinellidae were taken into consideration in this paper. forty one species were described, belonging to the following genera: quinqueloculina d'orbigny, articulina d'orbigny, massilina schlumberger, pyrgo defrance, pyrgoella cushman & white, biloculinella wiesner, spiroloculina d'orbigny, sigmoilina schlumberger. ilauerina d'orbigny, flintia schubert, triloculina d'orbigny, peneroplis montfort, archaias montfort, sorites ehrenberg, borelis montfort. two regions were established from the observation of the distribution of the miliolidae, peneroplidae and alveolinellidae. the animals collected from 23o s northwards were captured in warm tropical waters and those found on banco s. tomé, barra de s. jo?o, and trindade island are the same which occur in the west indies. according to ekman (1953, p. 4) the coral regions are not always formed by animals. they may also be formed by algae belonging to the genus làtliothamnion. the samples i, ii, and iii were collected in such coral formations. the following species occurred from this southern limit northwards : articulina multilocularis, a. sagra, a. sulcata, hauerina bradyi, h. ornatissima, quinqueloculina funafutiensis, o. tricarinata, miliolinella oblonga, massilina crenata, spiroloculina antillarum, s. caduca, triloculina planciana, peneroplis bradyi, p. carinatus, p. pertusus, p. proteus, archaias angulatus, sorites marginalis and borelis pulchra.

Abstract: a description is given of dentostomina besnardii, new species, from banco de s?o tomé (lat. 22o 08' s - long 40o 39' w) and trindade (lat. 20o 33' s - long. 29o 22' w), coast of brazil. the species is similar to d. guraboensis bermudez, 1949, differing from it by the position of the bifid tooth and the size of the grains which form the test.

Abstract: the aim of this study was to analyze delivery care by nurses in six public hospitals of the east zone in the municipality of s?o paulo. a descriptive-exploratory study was carried out from october 2006 to june 2007, with nursing leaders and 38 nurses who answered the questionnaire, indicating frequency they performed delivery care related activities, as well as any possible obstacles or difficulties to exercise their functions. the results showed that nurses had no means to operationalize their work due to institutional and organization barriers that they had to deal with. it was concluded that to completely perform their work regarding delivery care, nurses must count with more favorable structures and to strengthen their professional identity, thus assuming greater qualification and power.

Abstract: the objective of this study was to analyze the exercise of competences in prenatal care performed by nurses in the east zone of the city of s?o paulo through the identification of the activities performed by them and their frequency as well as possible difficulties found. this quantitative, descriptive-exploratory study was carried out in 59 health centers, from october 2006 to december 2007, with a sample of 131 nurses. the results showed that nurses did not perform essential competences necessary to obtain a qualified prenatal care due to personal and institutional barriers found in their job. it was concluded that there is a need to review the policies of public health structures in s?o paulo to guarantee the implementation of the unique health system guidelines regarding the improvement of prenatal care and to provide human and financial resources to reach this purpose.

Abstract: descriptive and exploratory research carried out from october, 2006 to december, 2007 with the analyzing the exercise of midwifery competences by non-physicians working in the public healthcare services of the municipality of s?o paulo. this was achieved by means of the characterization of the services of the professionals, the identification of the activities they performed during such assistance and their frequency, as well as any possible obstacles or difficulties they faced to exercise their functions. the field of study was all the public health institutions with midwifery care provided by non-physicians in the east zone of the city of s?o paulo, that is, 59 primary care units and six hospitals. the sample was composed of 131 nurses that worked in the prenatal assistance of the primary care units and 42 nurses who worked in the hospitals: six nursing coordinators and38 nurses who provided care directly for women during labor. the data were collected through a form for the coordinators and two questionnaires, one for antenatal and post-delivery caregivers and another for delivery caregivers. the results showed that the non-physicians who assist women during pregnancy, delivery and post-delivery do not put into practice the essential competences for midwifery care because they met institutional and personal resistances and also due to the of protocols based on best practices to the application of the humanized, woman-centered care model. the conclusions show that the public structures of s?o paulo need to review their policies, so as to assure the improvement in maternal and child care .
Occurrence and recording of genitourinary tract infections during pregnancy

Abstract: A quantitative descriptive research, whose objective was to verify the occurrence and recording of genitourinary tract infections in pregnant women. According to the data collected through interviews with 100 puerperae of a philanthropic maternity hospital and analyses of their prenatal care cards, 51% of the women reported having had urinary tract infection during pregnancy. Of these, only 70% reported having been treated with antibiotics and 47% having repeated urine tests after the treatment. Of the total cases of reported urinary tract infections, only 41% had been entered on the prenatal care cards, which in only 25% of the cases indicated uroculture as being the test of cure. Of the 8% of women who reported genital infections, 50% had been treated with vaginal cream. Concerning data entries, 11% of the prenatal care cards indicated genital infection and 64% of those did not report any treatment. All the data demonstrated high percentages of both failure to record data and inadequate knowledge of the women as to the actual presence of such infections, the need for treatment and the proper follow-up. It was concluded that health care professionals need to reevaluate the way they assist patients during the prenatal care period, especially regarding the women’s information and the complete recording of data on the prenatal care card.

Abstract: this study aims at describing the design used in the evaluation of technical nursing courses in s?o paulo state's education system by means of quantitative and qualitative institutional performance indicators. such indicators, established by former students and by the labor market, showed to be useful in the evaluation and demonstrated the need for increasing, by means of similar evaluation studies, parameters related to the academic staff's performance as well as that of practical teaching fields. it is concluded that the use of a reasonable number of indicators allowed the articulation among data and a better understanding of the evaluated institutions.
