Learner evaluation of an online continuing medical education course for general practitioners

Abstract: Societal expectations for quality medical care have made it imperative that doctors undergo Continuing Medical Education (CME) to keep abreast of new developments in medicine. Taking cognizance of the potential of ‘Online Learning’, the OUSL offered an online CME course on cardiovascular health for physicians.The course purchased from Monash University, Australia was updated and adapted to suit the Sri Lankan context. The course which was available in print format was converted into an online format on the learning management system ‘Moodle’. While designing the online format, the course was enriched by using audio visual material, discussion forums, self assessment activities and an online reflective journal. Online assessments included a variety of assessment methods. The course team functioned as tutor mentors to facilitate learning.All learners (n=21) participated in the evaluation of the course by completing an online questionnaire. The questionnaire had 16 closed questions and seven open ended questions. Responses to the closed questions were gathered on a four point Likert scale. Frequencies were computed for closed questions and content analysis was carried out for open-ended questions.Learners perceived interactive and collaborative learning as the most liked features. They agreed that the audiovisual material enriched the course and that sessions were well structured, easy to understand and interesting. Some disliked essays and reflective writing while others suggested more interaction and audio-visual material to improve the course. Problems faced by learners were mainly of a technical nature. Finally, learners expressed an overall satisfaction with the course and thought that online learning was a rewarding experience. Learner evaluation showed that pedagogy, social environment and technical underpinning should be properly utilised in optimising learning in an online learning environment. The course had provided an enjoyable, satisfying and useful experience for physicians who lack the opportunity of undergoing face to face CME.
Peptidoglycan and Bacterial DNA Induce Inflammation and Coagulation Markers in Synergy

Abstract: Bacterial compounds signal the presence of foreign pathogens in the innate immune system. These microbial components are key players in infectious diseases and implicate toll-like receptors in the activation of inflammation and coagulation. Nevertheless, the existence of a synergistic relationship between peptidoglycan and bacterial DNA on these two physiological responses has not been investigated. The present study reports new findings on the regulation of tumor necrosis factor alpha and tissue factor in peripheral blood mononuclear cells by peptidoglycan and bacterial DNA. These were found to induce tumor necrosis factor alpha and tissue factor simultaneously and in a synergistic manner. These findings provide a new proinflammatory and procoagulant mechanism likely to play a role in sepsis pathogenesis.

Abstract: The incidence of extrapulmonary tuberculosis has been increasing especially among immunocompromised patients. Mycobacterium tuberculosis skin infection is rare, accounting for 1.5% of all forms of the disease. We present a clinical case of 38 years old woman, Brazilian; with sexual risk behaviours that develops a sudden thoracoabdominal tumefaction of 4 - 5 cm. The investigation confirmed a metastatic tuberculous abscess in a patient with acquired immunodeficiency syndrome.

Abstract: the recommendation of sugarcane varieties that show superior performance in different locations is a challenge for breeders and crop managers due to the response of genotypes across environments. researches on the stability of sugarcane genotypes under a twelve-months planting cycle are scarce. the objectives of this work were to assess the magnitude of the genotype x environment interaction and identify stable genotypes under this cycle. phenotypic stability was estimated by two methods of analysis, linear regression coefficient and coefficient of variation. ten genotypes and two control varieties were evaluated for stalk height, stalk diameter, stalk number per meter, pol% cane and yield of cane and sugar in three locations during the years 2004 and 2005. the results showed that there was genotype x environment interaction for stalk diameter, number of stalk and yield of cane and sugar, indicating that genotypes ranked differently at individual location. number of stalk per meter was the biometric attribute more affected by the genotype x environment interaction. the genotypes iac87-3396, iac91-1099 and iacsp94-4004 can be recommended for planting under a twelve months cycle at jaú, piracicaba and s?o jo?o da boa vista regions of s?o paulo state, brazil.

Abstract: according to the world health organization, rational drug use means that patients receive medicines appropriate to their clinical needs, in doses that meet their individual requirements, for an adequate period of time, and at the lowest cost to them and their community. however, as far as one can see, the reality is very different. in brazil, at least, 35% of medicines are purchased for self-medication. medicaments are responsible for 27% of intoxications in brazil and for 16% of deaths due to poisoning are caused by medicaments. moreover, 50% of medicaments are prescribed, dispensed or used inappropriately and hospitals spend about 15 to 20% of their budgets to deal with the troubles caused by misuse of medicaments. the promise of immediate relief of suffering is a very attractive appeal, but it has its price. sometimes the price we pay is our health. rational drug use requires a very complex logical chain involving a number of variables and participation of different social actors including patients, health professionals, legislators, policy makers, industry, commerce and the government.

Abstract: the political culture of a peoples is a relevant aspect to understand the relationship they maintain toward its state, as well as their demands based on particular justice and democracy notions. in the bolivian case, my approach will privilege two factors that help understanding cultural and political orientation of the indigenous majority: (1) the argument around the right to cultivate coca leaf, even if it goes against international and national legislation; and (2) the present influence on political action standards based on ancestral principles of pre-colombian andean politics. my goal is to show how both long-lasting structures and the implementation of international anti-drug legislation influence the conformation of the bolivian political culture.

Abstract: the text discusses how "indians" are presented as distinct based on ways of looking as materialised in textbooks, stamps and postcards, etc. marking the pedagogical function of apparently banal artefacts. the article is divided into three thematic segments: the first is related to the diverse ways in which the 'other' is seen, the second, to the transposition of meanings attributed to objects and the third discusses the representation of indians by means of postcards, phonecards and national coins, as 'authentic' brazilians and patrimony of the nation. the framework that supports this approach is related to the field of cultural studies in a post-modern perspective. in this sense, the concepts articulated in these productions were analysed as a set of established discursive practices based on 'relations of power'.

Abstract: O presente trabalho tem como objetivo conceituar a media o da leitura literária para crian as, exaltando sua importancia na forma o da crian a, preparando-a para contribuir com a transforma o da realidade em que vive, e investigar as livrarias como espa o de media o, demonstrando aos profissionais da área que n o é somente a biblioteca que deve ser usado como espa o para que esta aconte a. Tem como metodologia – identificar as livrarias londrinenses que realizam a media o da leitura literária para crian as, bem como avaliar as atividades desenvolvidas nesse sentido, ou seja, hora do conto, saraus, atividades com sucatas etc., verificando quais as atividades voltadas para despertar nas crian as o desejo de ler. Em seu marco teórico, discute a media o da leitura literária, a rela o entre autor, obra e o leitor infantil. Além disso, defende a necessidade de o bibliotecário expandir sua atua o para além das fronteiras das bibliotecas, bem como efetivar uma das suas fun es primordiais que é a media o da leitura.
