Residual platelet reactivity during therapy with inhibitors of cyclooxigenase or adenosine diphosphate receptors

Abstract: Aim. To compare effects of acetylsalicylic acid (ASA) and two clopidogrel drugs on residual platelet aggregative reactivity (RPAR).Material and methods. Patients (n=40) with ischemic heart disease aged under 70 years were involved into the crossover study. Clinical examination included questionnaire survey, blood pressure (BP) measurement, ECG registration, 24-hour ECG and BP monitoring, determination of blood levels of total cholesterol, high density lipoproteins, triglycerides, transaminases, and creatinine, complete blood cell count, including platelets number and hemoglobin level. Besides evaluation of the platelet aggregation by optical aggregometry was performed initially, after one week ASA treatment and after every next 3 week clopidogrel treatment period.Results. RPAR during ASA monotherapy was 56.4±0.3%. There were no significant differences in effects of original and generic clopidogrel on RPAR. Сlopidogrel therapy reduced RPAR more significantly (42.2±0.2%) than ASA monotherapy did (p=0.0003). Authors proposed definition for high level of RPAR during therapy - it is platelet aggregation more than 46%. Data analysis taking into account this criterion showed that a number of patients with high RPAR was 70 and 30% among patients treated with enterosoluble ASA and clopidogrel, respectively.Conclusion. Study results show that a significant number of patients receiving antiplatelet monotherapy does not achieve the target level of RPAR(<46%). These results may be a rationale for combined therapy in patients of this type.
Tissue Doppler imaging of longitudinal movement of a fibrous ring of mitral valve during isovolumic periods in left ventricular hypertrophy

Abstract: Aim. To study change of rate and duration indicators of longitudinal movement of a fibrous ring of mitral valve (MFR) during isovolumic contraction (IVC) and relaxation (IVR) in hypertensive patients with various degree of a left ventricular hypertrophy (LVH).Material and methods. 80 hypertensive patients with moderate LVH (n=40) and severe LVH (n=40) are examined. The control group was presented by 30 healthy volunteers. Transthoracic echocardiography and Tissue Doppler imaging has been performed with ultrasonic tomograph “HDI 5000” (Philips).Results. Increase in LVH (Smm) and Е/Еmm associates with reduction in systolic velocity of movement of medial MFR (Smm). There is direct relation with duration of IVC-negative and IVR-positive components and myocardium mass index. Maximal velocity of IVC-positive component increases and maximal velocity of IVR-negative component decreases when LVH is growing.Conclusion. Velocities curves of IVC and IVR were bi-phase both in healthy persons and in hypertensive patients with LVH. Velocity and duration of positive and negative components of IVC and IVR depended on LVH degree.
Anti-inflammatory effect of laser therapy at complex treatment of the chronic bronchitis

Abstract: The research goal is to reveal influence of laser therapy on endobronchial changes and integrated indicator of inflammatory process in complex treatment of patients with chronic bronchitis. 64 patients with chronic bronchitis divided into 2 clinical groups have been under study: 33 patients with chronic purulent obstructive bronchitis and 31 - with chronic purulent nonobstructive bronchitis. Bronchial fibroscopic investigation of mucous membrane of bronchial tubes and biochemical research of blood have been performed to reveal endobronchitis degree and integrated indicator of inflammatory process. Indicators have been expressed in points: under 1 - moderated, 1-2 - expressed and more than 2 points - sharp changes. Results have demonstrated that the combination of drug therapy with endobronchial laser irradiation has an optimum anti-inflammatory effect indicated by decrease of endobronchitis degree - 1,12 and 0,81 points and integrated indicator - 1,75 and 0,75 (Р <0,01). That allows to recommend introduction of endobronchial laser irradiation into the complex therapy of chronic bronchitis

Phosphoric and Calcium Metabolism Abnormality as an Additional Factor in Anemia Genesis in Patients with End-stage Chronical Renal Failure

Abstract: The objective of the investigation is to study the relations of the main parameters of phosphoric and calcium metabolism and anemia in patients with an end-stage chronic renal failure (CRF). Materials and methods. There have been analyzed the characteristics of peripheral blood and the parameters of phosphoric and calcium metabolism in 108 patients with end-stage CRF. Results. In patients with end-stage chronic renal failure before the hemodialysis treatment, the severity of anemia correlates predominantly with the state of phosphoric and calcium metabolism. Anemia before the dialysis is to be treated not only by the preparations stimulating erythropoesis and iron preparations but also by the remedies correcting phosphoric and calcium metabolism abnormality, preferably without using calcium preparations.

Abstract: Living imaginatively: Imagination in the pastoral narrative therapeutic approach and the theology of Walter Brueggemann The article focuses on the relevance of Walter Brueggemann’s work for the pastoral narrative therapeutic approach. The concept of imagination and its use in both pastoral narrative therapy and Brueggemann’s work are described. Case studies are submitted as possible descriptions of how imagination in pastoral narrative therapy can be “funded” by Brueggemann’s insights. It is followed by cursory remarks on the possible relevance of Brueggemann’s use of the concept imagination for the pastoral narrative therapeutic approach.

Development of L. Kohlberg's idea of moral formation of cross-cultural universality of judgments in modern research

Abstract: We examine the ideas of foreign researchers (J. Gibbs, K. Basinger, J. Flavel, J. Snarey), who reconsider L. Kohlberg's statements on cognitive development, which forms the basis of moral judgments formation, recognizable usually across cultures. We show the intersection points for various contemporary approaches. We describe the results of empirical studies of moral judgments development in children of primary school age. The sample consisted of Moscow secondary schools students and students of National Jewish Education Center (total 367 people: 198 girls and 169 boys aged 8 to 10 years). We used method of presentation of moral dilemma situations. The study results confirm the relevance of L. Kohlberg main statements on the content of pre-conventional level of moral reasoning in children, on the laws of transition to conventional level. They also demonstrate the features of interpretation of crosscultural universality, discussed by modern foreign authors.

Special attention is paid to organoprotective lisinopril effects and lisinopril implementation at special clinical conditions (chronic obstructive pulmonary disease comorbidity, elderly patients, and concomitant liver diseases).Pharmacoeconomic aspects of lisinopril usage in arterial hypertension are also considered.
