Prevalence and Risk Factors of Osteoporosis in Postmenopausal Women with Rheumatoid Arthritis: A Brief Report

Abstract: Background: Low bone mass is a serious health problem mostly seen in postmeno-pausal women with rheumatoid arthritis. The purpose of this study was to determine the prevalence of osteoporosis and some related risk factors in postmenopausal women with rheumatoid arthritis.Methods: The data for this descriptive analytical study was extracted from the medical records of 98 postmenopausal women with rheumatoid arthritis who had attended the 5th of Azar Teaching Hospital affiliated to Gorgan University of Medical Sciences, in Iran, in 2009.Results: The mean durations of menopause and rheumatoid arthritis were 9.39 and 5.13 years, respectively. The overall prevalence of osteoporosis was 13.3%. We found a significant correlation between age, disease duration, and duration of menopause with bone mineral density (P<0.05).Conclusion: Our results indicate a high prevalence of osteoporosis at the lumbar spine of postmenopausal women with rheumatoid arthritis.
Project selection problem under uncertainty: An application of utility theory and chance constrained programming to a real case

Abstract: Selecting from a pool of interdependent projects under certainty, when faced with resource constraints, has been studied well in the literature of project selection problem. After briefly reviewing and discussing popular modeling approaches for dealing with uncertainty, this paper proposes an approach based on chance constrained programming and utility theory for a certain range of problems and under some practical assumptions. Expected Utility Programming, as the proposed modeling approach, will be compared with other well-known methods and its meaningfulness and usefulness will be illustrated via two numerical examples and one real case.
Minimizing the Time of Spam Mail Detection by Relocating Filtering System to the Sender Mail Server

Abstract: Unsolicited Bulk Emails (also known as Spam) are undesirable emails sent to massive number of users. Spam emails consume the network resources and cause lots of security uncertainties. As we studied, the location where the spam filter operates in is an important parameter to preserve network resources. Although there are many different methods to block spam emails, most of program developers only intend to block spam emails from being delivered to their clients. In this paper, we will introduce a new and efficient approach to prevent spam emails from being transferred. The result shows that if we focus on developing a filtering method for spams emails in the sender mail server rather than the receiver mail server, we can detect the spam emails in the shortest time consequently to avoid wasting network resources.

The relationship between coronary risk factors and coronary artery involvement based on angiogrpahy findings

Abstract: Introduction: Obstructive coronary diseases have been found to be the most important for theassessment of cardiovascular diseases. This survey was designed to determine the relationship betweenischemic risk factors and tract disease coronary artery.Materials and Methods: This cross-sectional study was conducted on 5442 patients who referred toGolestan’s Kosar center for angiography in the years 2008 to 2009. The outcome variable was the resultsof angiography, positive or negative. Risk factors including age, gender, body mass index (BMI), waistcircumference (WC), waist to hip ratio (WHR), blood pressure, diabetes mellitus, smoking and drugaddiction in the samples were evaluated and recorded for all patients. Multivariate analysis was performedby logistic regression.Results: We found significant differences between age, diabetic mellitus and hypertension status inpatients with positive coronary angiogram versus patient with negative coronary angiogram in both sexes.Also, regression analysis showed hypertension (OR=1.8), abnormal fasting blood sugar (OR=1.2), malegender (OR=2.2), WHR (OR=1.4) and opiates addiction (OR=1.3) increased significantly the probability ofcoronary arteries involvement.Conclusion: In patients with positive coronary angiogram, fasting blood sugar, hypertension and age inboth sexes, and WHR and opium addiction in females may be stronger predictors of extent of CAD.Prevention of these risk factors may be effective in controlling the progress of CAD.
Evaluation of the overlapping of posterior teeth in two techniques of improved interproximal panoramic program and standard panoramic

Abstract: "nBackground and Aims: Overlapping of the proximal surfaces of posterior teeth in the panoramic radiography is a major concern. Therefore, an option has been developed in the panoramic unit of Planmeca Promax, namely improved interproximal mode. This mode causes lower horizental angle with the teeth contact region during the unit rotation decreasing overlapping of the panoramic images of the posterior teeth especially premolar teeth. The present study was done to compare the overlapping of posterior teeth using two techniques of improved interproximal panoramic program and standard panoramic. "nMaterials and Methods: In this diagnostic study, 32 patients requiring panoramic radiographies at their posterior teeth during their routine diagnosis and treatment process with the mean age of 27.3 years were participated. No patients showed crowding of posterior teeth or missed and restored posterior teeth. The participants' panoramic radiographies were randomly taken by two techniques of improved interproximal panoramic and standard panoramic using Planmeca Promax device. The overlapping of the panoramic images was blindly assessed by an oral radiologist. The overlapping in both techniques was reported by frequency and percentage. The comparisons were done by Chi-square test between two techniques and the odds ratio of overlapping was estimated using regression analysis. "nResults: In standard panoramic techniques, 38.5% (148 contacts of 384 contacts) of the proximal surfaces overlapped while the overlapping of the proximal surfaces was observed in 18.8% (72 contacts of 384 overall contacts) in improved interproximal technique. Significant differences were noted between two techniques regarding overlapping (P<0.001). Also 66.4% and 39.1% of 4-5 teeth contacts overlapped in standard and improved techniques. The values were reported to be 39.1% and 12.5% in contacts of 5-6 teeth and 10.2% and 4.7% in the contacts of 6-7 teeth in both techniques, respectively. The overlapping odds ratio in improved technique as compared to standard techniques was 2.72 more. While the odds ratio of overlapping in the contact regions of 4-5 and 5-6 teeth was 16.4 and 4.61 as compared to the contact region of 6-7 teeth (all: P<0.001). The lower or upper jaw with the patients' mouth side did not significantly influence the overlapping of the proximal surfaces in both panoramic techniques. A CASE REPORT OF CORONARY ARTERY DISEASE IN A TEENAGE GIRL

Abstract: Atherosclerosis is the leading cause of death in most parts of the world. This disorder affects mostly patients above the age 40 years. This case report introduces a 17 years old girl with early development of coronary artery disease who had severe coronary atherosclerosis that did not respond to medical and interventional treatment and underwent surgical operation in cardiac surgery department of Shahid Madani Hospital, Tabriz, Iran. Presence of risk factors for atherosclerosis were evaluated and the only findings were positive family history of cardiac death in her uncle at about 52 years of age and high level of lipoprotein (a) in one of her sisters. In follow up evaluation of this patient, high levels of lipoprotein (a) was documented which was controlled with medical therapy. We concluded that high level of lipoprotein (a) was the probable cause of atherosclerosis in this patient. This case report emphasizes the need to screen siblings of patients with premature myocardial infarction.
Optimization of fuzzy multi-company workers assignment problem with penalty using genetic algorithm

Abstract: In this paper, we proposed the fuzzy multi-job and multi-company workers assignment problem with penalty. Our purpose is obtaining the optimal solution the assignment problem, where n jobs are assigned to m workers (m>n), each job must be assigned to one and only one worker and each worker could be received one job or do not receive any job. Furthermore, there are k company where each worker belong a special company. For finding the optimal assignment, we must optimize total cost this problem assignment. This problem has three types of costs, direct cost company cost and penalty. In this paper, first the proposed assignment problem is formulated to the crisp model by using a suitable fuzzy ranking and fuzzy arithmetic operators. Finally, a heuristic genetic algorithm is designed for solving the proposed problem and an example is given to verify the efficiency of the algorithm.
Mutational Analysis of the PTEN/MMAC1 Tumor Suppressor Gene in Sporadic Glioblastoma Multiforme

Abstract: Background: In 1997 MMAC1 or the PTEN gene, was identified as a tumor suppressor gene on the long arm of chromosome 10.PTEN involves in the balance between proliferation, and differentiation, apoptosis and regulation of angiogenesis, and eventually mutation in this gene causes a strong potential for tumorigenesis cells. This study is the first report of the correlation between PTEN gene mutations with histological markers and the age of Glioblastoma patients with the purpose of using it to remove diagnostic ambiguities and identify the type of glioma.Methods: In this study, we screened for PTEN mutations in exon 4-8 in glioblastoma patients in Isfahanian population. Genomic DNA ware extract from 60 formalin fixed frozen glioblastoma tumors, 4 normal tissue sample and 60 blood samples. Mutational analysis was performed using polymerase chain reaction, single strand conformation polymorphism (SSCP) and heteroduplex mobility techniques. In order to augment the accuracy, the experiments (PCR, SSCP, HMA) was repeated at least 3 times. The correlation between PTEN mutations with Clinical Pathologic markers (pleomorphism, Necrosis, cellularity, mitosis and endothelial proliferation) in Glioblastoma tumors and the age of patients were investigated With the help of statistical tests (t-test, Manvitni and Fisher's exact test).Results: A total of 4 mutations (7%) were detected in 60 glioblastoma tumors. In this study, there was no significant relation between clinic pathologic markers and PTEN mutations (p-value > 0.05). Our data shows a positive association between the age of onset of cancer and PTEN mutations) p-value < 0.001. (Conclusion: Our results suggested that PTEN mutations are Low in Iranian glioblastoma patients. People with PTEN mutations are more likely to develop glioblastoma before the age of 20.
Effect of Cystoscopy on Prostate-Specific Antigen, New Words about Old Subject

Abstract: Background: Prostate Specific Antigen (PSA) is a protein currently used in conjunction with the digital rectal examination to detect and guide management of prostate cancer. There are several causes of increase in serum PSA including urological manipulations, medications, prostatic diseases, prostate cancer, prostate inflammation or infection, etc. some studies had reported cystoscopy to cause transient increases in PSA. The present study has been undertaken with an aim to find out the effect of rigid cystoscopy on serum PSA level. Methods: One hundred two men referring to clinic of urology of Imam Reza Hospital from 2008 to 2010 were included in this study. Their age ranged from 31-88 years with the mean of 65.85 years. Our inclusion criteria were having an indication for cystoscopy like BPH, erection disorder, signs related to prostate or bladder carcinoma. Patients with positive urine culture, any medication affecting PSA level, history of urinary retention, any urological manipulations a few days before and ejaculation 48-72 hours before PSA assessment were excluded from the study. Blood samples were taken before and 24 hours after cystoscopy for PSA measurement. PSA was measured by Sandwich ELISA test using PSA kit (pishtaz kit - made in Iran) containing monoclonal antibody. Results: PSA level of the study cases varied from 0.1 to 21 ng/ml. Mean PSA values before and after cystoscopy were 3.004± 3.34 and 2.92 ± 2.98 respectively indicating that after cystoscopy not only PSA value did not raise but also decreased to some extent. Conclusion: Our findings indicate that, cystoscopy has no effect on PSA value and does not lead to its elevation. It can be concluded that serum PSA after rigid cystoscopy is an accurate and reliable finding to decide about patients condition related to prostate cancer.
