Unusual Clinical Presentation of Ethylene Glycol Poisoning: Unilateral Facial Nerve Paralysis

Abstract: Ethylene glycol (EG) may be consumed accidentally or intentionally, usually in the form of antifreeze products or as an ethanol substitute. EG is metabolized to toxic metabolites. These metabolites cause metabolic acidosis with increased anion gap, renal failure, oxaluria, damage to the central nervous system and cranial nerves, and cardiovascular instability. Early initiation of treatment can reduce the mortality and morbidity but different clinical presentations can cause delayed diagnosis and poor prognosis. Herein, we report a case with the atypical presentation of facial paralysis, hematuria, and kidney failure due to EG poisoning which progressed to end stage renal failure and permanent right peripheral facial nerve palsy. 1. Introduction Ethylene glycol (EG) may be consumed accidentally or intentionally, usually in the form of antifreeze products or as an ethanol substitute. It is converted by alcohol dehydrogenase to active metabolites in the liver, and these metabolites cause metabolic acidosis with increased anion gap, renal failure, hypocalcemia, oxaluria, and damage to the central nervous system and cranial nerves [1–3]. Diagnosis of EG poisoning could be a challenge due to altered mental status and lack of poisoning history. While metabolic acidosis with increased anion gap is common, osmolar gap resolves within 24 to 72 hours as the EG is metabolized to toxic metabolites. Early initiation of treatment can reduce the mortality, and morbidity but different presentations especially with delayed cases could be a problem, so patients with acute renal failure require more attention [4]. In cases of renal failure with the suspicion of EG poisoning, kidney biopsy should be considered promptly. Histological examination of renal tissue often reveals widespread necrosis of the tubular epithelium and deposition of a multitude of doubly refractile oxalate crystals in the distal tubules and collecting ducts [5, 6]. Herein, we report a case with the atypical presentation of facial paralysis, hematuria, and kidney failure due to EG poisoning. 2. Case Report A 25-year-old man was admitted to the emergency department of our university hospital with sudden onset of nausea, vomiting, slight mental disability, and abdominal pain. Cardiovascular, respiration, and gastrointestinal system examination findings were normal. The neurological examinations of the cranial nerves and limbs were normal. He was clinically dehydrated. The evaluation of ten-year results after renal transplantation

Abstract: Puropse: To evaluate the outcomes of patients who were underwent renal transplantation between 05.05.1994 and 14.06.2005 at Erciyes University Faculty of Medicine.Material and Method: Survival rates were estimated by Kaplan-Meier test.Results: A total of 65 patients who underwent chronic hemodialysis or continuous ambulatory peritoneal dialysis, and those who had no contra-indications for transplantation were performed renal transplantation. The donor sources were living donor in 39 patients and cadaveric kidney in 26 patients. Acute rejection was diagnosed and treated in 10 patients. Chronic rejection was diagnosed in 14 patients. Primer graft failure was developed in 4 patients. Complications due to surgical procedure developed in 14 patients. Infection complications were determined in 38 patients. Urinary tract infections were the most common form of infection. A total of 16 patients died of various causes. 35 patients who have functioned graft is followed. Patient survival rates were 89.2%, 69.1%, 65.4% for the first year, 5 years and 10 years of follow up period, respectively. Graft survival rates were 86.0%, 52.0 %, 37.3 % for the first year, 5 years and 10 years of follow up period, respectively.Conclusion: In this study, in spite of the results of living donors are better than those of the cadaveric donors, number of the patients are not enough to claim such this result.

What Becomes Important in the Contemporary University?

Abstract: This article focuses on which issues should stand out in a contemporary university. For this purpose, the meaning of the word and concept university is pointed out; the function, aim and objective of the university are specified and the question of how this function will be realized is answered. In addition, issues including how universities influence community life, the main features of a contemporary university, the obstacles standing against the realization of contemporary university as well as how to overcome these obstacles and the characteristic the logic of university are addressed. How university's interest in the culture of the society where it originates will to contribute to the development of science and technology of the country as well as the world is emphasized. In addition, the importance of scientific mentality, rationality, and collaborative working in university is emphasized and the importance of native language is indicated. It is recommended for students to increase their knowledge and level of thinking, to acquire the methods of research and thinking, and to adopt critical thinking. The issue that students should save themselves from being torn between information provided by certain fields of science and canalize to trans-science mental unity is particularly emphasized. Finally, the way for one or several universities in our country to gain the identity of a national university and thus being a symbol of our understanding of education and higher education is specified.
The Effect of Students’ Learning Styles to Their Academic Success  [PDF]

Abstract: This study was aimed to evaluate the learning styles of education faculty students and to determine the effect of their success and relationship between their learning styles and academic success. The popula- tion of this study is comprised of the students of Education Faculty in 19 May University and the sample includes 140: 68 art, 72 pre-school teacher department students. Depending on the results obtained from pre-test, it was aimed to improve students’ knowledge and skills in studying. There was a significant dif- ference between the scores of pre- and post-tests. The significant relationship between the scores of post-test and the student success revealed that they learned how to study effectively. The validity and re- liability of the test were determined by considering the Cronbach alpha coefficients for each and all of the items. The study has found statistically significant differences between the results of the first and final applications of the subtests on learning styles and academic success; those subtests covered the items as learning, planned study, effective reading, listening, writing, note taking, using the library, getting pre- pared for and taking exams, class participation and motivation.

Abstract: Kerberos is one of the solutions for network security problems since it provides strong secret key cryptography over the insecure networks. Through the Kerberos authentication protocol, a client can prove its identity to a server (and vice versa) across an insecure network connection such as on Internet. In this comparative research paper, the Kerberos authentication protocol is extended and strengthened using x.509 with the integration of newer authentication system which is compared with previous authentication systems. In addition to this, RSA encryption mechanism used to provide authentication and security for the most communication systems replaced with Elliptic Curve Cryptography (ECC) encryption in Kerberos during authentication progress through simulation to expose possible efficient alternatives for key generation and to enhance security.
Comparison of Three Conventional Extraction Methods for the Detection of Plant Virus/Viroid RNAs from Heat Dried High-phenolic Host Leaves

Abstract: The presence of virus/viroid infections can go unnoticed since symptoms appear only if additional viruses are present. Detection of Plum bark necrosis stem pitting associated virus (PBNSPaV), Apricot latent virus (ApLV), Apple scar skin viroid (ASSVd), Prunus necrotic ringspot virus (PNRSV) and Potato virus Y (PVY) by reverse transcriptase polymerase chain reaction (RT-PCR) or nested-RT-PCR is possible; however, these assays could be unreliable if the tissue contains interfering compounds. This study reports on use of three extraction procedures in recently developed heat-dried virus/viroid preserved plant tissues. The methods tested were lithium chloride, silica-capture and citric buffer. The results showed that (1) the silica-capture RNA extraction method appears to be superior for total RNA extraction; (2) the increase in volume of silica improves the efficiency of RNA extraction from dried infected leaves and (3) the use of silica method minimizes the fragmentation of PCR products and improves the PCR detection of tested pathogens. The results of study indicate that the use of appropriate RNA extraction method is crucial for a successful PCR and an appreciable yield of PCR product from heat-dried infected leaves.

Abstract: In this study we utilize formal concept analysis to model association rules. Formal concept analysis provides a topological structure for a universe of objects and attributes. By exploiting the relationship between objects and attributes, formal concept analysis then introduces an entity called a concept. A concept is a set of attributes and objects. The attributes are maximally possessed by the set of objects and similarly the objects are the maximal set which all possess the set of attributes. Formal concept analysis deals with formal mathematical tools and techniques to develop and analyze relationship between concepts and to develop concept structures. We propose and develop a connection between association rule mining and formal concept analysis. We show that dependencies found by an association query can be derived from a concept structure. We have extended formal concept analysis framework to the association rule mining. We use analysis of market-basket problem, a specific case of association rule mining, to achieve this extension. This extension provides a natural basis for complexity analysis of the association rule mining. This extension can also help in developing a unified framework for common data mining problems.
