Abstract: pulmonary disease in paracoccidioidomycosis is very common and in some cases can be the sole manifestation of the disease. we studied the findings observed in 30 patients with pulmonary paracoccidioidomycosis submitted to high-resolution computed tomography in order to determine the most frequent findings, presentation patterns and distribution of paracoccidioidomycosis lesions in the lungs. the most frequent findings observed on high-resolution computed tomography were: interlobular septal thickening (96.7%), ground-glass opacities (66.7%), nodules (60%), irregular air-space enlargement (56.7%), bronchial wall thickening (46.7%), pleural thickening (36.7%), cavitation (36.7%), tracheal dilatation (33.3%), architectural distortion (30%), air-space consolidation (30%), parenchymal bands (23.3%), intralobular reticulate (13.3%) and hilar interstitial thickening (10%). chest x-rays allow limited evaluation of diffuse pulmonary diseases, and hence high-resolution computed tomography is an essential method for studying patients with pulmonary paracoccidioidomycosis.

Abstract: bronchioloalveolar carcinoma is a bronchogenic carcinoma that presents an insidious growth, arises from the walls of the distal air spaces and spreads using the alveolar septa as a stroma, preserving the lung architecture. in this work we analyze the high-resolution computed tomography of 17 patients with bronchioloalveolar carcinoma. conversely to the data presented in the literature, male preponderance (71%) and a higher frequency of association between multinodular and consolidation patterns (53%) in comparison to single nodular (12%), multinodular (12%) and consolidation (23%) patterns were observed. the most common findings were: consolidation areas (76%), air bronchogram (71%), low attenuation areas (60%), thickening of interlobular septa, ground-glass attenuation and confluent nodules (54% each), and crazy paving pattern (36%). cavitated nodules, atelectasis, halo sign and "tree in bud" pattern were observed in only one case each.

Abstract: although rare, transitional cell carcinoma of the urinary tract is the second most common tumor of the kidneys. we studied retrospectively the computed tomography images of 10 patients with confirmed transitional cell carcinoma of the urinary tract. there was involvement of the bladder in 60% of the cases whereas the calyceal system and the ureters were involved in 60% and 10% of the cases, respectively. concomitant tumors were present in 40% of the cases. the findings were sessile masses, ureteral wall thickening and renal infiltrative lesions. calcified tumors were seen in two patients and a transitional cell tumor arising from a bladder diverticulum was identified in another patient. local tumor dissemination was observed in 80% of the patients and distant dissemination in 20% of the patients. computed tomography allowed the detection of local and distant tumor dissemination, thus allowing a selected group of patients to be treated conservatively.

Abstract: introduction: several theories have been proposed for the morphofunctional analysis of the liver. objective: the purpose of this paper is to establish a standard and pratical procedure to understand the liver morphophysiology and hepatic alterations, based on the correlation of the predominant metabolism with the porto-central circulation pattern. methods: this study presents a grid model of morphological study (gmms), designed with the use of microsoft? powerpoint software. furthermore, it demonstrates its application in histopathological diagnosis and interpretation of liver physiopathology. results: the gmms application in three clinicopathological cases allowed a more accurate analysis of liver lesions from several circulatory and metabolic areas of the liver lobe, which were defined in the grid accordingly. moreover, it allowed a better understanding of the pathophysiology in each case and the correlation of histopathology diagnosis with clinical data. discussion: the gmms shows the importance of the metabolic zoning with the definition of a virtual interlobular septum. the gmms application demonstrates the lobular asymmetry in the liver and its correspondence with different areas of the regular lobe, enabling a better definition of morphofunctional unit boundaries and its respective areas of metabolic predominance. conclusion: the gmms allows an integrated analysis of the liver by correlating its microanatomy with different circulatory and metabolic gradients, which provides a useful tool for histopathological diagnosis. the gmms should be used in cases that do not involve abnormalities in the liver structure since its implementation depends on the relationship between the portal spaces and the centrilobular vein as reference points.

Abstract: This study develops and evaluates a pharmaceutical consultation program (PCP) to improve treatment for Type 2 diabetes patients (T2DP) and reduce risk factors for diabetic complications with possible application in other chronic diseases. We recruited T2DP receiving conventional medical treatment but with fasting glycemia >140mg/dl and/or glycated hemoglobin >7%. The PCP includes strategies obtained from Dader's method, the PWDT (Pharmacist's Workup of Drug Therapy method) model of pharmaceutical care, the SOAP (Subjective data, Objective data, Assessment, and Plan of care) method, and concepts based on a nursing care model. The PCP evaluated lifestyle, pharmacotherapy and monitoring it using laboratory tests, vital signs, and anthropometry. These procedures were repeated every 4 months for 1 year. Data obtained in each consultation were used to provide patient education focusing on healthy lifestyles and medications. Fifty patients completed the PCP. There were reductions in glycemia (P<0.0001), glycated hemoglobin (P=0.0022), cholesterolemia (P=0.0072), triacylglycerolemia (P=0.0204) and blood pressure (P<0.0001). Increased concordance with drug treatment and correction of drug-related problems contributed to improved treatment. We can therefore conclude that our PCP was suitable for improving health outcomes in T2DP by reducing risk factors for diabetic complications.
