Diversidade e psicoterapia: expectativas e experiências de pessoas LGBT acerca das competências multiculturais de psicoterapeutas

Abstract: the present papers addresses the issues related to psychotherapy practices with lesbian, gay, bissexual and transgendered (lgbt) individuals. a historical review of clinical interventions with non-heterossexual clients is presented, as well as the present international ethical guidelines for psychotherapy with lgbt people. the tridimensional model of multicultural competencies of psychotherapists or counsellors (awareness, knowledge and skills) is proposed as a reference base for the development of more ethical and effective practices with clients from minority groups, including lgbt. a study is presented, with a qualitative and exploratory approach. four interviews were conducted to four participants - one lesbian woman, one gay man, a bisexual woman, and a transgender male. the interviews aimed to explore their representations of psychological health and illness, as well as their experiences and expectations relative to psychotherapeutic processes. the results of the interviews are presented, voicing the participants by presenting excerpts of the interviews. discriminatory experiences were related as a central part of the lives of all the participants, including in the access to health care. the expectations they hold of psychotherapists, however, include the ability to separate their values and attitudes from their clinical work, and the capacity to help they in the resolution of problems resulting from discrimination and social isolation. theses expectations are discussed in light of the national and international literature, highlighting the fundamentally interpersonal nature of the therapeutic relationship and the relevance of the specific training of the psychotherapist.
The Moderation Effect of Social Support in the Relationship between Child Maltreatment and School Achievement

Abstract: This study investigated the relationship between child maltreatment, school achievement and social support, by exploring the adaptation of the stress buffering model of social support. Data was collected on 402 children from 5th - 9th grades (383 without history of child abuse and 64 with history of child abuse). An adapted version of the Social Support Appraisals Scale (Dubow & Ullman, 1989; Dubow et al., 1991) was used to analyze the role of social support in the relationship between child maltreatment and school achievement. Results indicated that abused children scored lower in Language and Math, and had more grade retentions than children without history of abuse. Findings also revealed that perceived social support is a promoter of school achievement. Results also indicated social support as a moderator between child abuse and school achievement, although only partially in the direction of the adaptation of the stress buffering model. Implications for school professionals are discussed.

Abstract: the present paper reflects on the development of culturally-sensitive mental health services and training of mental health professionals. national and international ethnic diversity is a growing reality, which demands different resources from health services toward the promotion of individuals’ global well-being. the specificities of these populations have led to an underutilization rate of counselling services, which are not adjusted in terms of their intercultural competence. we report an action-research project on access of child mental health care by immigrants, developed in switzerland, as well as the primary goals of a research project toward the development of culturally-sensitive mental health services in portugal. we discuss the importance of participative research, which involves and gives voice to ethnic minority groups and to their experiences, needs and perceived barriers in the access to support to their psychological well-being. finally, we defend the importance of intercultural competence training of clinicians, not as a need of the minorities, but of all individuals in an intercultural society.
A New Twist on Empirically Supported Treatments

Abstract: Este artículo presenta los resultados preliminares de un estudio experimental dise ado para desarrollar y validar un modelo integrador de intervenciones psicológicas. La psicoterapia prescriptiva basa sus procedimientos en principios de cambios meta-teóricos. Este enfoque enfatiza la adecuación o correspondencia de las intervenciones psicológicas a las características del paciente, planteando que esta correspondencia incrementa el poder predictivo de las contribuciones iniciales de los factores del paciente, tipos de intervención y alianza terapéutica de cara al resultado terapéutico. Este estudio piloto, en una muestra de 40 pacientes con trastornos del estado de ánimo y consumo excesivo de sustancias, confirmó que la correspondencia entre las características del paciente y las intervenciones psicológicas beneficia sustancialmente la eficacia del tratamiento, especialmente a los seis meses de finalizar el tratamiento. Los resultados sugieren que las características del paciente influyen en su pronóstico, que las intervenciones como tal influyen de forma modesta y que la alianza terapéutica incrementa el poder predictivo. Con todo, la adecuación de las intervenciones al paciente parece tener un valor predictivo superior al resto de variables, en particular para la depresión y la sensación de bienestar.

Abstract: the present study describes the composition, the qualification, the salary investment, the workforce produce, and discusses users' accessibility in terms of time at basic health units (bhus). the study was performed at two bhus from january to december 2008, and developed by analyzing administrative documents. in both, the composition of professionals according to education level revealed: 21% with a university degree, 27% with a secondary education, and50% with a primary education; showing a positive salary variation. the medical and nursing conducts were the majority at both. the production indicators confirmed: 25 and 37 min/person/month for accessibility, respectively for bhu a and b; r$ 8.43 and r$ 12.11/person/month for the salary investment at both bhus, and 0.07 appointments/person/month at both bhus. the professionals' available time is scarce compared to the potential of the demand. the production indicated an opportunity of care < 1 per person/month at a reduced cost.

Abstract: background: the clinical outcome routine evaluation - outcome measure (core-om) is a european self-report measure of psychological well-being in adults, which is already adapted for more than 20 countries. the core-om is applicable in primary care settings, in occupational health and educational contexts, as well as for evaluations of mental health services quality. objectives: to translate the core-om into and adapt it to the portuguese language. methods: the translation of core-om to portuguese was conducted in accordance with the core system trust international guidelines, in collaboration with the authors of the original core-om. this guidelines involved seven independent translations by individuals familiar/non-familiar with the field of mental health and by licensed translators. after the discussion of these translations in a focus group, a final version of the portuguese core-om was established. individuals from different age groups, socio-cultural backgrounds and educational levels participated in this translation process. results: preliminary results in a sample of 111 individuals (general population) show that the translated measure is confidential and has a good internal reliability. discussion: the portuguese version of the core-om is a valid and adequate instrument to evaluate psychological changes, both in research and clinical practice contexts.

Abstract: the internal medicine service of the hospital general de vic (barcelona) takes part in the mortality committee by revising and discussing in-hospital mortality. background: to establish the characteristics of the deceased, death causes and to revise possible changes in the last six-years time or problems related to the exitus, to evaluate and improve hospitalized patients' assistance. methodology: every case was revised following a specific register: demographical data, diagnosis and death cause, hospital death, documentation data, terminal or agonic situation when hospitalized, autopsies and death quality data. exitus due to hospital problems were analyzed and classified in different groups. the statistical analysis was performed with measures of central tendency and of standard deviation. results: during the revised six years, there were 819 exitus (5.1%). global average death age was 79 ± 1.8 years: 52.5% were men and 47.4% were women; 22.8% died in less than forty-eight hours after hospitalization. the most frequent death causes were cerebrovascular accident (24%), chronic obstructive pulmonary disease (14.4%) and pneumonia (9.6%). there were a small number of autopsies (4.8%). ratio of exitus due to hospital problems was stable during the six years (0.5%), in which nosocomial infection was the severest problem. conclusions: the total percentage of exitus was 5.1%, higher than the common standards. mortality causes coincide with other series. ratio of exitus due to hospital problems was according to recommended objectives. the number of autopsies was very small. a correct completing and revision of the clinical recording is indispensable to spot a shortage in the hospitalized patients' assistance.
Determinantes cardíacas do tempo de ventila??o mecanica e mortalidade de doentes com insuficiência respiratória crónica exacerbada: A importancia dos parametros ecocardiográficos

Abstract: objective: to study the influence of cardiac status on the length of mechanical ventilation, outcome and disease severity in patients admitted to an intensive care unit (icu) with exacerbation of chronic respiratory failure. design and setting: a 30-month prospective study in a 14 bed icu patients and methods: fifty nine patients were enrolled, with a mean age 74.7 +/- 9.7 years, mean length of ventilator support 10.8 +/- 12.6 days, and mean apache ii score 23 +/- 8.3. within the first 24 hours of admittance, cardiac chamber dimensions, inferior vena cava (ivc), and mitral transvalvular doppler were evaluated using transthoracic echocardiography; the cardiac rhythm was recorded (presence of sinus rhythm or atrial fibrillation). blood gases were evaluated at dis charge. results: greater length of ventilation was observed in patients presenting atrial fibrillation (p=0.027), particularly when a dilated ivc was also present (>20mm, p=0.004). a high level of serum bicarbonate (>35meq/l), was also related with longer ventilation (p=0. 04). twelve patients died. mortality was related to the presence of a dilated right ventricle (p=0. 03) and a ratio between right and left ventricle> 0. 6 (p=0.04). conclusion: patients submitted to mechanical ventilation due to exacerbation of chronic respiratory failure which present atrial fibrillation require a longer ventilation period, particularly if a dilated ivc is also present. patients with dilated right cardiac chambers are at an increased risk of a fatal outcome.

Abstract: bioethics is supported in the principle of autonomy, the respect of the human dignity and it is stated by means of the informed consent; however dentists in pinar del rio do not know how to apply it. a descriptive and cross-sectional study was carried out to determine the level of application of the informed consent by the dentists of the primary health care in pinar del rio municipality on june 2007, which is considered a matter of vital importance to practice the profession implying the fulfillment of the bioethical principles, mainly the informed consent. out of a universe of 134 dentists, 100 were included into the research applying a survey and an observation guide. data were processed using descriptive statistical methods. findings showed incomplete knowledge about the proper application of the informed consent. the most violated elements were request for the authorization and the incorporation of the patents to decision-making about the treatment to be followed
