Structured exercise interventions for type 2 diabetes mellitus: Strength of current evidence

Abstract: Exercise, along with medical nutrition therapy and pharmacological interventions, is an important component in the clinical management of type 2 diabetes mellitus (T2D). Current clinical guidelines on type 2 diabetes provide no detailed information on the modalities of effective exercise intervention in the treatment of type 2 diabetes. Both endurance and resistance types of exercise seem to be equally effective in improving metabolic control in patients with type 2 diabetes. Determining the best method of providing exercise is clinically relevant to this population. This paper reviews the epidemiology of diabetes and problems of physical function associated with type 2 diabetes and discuss the benefits of exercise therapy on the parameters of glycemic control and function in type 2 diabetes patients, with special reference to Asian Indians. Based on the currently available literature, it is concluded that type 2 diabetes patients should be encouraged to participate in specifically designed exercise intervention programs. Attention should be paid to the avoidance of cardiovascular and musculoskeletal deconditioning. More clinical research is warranted to establish the efficacy of different dosages of exercise intervention in a holistic approach for type 2 diabetes subpopulations within different stages of the disease and various levels of co-morbidity.
Intrathecal baclofen withdrawal syndrome- a life-threatening complication of baclofen pump: A case report

Abstract: A 24-year-old man with a past medical history of cerebral palsy and spastic quadriparesis developed hyperthermia, disseminated intravascular coagulation, rhabdomyolysis, acute renal failure and multisystem organ failure leading to a full-blown intrathecal baclofen withdrawal syndrome. Intrathecal baclofen pump analysis revealed that it was stopped due to some programming error. He was treated effectively with supportive care, high-dose benzodiazepines and reinstitution of baclofen pump.The episodes of intrathecal baclofen withdrawal syndrome are mostly caused by preventable human errors or pump malfunction. Educating patients and their caregivers about the syndrome, and regular check-up of baclofen pump may decrease the incidence of intrathecal baclofen withdrawal syndrome. Oral baclofen replacement may not be an effective method to treat or prevent intrathecal baclofen withdrawal syndrome. Management includes an early recognition of syndrome, proper intensive care management, high-dose benzodiazepines and prompt analysis of intrathecal pump with reinstitution of baclofen.Baclofen is a gamma-aminobutyric acid (GABA) analog that has inhibitory effects on spinal cord reflexes and brain. Intrathecal baclofen (ITB) therapy consists of long-term delivery of baclofen to the intrathecal space. Intrathecal baclofen has been used to treat spasticity due to cerebral palsy, brain or spinal cord injury, multiple sclerosis, dystonia, stroke and stiff-man syndrome, particularly for those patients who are unresponsive to conservative pharmacotherapy or develop intolerable side effects at therapeutic doses of oral baclofen [1]. Side effects such as drowsiness, nausea, headache, muscle weakness and light-headedness can occur as a result of the pump delivering an incorrect dose of baclofen. Media - Oriented Rate Selection Algorithm for IEEE 802.11 WLANs

Abstract: Current wireless LANs treat multimedia flows and classical data flows alike. Typically, the same error control mechanisms are used for video flows, which are generally error tolerant but delay sensitive, and TCP flows which are error-intolerant and delay insensitive. The performance of multimedia applications can be significantly improved by some degree of cross-layer awareness. In this paper, we focus on the optimization of real-time multimedia transmission over 802.11-based wireless networks. In particular, we propose a simple and efficient cross-layer mechanism for dynamically selecting the transmission mode that considers both the channel conditions and characteristics of the media. This mechanism called media-oriented rate selection algorithm (MORSA) targets loss-tolerant applications such as audio/video conferencing or VoD that do not require full reliable transmission. We provide an evaluation of this mechanism for MANETs using simulations with NS-2 and analyze the video quality obtained with a fine-grain scalable video encoder based on a motion-compensated spatio temporal wavelet transform. Our results show that the proposed cross-layer approach achieves up to 4 Mbps increase in throughput and that the routing overhead decreases significantly. The transmission of a sample video flow over an 802.11a wireless channel has been evaluated with MORSA and compared with the traditional approach. Significant improvement is observed in throughput, latency, and jitter while keeping a good level of video quality.
An Efficient Approach for Countering Black Hole Attack in MANETs

Abstract: Mobile Ad-Hoc Networks(MANETs) are autonomous and decentralized wireless systems. MANETs consist of mobile hosts that are free in moving in and out in the network. Security in MANET is the most important concern for the basic functionality of network. MANETs often suffer from various security attacks because of its features like open medium, changing its topology dynamically, lack of central monitoring and management, cooperative algorithms and no clear defense mechanism. In this paper, we consider only black hole attak. Black hole attack in MANETS is a serious security problem to be solved. In this attack, a malicious node uses the routing protocol to advertise itself as having the shortest path to the node whose packets it wants to intercept. In flooding based protocol, if the malicious reply packet reaches the requesting node before the reply from the actual node, a forged route has been created. This malicious node then can choose whether to drop the packets to perform a denial-of-service attack(DOS) or to use its place on the route as the first step in a man-in-the-middle attack. This paper provides a solution for countering black hole attack based on data routing information.

Abstract: This paper represents the literature survey report on growth and success of e-commerce. It helps to analyze the growth of e-commerce and the success factors to be considered in the growth of business. In E-congress survey, the author (Nick Bontis 2000) had analyzed the trends in shopping on-line. The accelerating total revenue obtained from 1999 to 2003 was analyzed. The analysis concluded that software, music (CDs) and literature (books and magazines) continue to dominate on-line purchases; however, new industries are finding their place on the web as well. Travel and tourism companies, grocery and department store chains and non apparel specialty stores were making their presence known. It was stressed that the companies must undertake the beginning of an implementation plan to involve in e-commerce.
Improved Multiple Feedback Successive Interference Cancellation Algorithm for Near-Optimal MIMO Detection

Abstract: In this article, we propose an improved multiple feedback successive interference cancellation (IMF-SIC) algorithm for symbol vector detection in multiple-input multiple-output (MIMO) spatial multiplexing systems. The multiple feedback (MF) strategy in successive interference cancellation (SIC) is based on the concept of shadow area constraint (SAC) where, if the decision falls in the shadow region multiple neighboring constellation points will be used in the decision feedback loop followed by the conventional SIC. The best candidate symbol from multiple neighboring symbols is selected using the maximum likelihood (ML) criteria. However, while deciding the best symbol from multiple neighboring symbols, the SAC condition may occur in subsequent layers which results in inaccurate decision. In order to overcome this limitation, in the proposed algorithm, SAC criteria is checked recursively for each layer. This results in successful mitigation of error propagation thus significantly improving the bit error rate (BER) performance. Further, we also propose an ordered IMF-SIC (OIMF-SIC) where we use log likelihood ratio (LLR) based dynamic ordering of the detection sequence. In OIMF-SIC, we use the term dynamic ordering in the sense that the detection order is updated after every successful decision. Simulation results show that the proposed algorithms outperform the existing detectors such as conventional SIC and MF-SIC in terms of BER, and achieves a near ML performance.
The Effects of Sintering Temperature on the Densification of Mechanically Alloyed W-Brass Composites  [PDF]

Abstract: Prealloyed (PA) and premixed (PM) W-brass with the composition of 60 wt% W, 1 wt% Ni and 39 wt% brass was sintered at the temperature of 800°C, 920°C and 1000°C each. As a result of difficulties in the densification of W-Cu and W-Cu alloys, mechanical alloying (MA) and activated sintering were combined. The powders were mechanically alloyed for 13 hours to produce nanosized W grains embedded in brass. The microstructure and properties of these composites with increase in sintering temperature has been studied. Both prealloyed and premixed composites sintered at 800°C (solid state sintering) and 920°C (sub-solidus state sintering) have lower sintered densities and hardness. The densification rate in the premixed composites was observed to be higher than that of the prealloyed composites. Their densification and properties increased with the increase in the sintering temperature. Premixed composite sintered at 1000°C had 91.0% sintered density, 180 Hv microhardness against 76.0% and 133 Hv respectively for prealloyed composite at the same temperature. The values of electrical conductivity in both prealloyed and premixed composites increased with increase in temperature.

Abstract: Fabrication of full-density W-brass composites is very difficult to achieve because of evaporation of zinc, insolubility of W and brass and compacts expansion. In this study, to achieve full-density W-brass composites, mechanical alloying (MA) and activated sintering process were utilized. Mechanical coating of W with Ni using high energy planetary ball mill was carried out. The milling was divided into two stages: to alloy and modify the surface of W with Ni for enhanced activation. The microstructure of the milled powders and sintered compacts, elemental composition and phases present were studied by using scanning electron microscopy (SEM), energy dispersive X-ray spectroscopy (EDX) and X-ray diffraction (XRD) respectively. As-received powder compacts was also sintered under the same condition for comparison purpose. The effects of milling time on the microstructure, sinterability and the hardness of the composites were investigated. It was observed that the samples produced from 8 h milled powder had the highest relative sintered density (98% TD) and microhardness (234 Hv). On the other hand, the samples from the as-received powders expanded and had a relative sintered density of (67% TD) and microhardness as low as 24 Hv. The significance of this study is the possibility of producing W-brass composites as a cheaper alternative to W-Cu composites.
High incidence of microsatellite instability and loss of heterozygosity in three loci in breast cancer patients receiving chemotherapy: a prospective study

Abstract: Background The aim of the study was to evaluate potential chemotherapy-induced microsatellite instability, loss of heterozygosity, loss of expression in mismatch repair proteins and associations with clinical findings in breast cancer patients, especially resistance to chemotherapy and/or development of other tumors in the four years following chemotherapy treatment. Methods A comprehensive study of chemotherapy-related effects with a follow-up period of 48 months post treatment was conducted. A total of 369 peripheral blood samples were collected from 123 de novo breast cancer patients. Microsatellite instability and loss of heterozygosity in five commonly used marker loci (including Tp53-Alu of the tumor suppressor gene TP53) were analyzed in blood samples. Sampling was conducted on three occasions; 4–5 weeks prior to the first chemotherapy session (pre-treatment), to serve as a baseline, followed by two consecutive draws at 12 weeks intervals from the first collection. Mismatch repair protein expression was evaluated in cancer tissues using immunohistochemistry for three mismatch-repair related proteins. Results A total of 70.7% of the patients showed microsatellite instability for at least one locus, including 18.6% marked as high-positive and 52.1% as low-positive; 35.8% showed loss of heterozygosity in addition to microsatellite instability, while 29.3% exhibited microsatellite stability. The following incidence rates for microsatellite instability and loss of heterozygosity were detected: 39.1% positive for Tp53-Alu, 31.1% for locus Mfd41, and 25.3% for locus Mfd28. A higher occurrence of loss of heterozygosity was noted with alleles 399 and 404 of Tp53-Alu. The mismatch repair protein expression analysis showed that the chemotherapy caused a loss of 29.3% in hMLH1 expression, and 18.7% and 25.2% loss in hMSH2 and P53 expression, respectively. A strong correlation between low or deficient hMSH2 protein expression and occurrence of mismatch repair/loss of heterozygosity events in Mfd41, Tp53-Alu, and Mfd28 was evident. A significant association between mismatch repair/loss of heterozygosity and incidence of secondary tumors was also established. Conclusion Our results suggest that microsatellite instability, loss of heterozygosity, and deficiency in mismatch repair may serve as early prognostic factors for potential chemotherapy-related side effects in breast cancer patients.
