Abstract:

The use of contraceptive method to avoid pregnancy is a multifaceted phenomenon. Worldwide, 20 percent of married women of reproductive age are currently using periodic abstinence. To assess the knowledge and practice of periodic abstinence among currently married pregnant women, data were collected from the pregnant women attending the Department of Obstetrics and Gynaecology, All India Institute of Medical Sciences, New Delhi, India. One thousand three hundred and thirty one (1331) currently married pregnant women were interviewed face-to-face. Approximately 29.3 percent of women were aware of the abstinence method, 11.5 percent women had history of using it. In-depth finding shows that 84.9 percent women did not have correct knowledge of fertile window of a menstrual cycle. Counseling about the correct fertile window of a menstrual cycle may increase the effectiveness of methods and hence reduce the incidence of unwanted pregnancies.

Abstract: We report a case of invasive hydatidiform mole presenting as an acute primary haemoperitoneum.The patient presented with acute abdominal pain and signs of haemoperitoneum. Emergencylaparotomy revealed a molar pregnancy perforating through the uterine fundus, resulting in massivehaemoperitoneum. The serum beta chorionic gonado-tropin ( -hCG) levels regressed spontaneouslyfollowing evacuation of the molar pregnancy.
Uterine Volume & Blood Flow Changes in the Uterus Induced by GnRH Analogues in Cases of Fibroids

Abstract: This prospective study was conducted to study the effect of gonadotropin releasing hormone( GnRH) agonist on uterine and fibriod volume and the changes in blood flow in the uterine vessels.Nine patients with symptomatic fibriods, not willing for surgery, were enrolled. All patients recievedminimum 3 injections of GnRH agonist at 4 weeks interval. Uterine and fibroid volume and bloodflow in the utrine vessels was measured before and three months after treatment. There wasdecrease in the fibriod volume and increase in resistance index in uterine arteries. There was nosignificant decrease in uterine volume. Out of four perimenopausal women two achieved menopause.GnRH agonist may have a role in perimenopausal women with symptomatic fibroid.

Abstract: Amac: All India Institute of Medical Sciences bunyesindeki aile planlamasi klinigine basvuran kadinlarda kontrasepsiyon ve medikal abortusa yaklasimlari, bilgileri ve uygulamalari degerlendirmek. Materyal ve Metot: Yeni Delhi All India Institute of Medical Sciences bunyesindeki aile planlamasi merkezlerine 1 Agustos 2005 ile 31 Ekim 2005 tarihleri arasinda 284 kadin basvurdu. Bunlardan gebeliklerinin sonlandirilmasini isteyen 158 kadin sorgulandi ve anket calismasina alindi. Kadinlarin yaslari 20-45 arasindaydi; 83 ¢ € u (%29.2) okuma yazma bilmiyordu, 63 ¢ € u (%22.2) ilkokul mezunu ve 138 ¢ € i (%48.6) de lise veya yuksekokul mezunuydu. Kadinlar modifiye Kuppuswamy Sosyo-Ekonomik Duzey Skalasina gore dusuk ve yuksek sosyoekonomik sinif olarak gruplandirildi: I. Ust sinif, II. Orta-ust sinif, III. Orta sinif, IV. Alt orta sinif, V. Alt sinif. Eslerin her ikisinden de onam alindi. Cesitli kontrasepsiyon yontemleri sunuldu ve memnun olduklari yontem secimi kendilerine birakildi. Sonuclar: Kadinlardan 111 ¢ € i (%39.8) korunma yontemi kullanmadi; 108 ¢ € i (%3.87) bariyer yontemi, 52 ¢ € si (%18.31) rahim ici arac, 11 ¢ € i (%3.87) dogum kontrol haplari ve 2 ¢ € si de (%0.7) diger yontemleri tercih etti. Gebelik sonlandirilmasini isteyenlerin %56.96 ¢ € sinda istenmeyen gebelik, %43.03 ¢ € unde basarisiz kontrasepsiyon sonucu gebelik olusmustu. Saglik problemi olan bir olgu haricinde kalitimsal bir endikasyon yoktu. Olgulardan 4 ¢ € u (%1.4) hic kullanmasalar da acil kontrasepsiyonu ve 22 ¢ € si medikal abortusu duymustu. Olgularin 15 ¢ € i ise (%5.28) daha onceden medikal abortus islemi gecirmisti. Abortus sonrasi kadinlarin 9 ¢ € u (%3.17) dogum kontrol haplari ile, 81 ¢ € i (%28.5) rahim ici arac ile ve 62 ¢ € si (%21.83) tup ligasyonu ile korunmayi tercih etti. Diger bir grupta 38 kadinda (%13.38) RIA cikarildi ve yeniden takildi, 35 ¢ € ine (%12.32) sterilizasyon operasyonu ve 6 ¢ € sina (%2.4) gebelik istemi nedeniyle spiral cikarma islemi uygulandi. Genome-wide expressions in autologous eutopic and ectopic endometrium of fertile women with endometriosis

Abstract: Background In order to obtain a lead of the pathophysiology of endometriosis, genome-wide expressional analyses of eutopic and ectopic endometrium have earlier been reported, however, the effects of stages of severity and phases of menstrual cycle on expressional profiles have not been examined. The effect of genetic heterogeneity and fertility history on transcriptional activity was also not considered. In the present study, a genome-wide expression analysis of autologous, paired eutopic and ectopic endometrial samples obtained from fertile women (n = 18) suffering from moderate (stage 3; n = 8) or severe (stage 4; n = 10) ovarian endometriosis during proliferative (n = 13) and secretory (n = 5) phases of menstrual cycle was performed. Methods Individual pure RNA samples were subjected to Agilent’s Whole Human Genome 44K microarray experiments. Microarray data were validated (P < 0.01) by estimating transcript copy numbers by performing real time RT-PCR of seven (7) arbitrarily selected genes in all samples. The data obtained were subjected to differential expression (DE) and differential co-expression (DC) analyses followed by networks and enrichment analysis, and gene set enrichment analysis (GSEA). The reproducibility of prediction based on GSEA implementation of DC results was assessed by examining the relative expressions of twenty eight (28) selected genes in RNA samples obtained from fresh pool of eutopic and ectopic samples from confirmed ovarian endometriosis patients with stages 3 and 4 (n = 4/each) during proliferative and secretory (n = 4/each) phases. Results Higher clustering effect of pairing (cluster distance, cd = 0.1) in samples from same individuals on expressional arrays among eutopic and ectopic samples was observed as compared to that of clinical stages of severity (cd = 0.5) and phases of menstrual cycle (cd = 0.6). Post hoc analysis revealed anomaly in the expressional profiles of several genes associated with immunological, neuracrine and endocrine functions and gynecological cancers however with no overt oncogenic potential in endometriotic tissue. Dys-regulation of three (CLOCK, ESR1, and MYC) major transcription factors appeared to be significant causative factors in the pathogenesis of ovarian endometriosis. A novel cohort of twenty-eight (28) genes representing potential marker for ovarian endometriosis in fertile women was discovered. Abstract: Large chorioangiomas are rare and associated with significant fetal and maternal risks. A case ofchorioangioma syndrome is presented with polyhydramnios, pre-eclampsia, preterm labor and fetomaternalhemorrhage. Antenatal diagnosis is possible with ultrasound, and the prognosis for pregnancyoutcome can be predicted by evaluating the vascularity of the tumor on color doppler sonography.
