Abstract: the modes of transmission of hepatitis c virus are parenteral, sexual and maternal-fetal. it affects 3% of the population worldwide. currently, vertical infection is the main way of virus spreading. most children are clinically asymptomatic, but progression of liver disease has been described. the positivity of antibodies against hepatitis c virus does not discriminate between active and resolved infection, so determination of serum hcv-rna is necessary. the combination of peginterferon and ribavirin appears to be the most effective treatment. future challenges are the development of new drugs and a vaccine.

Abstract: the mortality of acute bacterial meningitis (bm) has remained stable in the last decades in spite of the use of new antibiotics, probably due to vascular complications. we report a 68-year-old woman with bm who had a malignant infarction of left middle cerebral artery territory 72 hours after admission to the hospital. the patient experienced a bad evolution and died four days after admission. the arterial involvement in patients with bm is explained by vasospasm of large arteries and vasculitis of small arteries. the medical treatment of a malignant middle cerebral artery infarct has a high mortality (rev méd chile 2004; 132: 1217-20)

Abstract: Algunas enfermedades del sistema nervioso central pueden imitar el patrón tomográfico de la hemorragia subaracnoidea, la más frecuente es la encefalopatía hipóxico isquémica. Su reconocimiento por especialistas es trascendente ya que evita el estudio y tratamiento agresivo en este grupo de pacientes. Presentamos un caso con diagnóstico clínico y tomografico probable, pero sin confirmación ya que no se realizó estudio de LCR ni autopsia. Efecto de la temperatura del ba?o de hemodiálisis en diabéticos

Abstract: during the dialysis procedure, arterial hypotension is one of the most common problems and it has been object of many studies. in hemodialysis, changes are produced in body volume through ultrafiltration that generate an increase in the production of thermic energy, which is removed during the treatment. the hypovolemia resulting from the removal of volume activates the sympathetic system, avoiding in this way heat loss and increasing body temperature that promotes vascular vasodilatation and interferes with the compensatory constrictive response to volume fall with consequent arterial hypotension. patients with autonomic neuropathy would be the most affected by volume depletion and they are usually the ones that show the highest frecuency of hypotension episodes, typical of patients with diabetes. it has been proved before that the use of a cold bath does not decrease the efficiency of the dialysis treatment and improves the cardiovascular stability as well, mostly in patients proned to it, such as diabetics, elderly, and patients with cardiac failure. in this study, it was observed that patients showed low basal temperatures before dialysis treatment and that the use of bath temperature of 35.5 °c increased the temperature post dialysis less than with the standard bath at 37 °c. the bath at 35.5 °c decreased the episodes of arterial hypotension, with an improvement in patient's welfare, and lower requirement of attention and treatment costs.

Abstract: many non-alcoholic drinks commercially available do not emphasize on their labels that they correspond to light drinks. the objective of the present paper was to estimate the daily intake of non nutritive sweeteners by healthy children and adolescents through the consumption of non-alcoholic drinks. food frequency questionnaires of non alcoholic drinks from 190 children and adolescents (3 to 18 years old) were analysed. the brands of the most consumed non-alcoholic drinks were requested in order to consider the quantity of the declared sweetener in the correspondent labels to estimate the daily intake. moreover, the daily intake of nonalcoholic drinks and the body weight declared in each questionnaire was taken into account. the estimated daily intake of students from 3 to 18 years old was 2,61 mg/kg of cyclamate, 0,28 mg/kg of saccharin, 0,69 mg/kg of acesulfame-k and 2,78 mg/kg of aspartame. only in the case of cyclamate there were three individuals with an estimated daily intake greater than the admissible daily intake.
Efecto de la temperatura del ba o de hemodiálisis en diabéticos Effect of the temperature of the dialysis bath in diabetics

Abstract: Durante el procedimiento dialítico la hipotensión arterial es uno de los problemas más comunes y ha sido objeto de numerosos estudios. En la hemodiálisis se producen cambios en el volumen corporal a través de la ultrafiltración, que generan aumento en la producción de energía térmica, la cual se remueve en el transcurso del tratamiento. La hipovolemia resultante de la remoción de volumen activa el sistema simpático impidiendo de esta manera la pérdida de calor originando aumento de la temperatura corporal que promueve vasodilatación vascular e interfiere con la respuesta constrictiva compensatoria a la caída del volumen con la consecuente hipotensión arterial. Los pacientes que padecen neuropatía autonómica serían los más afectados por la depleción de volumen y habitualmente son los que presentan mayor cantidad de episodios hipotensivos, típico caso de la población con diabetes. Se comprobó anteriormente que el uso de ba o frío no sólo no disminuye la eficiencia del tratamiento dialítico sino que mejora la estabilidad cardiovascular sobre todo en las poblaciones con mayor predisposición a padecerla: diabéticos, a osos, cardiópatas. En este trabajo se observó que la población con diabetes presentó temperaturas basales bajas antes del tratamiento dialítico, que el uso de ba o a 35.5 °C aumentó menos la temperatura post diálisis que con el ba o estándar de 37 °C. El ba o a 35.5 °C disminuyó los episodios de hipotensión arterial desarrollando un mejor bienestar en los pacientes y modificando las características en el tratamiento por generar menor requerimiento de atención y eventualmente observar repercusión en los costos del tratamiento. Evolucion de ni os post-trasplante hepático luego del primer a o de sobrevida Evolution of children one year post liver transplant

Abstract: El trasplante hepático constituye la única alternativa terapéutica para numerosas enfermedades hepáticas avanzadas. En el seguimiento post-trasplante pueden observarse complicaciones de diversa gravedad. El objetivo de este estudio fue analizar la evolución a largo plazo de los pacientes trasplantados con un seguimiento mayor de un a o post quirúrgico. En el período 11/92-11/01 se realizaron 264 trasplantes en 238 pacientes; incluyéndose en este trabajo 143 ni os con más de un a o post-trasplante. La mediana de edad ± DS fue de 5.41 ± 5.26 a os (r: 0.58-21.7 a os). Catorce (9.79%) fueron retrasplantados. Todos los pacientes recibieron ciclosporina (inmunosupresión primaria). Las indicaciones de trasplante fueron: falla hepática fulminante (n: 50); atresia de vías biliares (n: 38); cirrosis (n: 37); colestasis crónica (n: 13) y otras (n: 5). Las indicaciones de retrasplante fueron: cirrosis biliar (n: 7); trombosis de la arteria hepática (n: 4) y rechazo crónico (n: 3). Fueron utilizados injertos reducidos en 73/157 trasplantes(14 donantes vivos relacionados y 11 biparticiones hepáticas). La sobrevida global fue de 93%, paciente y 86%, injerto. El retrasplante y el injerto reducido fueron las variables de mayor significación para el aumento del riesgo de muerte. El déficit de talla y masa ósea se recuperó antes de los 3a os post-trasplante. La incidencia de síndrome linfoproliferativo fue del 7.69%. Se diagnosticó hepatitis B de novo en 7 pacientes (4.8%). El riesgo social no afectó la sobrevida. La prevención, detección y tratamiento precoz de las complicaciones en el seguimiento a largo plazo permitió mejorar la evolución de los pacientes. Orthotopic liver transplantation is the only definitive mode of therapy for children with end-stage liver disease. However, it remains challenging because of the necessity to prevent long-term complications. The aim of this study was to analyze the evolution of transplanted patients with more than one year of follow up. Between November 1992 and November 2001, 238 patients underwent 264 liver transplantations. A total of 143 patients with more than one year of follow up were included. The median age of patients ± SD was 5.41 years ± 5.26 (r: 0.58-21.7 years). All children received primary immunosuppression with cyclosporine. The indications for liver replacement were: fulminant hepatic failure (n: 50), biliary atresia (n: 38), cirrhosis (n: 37), chronic cholestasis (n: 13) and miscellaneous (n: 5). Abstract: abstract training activity for all the workers of the cuban health system is permanent. an applied, with educative intervention, almost experimental having a cross-sectional character research was carried out with the purpose of assessing the level of knowledge and skills that nursing assistants (na) had about breastfeeding procedures; this allowed the implementation of a new 40 hours-duration strategy establishing an initial and final evaluation at "hermanos cruz" university outpatient clinic from february to june 2008. the universe (49 na) was chosen by means of an intentional sample. the research methods used were empirical, surveys (questions and verbal interview), descriptive and deductive statistics ( odds ratio test 95% of certainty). results: before the intervention the < of the nursing assistants had knowledge of breastfeeding procedures. after the educative intervention, skills, level of information, duration, manual extraction and ways of conservation increased (p<0,01) significantly. the training course to teach breastfeeding techniques for nursing assistants showed a very significant change in the achievement of knowledge and skills.
