Significance and challenges of the tourism policy in the modern world

Abstract: Raising awareness in expert circles and the general public about the heterogeneity of effects caused by tourism (economic, social, cultural, ecological), imposed the need for organized guidance of development of this activity and its placement under the framework of public (government) policy. Accordingly, today in almost all developed tourist countries are being used different instruments to control the development of this complex phenomenon of the modern age. In this paper, special attention is paid to the analysis of the role, importance, models, processes of development and implementation of tourism policy, in the context of the general policy and other sectoral policies brought by governments in developed tourist countries and countries trying to become such. Tourism policy today is the starting point of the thinking process in the tourism industry. It is focused on directing the development of tourism at all levels (international, national, regional, local), that is, it must create a long-term and integral development perspective.
Reduction of myocardial hypertrophy after aortic valve replacement

Abstract: Background/Aim. Aortic valve disease - stenosis and regurgitation are the cause of increased homodynamic stress of the left ventricle (LV) which then develops an adaptive mechanism of cardiac muscle hypertrophy. The aim of this study was to establish if aortic valve replacement procedure (AVR) reduces myocardial hypertrophy and if it does in what period of time. Methods. Eighty-six patients who had been operated for AVR in the Clinical Center of Serbia were included in this investigation. In the every patient the aortic valve had been replaced with a mechanical valve prosthesis. Transthoracic echocardiography examination (TTE) was performed in all of the patients before, and one week after the operation, while 22 patients were followed-up on a long term basis. The LV mass was determined with the formula according to the Pen convention. Results. In the tested group there was significantly more male than female individuals (n = 57-66.3%, 29-337%). Twelve patients (14%) were operated for isolated aortic stenosis, 22 patients (25.6%) for aortic regurgitation, 48 patients (55.8%) for combined aortic valve disease, while 4 patients (4.7%) for endocarditis. Student t test did not show any significant difference in diastolic septal thickness before and after the operation (p = 0.88), while it did show that the difference in the LV mass before and after the operation was highly significant (p = 0.000). This test also showed that, taking the mass of 240 g as the border value for hypertrophy of LV, the reduction of LV mass between preoperative and early postoperative finding was not significant (p = 0.5), while the reduction in LV mass between late and early postoperative examination was statistically significant (p = 0.000). In 19 of 22 patients who were followed-up postoperatively over a long period (84 months after the operation) significant reduction of LV mass was registered. The mean time of the reduction was 27.5 months. Conclusion. This study showed the presence of a significant reduction in the LV mass after AVR, and that the mean time required for this process was more than two years.
Papillary muscle head rupture in a patient with normal coronarography findings

Abstract: Background. Rupture of papillary muscle generally happens during acute myocardial infarction and is the cause of acute mitral regurgitation, pulmonary oedema, so it should be promptly recognized and managed. Case report. A patient, 52 year-old, was admitted to the Thoracic Department with fever, general weakness, dyspnea and cough as a case of suspected pneumonia. Two days before the admission he was treated with antibiotics. At thoracic ward, his clinical status got serious and he transferred to Intensive Care Unit (ICU) as pulmonary oedema. At the time of admission to ICU the patient was seriously ill with tachycardia, tachydyspnea, orthopnea and cyanosis image. Auscultatory, he showed pulmonal stasis at both sides and a tachyarrhythmic action, with a systolic murmur 5/6 grade above the mitral valve. Echocardiography showed grave mitral regurgitation with prolapsus of posterior leaflet with suspected chordal rupture. At coronarography no significant lesions of coronary arteries were found. After hemodynamic stabilization the patient was operated. During the operation, Transesophageal echocardiography (TEE) examination showed a rupture of the head of the posteromedial papillary muscle. He was surgically treated with atypical quadrantectomy of posterior leaflet with homologous pericardial patch anuloplasty. Conclusion. The recognition of acute mitral regurgitation caused by the papillary muscle rupture and prompt surgical treatment is of vital interest for the survival of patients.

Abstract: The research was made on a sample of 151 female volleyball players at the age of 14-18, who took part in the youth tournaments in the Republic of Macedonia, in the season 2009/10. The study applied 18 variables to estimate the anthropometric characteristics. In order to determine the structure of anthropometric characteristics it has been applied a component factor analysis. Based on the obtained results it can be determined that in the structure of anthropometric characteristics there are three latent factors existing: factor of the volume of the body and subcutaneous fat, factor longitudinal dimensionality of the body and factor of the transverse dimensionality of the body.

The role of Streptococcus mutans group and salivary immunoglobulins in etiology of early childhood caries

Abstract: Dental caries is a bacterial infective disease modified by carbohydrates from food. Early childhood caries is a special type of primary teeth caries in infants and toddlers. Appearance of early childhood caries (ECC) is related to mistakes in nutrition of infants, but the mechanism of beginning and progression of this disease is very complex. To understand etiology of caries, it is necessary to understand ecology of oral environment and to determine which factors are responsible for conversion of non-pathogenic microorganisms of the oral cavity into pathogenic. Among host factors, salivary immunoglobulin (sIgA) constitutes the main specific immune defense mechanism in saliva and may play an important role in the oral homeostasis. Basic role of salivary immunoglobulins is in control of bacterial oral flora and constitution of balance relationship between oral bacteria and organism as whole.

Abstract: This study examined the uptake, translocation, distribution and accumulation of Cd, Zn and Fe and the interaction of these elements in winter wheat. The objectives were: (1) to characterise specificity among winter wheat genotypes in terms of accumulation of Cd, Zn and Fe in various organs and identify genotypes combining low accumulation of Cd with high accumulation of Zn and/or Fe, (2) to determine genetic specificity of winter wheat genotypes in terms of translocation of Cd, Zn and Fe from the vegetative parts to the grain, and (3) elucidate an effect of soil cadmium contamination on the distribution and accumulation of Cd, Zn and Fe in various organs of wheat. In the 2007/2008 vegetation season, 52 winter wheat varieties (34 Croatian, 7 Austrian, 5 Hungarian, 3 French and one variety of Russian, Italian and German descent) were investigated. The pots were arranged, according to a completely randomized design with two levels of soil Cd contamination (0 and 20 mg kg-1 soil) in four replicates. In the vegetation season of 2008/2009 the experiment was set up to a completely randomized block design with 10 varieties of wheat and three levels of soil Cd contamination (0, 2 and 5 mg kg-1 soil) in four replicates. The concentration of Cd, Zn and Fe in the solution of plant samples was determined by ICP-OES. The concentration of Cd, Zn and Fe was determined at the flowering stage in the root, stem,leaves, flag leaf and spike and in a full maturity in the straw, leaves, glumes and grain. Collected data were statistically analyzed with SAS software 9.1.3. Analysis of variance identified a statistically significant difference in the concentrations of Cd, Zn and Fe in vegetative parts and grain between the tested wheat varieties at all levels of soil Cd contamination. It was also found that the soil Cd contamination had a significant effect on the accumulation of Cd in grain while the accumulation of Zn and Fe in the grain hasn’t been influenced by soil Cd contamination. The tested genotypes differed significantly in the Cd, Zn and Fe plant uptake factor as well as in the coefficient of translocation from roots to stems and from roots to the flag leaf. According to correlation coefficient between the concentrations of Cd, Zn and Fe in the grain, the tested cultivars were divided into six groups which differ in the interrelation of these elements. Abstract: Odnos profesionalnih nogometa a i njihovih klubova esto biva razli ito definiran. U pojedinim dr avama rije je o radnopravnom, a u drugim o gra anskopravnom odnosu. U navedenom radu autori iznose tezu da ugovor zaklju en izme u profesionalnog nogometa a s jedne strane i njegova kluba s druge strane, ima elemente radnopravnog odnosa, pogotovo u kontekstu europskog (radnog) prava. Navedeno autori razra uju izu avanjem sudske prakse Europskog suda u Luksemburgu, mnogobrojnih akata tzv. mekog prava EU“, komparativnopravne usporedbe pojedinih dr ava EU (Italije, Velike Britanije i Njema ke), te prakse Arbitra nog suda za sport (CAS) i nadle nih FIFA tijela.Analiza samog radnog odnosa i njegove transformacije iznimno je bitan imbenik koji poma e u definiranju odnosa profesionalnog nogometa a kao radnika s jedne strane i profesionalnih nogometnih klubova kao poslodavaca s druge strane. U radu autori iznose svoje vi enje problematike definiranja odnosa sporta a i klubova u obliku ugovora gra anskog prava, analizom nedostataka takvog definiranja, te iznose svoje prijedloge de lege ferenda.
