Perfil e adequa o dos medicamentos prescritos para idosos internados em hospital de ensino da cidade de Fortaleza - CE

Abstract: O presente trabalho foi desenvolvido entre outubro de 2006 e maio de 2007 em um hospital público de ensino da cidade de Fortaleza onde participaram 50 idosos com idade igual ou superior a 60 anos. O objetivo do estudo foi avaliar o perfil e adequa o dos medicamentos prescritos para pacientes idosos internados tendo como base os Critérios de Beers, Critérios Canadenses e indicadores suecos de qualidade da farmacoterapia em idosos. Do total de 50 pacientes idosos admitidos no estudo, 42% eram homens e 58% eram mulheres. Metade dos idosos se encontravam na faixa etária dos 60 a 69 anos. Considerando-se os Critérios de Beers e Canadenses, 84% dos pacientes tiveram prescri es de medicamentos a serem evitados. Foram contabilizados 17 medicamentos com potencial de inadequa o em idosos e o mais freqüentemente prescrito foi metoclopramida, seguido de óleo mineral e diazepam. Os resultados mostraram que um percentual elevado recebeu prescri o de medicamento inadequado, demonstrando a necessidade de um melhor acompanhamento de prescri es voltadas para o paciente idoso.

Abstract: objective: to analyze polypharmacy (daily use of two or more) of continuous use medication and its associated factors in the elderly. methods: a cross sectional study conducted with elderly individuals living in an urban area of the municipality of quixadá, ceara, brazil from may to december, 2009. the sample consisted of 384 individuals who were 60 years old or older and users of continuous medication. the variables studied were sociodemographic, economical, and health characteristics related to use of medications. for the analysis of the association between the dependent variable polypharmacy of continuous-use medication and the independent variables a logistic regression model was developed. results: results showed a predominance of females, between 60 and 69 years old, married, with incomplete elementary school education, non-paid job, family income of one minimum salary, living in their own home and together with up to three individuals. a prevalence of 70.6% of polypharmacy in the elderly was found, and it was higher in women (66.4%). factors positively associated with the use of two or more continuous-use medication were: family income over one minimum salary (or 2.83; ci95% = 1.54-5.32); two or more self-reported chronic conditions (or 17.71; ci95% = 9.80-31.990) and self-perception of regular to poor quality of life (or 2.85; ci95% = 1.60-5.07). conclusions: the study observed a practice of polypharmacy of continuous-use medications in the elderly with family income over one minimum wage, two or more chronic conditions and self-perception of regular to poor quality of life. such status leads to questions on social, cultural, economic and health aspects.

Abstract: background: the morbi-mortality by the use of medicines is a major health problem. the drug adverse reactions may result in death, increased hospitalizations and healthcare costs. objectives: describe and analyze reports of suspected adverse reactions caused by drugs that act on the nervous system (sn-adr), registered in the database of the pharmacovigilance centre of ceará, from january 1997 to march 2008. methods: all the ns-adrs were classified according to criteria of the world health organization. the causality between the drug administered and the adverse reaction was established, as well as the analysis regarding to the severity of the reaction. results: the centre recorded 176 notifications of ram-sn. the most of the reactions occurred in the hospital. the main reporter was the pharmacist. all the ram-sn were classified as possible (n = 110), probable (n = 37) and definite (n = 17). with regard to severity, the ns-adrs were considered: light (n = 21), moderate (n = 127), serious (n = 15) and fatal (n = 1). the fatal case was reported by physician, and involved anesthetic drugs. in general the adverse reactions observed were caused predominantly by analgesics, anesthetics and antiepileptics. discussion: the data demonstrate the potential values of accessing to a local system of pharmacovigilance to report possible risks with the use of nervous system drugs.

Abstract: this paper presents a comparative analysis of specific aspects of concession agreements for water supply and sewage collection and treatment services in four brazilian counties, undertaken in recent years, within the context of overcoming the legal model instituted by planasa as of the 1970s. the contracts were analyzed about permission payment, tariffs, coverage and investment goals, regulation, supervision and social control, and the right of consumers. the analysis performed showed the lack of uniformity in regard to relevant concession issues, suggesting that this may result from failure to define a new regulatory benchmark applicable to the sanitation sector.
O estágio extracurricular na forma o profissional: a opini o dos estudantes de fisioterapia The academic training for the professional education: the opinion of the Physiotherapy students

Abstract: Investigar a opini o dos estudantes de Fisioterapia em rela o ao estágio extracurricular e a influência dessa prática para sua forma o profissional. Realizou-se um estudo transversal com alunos de Fisioterapia de uma institui o de ensino superior do estado do Ceará, Brasil. A amostra foi composta de 157 estudantes, sendo 29 homens (18,5%) e 128 mulheres (81,5%), com média de idade 22,6±4,02 anos, do segundo ao último semestre da gradua o. Aplicou-se um questionário com 20 quest es objetivas abordando a vivência e repercuss es dessa prática. Do total de alunos pesquisados, 61 (38,9%) afirmaram ter participado de estágio extracurricular. Desses, 55 (90,2%) foram motivados pela necessidade de adquirir experiência; 53 (86,9%) realizaram interven es nos pacientes; e 13 (21,3%) com carga horária de 15 a 20 horas/semanais. Porém, 36 acadêmicos (59,0%) o fizeram em desacordo com as normas do Conselho Federal de Fisioterapia e Terapia Ocupacional. Os acadêmicos consideraram que o estágio extracurricular foi uma ferramenta de aprimoramento para sua forma o profissional, pois propiciou o exercício de procedimentos e técnicas mais comuns da atua o do fisioterapeuta. Entretanto, alerta-se para a necessidade de fiscaliza o dos órg os competentes, para que o aluno exer a fun es compatíveis com o seu nível de qualifica o. This study aimed to investigate the opinion of the Physiotherapy students on the extracurricular internship and the influence of this practice on their professional formation. A cross-sectional study was performed with Physical Therapy students from a higher education institution in Ceará, Brazil. The sample consisted of 157 students, 29 men (18.5%) and 128 women (81.5%), with mean age of 22.6±4.02 years, from second to last semester of graduation. A questionnaire was applied with 20 objective questions covering the experiences and repercussions of this practice. A total of 61 (38.9%) surveyed students affirmed to have participated of extracurricular internship. Out of these, 55 (90.2%) were motivated by the necessity of acquiring experience; 53 (86.9%) were performing therapeutic interventions in patients; and 13 (21.3%) had 15 to 20 working hours. However, 36 academics (59.0%) did work in disagreement with the rules of the Federal Council of Physical Therapy and Occupational Therapy. The students considered the extracurricular internship as a tool of improvement of their professional formation, because it allowed the practice of common physical therapist daily life activities procedures and techniques. Abstract: It is known that temperature and thawing velocity can affect sperm quality during freeze-thawing procedures. Thus, the aim of this research was to compare different thawing protocols for canine semen extended in ACP-106 extender and cryopreserved. Ten ejaculates were collected from eight dogs and sperm rich fraction was evaluated regarding volume, concentration, motility, vigor, pH, morphology, acrosomal integrity and hyposmotic swelling test. Then, semen was extended in ACP-106 with 20% egg yolk and 6% glycerol, and frozen in 0.25 mL straws. After one week, one straw was thawed at 37°C/60s, another one at 55°C/5s and a third one at 75°C/8s, and the seminal characteristics were reevaluated. Similar results were observed for all the groups, except for sperm motility which was better preserved at 55°C/5s for up 30min than at 37°C/60s or 75°C/8s. From these results, it is concluded that canine semen extended in ACP -106 is better thawed at 55°C/5s.
Carbon Capture and Storage and the UNFCCC: Recommendations to Address Trans-Boundary Issues  [PDF]

Abstract: This paper is mainly focused on the eligibility of trans-boundary Carbon Dioxide Capture and Storage (CCS) as a Clean Development Mechanism (CDM) project activity and it aims to explore a broad range of CCS transboundary issues whereby the United Nations Framework Convention on Climate Change (UNFCCC) and national governments could take into consideration. Much work is still to be done by the CCS community (including the Institute) to ensure that the implementation of CCS under the CDM is both environmentally effective and commercially attractive. The recommendations focus on technical issues, with the aim of helping Parties evaluate a robust strategy for CCS as part of international negotiations and establish CCS best practice criteria for governments and the international process, thereby enhancing transparency and ensuring that CCS deployment is safe and effective. When considering CDM projects activities with a transboundary component, it is recommend that the rules of the main international treaties related to CCS be considered by the UNFCCC, especially the London Protocol guidelines for risk assessment and management and the 2006 IPCC Guidelines. Additionally, national governments should apply the rules and guidelines as delineated under the relevant existing international treaties and CCS national regulations.

Abstract: In this paper, we present a brief synthesis of the record of Fell or Fishtail points found in northeast Argentina, including the first two records for Misiones province. To date, 11 pieces have been identified in the region, which demonstrate techno-morphological similarities with specimens recovered in different areas of South America, which are linked to Paleoamerican groups of the Pleistocene-Holocene boundary. Although these points are still scarce in this area, this is probably due to the lack of research and issues related to archaeological visibility. These new records increase the density of findings of these early hunter-gatherers on the South American Atlantic slope.

Abstract: purpose: to standardize recordings of pattern electroretinograms (perg) in normal human subjects. methods: the standardization followed the model proposed by the international organization of eletroretinography and was specific for the laboratory of evoked potentials of the department of neurology, faculty of medical sciences, state university of campinas. two types of stimuli were used, one of 30 minutes of arch of visual angle and another of 60 minutes of arch of visual angle. results: in all patients a positive wave, defined internationally as p50, and a negative, called n95, were obtained without interferences. the normal range for latency, amplitude and duration of each wave was obtained. there were significative differences in amplitude, latency and duration of p50 and n95 waves among the several age groups. there was an increase in latency and a decrease in amplitude with increasing age. no significative differences were found among the variables comparing different stimuli, sex or when eyes were compared with each other. conclusions: the standardization of pattern electroretinograms yield reproductibility of results and the possibility of further comparative studies.
Functional assesment of patients with AIDS disease

Abstract: we documented the types and degree of functional disability in 74 patients with aids at the hospital de clínicas of the federal university of paraná, brazil. few of these patients are referred for rehabilitation services and there is only a limited team approach in their care. we found that 91% of the patients had some degree of functional impairment; in 81% the complaint was weakness and in 47% it was neurological involvement. according to the barthel index, 79% were considered functionally independent, 14% partially dependent and 7% dependent. for most of them, independence requires effort and their quality of life is reduced. severe disability was rather unusual, while mild or moderate disability levels were not. the functional physical disability found in 21% of the patients required management by a rehabilitation team. we conclude that it is important to focus attention on the quality of life of patients with aids.
