Abstract: Este artículo representa una parte de los análisis del proyecto de investigación “Dinámicas de privatización exógenas y endógenas en y de la educación: la implantación del modelo de cuasimercado en Espa a”, financiado por el Ministerio de Educación a través del Plan Nacional de I+D (REF. EDU2010/20853). Una de las líneas de investigación del trabajo se está desarrollando en la Comunidad de Madrid y tiene por objetivo principal rastrear y hacer explícitas las dinámicas de privatización endógenas y exógenas en el sistema educativo local, para el posterior análisis de los efectos pedagógicos de estas dinámicas. La Administración educativa de la Comunidad de Madrid tiene como objetivo prioritario la promoción del derecho de los padres a elegir el centro escolar. De la ampliación de las posibilidades de elección de centro se derivarán a su vez, según defiende la Administración regional, la promoción de la competencia entre los centros, que se acompa ará de la mejora de la calidad de la educación. Para lograr este objetivo final, está llevando a cabo dos líneas de actuación fundamentales en su agenda política educativa: la promoción de la autonomía de los centros escolares en aras de su especificidad y la publicación de los resultados de los centros educativos. En este artículo se presentan los resultados del análisis de la efectividad de estas políticas en relación con la mejora de la calidad de la educación desde tres perspectivas: la implementación de estas acciones por parte de la Administración educativa, lo que constituye un análisis de coherencia interna de las políticas educativas en la Comunidad de Madrid, la efectividad real que están teniendo, es decir, su influencia en la calidad educativa del sistema como un conjunto, y por último, la recepción de estas reformas tanto por las familias como por los centros educativos, que constituyen respectivamente la demanda y la oferta en los procesos de elección que la Administración regional trata de fomentar.

Ursodeoxycholic Acid Is Conjugated with Taurine to Promote Secretin-Stimulated Biliary Hydrocholeresis in the Normal Rat

Abstract: Background & Aims Secretin induces bicarbonate-rich hydrocholeresis in healthy individuals, but not in untreated patients with primary biliary cirrhosis (PBC). Ursodeoxycholic acid (UDCA) – the first choice treatment for PBC – restores the secretin response. Compared with humans, secretin has poor effect in experimental normal-rat models with biliary drainage, although it may elicit hydrocholeresis when the bile-acid pool is maintained. In view of the benefits of UDCA in PBC, we used normal-rat models to unravel the acute contribution of UDCA (and/or taurine-conjugated TUDCA) for eliciting the biliary secretin response. Methods Intravascular and/or intrabiliary administration of agonists and inhibitors was performed in normal rats with biliary monitoring. Secretin/bile-acid interplay was analyzed in 3D cultured rat cholangiocytes that formed expansive cystic structures with intralumenal hydroionic secretion. Results In vivo, secretin stimulates hydrocholeresis upon UDCA/TUDCA infusion, but does not modify the intrinsic hypercholeretic effect of dehydrocholic acid (DHCA). The former effect is dependent on microtubule polymerization, and involves PKCα, PI3K and MEK pathways, as shown by colchicine (i.p.) and retrograde biliary inhibitors. In vitro, while secretin alone accelerates the spontaneous expansion of 3D-cystic structures, this effect is enhanced in the presence of TUDCA, but not UDCA or DHCA. Experiments with inhibitors and Ca2+-chelator confirmed that the synergistic effect of secretin plus TUDCA involves microtubules, intracellular Ca2+, PKCα, PI3K, PKA and MEK pathways. Gene silencing also demonstrated the involvement of the bicarbonate extruder Ae2. Conclusions UDCA is conjugated in order to promote secretin-stimulated hydrocholeresis in rats through Ae2, microtubules, intracellular Ca2+, PKCα, PI3K, PKA, and MEK.

Abstract: objectives: classically, it was understood detrusor contractility was affected in bladder areflexia but not in hyperreflexia. we aim to demonstrate that there is some detrusor contractility affectation in bladder hyperreflexias, although it may seem paradoxical at first sight. methods: neuro-urologic study was performed in a series of 68 consecutive patients with bladder hyperreflexia (neurogenic detrusor hyperactivity), referred to the unit of functional urology for urodynamic evaluation. all patients underwent clinical history, post void residual determination, cystomanometry, voiding pressure-flow test, videourodynamics and selective emg of the periurethral sphincter. detrusor contractile potency was measured with w 80-20. terminology follows the ics standard. data processing was performed using the statistical software spss 12.0. results: this series includes 14 males (58.8%) and 28 females (41.2%), of all ages (adults more frequent). 88.2% of the cases had acquired neurological diseases (trauma 30.9%; medical 33.8%; degenerative 35.3%), and 11.8% congenital neurological diseases (myelodysplasia). most frequent type of neurological lesion was detrusor-sphincter dyssynergia: 59 cases (79.4%); 22 patients (32.4%) had undergone previous treatment of their vesicourethral dysfunction. the amplitude of detrusor involuntary contractions was wider in detrusor-bladder neck dyssynergia. detrusor isotonic contractility (w 80-20) was significantly affected, mainly in neurological lesions of traumatic nature and less in degenerative lesions (p = 0.01). conclusions: we demonstrated that bladder contractility is affected in bladder hyperreflexias (neurogenic detrusor hyperactivity), more important in traumatic lesions without relationship with the type of neurological associated lesion, which could be related to a bladder structural change, or with a change in the central neurological tracts (without requirement of presence of dyssynergia).
Función pulmonar en pacientes con anemia drepanocítica

Abstract: in order to assess the ventilatory capacity in patients with sickle cell anemia were selected and assisted at leon cuervo rubio provincial hospital hematology department, from january - april 2003. the selection of patients was randomized. along with the selection of clinical and hematological parameters obtained from the micro-histories made for the followup of patients; the patients were analyzed as for the lung performance. results from ventilarory study were related to the selected variants using microstat and applying chi ? square test. it was found all patients, exept one, presented ventilatory dysfunction, mostly restrictive. the obstructive component was present in 50 % cases, some of them as unique abnormality. there were no patients with chronic respiratory symptoms and the thoracic radiography was normal in all cases. there was no association between the typeof severity of the ventilatory dysfunction and the behaviour of the clinical and hematological variates evaluated. as a conclusion, the ventilatory repercussion in patients with sickle cell anemia must be taken into account even in the absence of clinical translation, and independently of the clinical and hematological behaviour of the disease.
Prevención y control del parasitismo intestinal en la atención primaria de salud

Abstract: the technical capacity of the general comprehensive physicians, parasitology laboratory technicians and epidemiologists from the health area of "centro habana" municipality was assessed from january to april, 1996, aimed at evaluating the quality of primary health care in relation to the prevention and control of intestinal parasitism. surveys, external quality controls and individual interviews were made. it was found that there was no integration in the actions of the evaluated groups. medical difficulties for the prevention and control of intestinal parasitosis as well as technical deficiencies in the laboratories were found. it was observed that epidemiologists had little knowledge about the program of intestinal parasitism. the training of technicians and professionals and the creation of a methodology that allows to reorient and optimize the national program of intestinal parasitism were recommended

Abstract: Se valoró la capacidad técnica de Médicos Generales Integrales, técnicos del laboratorio de parasitología y epidemiólogos de las áreas de salud del municipio Centro Habana, de enero a abril de 1996, con el objetivo de evaluar la calidad de la atención primaria de salud para la prevención y control del parasitismo intestinal. Se aplicaron eneustas, controles de calidad externos, y entrevistas individuales a profundidad. Se encontró que no existe integración en las acciones de los grupos evaluados, detectándose dificultades médicas para la prevención y control de las parasitosis intestinales, deficiencias técnicas en los laboratorios y desconocimientos del programa de parasitismo intestinal por los epidemiólogos.
