Abstract: this is a study conducted by the unit for health technology assessment of universidad simon bolivar, in order to assess the technology of 54 outpatient primary health centers for the government of miranda state, venezuela. the health network is of recent creation (2009), when the central government absorbed by presidential decree its original 250 outpatient facilities. in the discussion of the results, a proposal was made for the implementation of a health plan and technology management, to improve the performance of health programs developed by the government.

Abstract: the aim of this study was to evaluate the quality of the cardiology service at the dr. carlos arvelo military hospital based on the criteria of the venezuelan state cardiovascular program. during the study period (1990-2009), two completely different political systems were in effect and had an impact on the research results. the study had a retrospective approach. we used two types of instruments and had 1421 patients discharged from the institution with a diagnosis of acute myocardial infarction as the unit of analysis. for the assessment, we used the simple random sampling audit method. the results achieved were: a) during the 1990-1999 period the quality of service was 73%, b) during the 1999-2009 period the quality was quantified as 58%. the institution was not prepared for an increase of 75% in the population served during the second period, as a result of the implementation of a policy that worsened the quality of the service.
Manejo de desechos hospitalarios en un hospital tipo IV de Caracas, Venezuela

Abstract: medical wastes comprise the different varieties of waste generated in health establishments and, in view of their pathogenic potential and the inefficiency of its management, represent a health risk. a system for the internal management of the medical wastes in a type iv hospital in caracas, venezuela, was designed and validated. the current situation was diagnosed, based on decree 2218 of the republic of venezuela; a medical waste management system was proposed and validated. the results indicate that the hospital complies with less than the 50% of the existing legal regulation. the validation of the proposed waste management system revealed that the hospital generated at the beginning of the study a total of 724.06kg/day of waste, of which 181.33kg/day corresponded to infectious waste. after implementation of the proposed system the total quantity of infectious waste diminished to 129.98kg/day. it was verified that this type of system, diminishes significantly the quantity of infectious waste, reducing processing costs and the risks for the hospital population and for the public in general. for the establishment of this type of system, all the hospital community must be incorporated, since without the aid of all the personnel, the efforts to diminish the risk that this waste represents for health would be useless.

Parametrizing the abstract Ellentuck theorem

Abstract: We give a parametrization with perfect sets of the abstract Ellentuck theorem. The main tool for achieving this goal is a sort of parametrization of an abstract version of the Nash-Williams theorem. As corollaries, we obtain some known classical results like the parametrized version of the Galvin-Prikry theorem due to Miller and Todorcevic, and the parametrized version of Ellentuck's theorem due to Pawlikowski. Also, we obtain parametized vesions of nonclassical results such as Milliken's theorem.
A notion of selective ultrafilter corresponding to topological Ramsey spaces

Abstract: We introduce the relation of "almost-reduction" in an arbitrary topological Ramsey space R, as a generalization of the relation of "almost-inclusion" on the space of infinite sets of natural numbers (the Ellentuck space). This leads us to a type of ultrafilter U on the set of first approximations of the elements of R which corresponds to the well-known notion of "selective ultrafilter" on N, the set of natural numbers. The relationship turns out to be rather exact in the sense that it permits us to lift several well-known facts about selective ultrafilters on N and the Ellentuck space to the ultrafilter U and the Ramsey space R. For example, we prove that the Open Coloring Axiom holds in M[U], where M is a Solovay model. In this way we extend a result due to Di Prisco and Todorcevic which gives the same conclusion for the Ellentuck space.

Abstract: This article studies people relegated to invisibility: the LGTB (lesbians, gays, transsexuals and bisexuals) from constitutional and international law point of view. Thoughts will be focused on transsexuals and their rights to be treated according to an early internalized gender and some rights emerged from this treatment. Peruvian State role in this subject and the existing challenges will also be analyzed in the same way. El presente artículo es un estudio de una población relegada a la invisibilidad, los LGTB (lesbianas, gays, transexuales y bisexuales), desde el derecho constitucional y el derecho internacional. La reflexión girará entorno a los transexuales y al derecho que tienen estos a ser tratados como personas pertenecientes al género que han internalizado desde temprana edad. Asimismo, se discutirá algunos derechos que se desprenden de este trato. De la misma manera, se analizará el papel del Estado peruano en este tema y los retos que todavía existen. El ensayo es ganador del II Concurso de Investigación Jurídica de Responsabilidad Social, realizado en el segundo semestre de 2012.
Expression and function of beta1 integrins on human eosinophils

Abstract: eosinophils preferentially accumulate at sites of chronic allergic diseases such as bronchial asthma. the mechanisms by which selective eosinophil migration occurs are not fully understood. however, interactions of cell-surface adhesion molecules on the eosinophil with molecular counterligands on endothelial and epithelial cells, and on extracellular matrix proteins, are likely to be critical during the recruitment process. one possible mechanism for selective eosinophil recruitment involves the a4b 1 (vla-4) integrin which is not expressed on neutrophils. correlations have been found between infiltration of eosinophils and endothelial expression of vcam-1, the ligand for vla-4, in the lungs of asthmatic individuals as well as in late phase reactions in the lungs, nose and skin. epithelial and endothelial cells respond to the th2-type cytokines il-4 and il-13 with selective de novo expression of vcam-1, consistent with the possible role of vcam-1/vla-4 interactions in eosinophil influx during allergic inflammation. both b 1 and b 2 integrins on eosinophils exist in a state of partial activation. for example, eosinophils can be maximally activated for adhesion to vcam-1 or fibronectin after exposure to b 1 integrin-activating antibodies or divalent cations, conditions that do not necessarily affect the total cell surface expression of b 1 integrins. in contrast, cytokines like il-5 prevent b 1 integrin activation while promoting b 2 integrin function. furthermore, ligation of integrins can regulate the effector functions of the cell. for example, eosinophil adhesion via b 1 and/or b 2 integrins has been shown to alter a variety of functional responses including degranulation and apoptosis. thus, integrins appear to be important in mediating eosinophil migration and activation in allergic inflammation. strategies that interfere with these processes may prove to be useful for treatment of allergic diseases.
