Efficacy and safety of ERCP in a low-volume hospital Eficacia y seguridad de la CPRE en un hospital con bajo volumen

Abstract: Background and aims: there is little scientific evidence on the outcomes of endoscopic retrograde cholangiopancreatography (ERCP) performed in low-volume hospitals; however, in our country, it is growing up its implementation. The objectives of our study were to evaluate the efficacy and safety of this technique performed by two endoscopists with basic training in a center of this nature and analyze the learning curve in the first procedures. Patients and methods: single-center retrospective study of the first 200 ERCP performed in our hospital (analyzing the evolution between the first 100 and 100 following procedures), comparing them with the quality standards proposed in the literature. Results: from February 2009 to April 2011, we performed 200 ERCP in 169 patients, and the most common indications were: Choledocholithiasis (77 %), tumors (14.5 %) and other conditions (8.5 %). The cannulation rate rose from 85 % in the first 100 ERCP to 89 % in the next 100 procedures, clinical success from 81 % to 87 %, decreasing the post-ERCP acute pancreatitis rate from 11 % to 4 %, upper gastrointestinal bleeding (UGIB) from 3 % to 2 % and acute cholangitis from 4 % to 1 %. There was a death from a massive UGIB in a cirrhotic patient in the first group of patients and a case of biliary perforation resolved by surgery in the second one. Conclusions: the results obtained after performing 200 procedures support the ability to practice ERCP in low-volume hospitals obtaining levels of efficacy and safety in accordance with published quality standards. Introducción y objetivos: existe poca evidencia científica sobre los resultados de la CPRE realizada en hospitales con bajo volumen, sin embargo su puesta en marcha en nuestro medio es creciente. Los objetivos de nuestro estudio son evaluar la eficacia y seguridad de dicha técnica realizada por dos endoscopistas biliares noveles en un centro de estas características y analizar la curva de aprendizaje en los primeros procedimientos. Pacientes y métodos: estudio retrospectivo de las primeras 200 CPRE practicadas en nuestro hospital, analizando la progresión entre los 100 primeros procedimientos y los 100 segundos, comparándolos con los estándares de calidad propuestos en la literatura. Resultados: desde febrero de 2009 hasta abril de 2011 se realizaron 200 procedimientos a 169 pacientes con las siguientes indicaciones: coledocolitiasis (77 %), neoplasias (14,5 %) y otras patologías (8,5 %). Abstract: Las transformaciones que hoy se proponen en la Educación Médica, dada la descentralización de la ense anza en los municipios, harán posible un abordaje educacional más innovador, apoyado en las tecnologías de la información y la comunicación (TIC), acercando al estudiante al escenario profesional desde el inicio de la Carrera. Todo ello permitirá un equilibrio entre la docencia, la investigación y la asistencia. Como resultado de este redise o de programas en las Ciencias Médicas, donde el aprendizaje es a nivel del Policlínico Universitario, se ha hecho necesario realizar transformaciones en las Formas de Organización de la Ense anza (FOE), en la Atención Primaria de Salud. Con el presente trabajo, pretendemos ofrecer nuestros criterios sobre las características de estas FOE, así como brindar elementos metodológicos para la impartición de algunas de ellas, de forma tal que puedan ser utilizadas por igual por el claustro profesoral y poder elevar la calidad del proceso docente. The changes taking place nowadays in the medical education, due to the decentralization of the teaching in the municipalities, will make the educational implementation more innovative and be supported by the information and communication technologies (ICT). This gets the students closer to the professional scenarios from the beginning of the studies. This will allow a balance between teaching, research and medical assistance. Due to this program redesign in the Medical Sciences field where the learning is at the university polyclinic level, other changes at the Primary health care in the teaching organization forms (TOF) need to be implemented as well. This paper aims at providing our criteria about the characteristics of the TOF, as well as providing the methodological issues for their teaching in such a way they can be used by the teaching staff and increase the teaching process.

Abstract: Con la introducción, en la ense anza de las carreras de la salud, el video y la computadora, y la aplicación del Proyecto Policlínico Universitario se hizo necesaria la incorporación al trabajo en la televisión y el video de un grupo de profesores de las diferentes carreras de la salud, del ISCM-Habana, que requiere un nivel de preparación en aspectos propios del lenguaje de este importante medio para cumplir de manera satisfactoria la tarea encomendada. Este material tiene como objetivo acercar nuestro claustro al mundo del audiovisual y a la construcción de guiones, y proporciona un algoritmo de trabajo que facilita la creación de guiones para la filmación de videoclases en las carreras de la salud. Toguether with the introduction, in health careers of the video and the personal computer it was necessary the application in the Project Universitary Policlinic, the outmost incorporation to the work of television and video to a group of professors of different health careers of the University of Medical Sciences in Havana which require a level of preparation in the own kind of language of this media to accomplish, in a satisfactory way the commended task. This material has as a purpose to enclose our teachers to the audiovisual world and to the scripts process and purposes an algorithm of work that eases the script creation for the videoclasess in health careers.

Abstract: Uno de los recursos de aprendizaje más utilizado en la actualidad en los procesos docentes de las Ciencias de la Salud es la Videoclase. Para afrontar la elaboración de estos medios de ense anza es vital proveer al profesor de herramientas de apoyo sencillas y flexibles que le permitan lograr sus objetivos pedagógicos, ofreciendo a su alumnado el soporte y apoyo adecuado para cubrir sus necesidades. Este trabajo brinda una serie de recomendaciones para tratar de unificar los criterios acerca de la elaboración de las Videoclases, así como orientar a los profesores que van a enfrentar este reto dentro del proceso docente. Se analizan los aspectos fundamentales que deben tenerse en cuenta en el momento de la confección de la Videoclase, así como los procesos comunicacionales que están involucrados y las características y responsabilidades que debe tener un teleprofesor. Herpes virus infection associated with interstitial nephritis in a beaked whale (Mesoplodon densirostris)

Abstract: On April 2004, an old adult male Blainville’s beaked whale (Mesoplodon densirostris) 420 cm long with a poor body condition was stranded on Tenerife Island. During necropsy, no gross lesions were observed in the kidneys. However, membranous glomerulonephritis, multifocal interstitial lymphoplasmacytic nephritis and acute multifocal necrotizing tubulointerstitial nephritis with intranuclear inclusion bodies was diagnosed by histological analysis. Tissue samples were submitted for bacteriological analysis and molecular viral screening.A novel alpha herpesvirus associated with interstitial nephritis was identified in an old adult male Blainville's beaked whale (M. densirostris) with a poor body condition stranded in the Canary Islands. This report suggests that identification of herpesvirus infection could be used as a differential diagnosis for interstitial nephritis in cetaceans.The presence of herpesviruses (HV) in cetaceans was shown in the late 1980s by electron microscopy (EM) analysis demonstrating HV-like particles in skin biopsies from beluga whales (Delphinapterus leucas) [1,2]. EM analysis also demonstrated that HV was associated with encephalitis in a harbor porpoise (Phocoena phocoena) [3] and with skin lesions of dusky dolphins (Lagenorhynchus obscures) [4]. In addition to EM, immunoperoxidase staining [3], serum neutralization and enzyme-labeled immunosorbent assays have been used as indicators for the presence of HV in cetaceans [5]. Alpha-HV have been associated with fatal systemic infections in bottlenose dolphins (Tursiops truncatus) [6], a Cuvier’s beaked whale (Ziphius cavirostris) [7], and with cutaneous lesions in bottlenose dolphins [8]. Gamma-HV have been identified in mucosal lesions in bottlenose dolphins, Risso’s dolphins (Grampus griseus), dwarf sperm whales (Kogia sima) and Blainville’s beaked whales (Mesoplodon densirostris) [8-10].The capacity of HV to cause disease in cetaceans is unclear and may show variations depending on the differe

Abstract: background: the prevalence of disability shows a high geographical variability and the identification of factors that could explain these variations can be usuful to the heath and social welfare planning. here the analysis of disability variations among autonomous regions in spain is made taking into account individual and geographical setting factors together. methods: data come from the spanish disability, impairment and health status survey of 1999 and from inebase both of the national institute for statistics. the prevalence of disability crude and adjusted by age was calculated for each autonomous region. individual factors related to disability are identified by means of a logistic regression. the analysis accounting for both, individual and geographical setting factors was performed by means of a logistic regression of two levels. results: disability prevalence showed a maximum difference of 5.75 points among regions. in a logistic regression the region of residence was statistically significant (or: 3.35 in the highest rated region related to the lowest) beside several individual factors: age (or 40-64= 1,78 or 65-79= 1,87 and or >79= 3,34), sex (or women= 0,66), working status (or unemployment=2,25 or housewife/student=1,39 y or other=2,03), health status (or regular= 1,69 or bad/very bad= 2,05) and chronic diseases (or 1-3=1,56 or 4-6=1,82 or >6=2,59). individual factors accounted for a very few variance at the two levels regression model (s=0,261) and none of the regional variables improved the model. conclusions: individual factors do not explain enough the observed disability variations among the regions and none factor related to the geographical setting has been identified as statistically significant.
