Transfusiones en pacientes con pruebas de compatibilidad positivas y en aquellos con anemia hemolítica autoinmune

Abstract: positive compatibility tests in patients who need blood transfusions are common. patients with autoimmune hemolytic anemia may have warm or cool autoantibodies that react with red blood cell antigens and produce hemolytic transfusion reactions. multitransfused patients, and those with either chronic renal disease or sickle cell disease, have alloantibodies that make blood transfusion difficult. in some series the frequency of such antibodies is as high as 32%. detection and determination of specificity of autoantibodies and alloantibodies are now possible because of technical developments in laboratory. an integral approach to patients and the knowledge of these procedures are the best bases to offer secure blood transfusions.

Es frecuente la presencia de pruebas de compatibilidad positivas en pacientes que requieren soporte transfusional. Los pacientes con anemia hemolítica autoinmune presentan autoanticuerpos calientes o fríos que reaccionan contra antígenos de los eritrocitos y producen reacciones hemolíticas, que pueden ir desde leves hasta graves. Los pacientes multitransfundidos o con insuficiencia renal crónica o con anemia de células falciformes, presentan aloanticuerpos que dificultan el tratamiento con derivados de la sangre. La frecuencia de los mismos es hasta de 32% en algunas series. Dado el perfeccionamiento de las técnicas de laboratorio, en la actualidad son posibles la detección y determinación de la especificidad de los autoanticuerpos y aloanticuerpos; el enfoque integral del paciente y el conocimiento de dichas técnicas son los mejores fundamentos para un soporte transfusional seguro.

Abstract: Actualmente los consumidores no son pasivos, están altamente conectados y vinculados con las empresas. Por ello, algunas empresas aprovechan estas características para innovar productos a partir de los aportes de clientes. La co-creación como modelo colaborativo para la innovación se caracteriza por estar dividida en etapas relacionadas entre sí, con el fin de capturar apropiadamente los aportes de los clientes. La capacidad de valorar y clasificar los aportes de los agentes que participan en este proceso colaborativo es una tarea de crucial importancia para las organizaciones. El presente artículo propone un sistema difuso que permite valorar los aportes de los agentes que participan en forma colaborativa en la co-creación de productos y servicios. Now the customers are not passive, they are highly networked and connected with companies. Some companies take advantage of these features to innovate products from customer contributions. Co-creation like collaborative model to the innovation is characterized from being divided into interrelated stages in order to capture client's contributions in an appropriate way. The ability to assess and rank the contributions of the actors involved in this collaborative process is a crucial organizations task. The present paper proposes a fuzzy system that allows valuation for agent's contributions that participate in a collaborative way in products and services co-creation.
Seguridad y eficacia de la utilización de levosimendán sin dosis de carga en una unidad de falla cardiaca especializada

Abstract: objective: to evaluate the safety and effectiveness of levosimendan administration in patients with chronic heart failure, signs of low output, and indications for inotropic therapy. the protocol included a continuous infusion of levosimendan without loading dose, with a total dosage of 12.5 miligrams. methods: a retrospective, descriptive study was performed to evaluate changes in mean arterial pressure, the development of arrhythmias, adverse effects like death during the infusion, and functional changes in the two months after the treatment. results: the infusion was well tolerated, without side effects. mean arterial pressure before the infusion was 71± 21.6 vs. 75 ± 20.9 mmhg 24 hours after and 71 ± 21.6 vs. 71.6 ± 21.4 48 hours after. there was one death (4.7%), explained by non-cardiovascular causes (sepsis). an improvement in functional class was observed during the follow-up. conclusion: this report of an initial experience suggests that the administration of levosimendan infusion without loading dose is safe and can be used outside the intensive care unit (acta med colomb 2011: 36: 68-72).
Neumonía intersticial asociada a sirolimus en un paciente con trasplante hepático

Abstract: sirolimus is an immunosuppressive drug that has been used during the past few years. sirolimus is indicated in rescue therapies and to reduce the secondary toxic effects of calcineurin inhibitors. this drug has been associated with infrequent but severe pulmonary toxicity. cases of interstitial pneumonitis, bronchiolitis obliterans with organizing pneumonia, and alveolar proteinosis have been described. we describe a case of pulmonary toxicity associated with the use of sirolimus in a 59-yr-old liver transplant recipient. we also review all reported cases of sirolimus-associated lung toxicity among liver transplantation recipients, with the intention of understanding the risk factors, the clinical picture and the outcomes of this complication. five cases have been reported since january 2000, including the present case. clinical presentation is similar, with fever, dyspnea, fatigue, cough, and hemoptysis. discontinuation of the drug led to resolution of clinical and radiographic findings. sirolimus-induced pulmonary toxicity is a serious condition and should be considered in the differential diagnosis of liver recipients presenting with respiratory findings. discontinuation of the drug is associated with resolution of the pulmonary compromise.

Abstract: Objetivo: evaluar la eficacia y seguridad del uso de levosimendán en pacientes con falla cardiaca severa crónica descompensada con signos evidentes de bajo gasto, e indicación de utilización de inotrópico, utilizando el medicamento sin dosis de carga, con una mayor concentración y en infusión continua hasta completar la dosis de 12.5 miligramos en total. Metodología: estudio retrospectivo, descriptivo. Se evaluaron los cambios en la presión arterial media, desarrollo de arritmias o muerte durante la infusión y clase funcional I (NYHA) antes y dos meses después de recibir el medicamento. Resultados: se observó cambio en la clase funcional durante el seguimiento a dos meses. La infusión del medicamento fue bien tolerada y no se observaron efectos adversos significativos durante la hospitalización. La presión arterial media antes de la infusión fue 71± 21.6 vs 75 ± 20.9 mmHg 24 horas después, y 71 ± 21.6 vs 71.6 ± 21.4 mmHg 48 horas después. Ningún paciente presentó arritmias inducidas por el inotrópico y se presentó solamente una muerte (4.7%) por sepsis. Conclusiones: el presente reporte es una experiencia inicial que sugiere que la administración de la infusión de levosimendan sin bolo inicial disminuye la incidencia de hipotensión y arritmias en los pacientes con falla cardiaca y puede emplearse en un servicio de hospitalización diferente a la Unidad de Cuidado Intensivo con seguridad (Acta Med Colomb 2011: 36: 68-72). Abstract: Con la utilización de sirolimus en pacientes trasplantados, se han reportado casos de neumonitis intersticial, bronquiolitis obliterante y neumonía organizada. Nosotros describimos un caso de toxicidad pulmonar asociada al uso de sirolimus en un paciente de 59 a os con trasplante hepático, revisamos los reportes de caso donde se describía toxicidad pulmonar asociada al uso de sirolimus en pacientes con trasplante hepático, con el objetivo de encontrar similitudes en factores de riesgo, manifestaciones clínicas y desenlaces. De los cinco casos reportados desde enero de 2000, incluyendo el presente, encontramos que las manifestaciones clínicas fueron similares presentándose fiebre, disnea, fatiga, tos y hemoptisis. Con la suspensión del medicamento, los pacientes resolvieron sus signos, síntomas y hallazgos imagenológicos. La toxicidad pulmonar asociada a sirolimus debe ser considerada como diagnóstico diferencial de aquellos pacientes con trasplante hepático que reciban este medicamento y presenten manifestaciones respiratorias, pues la suspensión del medicamento lleva a la resolución del cuadro. Abstract: cancer is currently a very serious public health problem worldwide. dendritic cell (dc) based immunotherapy has been one of the most active research fields during the last 20 years. however, results of several studies, mainly involving tumor antigen pulsed autologous dcs, have not been as successful as expected. induction of a tolerant behavior of dcs upon the contact with tumor derived antigens is one of the hypotheses proposed since other effector cells like cd8+, cd4+ t lymphocytes and natural killers, would be rendered to a non-responsive state seriously impairing tumor growth control. in this work we found additional evidence about a likely dc differentiation defect caused by lung adenocarcinoma specific antigens freshly obtained from a patient, as suggested by the presence of the co-receptor cd14 on the surface of mature dcs (cd14 is a known marker for initial stages of dc differentiation). our results confirm that some aspects in tumor antigen treated dcs resemble their immature phenotype, largely less efficient presenting cells, and very weak immune response inducers. interestingly, tumor antigens obtained from the lung adenocarcinoma derived a549 cell line were unable to cause the same effect. this highlights the requirement for a complete characterization of the functional and phenotypic individual conditions when planning a dc based immunotherapy in order to have good success likelihood at cancer patient′s treatment.
CARACTERíSTICAS, DINáMICA Y CAUSAS DEL MOVIMIENTO EN MASA DEL BARRIO EL SOCORRO (31 DE MAYO DE 2008) EN MEDELLíN

Abstract: the article presents the causes and dynamic of the mass movement which occurred on may 31, 2008 in the higher part of san pedro settlement, called las pe?itas, medellín. this tragic event left 27 dead people, 16 injured people and 20 houses destroyed of el socorro settlement. this event with rosellón (1927), media luna (1954), santo domingo (1974), villatina (1987) and el barro (2005) represents one of the most tragic events in the aburrá valley history. for the characterization of the area of the event previous work from different recognized, prestigious institutions was used; for the characterization, description of the predominant dynamics of the event and its causes ground evidence collected on the terrain and visual inspection the same day when it occurred were used, so that it did not change due to proper rescue works. the data of rainfall hydrometeorological stations of empresas públicas de medellín (epm) and colombian institute of hydrology, meteorology and environmental studies (ideam) were used, with the purpose of evaluating the influence of rain like triggering element. the mass movement was classified as simple rotational landslide on ne direction, with a longitude around 60 m, which due to high saturation levels, it changed to an debris flow moving 140 m ahead on se direction, and finally the mass turned ne. triggering causes are associated to geomechanical properties of soil and antecedent rainfall of previous 15 days and very intense rainfall event during may 30, when 83.3 mm fell down for just 2 hours and 30 minutes along the central part of the valley.
