Senning operation for correction of the transposition of the great arteries, results, long-term outcome and quality of life  [PDF]

Abstract: Objective: Long-term results after the Senning operation for transposition of the great arteries are little known. Sinus node dysfunction and systemic ventricular dysfunction are crucial in patient survival. We evaluated the results, long term outcome and quality of life in a group of 39 patients. Methods: The study was a retrospective analysis, of 39 (39/40 = 97.5%) surgical surviving patients, submitted to Senning operation, with a mean follow-up time of 14.7 +/– 3.1 years. Electrocardiogram, echocardiogram, and chest radiograph series were performed every 6 months. Thirty six patients of this series underwent Holter study and ergometric test, to evaluate the physical capacity. Three patients living overseas were excluded. Results: There was only one late death (1/39) (late mortality = 2.5%): a 16 year-old patient had a no cardiac death. The actuarial survival was 95.0% (38/40) (simple or with little VSD, TGA). The probability of staying in sinus rhythm, in 39 surviving patients was 77.1% (30) or normal right ventricular function was 76.5% (29), 10 to 20 years after operation. The incidence of sinus node and right ventricular dysfunction increased gradually over time. No re-operations and pacemaker implantation, was performed. Functional class: I = 30 (85.7%) cases and functional class II = 5 (14.3%) cases. Conclusions: Patients with simple TGA submitted to Senning procedure in our experience, presented during late follow-up: 1—Low incidence of right ventricular dysfunction and active arrhythmias; 2—Low mortality and no sudden death recorded; 3—Good quality of life and 4—Satisfactory surgical results (free of re-operation or definitive pacemaker implantation).

Abstract: The measure of management risk has been a constant in Economy in the recent past. This issue is present on any kind of economic or financial activity. The solution adopted for international authorities, increasing liquidity of financial market and generating new indicators to support making decisions, means the continuity of applying the monetary theory as general criteria to solve the actual crisis. In this labyrinth, the manuscript uses the Accounting Methodology of Edgeworth’s Box as a new methodology to measure the management of Banking Companies and analyzes the acquisition of the CAM and Bank Sabadell, two Spanish banking companies. Finally, the manuscript analyzes the financial crisis in Spain through the behavior of listed banking companies in IBEX 35. The manuscript concludes that accounting can explain equilibrium in economy, and it only needs to know how they can be measured properly.

Abstract: The analysis of economic policy is made through a new methodology using the information provided by businesses to the Bank of Spain. The financial statements are the result of economic and financial transactions in a year and show a behavior that is likely to be measured by applying the Edgeworth’s box for analysis. This means that it is possible to measure the response of firms against market disruptions through an accounting interpretation of the information contained in the financial statements. The indicators obtained through this methodology measure a position taken in the Edgeworth’s box. Consequently, the observations obtained are random and therefore may explain the evolution of the economy of any country.

Abstract: Open government is a term that has been built socially for a long time; however it is from the promotion that the Obama administration from the United States realizes that it has achieved a high penetration in the political and public agenda of the various countries. The main concern of open government is to articulate efforts of government capacities and modernization of public administration, following the principles of transparency with high openness, participation and collaboration. Abstract: In the last few decades structure optimisation has become a main task in a civil engineering project. As a matter of fact, due to the complexity and particularity of every structure, the great amount of variables and design criteria to considerate and many other factors, a general optimisation’s method is not simple to formulate. As a result, this paper focuses on how to provide a successful optimisation method for a particular building type, high-rise reinforced concrete buildings. The optimization method is based on decomposition of the main structure into substructures: floor system, vertical load resisting system, lateral load resisting system and foundation system; then each of the subsystems using the design criteria established at the building codes is improved. Due to the effect of the superstructure optimisation on the foundation system, vertical and lateral load resisting system is the last to be considered after the improvement of floor. Finally, as a case example, using the method explained in the paper, a 30-story-high high-rise residential building complex is analysed and optimised, achieving good results in terms of structural behaviour and diminishing the overall cost of the structure.

Abstract: one of de most difficult evaluations that a forensic doctor carries out is the one which determines, in a precise clear andjust way, whether or not a person is a handicap. many times, the expert not having an out line or a suitable frame of reference, produces a veredict that is nothing bur an enumeration of incoherent, contradictory and incongruent information. lt has been our concern, after many years of studying veredicts of such kind, after long and extensive conversations with my colleages of section a of the forensic medical counsil, to contribute with a fragme or reference qhich will allow these cases lo be analyzed with a greater closeness to the clinical reality of the patients who request a pension por disability. in any other way, the evaluations are made by mere calculations, most of the time with opinions (and not criteria) deduced front prejudices, stercootypes and personal anecdotic situacions. altrough the frame or reference is fundamental. it is important to mention that the correction and the vacue of medical-legal operations doesn't depend onlv on the methods and technicoves employed, but also on the moral and intellectual qualities of the expert, this article is dedicated especially lo the residents and to all good-willed people, and i would only want it to stimulate thinking on this topic wich in my opinion, is of extreme important.

Late outcome of right ventricular outflow tract repair using bicuspid pulmonary prosthesis in tetralogy of Fallot surgery repair: case report

Abstract: we present a case of a 19 year-old female patient, with tetralogy of fallot and hypoplastic pulmonary ring. diagnosis was performed by echocardiography and cardiac catheterization. the surgical repair was performed at the age of 2 years old. a right ventricular outflow tract remodeling was performed using a porcine pulmonary bicuspid prosthesis. the patient presented a uneventful follow-up during 17 years. postoperative assessment has shown: mild pulmonary insufficiency and mild pulmonary ventricle gradient, satisfactory right ventricular performance and prosthesis not presenting calcification. this is an option to correct the tetralogy of fallot adopted by our institution in the last two decades.
